Prediction of bone marrow fibrosis from complete blood count in myeloproliferative neoplasms (FIBOM-AI): a multicentre machine learning model development and validation study.

BACKGROUND: Bone marrow biopsy is essential for myeloproliferative neoplasms diagnosis, particularly for assessing bone marrow fibrosis. However, it is an invasive procedure that can be difficult to perform in some patients. This study aimed to develop an artificial intelligence (AI) model (FIBOM-AI) to predict grade 2-3 fibrosis using complete blood count (CBC) data and age. METHODS: In this machine learning model, histopathological and CBC data from patients (no age restriction) undergoing bone marrow biopsies, performed either at diagnosis or during follow-up for suspected or established m

Extensive Bone Marrow Involvement by BRAF V660E-Mutated Bi-Phenotypic Erdheim-Chester Neoplasm/Rosai-Dorfman Disease (Mixed Histiocytic Neoplasm) with Atypical Histological Features and Fulminant Hemophagocytosis.

BACKGROUND: Erdheim-Chester disease (ECD) is a recently recognized clonal hematopoietic neoplasm characterized by activating alterations in the MAPK pathway. It involves multi-organ accumulation of abnormal histiocytes, leading to nonspecific clinical manifestations due to inflammation and fibrosis caused by histiocytic infiltration. CASE PRESENTATION: We present a 45-year-old male with nonspecific clinical symptoms and progressive skin and abdominal lesions. Multiple tissue biopsies revealed fibrohistiocytic infiltration but provided an inconclusive diagnosis. Imaging studies showed extensiv

Bone marrow clonal hematopoiesis is highly prevalent in blastic plasmacytoid dendritic cell neoplasm and frequently sharing a clonal origin in elderly patients.

Myelodysplastic syndrome (MDS) and chronic myelomonocytic leukemia (CMML) are reported in up to 20% patients with blastic plasmacytoid dendritic cell neoplasm (BPDCN), where a shared clonal origin is shown in individual case studies. In this study, we performed targeted next generating sequencing on multiple bone marrow (BM), skin or sorted cells from 51 BPDCN patients (68.7 years,14.4-84.7), and detected mutations in BM hematopoietic cells in 65% (30/46) and BPDCN in 100% (27/27), both components showing similar high frequencies of TET2 (60% versus 58%) and ASXL1 (33% versus 40%). Evaluation of JAK2 in B and T cell neoplasms: identification of JAK2(V617F) mutation of undetermined significance (JMUS) in the bone marrow of three individuals.

BACKGROUND/AIMS: The JAK2(V617F) mutation, which has been found in patients with myeloproliferative disorders (MPD), has not yet been evaluated in lymphoproliferative disorders by any adequately sensitive techniques. METHODS: We investigated whether low levels of JAK2(V617F) are present in lymphoid neoplasms using a highly sensitive and highly specific amplification refractory mutation system PCR (ARMS-PCR) assay. RESULTS: While 234 of 237 cases did not carry the JAK2(V617F) allele, it was identified in the bone marrow of 3 B cell lymphoma patients. The mutation was found to be neither assoc

Systemic mastocytosis with an associated myeloid neoplasm: Report from the XVI European Bone Marrow Working Group Course and Workshop, Barcelona 2023.

Systemic mastocytosis with an associated myeloid neoplasm (SM-AMN) represents a diagnostic challenge. The first section of the XVI European Bone Marrow Working Group Workshop, held in Barcelona, Spain, in 2023, focused on such cases. Three main lessons were learned from the workshop. First, both the SM and the AMN components can mask each other. Second, because of their overlapping clinical and laboratory findings, it is usually impossible to recognize advanced systemic mastocytosis within an SM-AMN. In other words, unless the International Consensus Classification "C" findings were clearly ca

Indicators of an increased risk of therapy-related myeloid neoplasms in lymphoma patients: how can we best evaluate severe impairment of bone marrow function?

INTRODUCTION: The development of new lymphoma therapies in recent years has led to a significant increase in patient survival. However, in some cases, despite disease remission, the outcome of the applied therapy is diminished by the development of secondary cancers. These often have dismal outcomes and are unresponsive to standard therapies. Both therapy-related myelodysplastic syndrome (t-MDS) and therapy-related acute myeloid leukemia (t-AML) are well-known side effects of cytotoxic chemotherapy and/or radiation therapy. AREAS COVERED: This review summarizes the key factors associated with

Single-Cell Analysis of Bone Marrow CD8+ T Cells in Myeloid Neoplasms Reveals Pathways Associated with Disease Progression and Response to Treatment with Azacitidine.

ABSTRACT: CD8+ T cells are crucial for antitumor immunity. However, their functionality is often altered in higher-risk myelodysplastic neoplasms (MDS) and acute myeloid leukemia (AML). To understand their role in disease progression, we conducted a comprehensive immunophenotypic analysis of 104 pretreatment bone marrow (BM) samples using mass and flow cytometry. Our findings revealed an increased frequency of CD57+CXCR3+ subset of CD8+ T cells in patients who did not respond to azacitidine (AZA) therapy. Furthermore, an increased baseline frequency (>29%) of the CD57+CXCR3+CD8+ T-cell subset

Progression, transformation, and unusual manifestations of myelodysplastic syndromes and myelodysplastic-myeloproliferative neoplasms: lessons learned from the XIV European Bone Marrow Working Group Course 2019.

Disease progression in myelodysplastic syndromes (MDS) and myelodysplastic-myeloproliferative neoplasms (MDS/MPN) is a major source of mortality. The European Bone Marrow Working Group organized a dedicated workshop to address MDS and MDS/MPN progression, and myeloid neoplasms with histiocytic and lymphoblastic outgrowths in 2019 in Frankfurt, Germany. In this report, we summarize clinical, histopathological, and molecular features of 28 cases. Most cases illustrate that prognostic mutational profiles change during follow-up due to accumulation of high-risk mutations in the trunk clone, and th

Myelodysplastic/myeloproliferative neoplasms-unclassifiable with isolated isochromosome 17q represents a distinct clinico-biologic subset: a multi-institutional collaborative study from the Bone Marrow Pathology Group.

Classification of myeloid neoplasms with isolated isochromosome i(17q) [17p deletion with inherent monoallelic TP53 loss plus 17q duplication] is controversial. Most cases fall within the WHO unclassifiable myelodysplastic/myeloproliferative neoplasms (MDS/MPN-U) category. The uniformly dismal outcomes warrant better understanding of this entity. We undertook a multi-institutional retrospective study of 92 adult MDS/MPN-U cases from eight institutions. Twenty-nine (32%) patients had isolated i(17q) [MDS/MPN-i(17q)]. Compared to MDS/MPN without i(17q), MDS/MPN-i(17q) patients were significantly

Persistence of immunophenotypically aberrant CD34+ myeloid progenitors is frequent in bone marrow of patients with myelodysplastic syndromes and myelodysplastic/myeloproliferative neoplasms treated with hypomethylating agents.

AIMS: Hypomethylating agents (HMAs) exhibit clinical efficacy in patients with myelodysplastic syndromes (MDS) and myelodysplastic/myeloproliferative neoplasms (MDS/MPN). This study was performed to assess residual disease status by flow cytometry immunophenotyping (FCI) methods in patients with MDS or MDS/MPN treated with HMAs, and correlate the findings with clinical response. METHODS: CD34+ myeloid precursors were assessed in 85 patients with MDS and MDS/MPN treated with HMAs using FCI methods. Morphological, cytogenetic and molecular assessments were performed to evaluate the responses.

NPM1 -Mutated Myeloid Sarcoma/Leukemia Cutis Mimicking Blastic Plasmacytoid Dendritic Cell Neoplasm and Heralding Bone Marrow Acute Myeloid Leukemia.

Myeloid sarcoma and blastic plasmacytoid dendritic cell neoplasm (BPDCN) are hematologic malignancies of immature cells that frequently involve the skin. In the absence of bone marrow involvement or an associated myeloid neoplasm, distinguishing myeloid sarcoma with disseminated cutaneous involvement (leukemia cutis) from BPDCN is particularly challenging because of overlapping clinical and immunophenotypic features. We report a case of NPM1-mutated myeloid sarcoma presenting with rapidly progressive cutaneous lesions and no initial medullary disease, resulting in an initial misdiagnosis of BP

Myeloid/Lymphoid Neoplasms with ETV6::PDGFRB Fusion Gene: A Rare Case of Poor Response to Imatinib and Possible Transformation Mechanisms from Myeloid Neoplasms of Bone Marrow to T-Cell Lymphoblastic Lymphoma Invasion in Lymph Nodes.

The ETV6::PDGFRB fusion gene is commonly reported in chronic myelomonocytic leukemia with eosinophilia, yet patients with ETV6::PDGFRB presenting myeloid and lymphoid neoplasms successively have not been reported. Here, we report the first case of a 35-year-old man with myeloid and lymphoid neoplasms harboring an ETV6::PDGFRB fusion gene who demonstrated poor response to imatinib. The patient was diagnosed with an ETV6::PDGFRB fusion gene myeloid neoplasm on initial diagnosis at our hospital. After 5 months of treatment with imatinib, he was diagnosed with T-cell lymphoblastic lymphoma. ETV6::

CD5-Positive Mature B-Cell Neoplasm With Large-Cell Morphology, Predominant Bone Marrow Involvement, and a Leukemic Peripheral Blood Presentation: A Case Report.

A 65-year-old man presenting with pancytopenia and circulating large atypical cells was initially suspected of having acute leukemia. Flow cytometric analysis demonstrated a mature clonal B-cell phenotype, and the integrated findings supported a diagnosis of a CD5-positive mature B-cell neoplasm with large-cell morphology, predominant bone marrow involvement, and a leukemic peripheral blood presentation. Based on the available staging data, primary bone marrow DLBCL was suspected but could not be definitively confirmed. The neoplasm was characterized by CD5 expression, a BCL6 rearrangement, an

Bone Marrow Morphology in Hereditary Thrombocytosis May Show Features of Myeloproliferative Neoplasms: A Case Report of a Child with Homozygous MPL Baltimore Germline Variant and Review of Literature.

Hereditary thrombocytosis (HT) is a rare cause of elevated platelet counts in children, most commonly resulting from germline variants in THPO or MPL. We report the first description of bone marrow morphology in a pediatric patient with germline homozygous MPL Baltimore (K39N) variant, a functional polymorphism resulting in HT. The patient had persistent thrombocytosis from infancy without bleeding or thrombotic complications. Bone marrow biopsy demonstrated mild hypocellularity, increased megakaryocytes forming small clusters, and megakaryocytic atypia partially overlapping with the morpholog

The Rare Trio: Concomitant Bone Marrow Involvement by Smoldering Myeloma and Systemic Mastocytosis with an Associated Hematologic Neoplasm and ALECT-2 Renal Amyloidosis - A Case Report and Literature Review.

INTRODUCTION: Systemic mastocytosis (SM) is a clonal disorder characterized by abnormal accumulation of mast cells in various tissues and is categorized into distinct subtypes per the World Health Organization (WHO) 2022 classification. When SM is associated with another hematologic neoplasm, typically of myeloid lineage, it is classified as systemic mastocytosis with an associated hematologic neoplasm (SM-AHN). Coexistence with plasma cell dyscrasias is rare, with fewer than 15 published cases involving either smoldering multiple myeloma (SMM) or multiple myeloma (MM). ALECT-2 amyloidosis, a
