A 62 year-old male patient presented with isolated oculomotor nerve palsy following a small infarction of the medial ventral midbrain, documented by magnetic resonance imaging. There was a 12-year history of hypertension, but no diabetes mellitus. Angiography revealed atherosclerosis of the paramedian mesencephalic arteries. Magnetic resonance imaging may be useful in patients with small brain stem infarctions.

Cerebellar infarcts have long been underrecognized both clinically and neuroradiologically. In approximately a quarter of cases, they are oedematous and may require, unlike other cerebral infarcts, a sometimes life saving surgery when there are signs of brain stem compression. Benign infarcts are more frequent, but inaugural symptoms are identical: vertigo, headache, vomiting, unsteadiness, and dysarthria of sudden onset. Brain stem and occipital symptoms can be associated and are sometimes prominent. The infarct can be shown by CT but MRI is the method of choice for early recognition, precise

The serotonin-monoamine oxidase and histamine-diamine oxidase systems in the arteria and venous blood of the brain were examined in 65 patients with ischemic cerebral stroke. The examinations have shown that in the acute period of the disease the metabolism of serotonin and histamine is disturbed, the degree and the character of these disturbances depending substantially on the stroke clinical manifestations, process localization and the disease outcome. In infarctions in the large hemispheres without the secondary stem syndrome, the changes of the amine metabolism are determined by the state

Sixteen cases of cerebellar infarction with brain stem compression and tonsillar herniation were studied post-mortem. None was operated. Cardiac embolism was the most common cause (75% of the cases). Infarction involved the superior cerebellar artery territory in 8 cases, and multiple cerebellar artery territories (including in each case the posterior inferior cerebellar artery territory) in 8 cases. It involved the cerebellum only in 10 cases. In the 6 other cases, it was associated with an infarct in the paramedian territory of the brain stem. The oedematous swelling was linked to the large

Polycythaemia vera is a recognised cause of ischaemic stroke. If not treated, this condition may result in recurrent strokes. This is a case of a 61-year-old Caucasian man presenting with the inability to ambulate for 3 days. Brain imaging revealed acute and chronic infarctions in the brain stem and the cerebrum. Polycythaemia vera was diagnosed and treated during the admission. The unique mechanisms and management issues of ischaemic stroke associated with polycythaemia vera are discussed.

The paper gives details of the diagnostic value of MRI in 20 cases of non-tumorous lesions of the pons. MR findings in haematoma (3), vascular malformations (2), infarctions (6), encephalitis (1), demyelinations (6) and atrophies (2) resulted in signal changes typical of the kind of disease. A pontine volume increase or decrease was proved comparing the data of the normal and affected brain stem. The topography of lesions was clearly documented by MRI. Subacute haematomas were better outlined than by CT, while acute infarctions, foci of demyelination and one case of ponsencephalitis were prove

INTRODUCTION: Trousseau's syndrome is characterized as an unexpected, cancer-associated thrombotic event. We describe the first reported case of Trousseau's syndrome associated with rapidly emerging pancreatic cancer potentially triggered by esophagectomy.
PRESENTATION OF CASE: A 79-year-old asymptomatic male with clinical stage I esophageal squamous cell carcinoma underwent thoracoscopic subtotal esophagectomy. On postoperative day 46, the patient presented with weakness of his left upper extremity due to multiple cerebral and cerebellar infarctions, with no evidence of atherosclerotic or ca

This work is dedicated to the study of anatomo-physiological peculiarities of the construction and cerebrovascular circulation of the vestibulo-ocular formations of the brain stem of the human cerebrum. The analysis of anatomo-physiological data enable to convince that the arches of the vertical and horizontal vestibulo-ocular reflexes include many structures of the brain stem of the human cerebrum, which are located along its longitude from the superior colliculi to caudal sections of brain stem. There is an existence of separate mechanisms of horizontal and vertical vestibulo-ocular interact

The course and distribution of the facial corticobulbar tract (CBT) was examined by correlating MRI of brain stem lesions with neurological symptoms and signs including central (C-FP) or peripheral facial paresis (P-FP) in 70 patients with localised infarction of the lower brain stem. C-FP occurred more often in patients with lesions of the lower pons or upper medulla of the ventromedial brain stem. Some patients with dorsolateral infarcts of the upper medulla to the lower pons showed C-FP, mostly on the lesion side. P-FP on the side of the lesion was also seen in patients with dorsolateral in

An eleven year old boy presented with sudden onset right-sided hemiplegia and ipsilateral lower facial weakness. Two-dimensional echo revealed the diagnosis of idiopathic dilated cardiomyopathy with multiple intracardiac clots. MRI scan of head showed infarctions in the area of caudate nuclei, putamen, brain stem and cerebellum. On anticoagulation therapy, all thrombi, except one, disappeared. The child died 2.5 months later due to resistant cardiac failure.

In this report, we describe unilateral medial pontomedullary junction (MPMJ) syndrome as a novel brain stem stroke syndrome. A 68-year-old woman suddenly developed vertigo, ipsilateral facial paresis, contralateral thermal hypoalgesia (TH) and dysphagia without lateral gaze palsy, curtain sign and hoarseness. Magnetic resonance (MR) imaging showed a small infarction at the right MPMJ. MR angiography did not show vertebrobasilar arterial dissection, thrombosis or vasospasm. Finally, her dysphagia regressed over 4 weeks in synchronization with recovery of TH. To the best of our knowledge and bas

Cerebellar infarction is relatively infrequent and accounts for about 2% of all strokes. Its clinical presentation and course are variable. It may resemble vestibulitis in mild cases, but the presentation may be more dramatic in other cases. Cerebellar infarction may cause life-threatening complications such as acute hydrocephalus or brain stem compression, resulting from their mass effect in the posterior fossa or extension of the infarct to the brain stem. Clinical features alone are insufficient for the diagnosis and for follow-up of patients with cerebellar infarction. However the advent o

The authors report 10 patients with progressive neurological deterioration due to massive cerebellar infarctions. Computerized tomography scans confirmed obstructive hydrocephalus and brain stem compression. All 10 patients (seven men, three women; mean age, 59 years) were treated by external ventricular drainage and decompressive suboccipital craniectomy. After discharge from the hospital, they were followed up (23-101 months) and their functional independence was evaluated by the Barthel Index. The condition of three patients with brain-stem infarction had deteriorated despite decompressive

OBJECTIVE: Patients with acute central vestibular syndrome suffer from vertigo, spontaneous nystagmus, postural instability with lateral falls, and tilts of visual vertical. Usually, these symptoms compensate within months. The mechanisms of compensation in vestibular infarcts are yet unclear. This study focused on structural changes in gray and white matter volume that accompany clinical compensation.
METHODS: We studied patients with acute unilateral brain stem infarcts prospectively over 6 months. Structural changes were compared between the acute phase and follow-up with a group of health

We examined the occurrence of brain infarction with hepatic arterial infusion chemotherapy for liver cancer. One hundred and eighty-one cases of hepatic arterial infusion chemotherapy were carried out for liver cancer patients in 4 hospitals associated with Osaka University 2nd Dept. of Surgery. These included metastatic (n = 103) and primary (n = 78) liver tumors. The medication was mainly 5-FU with/without CDDP and IFN. Catheters were inserted via the left subclavian artery in 106 cases and via the femoral artery in 75 cases. Among these patients, brain infarctions occurred in seven patients

A 53-year-old man abruptly developed headache and unconsciousness. Brain computed tomography (CT) and CT angiography showed subarachnoid hemorrhage, intraventricular hemorrhage, and multiple tortuous vascular structures on the brain stem and upper cervical spinal cord. Four-vessel angiography displayed intradural ventral arteriovenous fistula, supplied by the left vertebral and occipital arteries. Drainage was via both sigmoid sinus and cervical venous plexus. He had been treated with transarterial coil embolization of the left vertebral artery. Subsequently, he suffered from left hemiplegia a

Pathological CR of the eyes and deviations of the SVV are among the most sensitive clinical brain stem signs. In acute unilateral brain stem infarctions, deviations of the SVV occur in 94% of cases and CR of one or both eyes is found in 88%. Deviations of CR and the SVV are typically ipsiversive with pontomedullary lesions and contraversive with pontomesencephalic lesions. They may involve a complete OTR, the triad of lateral head tilt, skew deviation and CR. There is a directional linkage between CR and the SVV (i.e. either right or left tilt), but the net tilt angles do not always match quan

Vascular accidents of the cerebellum include heamorrhages and infarctions. These two pathologies share the same clinical symptoms and the same potential severity due to the sudden creation of an expansive cerebellar process (haematoma or infarct). Owing to acute compression of the brain stem and of CSF channels in the posterior fossa, and to the resulting supratentorial obstructive hydrocephalus, cerebellar vascular accidents are acute neurosurgical emergencies. Although the diagnosis and prognosis of the lesions have been transformed by computerized tomography of the brain, they remain extrem

Central alveolar hypoventilation syndrome (CAH), or Ondine's curse, is a very rare disease characterized by dysfunction of respiratory center in the brain stem. Here, we report a case of CAH associated with cerebral infarction. A 59-year-old man developed right facial sensory deficit at age 56. Then, the facial sensory deficit spread to the left side and dysarthria and dysphagia also developed. Since age 58, he often developed respiratory failure and consciousness disturbance. Arterial blood gas analysis revealed alveolar hypoventilation and respiratory acidosis. Disorders of peripheral organs

BACKGROUND: Avellis' syndrome (lateral medulla oblongata ischaemia) is a rare neurological disorder. Early recognition of brain stem infarction may prevent referral to specialists other than a neurologist.
CASE DESCRIPTION: A 53-year-old woman was initially presented to an internist with acute onset of headache, dysphagia, nausea and vomiting. She was also hoarse and had a tingling feeling on the left side of her face, and in her left arm and leg. A neurologist was consulted and diagnosed Avellis' syndrome. MRI of the brain revealed ischaemia in the lateral medulla oblongata on the right side
