Mammographic density and survival in interval breast cancers.

INTRODUCTION: Mammographic density (MD) is the strongest risk factor for breast cancer. It is also strongly associated with interval cancers (ICs) due to decreased screening sensitivity and possibly by also giving rise to more aggressive tumors. With this information as background, we compared survival in interval and screen-detected cancers, taking MD into consideration. METHODS: The patients were postmenopausal women ages 50 to 74 years who were diagnosed with breast cancer in Sweden between 1993 and 1995. A total of 1,115 women with screen-detected cancers and 285 with ICs had available ma

Women's responses to information on mammographic breast density.

The objective of this study was to determine the negative and positive outcomes of providing mammographic breast density (MBD) information to participants of a screening program. A randomized experiment was conducted with a sample of 618 women 50 years or older with MBD greater than 50% of breast volume. The intervention consisted of reporting the presence of MBD in the screening mammography results letter that was sent along with an information pamphlet. Compared to the controls, more women in the intervention group described the term breast density correctly and recognized it as a risk facto

Is mammographic density a marker of breast cancer phenotypes?

PURPOSE: To investigate the association between mammographic density (MD) phenotypes and both clinicopathologic features of breast cancer (BC) and tumor location. METHODS: MD was measured for 297 BC-affected females using qualitative (visual method) and quantitative (fully automated area-based method) approaches. Radiologists' description, visible external markers, and surgical scar were used to establish the location of tumors. Binary logistic regression models were used to assess the association between MD phenotypes and BC clinicopathologic features. RESULTS: Categorical and numerical MD

Effect of danazol on the radiographic density of breast parenchyma.

We have given danazol to 25 women with mammary dysplasia. Clinical, cytologic and radiographic examinations were made before, during and after this treatment. In 23 patients breast pain disappeared completely. Danazol produced such a decrease in breast volume and density that on the mammogram the parenchyma was much more easily visualized. We suggest that danazol might be given before mammography to high risk women having dense dysplasic breasts, in order to ensure easier and more accurate interpretation.

Automated Breast Density Assessment for Full-Field Digital Mammography and Digital Breast Tomosynthesis.

Mammographic density is a strong risk factor for breast cancer and is reported clinically as part of Breast Imaging Reporting and Data System (BI-RADS) results issued by radiologists. Automated assessment of density is needed that can be used for both full-field digital mammography (FFDM) and digital breast tomosynthesis (DBT) as both types of exams are acquired in standard clinical practice. We trained a deep learning model to automate the estimation of BI-RADS density from a prospective Washington University clinic-based cohort of 9,714 women, entering into the cohort in 2013 with follow-up

Association of mammographic breast density measurements and hormone receptor status of breast cancer.

PURPOSE: Breast cancer is the most frequent cancer in women, with significant mortality. Mammography is a routine investigation for breast disease. A known risk factor for breast cancer is increased breast density. Here, we tried to observe if mammographic density also affects the hormone receptor status of breast cancer, which will help in the understanding of the biological mechanisms of breast cancer development. MATERIAL AND METHODS: Suspected breast cancer patients at Lok Nayak Hospital, Delhi, underwent mammography in the Department of Radiodiagnosis. The density of breast contralateral

Long-term excess risk of breast cancer after a single breast density measurement.

AIM: Breast density is a risk factor for breast cancer. As density changes across a woman's life span, we studied for how long a single density measurement taken in (post-)menopausal women remains informative. METHODS: We used data from Singaporean women who underwent a single mammography screen at age 50-64 years. For each case with breast cancer diagnosed at screening or in the subsequent 10 years, whether screen detected or diagnosed following symptoms, two age-matched controls were selected. We studied the excess risk of breast cancer, calculated as an odds ratio (OR) with conditional log

Prognostic Influence of Preoperative Mammographic Breast Density in Operable Invasive Female Breast Cancer.

We aimed to investigate the potential of preoperative mammographic breast density (MBD) as a prognostic factor in breast cancer. Data of 969 patients with primary breast cancer were analyzed. We defined low MBD as fatty or fibroglandular breast, and high MBD as heterogeneously dense or extremely dense breast, respectively. The high MBD group demonstrated a superior overall survival rate compared to the low MBD group (p < 0.001). Favorable prognostic effects of high MBD were observed in subgroups aged >50 years (p < 0.001) and with positive hormone receptor (HRc) and negative human epidermal gr

A prospective study of breast cancer risk using routine mammographic breast density measurements.

Mammographic breast density is a major risk factor for breast cancer but estimates of the relative risk associated with differing density patterns have varied widely. It is also unclear how menopausal status influences this association and to what extent the effects of density are due to its correlation with other risk factors. Most recent investigations of breast density have been case-control studies, which provide indirect estimates of relative risk. We have prospectively followed 61,844 women for an average of 3.1 years to directly estimate risk among women in the four mammographic breast

Impact of breast density on re-excision and recurrence of breast cancer following breast conserving surgery.

BACKGROUND: Mammographic breast density (BD) has garnered clinical interest due to its impact on breast cancer (BC) risk, sensitivity of mammographic screening, and tumour size at presentation. There is conflicting evidence as to BD's effect on re-excisions and recurrence following breast conserving surgery (BCS). We hypothesize that increasing BD may obscure disease, resulting in an underestimation of the extent of disease, and a greater risk of re-excision and local recurrence following BCS for invasive BC. METHODS: A retrospective, population-based review of patients undergoing BCS for inv

Temporal changes in mammographic breast density and breast cancer risk among women with benign breast disease.

INTRODUCTION: Benign breast disease (BBD) and high mammographic breast density (MBD) are prevalent and independent risk factors for invasive breast cancer. It has been suggested that temporal changes in MBD may impact future invasive breast cancer risk, but this has not been studied among women with BBD. METHODS: We undertook a nested case-control study within a cohort of 15,395 women with BBD in Kaiser Permanente Northwest (KPNW; 1970-2012, followed through mid-2015). Cases (n = 261) developed invasive breast cancer > 1 year after BBD diagnosis, whereas controls (n = 249) did not have breast

Mammographic Breast Density, Benign Breast Disease, and Subsequent Breast Cancer Risk in 3.9 Million Korean Women.

Background Mammographic breast density and benign breast disease are strong risk factors for breast cancer. Accordingly, women with both risk factors may have a markedly high risk for developing breast cancer. Purpose To investigate the risk of breast cancer associated with the combination of mammographic density and benign breast disease in Korean women, where population-based mammographic breast cancer screening is provided for all women aged at least 40 years. Materials and Methods This is a retrospective analysis of data from a nationwide breast cancer screening program linked with the nat

Mammographic radiomics and breast density for predicting PD-L1 expression in breast cancer.

BACKGROUND: Programmed death-ligand 1 (PD-L1) expression is a critical biomarker for guiding immunotherapy in breast cancer, particularly in triple-negative subtypes. However, conventional assessments rely on invasive biopsies and are limited by tumor heterogeneity. This study aims to develop a non-invasive approach for predicting PD-L1 expression using mammography-based radiomics features integrated with clinicopathological variables and breast density, and to evaluate its performance in both an internal development cohort and an independent external validation cohort. METHODS: A total of 121

Canadian Society Breast Imaging Position Statement on Mammographic Breast Density and Supplemental Screening.

Screening aims to detect breast cancer before it becomes clinically apparent, enabling identification of tumors when they are smaller and have not yet spread and when treatment options are more effective, less invasive, and more affordable. However, screening mammography has known limitations, with breast density being a primary challenge. Denser breast tissue not only increases the likelihood of cancer but also makes tumors harder to detect due to overlapping tissue. Strong evidence now exists to support updating our previous guidelines to recommend supplemental screening beyond mammography f

Impact of adding breast density to breast cancer risk models: A systematic review.

PURPOSE: Assessment of a woman's risk of breast cancer is essential when moving towards personalized screening. Breast density is a well-known risk factor and has the potential to improve accuracy of risk prediction models. In this study we reviewed the impact on model performance of adding breast density to clinical breast cancer risk prediction models. METHODS: We conducted a systematic review using a pre-specified search strategy for PubMed, EMBASE, Web of Science, and Cochrane Library from January 2007 until November 2019. Studies were screened using the Covidence software. Eligible studi

Independent association of lobular involution and mammographic breast density with breast cancer risk.

BACKGROUND: Lobular involution, or age-related atrophy of breast lobules, is inversely associated with breast cancer risk, and mammographic breast density (MBD) is positively associated with breast cancer risk. METHODS: To evaluate whether lobular involution and MBD are independently associated with breast cancer risk in women with benign breast disease, we performed a nested cohort study among women (n = 2666) with benign breast disease diagnosed at Mayo Clinic between January 1, 1985, and December 31, 1991 and a mammogram available within 6 months of the diagnosis. Women were followed up fo

Impact of breast density on the efficacy of radiofrequency ablation in early-stage breast cancer.

BACKGROUND: Radiofrequency ablation (RFA) is a minimally invasive technique employed in the management of small breast tumors. RFA involves the delivery of a high-frequency current through a needle electrode under ultrasound guidance. In Japan, RFA has been covered by insurance since 2023 as a localized treatment option for early-stage breast cancers. METHODS: We retrospectively analyzed the data of patients who underwent RFA at our institution between February 2016 and March 2025. Breast density was classified into four categories based on the Breast Imaging Reporting and Data System. Associ

Preoperative MRI in breast cancer: effect of breast density on biopsy rate and yield.

PURPOSE: Preoperative breast MRI is used to evaluate for additional cancer and extent of disease for newly diagnosed breast cancer, yet benefits and harms of preoperative MRI are not well-documented. We examined whether preoperative MRI yields additional biopsy and cancer detection by extent of breast density. METHODS: We followed women in the Breast Cancer Surveillance Consortium with an incident breast cancer diagnosed from 2005 to 2017. We quantified breast biopsies and cancers detected within 6 months of diagnosis by preoperative breast MRI receipt, overall and by breast density, accounti
