Inhibitory effect of 5β-pregnane-3α,20β-diol on transcriptional activity and enzyme activity of human bilirubin UDP-glucuronosyltransferase.

Prolonged unconjugated hyperbilirubinemia in infants associated with breast milk feeding is a common pediatric problem known as breast milk jaundice (BMJ). A polymorphic mutation (G71R) of bilirubin UDP-glucuronosyltransferase (UGT1A1) is a known cause of BMJ on the infantile side, but the responsible components of breast milk are not currently known. We analyzed the inhibitory effect of 5β-pregnane-3α,20β-diol (pregnanediol) on transcriptional activity and enzyme activity of UGT1A1. To this end, we constructed two types of expression vectors. One type comprised vectors including the upstream

Macronutrient and energy content of breast milk of mothers delivering prematurely.

The protein, lactose, fat and energy contents of the fore-milk of mother with term (n = 23) and preterm (n = 29) infants were estimated on postpartum days 3, 7, 14 and 21. During the first 4 weeks of lactation, the mean (+/- SD) energy (Kcal/dl), protein (g/dl), fat (g/dl) and lactose (g/dl) levels of the preterm milk were: 56.39 (+/- 7.99), 2.17 (+/- 0.66), 2.30 (+/- 0.48) and 5.78 (+/- 0.99), respectively. The same for term milk were: 59.39 (+/- 8.30), 1.99 (+/- 0.70), 2.48 (+/- 0.53) and 6.24 (+/- 1.08), respectively. The differences in composition between the term and preterm milk were not

Higher expression of SOX1, miR-155, and miR-21 in the colostrum of SARS-CoV-2-infected mothers.

The aim of this study was to assess the impact of SARS-CoV-2 infection during pregnancy on the molecular profile of human milk and to identify associations between the expression of regulatory genes and clinical parameters of newborns. The study included a group of 19 women who had COVID-19 during pregnancy and a healthy control group of 21 women. Expression levels of miR-21, miR-155, and the transcription factor SOX1 were determined in human milk using qPCR. The correlation of SOX1 expression with parameters in cord blood and infant blood was also analyzed. It was shown that maternal SARS-CoV

Breastfeeding Mothers' and Nurses' Perceptions of Breast Milk Sharing and Donation for Research in Bhutan: A Qualitative Study.

INTRODUCTION: Breast milk is globally recognized as the optimal source of infant nutrition due to its immunological and neurodevelopmental benefits. In low-and middle-income countries like Bhutan, limited availability of donor milk and absence of milk banking infrastructure hinder neonatal nutrition. While breast milk sharing for feeding has been explored elsewhere, little is known about maternal and nursing perspectives of milk donation for research use within Bhutanese sociocultural context. OBJECTIVE: To explore breastfeeding mothers' perceptions of breast milk sharing and donation for bot

Formula versus donor breast milk for feeding preterm or low birth weight infants.

BACKGROUND: When sufficient maternal breast milk is not available, alternative sources of enteral nutrition for preterm or low birth weight infants are donor breast milk or artificial formula. Donor breast milk may retain some of the non-nutritive benefits of maternal breast milk for preterm or low birth weight infants. However, feeding with artificial formula may ensure more consistent delivery of optimal levels of nutrients. Uncertainty exists about the balance of risks and benefits of feeding formula versus donor breast milk for preterm or low birth weight infants. OBJECTIVES: To determine

Pathological postpartum breast engorgement: prediction, prevention, and resolution.

BACKGROUND: Severe breast engorgement can cause substantial discomfort for mothers and interfere with an infant's ability to feed at the breast. This study explored the possibility of prediction of pathological postpartum breast engorgement in lactating women in relation to intense breast engorgement at the end of the luteal phase of the menstrual cycle and the possibility of prevention and resolution of postpartum breast engorgement with expression with a breast pump of colostrum before the appearance of transitional milk. SUBJECTS AND METHODS: The first group included 70 women with patholog

Human Breast Milk miRNA, Maternal Probiotic Supplementation and Atopic Dermatitis in Offspring.

BACKGROUND: Perinatal probiotic ingestion has been shown to prevent atopic dermatitis (AD) in infancy in a number of randomised trials. The Probiotics in the Prevention of Allergy among Children in Trondheim (ProPACT) trial involved a probiotic supplementation regime given solely to mothers in the perinatal period and demonstrated a ~40% relative risk reduction in the cumulative incidence of AD at 2 years of age. However, the mechanisms behind this effect are incompletely understood. Micro-RNAs (miRNA) are abundant in mammalian milk and may influence the developing gastrointestinal and immune

Relationship of insulin, glucose, leptin, IL-6 and TNF-α in human breast milk with infant growth and body composition.

BACKGROUND: Numerous appetite, growth, obesity-related hormones and inflammatory factors are found in human breast-milk, but there is little evidence on their relationship with infant body composition. OBJECTVIE: The purpose of the present cross-sectional pilot study was to assess the cross-sectional associations of appetite-regulating hormones and growth factors (leptin, insulin and glucose) and inflammatory factors (interleukin 6 (IL-6) and tumor necrosis factor alpha (TNF-α)) in human breast-milk with infant size, adiposity, and lean tissue at 1-month of age in healthy term infants. Evaluation of a breastmilk hand expression toolkit: the M.I.L.K survey study.

BACKGROUND: Breastmilk hand expression (BMHE) is recommended to promote lactation, relieve breast engorgement, and collect milk for future infant feedings. Resources to teach this skill are limited and infrequently developed in partnership with the obstetrical population. In collaboration with maternity care experts and individuals with recent breastfeeding experience, we designed a one-page toolkit that describes the process of BMHE and includes step-by-step instructions and images to illustrate the technique. This study aimed to evaluate the readability, clarity of content, layout, and infor

Partitioning of persistent organic pollutants (POPs) between human serum and breast milk: a literature review.

The literature was reviewed to assess the relationship between the lipid adjusted concentration in human serum and breast milk (expressed as the serum/milk ratio) of a broad range of POPs in paired samples. Thirteen studies were identified, including seven studies that reported serum/milk ratios for polychlorinated dibenzo-dioxins and -furans (PCDD/Fs), ten for polychlorinated biphenyls (PCBs), five for polybrominated diphenyl ethers (PBDEs), and five for organochlorine pesticides (OCPs). Mean serum/milk ratios ranged between 0.7 and 25 depending on the compound and congener. For PCDD/Fs, PCBs

Is prevention of food allergy worthwhile?

Several studies performed in high-risk babies have demonstrated a significant reduction in the prevalence and severity of atopic diseases with dietary and environmental manipulations. It has been demonstrated that prolonged breast-feeding and the avoidance of cow's milk, eggs and fish during the first three months of lactation significantly decrease both the prevalence and the severity of atopic disease up to the age of 5 years. We have shown a significant reduction in both the prevalence and the incidence of atopic dermatitis, food allergy and asthma in high-risk children followed up to the a

Maternal ω3 docosapentaenoic acid inhibits infant allergic dermatitis through TRAIL-expressing plasmacytoid dendritic cells in mice.

BACKGROUND: Maternal dietary exposures are considered to influence the development of infant allergies through changes in the composition of breast milk. Cohort studies have shown that ω3 polyunsaturated fatty acids (PUFAs) in breast milk may have a beneficial effect on the preventing of allergies in infants; however, the underlying mechanisms remain to be investigated. We investigated how the maternal intake of dietary ω3 PUFAs affects fatty acid profiles in the breast milk and their pups and reduced the incidence of allergic diseases in the pups. METHODS: Contact hypersensitivity (CHS) indu

Changes in the milk composition of nonpuerperal women.

Studies were undertaken to determine the effect of expressing small volumes of breast fluid (less than 3.0 ml) on the progressive changes in concentrations of lactose, proteins, and electrolytes in the mammary secretions of four nonpuerperal women with galactorrhea and two nonpuerperal women who artificially induced lactation by breast hyperstimulation. The changes in the concentration of the milk constituents in the mammary secretion of these women are compared to the levels in colostrum, transitional and mature milk obtained from normal lactationg women. In one woman galactorrhea was associa

Diabetes and antenatal milk expressing: a pilot project to inform the development of a randomised controlled trial.

OBJECTIVE: infants of women with diabetes in pregnancy are at increased risk of hypoglycaemia. If the infant's blood glucose is low and the mother is unable to breast feed/provide sufficient expressed breast milk, infants are often given formula. Some hospitals encourage women with diabetes to express breast milk before birth. However, there is limited evidence for this practice, including its impact on labour and birth, e.g. causing premature birth may be a concern. A pilot study was undertaken to establish the feasibility of conducting an adequately powered randomised controlled trial to eva

Manual expression and electric breast pumping in the first 48 h after delivery.

BACKGROUND: Early feeding for preterm infants via the mother's own milk is crucial for lowering morbidity and mortality. Obtaining the mother's milk in the first few days is sometimes difficult; an effective way of mediating this problem has not yet been established. The aim of the present study was therefore to investigate whether breast pumping using a hospital-grade electric pump was more effective in maximizing the available milk volume and more comfortable than manual expression in the first 48 h after birth. METHODS: A sequential cross-over study was performed in a maternity ward, in a
