A follow-up study of the progress of keratoconjunctivitis sicca and response to treatment in primary Sjögren's syndrome.

Thirty-four patients with Sjögren's syndrome were retrospectively examined in order to evaluate longitudinal alterations in objective ocular disease parameters and their possible relation to systemic bromhexine treatment. Twenty-three patients (68%) were initially found to respond to peroral bromhexine treatment and were subsequently treated with this agent in addition to tear substitutes. The other 11 patients (32%) were considered bromhexine non-responders and were treated with topical agents only. The bromhexine non-responders had a significantly (p less than 0.02) more reduced tear gland f

[Pharmacological effects of brovanexine hydrochloride (BR-222) on the respiratory tract system, particularly on the respiratory tract fluid, mucociliary transport and cough].

Pharmacological effects of brovanexine hydrochloride (BR-222), a new expectorant, on the respiratory tract system was studied in comparison with that of bromhexine hydrochloride. 1. When doses ranging from 5 to 40 mg/kg of BR-222 were given orally (p.o.) or intraduodenally (i.d.) to rats, rabbits and dogs, it caused a significant increase in the output volume of respiratory tract fluid (RTF). It was almost as potent as bromhexine, but its pattern of increasing RTF was different from that of bromhexine. The increase in the serous ingredient of RTF after BR-222 administration seemed to be more r

In vitro inhibition of human neutrophil histotoxicity by ambroxol: evidence for a multistep mechanism.

Neutrophils are major culprits for the protease/antiprotease imbalance during various lung diseases, that is, chronic obstructive pulmonary disease, cystic fibrosis, idiopathic pulmonary fibrosis and adult respiratory distress syndrome. Thus, these cells are presently considered an ideal target for the pharmacologic control of tissue injury during these diseases. This study was planned in order to investigate if ambroxol and its precursor bromhexine are actually capable of preventing alpha-1-antitrypsin (A1AT) inactivation by stimulated neutrophils and possibly to look into the mechanisms unde

Complete, Rapid Resolution of Severe Bilateral Pneumonia and Acute Respiratory Distress Syndrome in a COVID-19 Patient: Role for a Unique Therapeutic Combination of Inhalations With Bromhexine, Higher Doses of Colchicine, and Hymecromone.

An otherwise healthy, 35-year-old man was hospitalized for the management of acute respiratory failure due to coronavirus disease 2019 (COVID-19)-related severe bilateral pneumonia and acute respiratory distress syndrome (ARDS). The patient therapeutic regimen included the widely accepted standard combination of oxygen, anticoagulation therapy; corticosteroids, non-steroidal anti-inflammatory drugs (NSAIDs), and antibiotics. A novel combination of colchicine, hymecromone, and bromhexine inhalations was added to the therapeutic regimen as part of our unique COVID-19 management institutional pro

The negative effects of mucolytics in otitis media with effusion.

Much controversy still exists in the treatment of otitis media with effusion. A double-blind randomised prospective trial during which bromhexine hydrochloride was compared with a placebo is reported. Information from 60 patients was analysed and the results reveal interesting findings on the effects of bromhexine on middle-ear clearance. The data clearly reveal that bromhexine retards resolution of the effusion. Further discussion of the literature gives insight into the controversy surrounding the treatment of middle-ear effusion. Steroids are proposed for further research and clinical trial

Discovery of novel inhibitors for Pseudomonas aeruginosa lipase enzyme from in silico and in vitro studies.

Pseudomonas aeruginosa is an opportunistic pathogen prone to developing drug-resistance and is a major cause of infection for burn patients and patients suffering from cystic fibrosis or are hospitalized in intensive care units. One of the virulence factors of this bacterium is the lipase enzyme that degrades the extracellular matrix of the host tissue and promotes invasion. Bromhexine is a mucolytic drug and has recently been reported to function as a competitive inhibitor of lipase with an IC50 value of 49 µM. In the present study, an attempt was made to identify stronger inhibitors from the

Activity of some mucolytics against bacterial adherence to mammalian cells.

In this article, some mucolytic agents were tested for their activity to prevent bacterial adherence to mammalian cells. Preliminary screening for antiadherent activity showed that ambroxol, bromhexine, ammonium chloride, and ammonium acetate but neither guaiphenesin nor carbocysteine significantly reduced the adherence of the tested clinical isolates to cultured mammalian cells. The antiadherent effect of such agents was observed when mammalian cells were treated with these agents either before or after bacterial adherence, and this effect was exhibited in a dose-dependent manner. The minimum

[Experimental studies on the influence of selected preparations on secretion of tear fluid. I. Schirmer test I and lysozyme test].

In the paper the influence of ambroxol, betanechol and mesna on the production of tear fluid by the lacrimal gland in rabbits was studied and compared with the effectiveness of bromhexine hydrochloride. Tear secretion was stimulated by the Schirmer I and lysozyme tests. The obtained results indicate that all four examined drugs stimulate the production of tear fluid. The greatest stimulation of secretion was observed after mesna and then after betanechol and ambroxol. The least potent drug was bromhexine.

SARS-CoV-2 and SARS-CoV Spike-Mediated Cell-Cell Fusion Differ in Their Requirements for Receptor Expression and Proteolytic Activation.

Severe acute respiratory syndrome-related coronavirus 2 (SARS-CoV-2) infects cells through interaction of its spike protein (SARS2-S) with angiotensin-converting enzyme 2 (ACE2) and activation by proteases, in particular transmembrane protease serine 2 (TMPRSS2). Viruses can also spread through fusion of infected with uninfected cells. We compared the requirements of ACE2 expression, proteolytic activation, and sensitivity to inhibitors for SARS2-S-mediated and SARS-CoV-S (SARS1-S)-mediated cell-cell fusion. SARS2-S-driven fusion was moderately increased by TMPRSS2 and strongly by ACE2, while

Mucolytic agents and mucociliary activity.

The three substances bromhexine, NA 872 (bromhexine metabolite VIII) and the beta 2-receptor agonist NAB 365 (clenbuterol) and their effect on the mucociliary activity have been studied in vivo in rabbits. The substances were given parenterally in increasing dosages and 519 records were obtained from 12 rabbits. Not even at dosages in excess of recommended human dosages has any effect on the mucociliary activity been observed for any of the substances during the first 30 minutes after administration. The conclusion to be drawn is that the three substances have no effect on the mucociliary acti

Effects of expectorants on the canine tracheal ciliated cells.

Many expectorants are clinically effective as they reduce viscosity and facilitate expectoration. There are, however, few reports on effects of expectorants on tracheal ciliated cells. We investigated the effects of N-acetylcysteine, ethylcysteine and bromhexine on the ciliary activities of the canine trachea. Ciliary movement was estimated using a phototransistor, and intracellular electrical activity was measured with a microelectrode method, in vitro. N-Acetylcysteine, ethylcysteine and bromhexine in low concentrations under a low perfusion rate (0.1 ml/min) produced an increase in the ampl

Mucoactive Agents in the Therapy of Upper Respiratory Airways Infections: Fair to Describe Them Just as Mucoactive?

BACKGROUND: Upper and lower respiratory tract infections are common conditions for which medical advice is sought, and their management relies on the use of prescription and over-the-counter (OTC) medicines. Ambroxol, bromhexine, carbocysteine, erdosteine, N-acetyl cysteine (NAC), and sobrerol are mucoactive agents for which clinical trials have been conducted, have been awarded well-established status by regulatory authorities, and are available as OTC or prescription products. OBJECTIVE: To briefly review the evidence-based efficacy and safety of these substances in the therapy of upper res

[Experimental studies on the influence of selected preparations on secretion of tear fluid. II: Light and transmission electron microscopy].

In the paper the influence of ambroxol, bethanechol, mesna and bromhexine hydrochloride on the morphologic appearance of the lacrimal glands in rabbits was studied with the use of light and transmission electron microscopy. The examination of the lacrimal glands in light microscopy has shown in comparison with the control group an increase in the number of secretory granules in rabbit glands after treatment with different drugs. In electron microscopy in animals, after treatment, much more prominent rough endoplasmic reticulum with a considerable distention of the cisternae and increase of the

Comparative in vitro analysis of inhibition of rhinovirus and influenza virus replication by mucoactive secretolytic agents and plant extracts.

BACKGROUND: Rhinoviruses and influenza viruses cause millions of acute respiratory infections annually. Symptoms of mild acute respiratory infections are commonly treated with over-the-counter products like ambroxol, bromhexine, and N-acetyl cysteine, as well as of thyme and pelargonium extracts today. Because the direct antiviral activity of these over-the-counter products has not been studied in a systematic way, the current study aimed to compare their inhibitory effect against rhinovirus and influenza virus replication in an in vitro setting. METHODS: The cytotoxicity of ambroxol, bromhex

Mucolytics for bronchiectasis.

BACKGROUND: Bronchiectasis is predominantly an acquired disease process representing the end stage of a variety of unrelated pulmonary insults. It is defined as a persistent irreversible dilatation and distortion of medium-sized bronchi. Patients diagnosed with bronchiectasis frequently have difficulty exporating the infected sputum. Mucolytic agents target hyper-secretion or changed physiochemical properties of sputum to make it easier to clear. One drug, recombinant human DNase, breaks down the DNA that is released at the site of infection by neutrophils. OBJECTIVES: The objective of this r

Effect of increasing doses of colchicine on the treatment of 333 COVID-19 inpatients.

BACKGROUND: Previous research done in Bulgaria demonstrated a fivefold reduction in mortality from COVID-19 with increased doses of colchicine from two hospitals in the country. We report here a further 333 cases of COVID-19 inpatients, treated with different doses of colchicine and its effect on mortality. MATERIALS AND METHODS: A case-control comparison from two additional hospitals was conducted between increased doses of colchicine and added bromhexine to standard of care (SOC) versus current SOC. Risk and odds ratio, as well as subgroup analysis, was conducted with newly reported data, a

Acute interstitial nephritis observed with three different triggering agents.

A 70-year-old female patient developed acute interstitial nephritis (AIN) after treatment with non-steroidal anti-inflammatory drugs (NSAIDs), proton pump inhibitors (PPI), and Bromhexine. Renal biopsy confirmed the diagnosis, and the patient was treated with oral prednisone. Careful attention to timing of acute kidney injury (AKI) is crucial to diagnosing AIN.

Mucolytics for bronchiectasis.

BACKGROUND: Bronchiectasis is predominantly an acquired disease process that represents the end stage of a variety of unrelated pulmonary insults. It is defined as persistent irreversible dilatation and distortion of medium-sized bronchi. It has been suggested that with widespread use of high-resolution computed tomography, more bronchiectasis diagnoses are being made. Patients diagnosed with bronchiectasis frequently have difficulty expectorating sputum. Sputum therefore is retained in the lungs and may become infected, leading to further lung damage. Mucolytic agents target hypersecretion or
