Effect of Bunion Surgery on Fall Risk: A Nationwide Database Study of 127,990 Older Adults with Hallux Valgus.

INTRODUCTION: The presence of a bunion (hallux valgus) deformity is a recognized risk factor for falls among older adults; however, it is uncertain whether surgical correction of the foot deformity can help mitigate this risk. METHODS: We conducted a retrospective cohort study of US adults aged 50 years and over with hallux valgus using commercial, state, and federal healthcare claims obtained from a national database (PearlDiver, Inc.). Patients who underwent bunion surgery were compared to those treated nonoperatively for their bunion deformities from January 2011 to April 2022. Claims-base

Is our current paradigm for evaluation and management of the bunion deformity flawed? A discussion of procedure philosophy relative to anatomy.

Of the >100 procedures that have been proposed to treat hallux valgus or the "bunion" deformity, most have focused on correction through metatarsal osteotomies at various levels combined with soft tissue balancing procedures at the first metatarsophalangeal joint. This paradigm of metatarsal osteotomy and soft tissue balancing has been so commonplace, any argument for a fundamental change to the approach becomes uncomfortable and seems unwarranted to most foot and ankle surgeons. However, the simple fact that so many procedures exist, with so many modifications of these procedures, can be inte

Complete vs Incomplete Percutaneous Oblique Distal Closing Wedge Osteotomy for Bunionette (Tailor's Bunion) Deformity Correction: A Retrospective Comparative Study.

BACKGROUND: There has been increasing interest in the use of percutaneous osteotomy techniques for bunionette (tailor's bunion) correction. This study evaluated clinical and radiographic outcomes following an unfixed, percutaneous oblique distal metaphyseal-diaphyseal osteotomy and compared outcomes between complete and incomplete osteotomy groups. METHODS: A total of 43 feet (mean age 54.2 ± 17.1) underwent percutaneous oblique distal osteotomy by a single surgeon over a 4-year period. The primary outcome was the presence of significant postoperative hypertrophic callus at the osteotomy site

A passing glance? Differences in eye tracking and gaze patterns between trainees and experts reading plain film bunion radiographs.

Eye tracking and gaze pattern studies have been used to evaluate human behavior for decades. This is because of its ability to reveal conscious and subconscious behaviors when subjects are tasked with observation, decision making, and surgical performance. Many have popularized the use of this technology for radiographic assessment while evaluating radiologist behaviors, but little has been described for surgeon behavior patterns when evaluating preoperative deformities by radiograph. Because the radiographic assessment strongly influences surgical selection, the present study was designed to

Can artificial intelligence provide reliable patient education in minimally invasive bunion surgery? A comparative study of ChatGPT 5.2 and Gemini.

BACKGROUND: Patients use artificial intelligence-based large language models (AI-LLMs) to research minimally invasive surgery (MIS) for hallux valgus; however, their reliability remains uninvestigated. PURPOSE: To compare the quality and readability of ChatGPT and Gemini responses regarding MIS bunion surgery. METHODS: Ten frequently asked questions reflecting diverse clinical and procedural inquiries were submitted to ChatGPT and Gemini. Quality was assessed via DISCERN and 5-point Likert scales. Readability and actionability were evaluated using the Patient Education Materials Assessment T

Association of Tarsometatarsal Joint Slippage With Distal Screw Removal After Third-Generation Minimally Invasive Bunion Surgery: A Retrospective Controlled Study.

BackgroundFollowing Minimally Invasive Bunionectomy (MISB), some patients undergo the removal of hardware (ROH) due to medial-sided pain near the distal metatarsal screw site. Medial pain following MISB has been attributed to multiple factors, including hardware prominence, residual medial bony prominence, and construct-specific considerations. One potential radiographic finding observed postoperatively is medial translation of the first metatarsal base at the tarsometatarsal (TMT) joint, referred to here as "TMT slippage." This study investigated the association between postoperative TMT slip

Surgical correction of tailor's bunion with resection of fifth metatarsal head and silastic sphere implant: an 8-year follow-up study.

This paper introduces a fifth metatarsal head surgery with silicone sphere implant to correct tailor's bunion deformities and prevent digital retraction that may occur after fifth metatarsal head resection. The authors present an 8-year follow-up of 35 patients who underwent 50 procedures. The authors had an 84% success rate, but they submit that this procedure is not indicated in every case of tailor's bunion.

Time to Callus Formation With Minimally Invasive Bunion Surgery: Association of Intramedullary Plate Fixation With Earlier Consolidation Compared to Screw Fixation Techniques.

BACKGROUND: Hallux valgus (HV) remains a common forefoot deformity with a global prevalence of 19%. It is largely agreed that surgical correction of HV with minimally invasive techniques have good outcomes. Options include the percutaneous minimally invasive screw fixation (SF) and the intramedullary plate fixation (IPF) technique, with both employing different fixation techniques and hence stability of fixation. In this article, we aim to compare these 2 minimally invasive correction techniques and analyze the radiologic and clinical outcomes, including their time to consolidation. METHODS:

Medial and lateral exostoses of the distal phalanx of the hallux: A potentially painful bunion-like structure. Part 1: Incidence and clinical application.

BACKGROUND: Exostoses at the base of the distal phalanx of the great toe are usually asymptomatic. The literature has not generally considered them as the origin of a possible problem resulting from a pressure conflict between hallux and shoe (medial aspect) or second toe (lateral aspect) nor a potential complication of surgical correction of hallux valgus deformity. No studies, to our knowledge, have evaluated its possible correlation with other foot disorders. When one of these neglected exostoses became painful after surgical correction of hallux valgus, we decided to start a study to deter

Observed changes in radiographic measurements of the first ray after frontal and transverse plane rotation of the hallux: does the hallux drive the metatarsal in a bunion deformity?

It is well known that the pathologic positions of the hallux and the first metatarsal in a bunion deformity are multiplanar. It is not universally understood whether the pathologic changes in the hallux or first metatarsal drive the deformity. We have observed that frontal plane rotation of the hallux can result in concurrent positional changes proximally in the first metatarsal in hallux abducto valgus. In the present study, we observed the changes in common radiographic measurements used to evaluate a bunion deformity in 5 fresh frozen cadaveric limbs. We measured the tibial sesamoid positio

A 28-Year-Old Woman with Down Syndrome, Congenital Heart Disease, and a History of Knee Surgery and Plantar Fasciitis, with Hallux Abducto Valgus (Bunion) and Lapiplasty Three-Dimensional Correction Surgery.

BACKGROUND Tarsometatarsal joint (TMJ) arthrodesis is common method used for correcting hallux abductus valgus (HAV). Its popularity has grown due to studies revealing HAV's triplanar deformity with frontal plane rotation. This case report presents a 28-year-old woman with Down syndrome, congenital heart disease, and a history of knee surgery and plantar fasciitis, with severe HAV deformity and flexible valgus flatfoot associated with ligamentous hyperlaxity. CASE REPORT Examination revealed severe foot deformities, and radiographic studies confirmed the condition. A surgical intervention was

The influence of concomitant hammertoe correction on postoperative outcomes in patients undergoing hallux valgus correction.

BACKGROUND: Patients with hallux valgus often develop secondary hammertoe deformities of the lesser toes. Operative management of bunions with hammertoe can be more extensive; however, it is unclear whether this affects patient-reported outcomes. The aim of this study was to compare postoperative patient-reported outcome measures and radiographic outcomes between patients who underwent isolated bunion correction and patients who underwent simultaneous bunion and hammertoe correction. METHODS: Preoperative, postoperative, and change in Patient-Reported Outcomes Measurement Information System (

Minimally invasive bunionette correction.

OBJECTIVE: Bunionette or 'tailor's bunion' is a deformity of the fifth ray, which comes along with a metatarsus quintus valgus and a varus deformity of the fifth toe with subluxation of the metatarsophalangeal joint. A minimally invasive osteotomy of the fifth metatarsal without internal fixation using burrs is an increasingly used alternative for symptomatic tailor's bunion deformity. Similar to open surgery procedures the type of osteotomy complies with the underlying pathology. Minimally invasive surgical (MIS) procedures allow comprehensive treatment of various types of tailor's bunion def

Combination chevron plus Akin osteotomy for hallux valgus: should age be a limiting factor?

Current literature consistently precludes the chevron osteotomy for bunion correction in patients older than age 50 years. We retrospectively reviewed 47 patients (73 bunions) with an average age of 62 years (range, 55-81 years old) who had a chevron-Akin double osteotomy. The mean follow-up was 4 years, 6 months (range, 2 years, 3 months to 8 years, 2 months). The overall satisfaction rate was 95%. No significant pain or stiffness in the first metatarsophalangeal joint occurred in comparison with other bunion procedures. Radiographic results were better for patients with a preoperative interm

Exploring the Relationship Between Clinical Presentation in Hallux Valgus and Response to AbobotulinumtoxinA Treatment.

The relationship between pain/disability and angular deviation of the hallux valgus (HV), and the impact of orthotic use, laterality, and pain variability on treatment outcomes remain unclear. This was explored in post hoc analyses of a placebo-controlled trial of abobotulinumtoxinA (aboBoNT-A; Dysport®) for HV-associated pain (NCT03569098). The primary endpoint was not met in this study (change from baseline Numeric Pain Rating Scale [NPRS] score vs placebo at week 8); however, there was a greater reduction from baseline in mean NPRS score at week 12 with aboBoNT-A 500U versus placebo (p = .0

Operative treatment for hallux valgus in children with cerebral palsy.

All patients with cerebral palsy who had hallux valgus and bunion deformities surgically corrected between 1986 and 1995 were reviewed to determine whether techniques other than arthrodesis of the first metatarsophalangeal joint would adequately correct these deformities. Twenty-six feet of 17 patients were surgically corrected for their bunions and/or their hallux valgus deformity. The mean hallux valgus angle for all children was 30 degrees, preoperatively, with 70% correction achieved. Four techniques of surgical correction were used.
