Transdermal fentanyl for pain control in adults with chronic cancer pain.

The transdermal therapeutic system (TTS) for fentanyl is a drug-delivery system for use in patients with chronic pain who require an opioid analgesic. A multicentre, randomized, double-blind, placebo-controlled study was performed to evaluate the efficacy and safety of TTS-fentanyl as an analgesic for chronic cancer pain. One hundred and thirty-eight patients entered a 15-day dose-titration period, followed by a 9-day double-blind period (95 patients) with TTS-fentanyl or placebo. Fifty-five patients entered a follow-up period of indefinite duration. For the majority of patients, TTS-fentanyl

Intrathecal Infusion in Cancer Pain Surgery.

BACKGROUND: Cancer pain is a complex and multifactorial phenomenon that significantly impairs quality of life, particularly in advanced stages of malignancy. Although systemic pharmacologic therapies remain the cornerstone of management, a substantial proportion of patients experience refractory pain or intolerable side effects. Intrathecal drug delivery (ITDD) offers a targeted method of analgesia by administering medications directly into the cerebrospinal fluid, enabling effective pain control at lower doses while minimizing systemic toxicity. SUMMARY: This review explores the physiologica

Cancer pain: molecular mechanisms and management.

Cancer pain, a highly prevalent and distressing symptom among cancer patients, has a seriously harmful effect on their life and presents a complex challenge in clinical management. Despite extensive research efforts and the existence of clinical guidelines, significant controversies persist regarding the molecular mechanisms underpinning cancer pain as well as the most effective management strategies. This review systematically delves into the neurobiological underpinnings of cancer pain, centering on the interplay of peripheral and central sensitization, cellular stress and dysfunction, as we

Cancer Pain and Opioid Use Disorder.

Opioid use disorder (OUD) is increasingly recognized and co-present in patients with cancer. Unfortunately, OUD is not addressed or treated adequately in oncology settings. In addition, patients with cancer-related pain treated with narcotic pain medications are at risk for nonmedical opioid use (NMOU). More than two-thirds of patients with advanced cancer have pain. Both OUD and NMOU need to be concomitantly addressed alongside cancer-related pain management to avoid complications such as overdose. We review the approach to identifying and treating OUD and NMOU in patients with cancer and can

Methadone switching for refractory cancer pain.

BACKGROUND: Methadone is commonly considered an alternative opioid treatment for refractory cancer pain. This study aims to investigate the efficacy, safety, and cost of methadone in the treatment of refractory cancer pain. METHODS: A retrospective study was conducted in patients who used methadone for refractory cancer pain from April 2016 to December 2020 at a cancer specialized hospital. Pain control, evaluated via pain score and breakthrough pain frequency, and adverse events of methadone were compared with analgesic regimens prior to methadone administration. The factors potentially affe

Neurosurgical Management of Cancer Facial Pain.

Facial pain occurs in approximately 80% of patients with head and neck cancers. Pain in these settings may result directly from the tumor, or indirectly as a side effect of oncological treatment of the tumor. Optimizing treatment for cancer pain of the face, therefore, involves a variety of diagnostic and treatment considerations, with the development of a successful treatment algorithm dependent on accurate diagnosis of the anatomical location of the pain, its relationship to the facial pain pathway, the type of pain being treated and, finally, patient's prognosis and preference for treatment

Surgical/radiological interventions for cancer pain.

Pain is a major morbidity associated with cancer and up to 20% patients require invasive procedures for pain relief. Ablative techniques can be directed towards the spinal cord and brain to palliate pain or modify its perception. Anterolateral cordotomy, myelotomy, DREZotomy and cingulotomy are useful interventions for the management of refractory cancer pain. Advanced imaging modalities, including intraoperative computed tomography (CT) guidance, have increased safety and efficacy of these interventions. In this paper, authors review the recent literature regarding surgical interventions for

Acupuncture for cancer-induced bone pain?

Bone pain is the most common type of pain in cancer. Bony metastases are common in advanced cancers, particularly in multiple myeloma, breast, prostate or lung cancer. Current pain-relieving strategies include the use of opioid-based analgesia, bisphosphonates and radiotherapy. Although patients experience some pain relief, these interventions may produce unacceptable side-effects which inevitably affect the quality of life. Acupuncture may represent a potentially valuable adjunct to existing strategies for pain relief and it is known to be relatively free of harmful side-effects. Although acu

[Non-opioid analgesics in cancer pain].

Non-opioid analgesics (NSAIDs, acetaminophen) are the most basic drugs used from the first to third steps according to the WHO analgesic ladder. Even after the initiation of the use of opioids, improvement in pain is observed by the continuation of non-opioid analgesics. NSAIDs have anti-inflammatory effects and are particularly effective against somatic pain represented by that due to bone metastasis. On the other hand, NSAIDs should be prescribed with adequate attention being paid to side effects (gastrointestinal disorders and nephropathy) due to long-term administration. Acetaminophen with

Abdominal pain in neutropenic cancer patients.

A review of 58 patients with malignancies (age range, 14-73 years), who required surgical consultation for acute abdominal pain in the setting of neutropenia (granulocyte count less than 1000/mm3) after chemotherapy was conducted. Ninety percent had fevers greater than 37.8 degrees C, 30% had diarrhea or melena, and 25% had diminished bowel sounds. Five of the 29 patients (17%) with localized pain had surgical intervention; 3 of 29 patients (10%) with generalized pain underwent operations (2 for x-ray findings). All eight of these surgically treated patients survived to leave the hospital. Eig

Myofascial pain associated with oropharyngeal cancer.

Since the early detection of neoplastic disease may have a significant impact on the morbidity and mortality of the disease, all health professionals are urged to consider this in their differential diagnosis. The cases presented point up the difficulty of doing this because of the complex nature of the neural patterns that innervate the head and neck area. This becomes even more disturbing when neoplasia has been ruled out and the findings point to a myofascial basis for the complaint. It was only after the failure of treatment and the emergence of more sinister symptoms that a second evaluat

Pain and Fear of Cancer Recurrence in Survivors of Childhood Cancer.

OBJECTIVES: Theoretical models suggest that anxiety, pain intensity, and pain catastrophizing are implicated in a cycle that leads to heightened fear of cancer recurrence (FCR). However, these relationships have not been empirically examined. The objective of this study was to examine the relationships between anxiety symptoms, pain intensity, pain catastrophizing, and FCR in childhood cancer survivors and their parents and to examine whether pain catastrophizing predicts increased FCR beyond anxiety symptoms and pain intensity. METHODS: The participants were 54 survivors of various childhood

Comparison of parenteral fentanyl and morphine infusion in chronic cancer pain management: Parenteral opioid treatment for cancer pain.

BACKGROUND: The World Health Organization pain ladder treatment is recognized as the gold standard for moderate and severe chronic cancer pain palliation in hospitalized patients. The third step of this treatment includes powerful opioid agents, which in our case are morphine and fentanyl. METHODS: The primary purpose of our study is to measure pain symptom relief by evaluating the pain scores after intravenous continuous infusion of these opioids in palliative care unit to ensure pain control of the patients. Our second main goal is to compare the morphine equivalent dose (MED) administered

Pain in cancer. An outcome research project to evaluate the epidemiology, the quality and the effects of pain treatment in cancer patients.

BACKGROUND: Management of pain related to advanced or metastatic cancer, although the availability of several pharmacological and non-pharmacological interventions and the existence of well-known guidelines and protocols, is often difficult and inadequate. Evidence of the relative effectiveness of current options for treating cancer pain from comparative randomized studies is scanty. METHODS: In the context of a wider project, a multicenter, open label, prospective Outcome Research study will be launched in Italy in 2006 to investigate the epidemiology of cancer pain and of its treatments, th

[Patients with cancer-related pain and other chronic pain. Priorities and assessment].

In a survey completed at our hospital, 519 doctors and nurses were asked how pain treatment was estimated whether it received priority, and to what degree patients' pain syndromes were assessed. A total of 473 responded to the questionnaire. In the study cancer-related pain and pain from causes other than cancer were assessed in separate population groups. The responders considered that the staff gave higher priority to patients with cancer-related pain, than to patients with other pain. There was a discrepancy between the physicians' and the nurses' answers to the question whether optimal pai
