An RCT to compare high flow nasal cannula with standard nasal cannula to prevent desaturation in subjects undergoing EBUS-TBNA: HIFLO-EBUS trial.

BACKGROUND AND OBJECTIVE: Oxygen desaturation events occur commonly during Endobronchial endobronchial ultrasound (EBUS) guided-transbronchial needle aspiration (TBNA) performed under conscious sedation. We hypothesized that high-flow nasal cannula (HFNC) would be superior to conventional nasal cannula (NC) in preventing these hypoxemic episodes. METHODS: We randomized consecutive subjects ≥18 years undergoing EBUS-TBNA to receive oxygen with HFNC or NC. The primary objective was to compare the proportion of subjects experiencing oxygen desaturation events (defined as SPO2 < 90% for at least

High-Flow Nasal Cannula Liberation: Who, When, and How?

High-flow nasal cannula (HFNC) is a first-line therapy for patients with acute respiratory failure. Despite increased HFNC utilization over recent years-accelerated in part by the COVID-19 pandemic-high-quality evidence to guide HFNC discontinuation is lacking. Decisions about when and how quickly to reduce flow rate, Fio2, or both simultaneously are frequently left to clinicians' discretion without clear guidance on an optimal approach. Failure to de-escalate HFNC support when clinically appropriate has many potential consequences, such as prolongation of ICU/hospital length of stay, increase

Enhanced Aerosol Delivery During High-Flow Nasal Cannula Therapy.

BACKGROUND: Aerosolized drug delivery via high-flow nasal cannula (HFNC) decreases as gas flow is increased. To improve aerosol delivery, breath-enhanced jet nebulizer may increase aerosol output. This study tested that hypothesis and compared breath-enhanced jet nebulizer to vibrating mesh nebulizer technology. METHODS: First, in an isolated circuit, breath-enhanced jet nebulizer and vibrating mesh nebulizer aerosol outputs were measured during simulated HFNC by using infused saline solution at rates of 5-60 mL/h. Limits were defined when nebulizer filling was detected. The devices were then

Modified Intravenous Cannula Ventilation Tubes for Otic Barotrauma Prevention.

Objectives: Middle ear barotrauma is a common complication of hyperbaric oxygen therapy (HBOT). Tympanostomy tube placement prevents barotrauma by facilitating middle ear equalization. We describe variations on techniques published by Mooney et al and Zhang et al for placing modified peripheral intravenous cannula (PIVC) ventilation tubes, which can be performed at the bedside under topical anesthesia. We aim to evaluate the safety and efficacy of this technique when performed to prevent otic barotrauma in patients undergoing HBOT. Methods: We performed an analytical observational study at a t

[The perioperative role of high-flow cannula oxygen (HFNO)].

High-flow nasal cannula oxygen (HFNO) is commonly used during the perioperative period. Its numerous physiological benefits, satisfactory tolerance and ease of use have led to its widespread application in intensive care and post-anesthesia care units. HFNO is also used in the operating theater in multiple indications: as oxygen supplementation (associated with pressurization) prior to orotracheal intubation; in digestive and bronchial endoscopies, especially in patients at risk of hypoxemia; and in intraoperative surgery requiring spontaneous ventilation (ENT, thoracic surgery…). During the p

High-flow nasal cannula oxygenation utilization in respiratory failure.

High flow nasal cannula (HFNC) represents a new oxygenation system to be used in the treatment of respiratory emergencies. During HFNC therapy, the active humidification and air heating system allow the patient to tolerate higher flows by favouring physiologic mucociliary clearance and improving fluidity of respiratory secretions. Following this, FiO2 values are more stable and reliable, by reducing losses and minimizing ambient air entrainment. Several clinical trials in acute respiratory failure patients have suggested lower rate of invasive mechanical ventilation, improved comfort and enhan

[High flow nasal cannula in patients after trachea extubation].

Acute respiratory failure (ARF) is one primary cause of intensive care unit (ICU) admittance, which is usually treated with intubation and mechanical ventilation in order to maintain a necessary ventilation and oxygenation function. After the remove of trachea tube, available oxygenation supports include nasal cannula, venturi mask, non-invasive ventilator and high flow nasal cannula (HFNC). In this article we reviewed the physiological mechanism related to HFNC therapy and trails about the application of HFNC in extubated patients. HFNC can provide well heated and humidified high flow gas wit

High-Flow Nasal Cannula Utilization in Pediatric Critical Care.

BACKGROUND: High-flow nasal cannula (HFNC) is increasingly utilized in pediatrics, delivering humidified air and oxygen for respiratory conditions causing hypoxia and distress. In the neonatal ICU, it has been associated with better tolerance, lower complications, and lower cost. Few data exist regarding indications for use and the epidemiology of disease/pathology that warrants HFNC in the pediatric ICU. METHODS: This study is a retrospective cohort study of patients admitted to a tertiary children's hospital pediatric ICU and placed on HFNC from October 1, 2011 to October 31, 2013. Descript

Temporomandibular joint arthroscopy technique using a single working cannula.

The traditional arthroscopy technique includes the creation of three ports in order to enable visualization, operation, and arthrocentesis. The aim of this study was to assess an advanced temporomandibular joint (TMJ) arthroscopy technique that requires only a single cannula, through which a one-piece instrument containing a visualization canal, irrigation canal, and a working canal is inserted, as an alternative to the traditional double-puncture technique. This retrospective study assessed eight patients (13 TMJs) with pain and/or limited range of movement that was refractory to conservative

Left atrial dissection related to retrograde cardioplegia cannula insertion.

Left atrial dissection (LAD) is a rare complication and is defined as a gap from the mitral or tricuspid annular area to the interatrial septum or left atrial wall. Because of its low incidence, this entity is not fully understood. LAD is related to mitral valve surgery as well as coronary artery disease, arrhythmia, trauma, and tumors, and occurs spontaneously. Transesophageal echocardiography is the most useful diagnostic modality for LAD, but multimodality investigation supports accurate diagnosis. We experienced a case of LAD related to retrograde cardioplegia cannula insertion which was t

Use of self-expanding venous cannula in tricuspid reoperation.

Significant tricuspid regurgitation requiring surgical correction is associated with poor survival in patients undergoing tricuspid valve reoperations. Right chamber dilatation increases the risk of injury during resternotomy. The novel technique of peripheral cannulation using an expandable venous cannula for cardiopulmonary bypass can help to reduce the risk of complications and associated morbidity, thus enhancing the short-term outcomes.

Heated humidified high-flow nasal cannula therapy in children.

Heated humidified high-flow nasal cannula therapy (HHHFNC) was originally described as a mode of respiratory support in premature neonates and is now increasingly used in the management of acute respiratory failure in older infants and children. Heating and humidification of gas mixtures allow comfortable delivery of flow rates that match or exceed the patient's inspiratory flow rate. Emerging evidence from observational studies suggests that the use of HHHFNC therapy may be associated with reduced work of breathing, improved ventilation efficiency and a decreased need for intubation in childr

Femoral cannulation with long arterial cannula in aortic dissection.

The optimal cannulation site in repair of DeBakey type I aortic dissection is controversial, and malperfusion during cardiopulmonary bypass is facilitated by retrograde flow. We propose the use of a long arterial cannula through the femoral artery to achieve a proximal antegrade perfusion. The tip of the cannula is placed in the true lumen of the distal aortic arch through the common femoral artery (Seldinger technique and transesophageal echography guidance). In 9 patients, there was one case of operative mortality (cardiac death), and no cases of perioperative stroke, bowel ischemia, severe

Improving peripheral IV cannula care: implementing high-impact interventions.

Infection prevention and control measures have been recognized as effective in minimizing the risk of infection from peripheral intravenous (IV) cannulas. However, this relies on health professionals' compliance with guidelines for the care of patients with IV catheters and at times it may be that practice is inconsistent with guidelines. This article discusses the care required for peripheral cannulas and shows how implementing the high-impact interventions can improve peripheral IV catheter care on insertion and its management afterwards.

Cannula-probing combined with nasal procedure for dacryocystitis neonatorum.

Between 1973-1976 a total of 2063 cases of dacryocystitis neonatorum have been treated by cannula-probing in our department. In 37 cases the cannula-probing had to be combined with a nasal procedure for opening the closed nasolacrimal membrane: a periosteal elevator was passed into the inferior meatus of the nasal cavity and its tip was rubbed against the tip of the cannula-probe through the imperforate mucosa of the ostium. The patency of the duct was tested by simultaneous syringing. In 34 cases the lacrimal pathway became permanently patient after one nasal procedure and in 2 cases after tw

Heated humidified high-flow nasal cannula versus low-flow nasal cannula as weaning mode from nasal CPAP in infants ≤28 weeks of gestation.

UNLABELLED: Despite the paucity of evidence, the practice of weaning nasal continuous positive airway pressure (NCPAP) is widespread. However, the most clinically effective non-invasive ventilatory support strategy remains to be determined. We compared the outcome of very premature infants with respiratory distress syndrome treated with a combination of NCPAP and heated humidified high-flow nasal cannula (HHFNC) versus NCPAP and low-flow nasal cannula (LFNC). Between 2004 and 2008, patients ≤28 weeks of gestation and <1,250 g of birth weight were treated with NCPAP + HHFNC or NCPAP + LFNC. The
