["Bark-gras" syndrome? Case presentation on the differential diagnosis of delusional misidentifications].

In this case report, we present a young female patient exhibiting pathological misidentification symptoms, in which we employ differential diagnostic methods to distinguish between Capgras syndrome and Fregoli syndrome. The uniqueness of the case lies in the fact that the object of misidentification is not a human being but an animal - in this instance, a dog. Moreover, the underlying psychiatric disorder does not conform to the typical patterns commonly associated with such symptom formations. Following the presentation of findings from a comprehensive psychodiagnostic evaluation, we interpre

[Treatment of psychological and behavioral symptoms in dementia with Lewy bodies and Parkinson's disease dementia: Review of the literature and consensus using the Delphi method].

Dementia with Lewy bodies (DLB) and Parkinson's disease dementia (PDD) have frequent psychological and behavioral symptoms (PBS). Treatment of these PBS is variably validated We carried out a review of the treatment of PBS in DLB and PDD from 2009 to 2023. Where the literature allowed, we made treatment recommendations. When the literature was insufficient, we used the Delphi method. The following proposals were made: for pervasive and criticized hallucinations, donepezil or rivastigmine; for delusion, and Capgras syndrome (Delphi), low doses of clozapine; for depression, sertraline, venlafaxi

[Psychiatric manifestations of vitamin B12 deficiency: a case report].

Psychiatric manifestations are frequently associated with pernicious anemia including depression, mania, psychosis, dementia. We report a case of a patient with vitamin B12 deficiency, who has presented severe depression with delusion and Capgras' syndrome, delusion with lability of mood and hypomania successively, during a period of two Months. Case report - Mme V., a 64-Year-old woman, was admitted to the hospital because of confusion. She had no history of psychiatric problems. She had history of diabetes, hypertension and femoral prosthesis. The red blood count revealed a normocytosis with

Delusion of inanimate doubles: description of a case of focal retrograde amnesia.

This paper reports the case of a patient, M.P., who developed delusion of inanimate doubles, without Capgras syndrome, after traumatic brain injury. His delusional symptoms were studied longitudinally and the cognitive impairments associated with delusion were investigated. Data suggest that M.P. did 'perceive' the actual differences between doubles and originals rather than 'confabulate' them. The cognitive profile, characterized by retrograde episodic amnesia, but neither object processing impairment nor confabulations, supports this hypothesis. The study examines the nature of object miside

Measles, magic and misidentifications: a defence of the two-factor theory of delusions.

Corlett (Corlett, P. (this issue). Factor one, familiarity and frontal cortex: A challenge to the two-factor theory of delusions. Cognitive Neuropsychiatry) provides a robust critique of the two-factor theory of delusions. The heart of his critique is two challenges he derives from a paper by Tranel and Damasio (Tranel, D., & Damasio, H. (1994). Neuroanatomical correlates of electrodermal skin conductance responses. Psychophysiology, 31(5), 427-438), who illuminate the autonomic responses and brain damage of four patients often cited in support of the two-factor theory of Capgras delusion. I d

[Psychopathology of stroke].

The psychopathology of stroke encompasses several psychiatric and behavioral disorders that have high prevalence in the geriatric population, reduce the patient autonomy and increase the caregiver's burden. These disorders are usually associated with other cognitive and neurological deficits, and are labelled as neuropsychiatric when the whole clinical picture is consistent with the specific dysfunction of a neural system or brain region. Thus the neuropsychiatry of stroke comprises disorders of the perception/identification of the self and the environment (anosognosia of hemiplegia, misidenti

Delusional misidentifications and duplications: right brain lesions, left brain delusions.

When the delusional misidentification syndromes reduplicative paramnesia and Capgras syndromes result from neurologic disease, lesions are usually bifrontal and/or right hemispheric. The related disorders of confabulation and anosognosis share overlapping mechanisms and anatomic pathology. A dual mechanism is postulated for the delusional misidentification syndromes: negative effects from right hemisphere and frontal lobe dysfunction as well as positive effects from release (i.e., overactivity) of preserved left hemisphere areas. Negative effects of right hemisphere injury impair self-monitori

[A unique hallucination in the context of Parkinson's disease: reduplication or misidentification?].

We discuss a case involving a 75-year-old man with Parkinson’s disease (PD) who experienced a psychotic duplication phenomenon. This resembled Capgras syndrome, a misidentification delusion characterized by the false belief that a familiar person has been replaced by a double. However, our patient did not exhibit a delusion but rather a hallucination in which he simultaneously perceived his partner in two locations. He consistently engaged with the hallucinatory experience of his partner, whom he saw duplicated at the table, without awareness of the inaccuracy of this perception. Therefo

[Schroeder van der Kolk and Capgras's delusion].

BACKGROUND: About one hundred years prior to Capgras' description of the eponymous misidentification psychosis, this clinical picture had already been described by Schroeder van der Kolk. To this day, this publication has received unjustly little attention. AIM: To highlight Schroeder van der Kolk's case report and to bridge past and present, with emphasis on psychiatric care. METHOD: Analysis of the original case history alongside Capgras' three publications and presentation of a modernized adaptation. RESULTS: The pathogenetic factors emphasized by Capgras - alienation, heightened affecti

Delusional Misidentification Syndromes in Postpartum Psychosis: A Systematic Review.

INTRODUCTION: Delusional misidentification syndromes (DMS) are a group of psychopathological experiences occurring in psychosis, involving the misidentification of a person or place. DMS are often accompanied by hostility towards the object of delusional misidentification. This is of a particular concern in perinatal mental illness due to the potential disruption of the mother-infant bond, and risk of neglect, violence, or infanticide towards a misidentified child. This review aimed to collate all published cases of DMS in postpartum psychosis to further understand how these syndromes present

[Novel advances in neuropsychology--forward to the "deconstruction" of psychiatry].

Neuropsychology has recently become a science which deals not only with instrumental disorders (e.g., aphasia, apraxia, and agnosia), but also with impairments of interpersonal relationships (e.g., emotional cognitions, social decision making, and understanding others), and many important paradigms are already provided. We could enumerate several representative instances: (1)application of the "theory of mind" to autistic or related disorders (-->"reasoning of psychological state of others"), (2) neuropsychological studies on the "social brain" concerning emotional recognition or social recogn

Cognitive world: Neuropsychology of individual differences.

It is proposed that depending upon the specific pattern of cognitive abilities, each individual lives in an idiosyncratic "cognitive world." Brain pathology can be associated with some disturbed abilities, and frequently experiential changes (i.e., how the world is understood) are observed. Because these patients often are aware of their intellectual changes, they may represent excellent models to illustrate the diversity of cognitive interpretations an individual can have about the surrounding environmental conditions. Four neuropsychology cases are presented to illustrate this point: (a) pro

Distinguishing Reality: A Case of Delusional Misidentification Syndrome in a 39-Year-Old Male.

Capgras syndrome (CS) is a type of delusional misidentification syndrome where an individual is under the impression that a person they know has been switched with an identical imposter. One theory for the development of CS is a disturbance among the frontal, limbic, and temporal areas, which creates an alteration in an individual's ability to recognize a person's face and provoke a response emotionally. The primary risk factors for the development of CS include having a neurological disorder and a diagnosis of schizophrenia. We present a case of a 39-year-old male with a past medical history

Finding the imposter: brain connectivity of lesions causing delusional misidentifications.

SEE MCKAY AND FURL DOI101093/AWW323 FOR A SCIENTIFIC COMMENTARY ON THIS ARTICLE: Focal brain injury can sometimes lead to bizarre symptoms, such as the delusion that a family member has been replaced by an imposter (Capgras syndrome). How a single brain lesion could cause such a complex disorder is unclear, leading many to speculate that concurrent delirium, psychiatric disease, dementia, or a second lesion is required. Here we instead propose that Capgras and other delusional misidentification syndromes arise from single lesions at unique locations within the human brain connectome. This hypo

The burning of Bridget Cleary: psychiatric aspects of a tragic tale.

BACKGROUND: Bridget Cleary was brutally burned to death by her husband in the presence of her father and several close relatives in rural Ireland in 1895. The story has attracted public attention for more than a century now, for numerous reasons. AIMS: The issue of psychiatric illness in this tragedy, and the role of fairy mythology in belief systems in 19th Century rural Ireland are reviewed, particularly in relation to providing explanations for physical and psychiatric illness, along with learning disability. METHODS: Reference was made to a wide range of sources featuring the burning of

Patricide and overkill: a review of the literature and case report of a murder with Capgras delusion.

Despite being an infrequent crime, parental homicide has been associated with schizophrenia spectrum disorders in adult perpetrators and a history of child abuse and family violence in adolescent perpetrators. Among severe psychiatric disorders there is initial evidence that delusional misidentification might also play a role in parricide. Parricides are often committed with undue violence and may result in overkill. The authors present the case of an adult male affected by schizoaffective disorder and Capgras syndrome who committed patricide. Forensic pathologists classify such cases as overk

Neuropsychiatric and neuropsychological correlates of delusions in Alzheimer's disease.

We examined the prevalence, phenomenology, and clinical correlates of delusions in a consecutive series of 103 patients with probable Alzheimer's disease (AD). Patients were examined with the Present State Exam and the Dementia-Psychosis Scale. Twenty-one patients (20%) met DSM-III-R criteria for a delusional disorder. The most frequent delusion type was paranoid (71%), followed by hypochondriacal (67%), the Capgras syndrome (29%), house misidentification (29%), and grandiose delusions (29%). Out of the 21 AD patients with delusions, 76% had three or more different types of delusions simultane

A 31-year-old man with ankylosing spondylitis, receiving treatment with infliximab, presented with a large progressive cutaneous ulcer at the right knee. Biopsies showed Leishmania amastigotes, and polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) analysis showed Leishmania infantum as the causative agent. After treatment with miltefosine, the ulcer resolved completely, and infliximab was reinstituted because of progression of spondylitis. After 1 year, there was a recurrent ulcer at the same site being positive for Leishmania DNA by PCR. Local treatment with sodium

Psychiatric, neurological and medical aspects of misidentification syndromes: a review of 260 cases.

Two hundred and sixty case reports of misidentification syndromes were evaluated. One hundred and seventy-four patients had a Capgras syndrome misidentifying other persons, 18 a Fregoli syndrome, 11 intermetamorphosis, 17 reduplicative paramnesia and the rest had other forms or combinations of mistaken identification. Schizophrenia (127 cases), mostly of paranoid type, affective disorder (29), and organic mental syndromes including dementia (46) were the most common diagnoses in patients who misidentified others or themselves. The patients with reduplicative paramnesia more frequently suffered
