Clinical test to confirm tracheal intubation: a new method to confirm endotracheal intubation in the absence of capnography.

BACKGROUND AND OBJECTIVE: Advancing an uncut endotracheal tube into the right main bronchus produces unilateral breath sounds. We wanted to test the validity of using this method to distinguish oesophageal from tracheal intubation. METHODS: Forty-two patients were randomized into two groups. The first group was randomized to receive an endotracheal tube that was advanced into the right main bronchus. The second group of patients had their tracheas intubated as normal and then a second endotracheal tube was placed in the oesophagus. Blinded observers were then asked to decide by auscultation i

Evaluation of the clinical usefulness of capnography curves during a hyperventilation provocation test in the diagnosis of hyperventilation syndrome.

We evaluated the diagnostic usefulness of capnography curves during and following a hyperventilation provocation test (HVPT) in the hyperventilation syndrome (HVS). The diagnosis of HVS was based on the Nijmegen questionnaire and on the reproduction of symptoms during HVPT. Capnography curves of 40 HVS patients, 40 non-HVS patients with psycho-somatic complaints and 26 healthy controls were analyzed. There was no difference in baseline end-tidal CO2-level (FETCO2) between the 3 groups. The spontaneous fall of FETCO2 during the adaptation phase was clearly different in HVS patients versus non-H

End-tidal carbon dioxide pressure in neonates and infants measured by aspiration and flow-through capnography.

In 25 anesthetized, intubated, artificially ventilated, and paralyzed healthy neonates and infants, end-tidal PCO2 (PETCO2) measured by remote multiplexed mass spectrometry was 1.86 +/- 1.58 mm Hg lower than arterial PCO2 (PaCO2). PETCO2 measured by a flow-through cuvette was 1.02 +/- 1.64 mm Hg lower than PaCO2. The difference between the two methods of capnography was not significant. Values for PETCO2 obtained by mass spectrometry changed -0.43 +/- 1.43 mm Hg from baseline after 15 minutes of aspiration at a sample flow rate of 240 ml/min. Values for PETCO2 obtained with flow-through capnog

Prehospital Normoventilation in Critically Ill Anaesthetised Patients Cannot Be Reliably Monitored by Capnography-A Retrospective Observational Study.

BACKGROUND: End-tidal carbon dioxide capnography (EtCO2) is widely used to guide intubated patients' ventilation. Current guidelines for prehospital advanced airway management assume a gradient of approximately 0.67 kPa between EtCO2 and arterial carbon dioxide partial pressure (PaCO2). However, concerns have been raised about the accuracy of capnography. We aimed to assess the agreement between EtCO2 and PaCO2 in different patient groups to see if EtCO2 could be accurate enough to guide ventilation during prehospital care. METHODS: We carried out a retrospective study of all anaesthetised an

Dynamic Assessment of Exercise Gas Exchange Efficiency by Breath-by-Breath Volumetric Capnography in Mild-Moderate COPD.

A sizable fraction of dyspneic patients with only mild to moderate COPD exhibit a heightened ventilatory response to exercise relative to metabolic demands, i.e. a high ventilation (V.E)/CO2 output (V.CO2). The lack of continuous assessment of gas exchange efficiency and estimates of arterial CO2 partial pressure has hindered our understanding of the physiological underpinnings of this dynamic phenomenon. We compared key indices of gas exchange efficiency relative to the intra-breath CO2 profile as a function of expired volume using breath-by-breath volumetric capnography in 30 patients (FEV1

Capnography sensor use is associated with reduction of adverse outcomes during gastrointestinal endoscopic procedures with sedation administration.

BACKGROUND: Evidence to date suggests that capnography monitoring during gastrointestinal endoscopic procedures (GEP) reduces the incidence of hypoxemia, but the association of capnography monitoring with the incidence of other adverse outcomes surrounding these procedures has not been well studied. Our aims were to estimate the incidence of pharmacological rescue events and death at discharge from an inpatient or outpatient hospitalization where GEP was performed with sedation, and to determine if capnography monitoring was associated with reduced incidence of these adverse outcomes. METHODS

Control of carbon dioxide levels during neuroanaesthesia: current practice and an appraisal of our reliance upon capnography.

With the widespread availability of capnography, many anaesthetists have swung away from formally verifying hypocapnia by intraoperative arterial blood gas analysis and, instead, have come to rely upon capnography as an acceptable and constant predictor of arterial CO2 tension (PaCO2) during neurosurgery. However, the nature of the arterial-end-tidal CO2 gradient is complex, and is frequently unexpectedly large, or even negative. The importance of close intraoperative CO2 control during neurosurgery--more specifically, routine hyperventilation, and our reliance upon capnography to guide intrao

Use of capnography in diagnosis of pulmonary embolism during acute respiratory failure of chronic obstructive pulmonary disease.

In chronic obstructive pulmonary disease (COPD) patients, there is a difference between PaCO2 and end-tidal partial pressure of CO2 (PetCO2). This gradient P(a-et)CO2 is due to ventilation/perfusion mismatching and deadspace, and is usually abolished by forced and prolonged expiration. We hypothesized that this gradient might not be canceled by forced expiration in the case of acute respiratory failure (ARF) related to pulmonary embolism (PE). Forty-four adult COPD patients were prospectively entered into this study; they were suspected of having ARF related to PE on the basis of clinical and

Capnography rapidly confirmed correct endotracheal tube placement during resuscitation of extremely low birthweight babies (< 1000 g).

During neonatal resuscitation, the routine use of capnography to verify correct placement of the endotracheal tube is not an established international practice. We present four cases that illustrate the successful use of immediate capnography to verify correct tracheal tube placement even in extremely low birthweight (ELBW) prematures (< 1000 g) during resuscitation. Based on this limited experience, we reached institutional consensus among paediatricians and anaesthesiologists that capnography should become standard monitoring during all endotracheal intubations in premature babies.

Increased volume of conducting airways in idiopathic pulmonary fibrosis is independent of disease severity: a volumetric capnography study.

Bronchiectasis, bronchiolectasis, and bronchiolisation of alveolar regions are salient features of idiopathic pulmonary fibrosis (IPF). We asked whether IPF was associated with physiological changes consistent with increases in the volume of conducting airways, and whether airway volume was related to the severity of lung fibrosis. Patients with IPF (N  =  57, vital capacity-VC: 73  ±  20%), patients with non-IPF interstitial lung disease (non-IPF ILD, N  =  24, VC  =  78  ±  18%) and controls without lung disease (N  =  51, VC  =  112  ±  21%) underwent volumetric capnography for the determin

Ventilation inhomogeneity assessed by nitrogen washout and ventilation-perfusion mismatch by capnography in stable and induced airway obstruction.

Few studies have been published on gas distribution in the lung during acute and stable airway obstruction in children. Multiple breath nitrogen (N(2)) washout is an established method for assessing ventilation inhomogeneity, while the tidal breathing capnogram may be used as an indicator of ventilation-perfusion (V(')(A)/Q) mismatch. We hypothesized that significant V(')(A)/Q mismatch is not seen in stable airway obstruction unless obstruction is severe, and that stable and induced airway obstruction of similar severity would result in different degrees of V(')(A)/Q mismatch. To test this hyp

Capnography With Integrated Pulmonary Index for Preventing Hypoxemia During Pediatric Urologic Surgery Under Sedation: A Randomized Controlled Trial.

BACKGROUND Procedural sedation and analgesia (PSA) can relieve pain and anxiety during invasive procedures. Carbon dioxide (CO₂) monitoring or integrated pulmonary index (IPI) can reduce respiratory depression in adults with PSA, but the effect of the combination of these 2 methods on intraoperative hypoxemia in pediatric urologic surgery is unclear. This study was conducted to determine whether capnography combined with IPI could lessen intraoperative hypoxemia during pediatric urinary procedures with PSA. MATERIAL AND METHODS We conducted a randomized controlled trial (ChiCTR 2300073943). Ch

Misleading Capnography in Acute Pulmonary Embolism: Case Series of Masked End-Tidal CO2 Response During Airway Management.

End-tidal carbon dioxide (ETCO2) monitoring is the standard method for confirming endotracheal tube placement during emergency airway management. However, massive pulmonary embolism can create paradoxical capnography findings that complicate airway confirmation and resuscitation efforts. We present two cases of cardiac arrest where massive pulmonary embolism masked expected ETCO2 changes despite confirmed endotracheal tube placement. In the first case, a 33-year-old male with diabetic ketoacidosis developed cardiac arrest with absent colorimetric ETCO2 change despite direct visualization of co

Utility of monitoring capnography, pulse oximetry, and vital signs in the detection of airway mishaps: a hyperoxemic animal model.

This study was undertaken to determine the time interval for changes in end-tidal CO2, oxygen saturation (SaO2), heart rate (HR), and blood pressure (BP) in response to an acute airway obstruction or hypopharyngeal extubation in a hyperoxemic model. Complete and partial airway obstructions were simulated with complete and partial cross-clamping of an endotracheal (ET) tube in five anesthetized, nonparalyzed, mechanically ventilated Yorkshire minipigs with initial PAo2 of > 400 mm Hg. Placement of the ET tube into the hypopharynx was performed to simulate accidental extubation. Both sidestream

"Decompression of tension pneumothorax in a trauma patient -first use of a novel decompression colorimetric capnography device in human patient".

Tension pneumothorax is a common cause of mortality in trauma. Tension pneumothorax is the confinement of respired gases within the pleural cavity at increasing pressure resulting in hemodynamic collapse. Decompression is crucial in management. Emergency needle thoracostomy is a life-saving maneuver that allows atmospheric pressure equilibration and partial restoration of cardiac filling. Needle decompressions are usually performed under noisy, tense, and stressful circumstances, and objective assessment of success is difficult in the field. A device which is simple that objectively informs op
