Routine Versus Selective Temporary Epicardial Pacing Wire Placement: A Scoping Review.

Temporary epicardial pacing wires (TEPWs) are used widely for rhythm management in cardiac surgery. However, their routine placement is being debated increasingly because of low utilization and potential procedural risks. We compared routine versus selective TEPW placement practices and assessed the utilization, complications, and predictors of pacing need. A scoping review was conducted in accordance with the PRISMA-ScR guidelines. We searched PubMed, Embase, Scopus, and Cochrane Library until June 2025. Studies were included if they examined TEPW strategies in adult or pediatric cardiac surg

[Cardiac stimulations for the treatment of supraventricular arrhythmias].

UNLABELLED: Permanent cardiac stimulation in patients with supraventricular tachycardia is used primary for treatment and prevention of atrial fibrillation (AF). Different strategies of stimulation have been design for this purpose. Among the most important are: preferential standard atrial pacing, multisite atrial pacing or septal atrial pacing and pacing algorithms for prevention or treatment. Multicentric, controlled and randomized studies design to explore this therapies have disappointing and controversial results. Current therapy is focused in the implant of physiological pacemakers (AAI

[Sleep apnea and cardiac pacing: mechanisms of action and perspectives].

In an adult population, the prevalence of sleep apnea is 4% for men and 2% for women. Generally, nasal positive pressure ventilation is the best therapeutic option. To date, and in spite of the possible presence of marked brady-arrhythmias during sleep apnea, there is no recognised indication for Pacemaker implantation. However, recent data show the potential benefit of permanent cardiac stimulation in these patients. Increasing heart rate (using atrial pacing) improves cardiac output, and reduces pulmonary congestion and pulmonary vagal afferent nerves are no longer stimulated. The incidence

Developments in transcutaneous and transthoracic pacing during bradyasystolic arrest.

Transthoracic cardiac pacing historically has been relegated to the role of the technique of last resort in treating cardiac arrest. Recent studies have shown that this technique has a high rate of successful electrical capture, but often without mechanical activity. Survival rates have been shown to be dismal when the technique is used late in cardiac arrest. Results of several recent studies of patients paced by the transcutaneous technique have suggested that electrical capture can often be rapidly obtained in asystolic or pulseless bradycardic patients. Even though electrical capture can o

[New method of percutaneous implantation of a definitive pacemaker].

The Authors present a new method for the installation of a definitive pacemaker with a percutaneous puncture in the sub-clavian vein. According to the Authors, this method is reserved, in alternative to the other stimulations, to the subjects with the venous anomalies or with difficulties of the electrocatheter introduction through the usual ways or for defect of disponibility of the venous vessels already sacrificed in previous operations. This allows to reduce the number of patients to send to the cardiosurgeon for a myoepicardic stimulation.

Randomized Pragmatic Trial of Pacemaker Versus Implantable Cardiac Monitor in Syncope and Bifascicular Block.

OBJECTIVES: In this study, the authors tested whether a strategy of empiric permanent pacing reduces major composite events more effectively than acting on the results of an implantable cardiac monitor (ICM). BACKGROUND: Syncope may be caused by intermittent complete heart block in patients with bifascicular heart block, but competing diagnoses coexist. Whether empiric permanent pacing or acting on investigative results provides best care is unknown. METHODS: This was a multinational, randomized, pragmatic clinical trial of patients ≥50 years of age with bifascicular block, preserved left ve

Pacing techniques in the treatment of tachycardias.

Pacemakers are playing an increasing role in the management of patients with tachycardia. Pacing to prevent tachycardia must be differentiated from pacing to terminate tachycardia. Pacing is of proven value in preventing ventricular tachycardia that occurs in the setting of complete heart block or the setting of drug-induced QT interval prolongation and may be valuable in some other patients with nonsustained ventricular tachycardia. Pacing termination has found its greatest use as an alternative to cardioversion in patients with atrial flutter, especially those with suspected digitalis toxici

[Cardiac remodelling and function after cardiovascular implantable electronic devices implantation].

Cardiovascular implantable electronic devices (CIED) encompass permanent cardiac pacemakers (PM) and implantable cardioverterdefibrillators (ICD). CIED play an important role in treatment of cardiac arrhythmias, including significant bradyarrhythmias and tachyarrhythmias. The conventional right ventricular endocardial leads as well as right ventricular pacing may increase valvular defects (especially dysfunction of tricuspid and mitral valve), cause adverse cardiac remodelling and lead to a decrease in left ventricular ejection fraction. These changes may be due to mechanical damage to the lea

Complications of Temporary Transvenous Cardiac Pacing by Access Site: A Systematic Review and Meta-Analysis.

INTRODUCTION: The femoral and internal jugular veins are the most commonly used access routes for TTVPM placement. Femoral access is associated with higher rates of infection and bleeding due to groin proximity, whereas jugular access may increase the risk of vascular injury or pneumothorax when performed without imaging guidance. Despite numerous observational studies comparing these access sites, the evidence remains inconclusive due to heterogeneous methodologies, small sample sizes, and varying definitions of complications. To address these limitations, we conducted a meta-analysis to comp

Risk of VF Undersensing in Extra-Vascular ICD Due to Concomitant Leadless Pacing.

Extravascular implantable cardioverter-defibrillators (EV-ICD) offer defibrillation, anti-tachycardia pacing (ATP), and backup pacing, packaged in a unit comparable to traditional transvenous ICDs in size, shape, and longevity. However, the pacing capabilities of EV-ICD are restricted to ATP, pause prevention and post-shock pacing. For EV-ICD patients who develop the need for pacing, having a concomitant leadless pacemaker such as Micra could provide the needed pacing support. This research examines the potential interference between Micra's pacing pulses and EV-ICD's arrhythmia detection func

T-wave oversensing after implantation of pacemakers with automatic sensitivity adjustment in patients with atrioventricular block.

T-wave oversensing in pacemakers is rare because the set sensitivity is generally fixed. However, several models of pacemaker employ automatic sensitivity adjustment. Here, we present two cases of atrioventricular block treated by implantation of the pacemaker with automatic sensitivity adjustment. After implanting the pacemaker with automatic sensitivity adjustment, ventricular pacing suppression due to T-wave oversensing occurred. In both cases, T-wave oversensing disappeared after adjusting the setting sensitivity from 0.9 to 2.0 mV.

Atrioventricular synchronous leadless pacing: Micra AV.

Since the arrival of leadless pacemakers (LPs), they have become a cornerstone in remedial treatment of bradycardia and atrioventricular (AV) conduction disorders, as an alternative to transvenous pacemakers. Even though clinical trials and case reports show indisputable benefits of LP therapy, they also bring some doubts. Together with the positive results of the MARVEL trials, AV synchronization has become widely available in LPs, presenting a significant development in leadless technology. This review presents the Micra AV (MAV), describes major clinical trials, and introduces the basics of

[Indications for leadless VVI and VDD pacing in clinical practice].

Leadless, intracardiac pacemakers are a major innovation in the management of bradyarrhythmia. Both safety and feasibility of leadless pacing have been assessed in acute settings and mid-term follow-up showing a significant reduction of pacing-related complications as compared with standard transvenous pacemakers. There are many advantages of the leadless approach including a reduction of infection risk and malfunction due to the absence of transvenous leads and the pacemaker pocket. Patients at high risk of endocarditis or pocket failure, with compromised venous access, can benefit from the l

A rare case of pacemaker mediated tachycardia caused by intrinsic conduction search function of a dual chamber pacemaker.

Pacemaker-mediated tachycardia (PMT) is a well-described cause of tachyarrhythmia in patients with dual chamber pacemakers (Barold, 1991 [1]). PMT is usually caused by common triggers including premature ventricular or atrial complexes (Alasti et al., 2018 [2]). However, PMT can also be triggered by other uncommon mechanisms. Intrinsic conduction search function is a feature available in some dual chamber permanent pacemakers that periodically looks for intrinsic conduction by gradually prolonging the preprogrammed AV delay in an attempt to minimize ventricular pacing (Calvi et al., 2017; Paka

[Cardiac resynchronization therapy].

Fifteen years after the birth of resynchronization therapy, this technique has become one of the recommended treatments in severe heart failure patients, with low ejection fraction (4 35%), and conduction disorders (ORS > or = 120 ms). Based on large randomized clinical trials, the European recommendation is class I type (level of evidence A), to improve symptoms and survival of this patient population. Recommendations have been extended to patients already implanted with a conventional pacemaker, and to those with chronic atrial fibrillation. Recent results

[Biosensors and rate-responsive cardiac pacing].

Patients with atrioventricular block and ventricular pacemakers at a fixed rate have their exercise ability limited. This situation has been solved through dual chamber pacing in those patients keeping an adequate sinus node function. For the remaining, showing chronotropic failure or atrial fibrillation, the ability to increase their pacing rate is only possible when signals other than atrial activity, and reflecting metabolic needs, are used as a guide. These signals detected by a sensor, act through an algorithm modifying the pacing rate and are the cornerstone of rate-responsive pacing. In
