Maintaining quality out-of-hours in an acute cardiology service in a UK primary PCI centre.

BACKGROUND: Many district general hospitals (DGHs) in the UK do not have resident on-call cardiology registrars overnight; in these situations, it is becoming more common for cardiac outreach practitioners (COPs) to provide specialist input for patients presenting with suspected cardiac events. The aim of this study is to evaluate the exact diagnostic concordance between the initial working diagnoses made independently by COPs and subsequent final discharge diagnoses in patients assessed out of hours and to assess the accuracy of identifying acute coronary syndrome (ACS). METHODS: A retrospec

Diabetic muscle infarction in a patient admitted to the cardiac ward.

Diabetic muscle infarction (DMI) is a rare complication of long-standing diabetes mellitus. This is the first case of DMI reported by cardiologists. A 49-year-old patient with a history of diabetes and hypertension for only two years was admitted to the cardiac ward due to pain in the left thigh with pitting edema in both lower extremities. Magnetic resonance imaging finally confirmed the presence of DMI in the left thigh, which was improved by treatment with anticoagulants, analgesics and rest. However, the typical clinical symptoms of DMI were unrecognizable at the start of treatment, which

Echocardiographic diagnosis of heart disease in Uganda.

Five hundred consecutive cardiological patients were reviewed and evaluated by echocardiography in Mulago Hospital as a referral service. All diagnostic problems in cardiology in the hospital and to a certain degree from other institutions were reviewed. The commonest diagnosis was endomyocardial fibrosis (EMF) 19.8%, followed by congenital heart disease (CHD) 15% and rheumatic heart disease (RHD) 11%. The anatomical distribution of fibrotic lesions in EMF (left, right or biventricular) correlated with that found in previous autopsy series from Mulago Hospital thus demonstrating the accuracy o

Aneurysmal 'pepper-pot' atrial septal defect in an older gentleman with multiple cerebrovascular attacks.

Our patient presented to a large university teaching hospital with a history of light-headedness, falls and multiple cerebrovascular ischaemic events. This caused a right sided hemiplegia and the patient experienced significant functional limitation. Extensive investigations were carried out to exclude any causative factors such as carotid artery disease and the patient had all identifiable cardiovascular risk factors identified and modified. No significant pathology was found and a referral was made to the cardiology service. Transthoracic echocardiography revealed a complex type 3 perforate

Size matters: a critical appraisal of current acute cardiac care unit sizing in a real-world Italian scenario.

BACKGROUND: Current sizing in terms of beds and the territorial distribution of acute cardiac care units (ACCUs) is presently defined in Italy by Ministerial Decree 70/2015. The performance of this model of care based on one ACCU with coronary-angioplasty facilities for every 150 000-300 000 inhabitants has been poorly evaluated in the real world so far. METHODS: This study is based on a real-world, single-center, cohort of patients urgently admitted to the ACCU (and annexed cardiology ward) for acute cardiovascular disease: admission diagnosis, hospital stay, and outcome were recorded and co

[Comparison of hospital prevalence of rheumatic heart diseases and acute rheumatic arthritis in France and Africa].

This study reports the results of a multicentre enquiry performed in France in 1990 which included 41,242 adults hospitalised in Cardiology Units, 33,907 children hospitalised in Pediatric departments and 8,868 soldiers. A comparative enquiry was also carried out in North Africa (Tunis) and West Africa (Abidjan, Ouagadougou). The results of the French arm of the enquiry showed that rheumatic heart disease (RHD) has become very rare in adults (3.1% of all cardiac disease) and that it tends to occur in older subjects (average 54.4 years of age). There is practically no RHD in young adults. This
