Journal of endovascular therapy : an official journal of the International Society of Endovascular Specialists2010eng
PURPOSE: To describe the endovascular treatment of an iatrogenic common carotid artery (CCA) dissection using the superficial temporal artery (STA) as the principal access.
TECHNIQUE: An 83-year-old man with a post-carotid endarterectomy left CCA dissection was admitted for a transient ischemic attack involving the left cerebral hemisphere that occurred despite proper anticoagulation. After excluding redo surgery and transfemoral stenting (owing to a partially compromised aortobifemoral bypass graft), stenting of the left CCA dissection was planned using a right transbrachial approach, with t

Porcelain aorta is associated to significantly increased risk of atheromatous embolization in patients who undergo cardiac surgery. We described three cases in which coronary artery bypass grafting was performed off-pump and the saphenous vein graft was anastomosed proximally to the left common carotid artery. This technique permitted avoidance of ascending aortic manipulation and reduce the risk of atheromatous embolization.

OBJECTIVE: Common carotid artery occlusion (CCAO) is a rare cause of cerebrovascular events. Symptomatic lesions are resistant to medical treatment and revascularization is often required, but there is no consensus on the treatment of CCAO at present. Riles type 1A CCAO is most likely to benefit from revascularization because it has patent outflow tract (internal carotid artery) which was supplied by patent external carotid artery (ECA) from collateral circulation. We described a novel surgical technique improved on the basis of the carotid endarterectomy (CEA) for treatment of Riles type 1A C

We evaluated duplex ultrasonographic velocity patterns from the common carotid artery in 156 patients with arteriographically verified internal carotid artery disease. Flow to zero during diastole in the common carotid was found in three groups of patients: (1) those with flow to zero in both common carotids not related to disease of the carotid bulb; (2) those with a tight (90% to 99%) stenosis in the internal carotid; and (3) those with total occlusion of the internal carotid. Although such flow to zero is commonly observed with total occlusion of the internal carotid artery, it may also be

OBJECTIVES: Acute type A aortic dissection complicated with brain ischemia is associated with significantly higher mortality risks. Even if rescued with central aortic repair, some patients develop permanent postoperative neurological deficiency postoperatively. We recently introduced direct common carotid artery perfusion for acute type A aortic dissection involving the common carotid artery. This study introduced this technique to prevent postoperative neurological deficiency by comparing brain protection strategies.
METHODS: Among 91 acute type A aortic dissection patients treated at our h

OBJECTIVES: To determine the incidence of antegrade internal carotid artery collateral flow in patients with common carotid artery occlusion, which artery supplies blood to the internal carotid artery, and whether the flow affects regional cerebral blood flow in the middle cerebral artery territory.
METHODS: We determined the incidence of antegrade internal carotid artery collateral flow and identified its arterial origins using carotid sonography in 10 patients with common carotid artery occlusion and evaluated middle cerebral artery territory regional cerebral blood flow by single-photon em

Duplex ultrasound is currently used both for screening and for preoperative assessment of the carotid arteries and has completely replaced carotid angiography for the latter purpose in many institutions. The study aimed to identify resistivity index abnormalities in the common carotid arteries in patients with occluded or severely stenosed internal carotid arteries. Sixteen patients with severe internal carotid artery stenosis or occlusion over a twelve-month period were studied by duplex ultrasonography. The resistivity index in the common carotid artery was 0.85 +/- 0.03 on the stenotic side

PURPOSE: Occlusion of the common carotid artery (CCA) is generally associated with occlusion of the ipsilateral internal carotid artery (ICA) and external carotid artery (ECA). Occasionally, however, collateral circulation to the ECA may preserve patency of the ICA via retrograde perfusion through the bulb. These patients may suffer ongoing transient ischemic attacks and risk for stroke. Recognition of this pathologic variant may allow for effective surgical intervention.
METHODS: We have performed seven operations in six patients with occluded CCAs and patent ECA and ICAs. The occluded CCA w

A 67-year-old man was admitted for evaluation of left homonymous hemianopsia. Carotid ultrasonography showed that the right common carotid artery (CCA) was occluded up to just proximal to the carotid bifurcation, and the patent external carotid artery showed retrograde flow to the patent internal carotid artery via the carotid bifurcation. The Doppler waveform pattern of the external carotid artery showed high end-diastolic flow velocity and low pulsatility index. The diagnosis was Riles type 1A CCA occlusion. Digital subtraction angiography and iodine-123 N-isopropyl-p-iodoamphetamine single

OBJECTIVES: To evaluate the role of the end-diastolic velocity (EDV) in the common carotid artery (CCA) as a marker of internal carotid artery (ICA) occlusion.
DESIGN: Validation of retrospective data in a prospective clinical study.
METHODS: The EDV in 94 patients with total ICA occlusion and in 24 patients with high grade (95-99%) unilateral ICA stenosis identified on extracranial carotid colour-flow Duplex imaging (CFDI) and arteriography was reviewed, and was retrospectively compared to the EDV of 176 normal individuals. Identification of patients with ICA occlusion was most accurate (99

BACKGROUND: Carotid artery stenting (CAS) using the 7F Optimo balloon guide catheter (BGC) allows for smooth navigation and facilitates proximal flow control. However, this method may allow antegrade flow in the internal carotid artery (ICA). This study aims to identify predictors of antegrade flow during CAS with the common carotid artery (CCA) occlusion.
METHODS: We retrospectively analyzed 102 lesions treated with CAS using the 7F Optimo BGC and distal filter protection. The ICA flow pattern was assessed via contrast injection during CCA occlusion.
RESULTS: Antegrade flow in the ICA was o

A case of agenesis of the left internal carotid artery, common carotid artery and main trunk of external carotid artery with multiple cerebral aneurysms is presented. No similar case has been reported before. A 70-year-old man who had severe headache was admitted to the neurological department of Matsuyama Shimin Hospital. On admission he demonstrated nuchal rigidity and motor aphasia. CT scan revealed subarachnoid hemorrhage with intracerebral hematoma in the left temporal area. Right common carotid angiography, bilateral retrograde brachial angiography and aortography demonstrated an agenesi

BACKGROUND: Proximal thoracic endovascular aortic repair extension near the aortic arch may compromise supra-aortic branch perfusion.
CASE SUMMARY: A 41-year-old man with acute Stanford type B/DeBakey III aortic dissection had persistent chest pain despite anti-impulse therapy. A persistent type IA endoleak after the first thoracic stent graft required a second proximal extension. Completion angiography showed resolution of the endoleak but immediate loss of left common carotid artery opacification. Carotid duplex ultrasonography confirmed absent flow. Ultrasound-guided direct left common car

The knowledge of both normal and abnormal anatomy of the veins of the neck may be important for surgeons performing carotid endarterectomy (CEA), to avoid inadvertent injury to vascular structures. We present three cases of abnormal course of the internal jugular vein (IJV) which run anterior to the common carotid artery, named twisted IJV, that usually run posterolateral to the common carotid artery in patients undergoing CEA. These twisted IJV cases were detected by preoperative multidetector computed tomography angiography evaluation. During the CEA procedure, before identifying the common

BACKGROUND: Cerebral ischemia associated with chronic CCA occlusion is a rare condition and raises strategic dilemma when the revascularization is needed.
METHODS: Two patients with CCA occlusion presented with ischemic symptom associated with the affected side. Both patients underwent vascular reconstruction by direct carotid endarterectomy to achieve primary restoration of CCA to ICA flow.
RESULTS: Successful reopening of the vessels was obtained in both patients without the evidence of postsurgical ischemic event. Follow-up MRA was obtained at later than 6 months after surgery, which demo

A proximal occluded vertebral artery (VA) with reconstitution by muscular collateral vessels is a relatively common finding. However, due to inadequate intracranial anastomosis and hypoplasia or stenosis of the opposite VA, a number of patients develop symptoms of brain ischemia. In the current case, a 63-year-old man presented with repeat neurological symptoms such as dizziness, nausea, vomiting, dysarthria, left hemiparesis, and right hemianopsia. Magnetic resonance imaging revealed multiple posterior infarctions. Angiography revealed the VA to be occluded and reconstituted by collateral ves

This case report highlights an exceptionally rare congenital anomaly where the left common carotid artery (LCCA) originates from the main pulmonary artery (MPA), a condition documented in only 12 cases globally. Typically, the LCCA arises from the aortic arch; however, in this situation, it attaches to the MPA, creating complications to cerebral blood flow. A patient in their first decade, with a history of symptomatic patent ductus arteriosus that required surgical closure shortly after birth due to persistent tachycardia and respiratory symptoms, presented to our clinic with recurrent headac

The thyroid gland is mainly supplied by the superior and inferior thyroid arteries, with the latter being its principal arterial supply in adults. The inferior thyroid artery usually arises from the thyrocervical trunk, and less frequently from the subclavian artery. Rarely, it may originate from the vertebral artery or the common carotid artery. In the present report, we describe a unique case of a 56-year-old patient, undergoing total thyroidectomy and level VI lymph node dissection for papillary thyroid carcinoma, with aberrant origin of both inferior thyroid arteries from the common caroti

PURPOSE: Carotid endarterectomy is a standard treatment for symptomatic high-degree internal carotid artery stenosis. The aim of this article is to present possible intimal lesions after carotid endarterectomy. These lesions could be manifested as intimal flaps, intimal steps or dissections with or without occlusion or stenosis of the artery.
METHODS: The evaluation of the frequency and characteristics of the asymptomatic dissecting intimal lesions of the common carotid arteries was performed in a sample of 100 patients who underwent endarterectomy for symptomatic high-grade stenosis of the i

In the bird the carotid body is located between the distal (nodose) ganglion of the vagus nerve and the recurrent laryngeal nerve at the beginning of the common carotid artery, that is, the organ is located at the cervicothoracic border. The chicken carotid body receives numerous branches from the vagus and the recurrent laryngeal nerves. In addition, dense networks of the peptidergic nerve fibers immunoreactive for substance P, calcitonin gene-related peptide (CGRP), vasoactive intestinal peptide (VIP), galanin, and neuropeptide Y (NPY) are distributed in and around the carotid body parenchym
