Carotid web ultrasonography and its associations with ischemic stroke: a preliminary observational study.

BACKGROUND: This study aims to characterize the carotid web (CW) via Ultrasonography and explore its structural correlations with cerebral ischemic stroke (CIS). METHODS: We retrospectively reviewed 11,656 patients hospitalized in the neurology department of our center who underwent carotid ultrasonography at our center between January 2018 and December 2024. Patients were included if they had CTA within 2 weeks of ultrasound. We calculated CW incidence with 95% confidence intervals (CIs) and compared baseline characteristics, lipid profiles, and ultrasonic features (length, thickness, web ang

Short-term recurrence of stroke following misdiagnosis of carotid web masked by thrombus.

BACKGROUND: Carotid webs are a potential cause of occult cerebral infarction. Although they occur frequently, proper diagnosis and treatment are crucial; surgery has been shown to provide a good outcome. Although thrombus on the carotid web have been reported, digital subtraction angiography (DSA)-confirmed cases of pre-dispersed thrombus are rare. In this study, we report a case in which a thrombus on the carotid web concealed a shelf-like defect, complicating its diagnosis. CASE DESCRIPTION: A 47-year-old woman without stroke risk factors presented to our hospital with aphasia and right hem

Carotid artery stenting for a carotid web revealed by shape change after adherent thrombus resolution following conservative treatment.

BACKGROUND: The carotid web (CW) is a shelf-like defect located on the posterior wall at the origin of the internal carotid artery. Abnormal blood flow in the CW causes thrombus formation, making it difficult to diagnose. Some reports have indicated that CWs become detectable only after thrombus resolution. We report a case of a patient who underwent carotid artery stenting (CAS) for a previously undetected CW with the resolution of an adherent thrombus resulting in a favorable outcome. CASE DESCRIPTION: A 39-year-old male presented with the left hemiparesis rushed to our hospital. Magnetic r

Management Dilemma of an Asymptomatic Carotid Web in a High-Risk Patient: A Case Report.

Carotid webs, particularly in high-risk patients, can lead to ischemic cerebral events mainly through thrombus formation. While the management of symptomatic carotid webs is relatively well-established, the management of asymptomatic patients remains poorly represented in the vascular literature. We report the case of a 44-year-old male smoker with a sedentary lifestyle and a strong family history of ischemic stroke who was referred by his general practitioner for routine vascular evaluation. Doppler ultrasound (DUS) of the supra-aortic vessels identified a shelf-like fibrous structure on the

Histologic differences between in situ and embolized carotid web thrombi: a case report.

BACKGROUND: The significance of carotid webs (CaWs) in ischemic stroke is becoming acknowledged. Histological features of clot composition in situ and secondary cerebrovascular embolized thrombi caused by CaW have not been described concurrently. Understanding clots' histological composition is essential for understanding the pathophysiology of clot formation in CaW. CASE PRESENTATION: A 50-year-old male patient with acute ischemic stroke, which was believed to be caused by ipsilateral CaW, was admitted to the hospital. Mechanical thrombectomy was used to retrieve thromboemboli from the middl

Delay in Carotid Web Diagnosis Remains Common and Associated With an Increased Risk of Stroke Recurrence.

BACKGROUND: Carotid web is a rare and likely underrecognized cause of ischemic stroke, particularly in young patients. Given the high risk of recurrence, diagnostic delays may have serious consequences. This study aimed to assess the incidence and impact of delayed carotid web diagnosis after a first ischemic event. METHODS: We conducted a retrospective analysis using data from the French ongoing multicenter prospective CAROWEB (Carotid Web registry). We included patients with a first-ever ischemic stroke or transient ischemic attack in the anterior circulation, attributed to an ipsilateral c

The case of treatment for carotid web - Double-layer micromesh stent implantation in our hospital.

BACKGROUND: A carotid web is a shelf-like structure on the posterior wall of the origin of the internal carotid artery, and it is believed to cause cerebral infarction due to thrombus formed by turbulent flow with stagnation of blood flow. Recently, it has been suggested that recurrent cerebral infarction cannot be prevented in patients with a symptomatic carotid web by conventional medical management alone. However, there is still no consensus on the treatment of carotid webs. Carotid artery stenting (CAS) with the CASPER stent (Microvention, Terumo, Tustin, CA, USA) was performed in six cons

Recurrent stroke shortly after mechanical thrombectomy secondary to carotid web: A case report.

RATIONALE: Carotid web, a known source of thrombus for embolic stroke, presents a considerable risk of stroke recurrence. While case reports have demonstrated the safety and effectiveness of mechanical thrombectomy in treating carotid web-related stroke, the need for concurrent carotid artery stenting to prevent recurrent stroke immediately after thrombectomy remains unclear. This study aims to underscore the importance of immediate carotid artery stenting in preventing recurrent stroke following mechanical thrombectomy in patients with carotid web-related stroke. PATIENT CONCERNS: A 43-year-

Carotid Web Phenotype Is Uncommonly Associated With Classic Fibromuscular Dysplasia: A Retrospective Observational Study.

BACKGROUND AND PURPOSE: Carotid web (CaW) is an intimal form of fibromuscular dysplasia (FMD) involving the carotid bulb which has been increasingly recognized as a potential cause of recurrent ischemic strokes. It is overlooked as a separate entity and often dismissed if no coexistent signs of classic FMD changes are observed. We aim to evaluate the frequency of classic FMD in high-yield vascular territories in patients with symptomatic CaW. METHODS: This was a retrospective analysis of a symptomatic CaW database of 2 comprehensive stroke centers (spanning September 2014-October 2020). The d

Comparative analysis between 1-D, 2-D and 3-D carotid web quantification.

BACKGROUND: Carotid webs (CaW) are now recognized as a cause of ischemic stroke in young patients. The thromboembolic potential appears related to the CaW's morphology and consequent impact on local flow dynamics. We aim to evaluate the reliability of different measurement methods for the quantification of CaW and their relationship to symptomatic status, presence of large vessel occlusion stroke (LVOS), clot burden and final infarct volume. METHODS: This was a retrospective analysis of the local comprehensive stroke center CaW database (September 2014-July 2019). CT angiograms (CTAs) were re
