Cast-forming Actinomyces israelii canaliculitis.

BACKGROUND: Primary chronic canaliculitis is an uncommon disease usually caused by Actinomyces israelii (streptothrix). Actinomyces israelii is a cast-forming Gram-positive anaerobe that is difficult to isolate and identify. We present a case that demonstrates the typical clinicopathological presentation of this unusual condition and discuss management options. METHODS AND RESULTS: A 10-year-old girl presented with a 6 month history of intermittent 'conjunctivitis' and discharge from her 'pouted' left lower punctum. Microbiology confirmed probable A. israelii infection, but topical treatment

Cost-effectiveness of cast treatment vs. surgery in elderly patients with substantially displaced intra-articular distal radius fractures: A trial-based economic evaluation.

BACKGROUND: The number of surgical procedures for distal radius fractures in the elderly has increased, even though most studies show little or no benefit over cast treatment. While medical costs of surgical treatment are higher than those of cast treatment, surgery may enable faster recovery and help patients maintain independence, potentially reducing their use of other healthcare resources and informal care. METHODS: We evaluated whether cast treatment is cost-effective compared to surgery for patients aged 65 years or older with substantially displaced intra-articular distal radius fractu

Efficacy of Waterproof Cast Protectors and Their Ability to Keep Casts Dry.

PURPOSE: The purpose of the study was to compare the efficacy of 6 different commercially available waterproof cast protectors in their ability to maintain a dry environment and evaluate whether cast protectors perform better than a plastic bag secured with tape in keeping casts dry. METHODS: We enrolled 23 adult participants to test 6 different commercially available cast protectors and a plastic bag. Participants trialed all cast protectors twice, with and without motion, by fully submerging each cast protector in water with a paper towel held between their index and middle fingers. Moistur

[Congenital clubfoot].

Congenital clubfoot is one of the most common congenital skeletal deformities with an incidence of 1-2/1000 newborns. The deformity is characterized by pathological changes of joints, bones (especially the talus), muscles, tendons and soft tissues which result in subtalar malpositions known as talipes equinus, varus adductus and cavus. Secondary clubfoot is always part of an underlying systemic or neurologic disease and can occur at birth or develop over time. The treatment of clubfoot should start early after birth and is primarily conservative, involving manipulation and serial casting. Amon

Serial casting: a method for treating burn contractures.

The purpose of this study was to assess the effectiveness of serial casting of burn contractures that were resistant to traditional methods of treatment such as paraffin therapy, massage, exercise, and splinting. Serial casting was used to increase the range of motion in 35 joints in 15 patients with burns. A mean increase of 54% was achieved. Casting provided immediate results with minimal complications and was accepted well by patients. Casts were easy to apply and effective even with noncompliant patients. They also delayed or eliminated the need for surgical correction.

Cast Saw Burn Prevention: An Evidence-Based Review.

Cast saw burns are an avoidable complication of cast removal and cast splitting. These iatrogenic injuries often lead to unacceptable clinical sequalae with significant financial and legal consequences. Therefore, a considerable body of research has been directed toward cast saw burn prevention. This review of currently published data provides clinicians with a summary of the literature to guide practice based on the best available evidence, with the goal of preventing iatrogenic cast saw burns. The PubMed database was queried for articles published from 1980 until present with the following k

Age-Stratified Outcomes of Mehta Casting in Idiopathic Early-Onset Scoliosis: A Multicenter Review.

BACKGROUND: This study provides age-stratified outcomes of cast treatment for idiopathic early-onset scoliosis. METHODS: This is a multicenter, retrospective review of patients with idiopathic early-onset scoliosis treated with Mehta casting at 4 tertiary children's hospitals between 2001 and 2016. The inclusion criteria were idiopathic early-onset scoliosis and a minimum follow-up of 2 years after casting ended. The exclusion criteria were <2 casts, >3 casts at an outside facility, and a major curve of <20° pre-treatment. Subjects were grouped by age at first cast: <18 months, 18 to <24 mont

Tibial shaft fractures from midshaft to distal metaphyseal region. CONTRAINDICATIONS: Proximal and middle forearm fractures. Complete dislocation. Articular fractures. Very young children (compliance problems). Open fractures. SURGICAL TECHNIQUE: Immediate cast application for fracture treatment as

Comparison of Growing Rod Instrumentation Versus Serial Cast Treatment for Early-Onset Scoliosis.

STUDY DESIGN: A comparison of 2 methods of early-onset scoliosis treatment using radiographic measures and complication rates. OBJECTIVES: To determine whether a delaying tactic (serial casting) has comparable efficacy to a surgical method (insertion of growing rod instrumentation [GRI]) in the initial phase of early-onset deformity management. SUMMARY OF BACKGROUND DATA: Serial casts are used in experienced centers to delay operative management of curves of surgical magnitude (greater than 50°) in children up to age 6 years. METHODS: A total of 27 casted patients from 3 institutions were m

Lower Extremity Cast Application.

UNLABELLED: This chapter will focus on lower extremity casting. Casting is widely recognized as the best treatment for many pediatric lower extremity fractures, unlike those in adults. Due to children's remodeling potential, the displacement threshold for surgery is often higher than that for skeletally mature patients, and immobilization in a cast with proper molding may be the only treatment needed.Lower extremity casts can be used to maintain the alignment of nondisplaced fractures. They are also utilized after closed reduction of a fracture, with appropriate molding to prevent any further

Comparison of cast materials for the treatment of congenital idiopathic clubfoot using the Ponseti method: a prospective randomized controlled trial.

BACKGROUND: The Ponseti method of congenital idiopathic clubfoot correction has traditionally specified plaster of Paris (POP) as the cast material of choice; however, there are negative aspects to using POP. We sought to determine the influence of cast material (POP v. semirigid fibreglass [SRF]) on clubfoot correction using the Ponseti method. METHODS: Patients were randomized to POP or SRF before undergoing the Ponseti method. The primary outcome measure was the number of casts required for clubfoot correction. Secondary outcome measures included the number of casts by severity, ease of ca

Wound complications from idiopathic clubfoot surgery: a comparison of the modified Turco and the Cincinnati treatment methods.

PURPOSE: Treatment protocols using the Turco and the Cincinnati incisions are widely used for the surgical correction of clubfoot deformity. However, it is unclear which surgical approach leads to fewer wound problems. We therefore sought to determine which treatment method led to a lower incidence of wound complications. STUDY DESIGN: A retrospective chart review of 217 consecutive patients (308 feet) who underwent a primary posteromedial release for the treatment of idiopathic clubfoot under the age of 24 months via either the modified Turco or Cincinnati treatment methods was used to docum

Selective surgical containment for Legg-Perthes disease: recognition and management of complications.

Surgical containment is often selected for older children with "at risk" Legg-Perthes disease because healing time is long and abduction bracing is poorly tolerated. This review of 19 older children (mean age, 9.6 years) treated by surgical containment, selected from a total of 167 patients with Legg-Perthes disease, demonstrates the frequency of complications when only older children with severe involvement are surgically contained. The radiographic results (37% good, 26% fair, 37% poor) are mediocre when compared with reported series in which patients were not selected critically for "at ris
