Hemodynamic monitoring and echocardiographic evaluation in cardiogenic shock.

Cardiogenic shock (CS) is characterized by the presence of a state of tissue hypoperfusion secondary to ventricular dysfunction. Hemodynamic monitoring allows us to obtain information about cardiovascular pathophysiology that will help us make the diagnosis and guide therapy in CS situations. The most used monitoring system in CS is the pulmonary artery catheter since it provides key hemodynamic variables in CS, such as cardiac output, pulmonary artery pressure, and pulmonary artery occlusion pressure. On the other hand, echocardiography makes it possible to obtain, at the bedside, anatomical

Incidence of cardiac dysrhythmias in patients during pulmonary artery catheter removal after cardiac surgery.

OBJECTIVE: No published studies focus on determining the frequency of dysrhythmias during pulmonary artery catheter removal by use of a standard technique. The objective was to assess the incidence and hemodynamic effect of dysrhythmias in patients who had recently undergone cardiac surgery (within 24 hours of when dysrhythmia was noted). METHODS: The study used an electrocardiogram recording by continuous rhythm strip during removal of the pulmonary artery catheter. Pulmonary artery catheters were removed by use of a standard procedure. Data were collected with respect to patients' serum pot

Pulmonary angiography via an indwelling four-channel Swan-Ganz catheter.

An indwelling Swan-Ganz catheter was utilized for pulmonary angiography in an extremely ill patient without the necessity for a further invasive procedure. The procedure allowed prompt exclusion of pulmonary embolism as a cause of her deteriorating condition, and confirmed the diagnosis of adult respiratory distress syndrome (ARDS).

[Influence of catheter type and site on infection incidence. Prevention techniques].

Relative risk of catheter-related sepsis varies according to the site and the catheter type chosen. Subclavian vein route is usually safer than internal jugular vein route. However internal jugular catheter can be tunnelled on the thorax which may decrease infections risk. Infection rate is very low during arterial cannulation whatever the site used, including femoral artery. The use of Swan-Ganz catheter is accompanied by a very low infection rate, despite repeated interventions on the catheter line. Prevention of infection is a major goal during vascular catheterization. A strict aseptic pro

[A comparative study of the effectiveness of 5% human albumin and 10% hydroxyethyl starch (HAES-steril) for correcting hemodynamics and O2 transport in surgical interventions].

Forty patients subjected to cavitary operations were examined. A high risk of hemodynamic disorders necessitated invasive monitoring; with this aim in view a catheter was inserted for measuring arterial pressure and the Swan-Ganz catheter for measuring the pressure in the pulmonary artery, in which helped monitor the hemodynamics and oxygen transport in the course of hypervolemic hemodilution. Plasma substitutes (one of two) were selected at random. After catheterization of the left radial artery and insertion of the Swan-Ganz catheter in the pulmonary artery through the internal right jugular

Indwelling cardiac catheters. An autopsy study of associated endocardial lesions.

An autopsy study was carried out to assess the relationship between indwelling intracardiac catheters and hemorrhagic, thrombotic, and infective lesions of the right heart endocardium and valve. Intracardiac catheters cause such lesions frequently, with a spectrum from uncomplicated valvular hemorrhage through nonbacterial thrombotic endocarditis to infective endocarditis. Pulmonary emboli were associated with the thrombotic, infective lesions. Endocardial lesions are more commonly seen with Swan-Ganz (SG) catheters; with these, lesions are more common and severe, with longer periods of cathet

Catheter-induced pulmonary artery haemorrhage resulting from a pneumothorax.

We present a case of pulmonary artery perforation in a patient who developed a pneumothorax after cardiac surgery. In the process of inserting a chest tube the patient became tachypnoeic, and developed haemoptysis. The trachea was intubated, and right bronchial intubation was performed with persistent bleeding. The pulmonary artery catheter was gently withdrawn and the balloon inflated, with cessation of bleeding. The patient was taken to the operating room, a bronchial blocker was placed in the right lower lobe bronchi, and the pulmonary artery catheter was removed. The bronchial blocker was

Innovative Use of Pulmonary Artery (PA) Catheter for Lung Isolation in a Pediatric Patient: A Case Report.

Lung isolation is essential for optimal surgical exposure and prevents contamination of normal lung. It is challenging in pediatric patients due to the anatomical and physiological differences. The most common methods of achieving lung isolation include the use of double-lumen endotracheal tubes (DLTs) or bronchial blockers (BBs). However, in pediatric patients, the size of the airways often restricts the use of DLTs, and in some cases, specific BBs may not be available or feasible due to the anatomy. Here, we report a case of right lower lobectomy in a two-and-half-year-old child using a pulm

Cerebral Arterial and Venous Air Embolism Following Removal of Percutaneous Sheath Introducer.

Cerebral air embolism after removal of central venous catheter (CVC) is a rare complication but can lead to fatal outcomes. We report a rare case of both cerebral venous and arterial embolism occurring in a patient with underlying scleroderma-related interstitial lung disease (SSc-ILD) and pulmonary hypertension following removal of percutaneous introducer sheath for pulmonary artery catheterization. We discuss the mechanisms, pathophysiology, management and prevention of cerebral air embolism.

[The pulmonary artery catheter in the intensive cardiac care unit].

More than 50 years after its introduction in clinical practice, the increase in the intensity of care offered by the cardiac intensive care units, the shift in the population of patients treated and the wider availability of circulatory supports, still makes the pulmonary artery catheter (PAC) an essential tool for diagnosis, monitoring and prognosis in patients suffering from cardiogenic shock. In this review, we will discuss how to identify those patients who can benefit most from its use, the configuration and the correct insertion technique of a PAC. A pragmatic guide will also be provided

Early pulmonary artery catheterization is not associated with survival benefits in critically ill patients with cardiac disease: An analysis of the MIMIC-IV database.

BACKGROUND: Many studies demonstrated no improved survival in patients with pulmonary artery catheter placement. However, no consistent conclusions have been drawn regarding the impact of pulmonary artery catheter in critically ill patients with heart disease. This study aimed to investigate the association of early pulmonary artery catheter use with 28-day mortality in that population. METHODS: The Multiparameter Intelligent Monitoring in Intensive Care IV (MIMIC-IV) database, a single-center critical care database, was employed to investigate this issue. This study enrolled a total of 11,88

Cardiopulmonary monitoring of shock.

PURPOSE OF REVIEW: We will briefly review the classification of shock and the hallmark features of each subtype. Available modalities for monitoring shock patients will be discussed, along with evidence supporting the use, common pitfalls, and practical considerations of each method. RECENT FINDINGS: As older, invasive monitoring methods such as the pulmonary artery catheter have fallen out of favor, newer technologies for cardiac output estimation, echocardiography, and noninvasive tests such as passive leg raising have gained popularity. Newer forms of minimally invasive or noninvasive moni

Minimally invasive or noninvasive cardiac output measurement: an update.

Although cardiac output (CO) by pulmonary artery catheterization (PAC) has been an important guideline in clinical management for more than four decades, some studies have questioned the clinical efficacy of CO in certain patient populations. Further, the use of CO by PAC has been linked to numerous complications including dysrhythmia, infection, rupture of pulmonary artery, injury to adjacent arteries, embolization, pulmonary infarction, cardiac valvular damage, pericardial effusion, and intracardiac catheter knotting. The use of PAC has been steadily declining over the past two decades. Mini
