A Pilot Study of Autonomic Function Screening Tests for Differentiating Complex Regional Pain Syndrome Type II and Traumatic Neuropathic Pain.

Background and Objectives: One of the most challenging tasks in a clinical setting is to differentiate between complex regional pain syndrome (CRPS) type II and traumatic neuropathic pain (NeP). CRPS is characterized by several dysautonomic manifestations, such as edema, hyper/hypohidrosis, skin color change, and tachycardia. This study compared the outcomes of autonomic function screening tests in patients with CRPS type II and traumatic NeP for diagnostic differentiation. Materials and Methods: CRPS type II was diagnosed according to the Budapest research criteria, while NeP was diagnosed ac

Visually evoked pain and its extinction using virtual reality in a patient with complex regional pain syndrome type II.

In this case report, we used virtual reality (VR) to explore pain evoked by only the appearance of being touched (rather than actually being touched) in a person with complex regional pain syndrome type II. Furthermore, we explored the degree to which this visually evoked pain could be extinguished by applying exposure principles in VR. In stage 1, we identified 4 specific scenarios where pain was triggered by visually simulated touch (without physical stimulation) and used these scenarios to quantify baseline sensitivity to visuotactile stimulation. In stage 2, the patient undertook a 12-week

Complex regional pain syndrome type II associated with lumbosacral plexopathy: a case report.

INTRODUCTION: Complex regional pain syndrome (CRPS) is a well-known clinical entity, first described in the 1800s, consisting of pain, hyperalgesia, edema, and sudomotor changes either without (Type I) or with (Type II) a definable nerve injury. CRPS Type II is most commonly caused by high velocity missile injuries, mononeuropathies, and partial nerve transections. CASE REPORT: In this case, a 25-year-old soldier who sustained a blast injury causing multiple spinal compression fractures, extremity fractures, and pelvic and sacral fractures was transferred to a U.S. Army medical center for sur

[Appearance of angry backfiring C-nociceptor (ABC) syndrome-like pain disorder in cross-side of one-side spinal anesthesia].

ABC (angry backfiring C-nociceptor) syndrome-like pain disorder categorized in sympathetic independent pain appeared after amputation during spinal anesthesia. A 69-year-old female who had undergone amputation of the right leg below knee 5 months before, received amputation of the left leg below knee this time because of diabetic neuropathy and gangrene of her both legs. There were no complications in her right leg after the former amputation. Spinal anesthesia was maintained in the left one-side by hyperbaric local anesthetic. After changing the body position from left lateral to supine, the

[Pain after nerve injuries (author's transl)].

After cut injuries to nerves, about 70% of the persons injured are stricken with painful hypersensitivity, and at least 50% with non-causalgic pain. Clinical reports on peace-time injuries have had scarce regard to both forms. In about 80% of the injured, the painful hypersensitivity sets in within 6 months of the surgical reconstruction and ceases in half of the the patients up to the end of the second year; in the other half the disturbances seem to persist much longer. Non-causalgic pain commence earlier than the painful hypersensitivity, often already shortly after the trauma. Complete rem

Case study: Gluteal compartment syndrome as a cause of lumbosacral radiculoplexopathy and complex regional pain syndrome.

We present the case of a 24 yr old male who was diagnosed with gluteal compartment syndrome and was subsequently found to have developed lumbosacral radiculoplexopathy and complex regional pain syndrome. The patient's gluteal compartment syndrome was diagnosed within 24 h of presentation to the emergency room, and he underwent emergent compartment release. While recovering postoperatively, persistent weakness was noted in the right lower limb. Results of electrodiagnostic testing were consistent with a lumbosacral radiculoplexopathy. After admission to inpatient rehabilitation, the patient com

[Posttraumatic dystonia. Review and legal aspects].

More attention should be paid to dystonia as a consequence of trauma, particularly with regard to legal aspects. The underlying pathophysiological mechanisms of dystonia following central or peripheral trauma are largely unknown. Hemidystonia after severe head trauma is regarded to be due to contralateral basal ganglia lesions, particularly of the putamen. Focal and segmental dystonias follow various kinds of peripheral trauma. Central synaptic reorganisation due to altered peripheral input may play a role in its genesis. Clinically, post-traumatic dystonia differs from the idiopathic disease

[Chronic pain syndrome of the penis following cerebrovascular insult].

Complex regional pain syndromes (formerly sympathetically maintained pain syndromes or reflex sympathetic dystrophy) encompass symptoms of pain, dysfunction and sympathetic disorder. They occur spontaneously or after peripheral or internal lesions (e.g. stroke or myocardial infarction) and predominantly affect the limbs, rarely the face or the trunk. This case report describes a 64-year old man who after a stroke suffered from heavy burning pain in the penis and perineum, which did not ameliorate after established conservative therapy. Sympathetic blockade, however, provided pain relief. The d

Lumbar sympathectomy by electrocoagulation; its use in the management of certain vascular and visceral disorders.

Although normally the sympathetic nerves aid vascular dilatation during effort, in certain diseases of the vascular system they have a reverse effect. Abolition of sympathetic vasoconstrictive impulses by sympathectomy is the most effective treatment in some chronic peripheral vascular conditions. The authors have used electrocoagulation for a number of years and found it quick, effective and more likely to prevent regeneration of the affected nerves. Improvement was obtained by sympathectomy in arteriosclerotic vascular insufficiency, thromboangiitis obliterans, Raynaud's disease, reflex symp

Motor-Evoked Potential Analysis of Femoral Nerve Status During the Direct Anterior Approach for Total Hip Arthroplasty.

BACKGROUND: During the direct anterior approach for total hip arthroplasty, a retractor is placed on the anterior wall of the acetabulum to retract the iliopsoas muscle. This step with the retractor has been reported to put the patient at risk for femoral nerve damage. The present study aimed to clarify the effects of the anterior acetabular retractor on the status of the femoral nerve during the direct anterior approach on the basis of transcranial motor-evoked potential (MEP) analysis. METHODS: Between July 2016 and February 2017, 22 patients underwent primary total hip arthroplasty via the

A woman with forearm amyotrophy.

We present the case of a 33-year-old woman with benign sporadic monomelic amyotrophy of the distal part of the arm, called Hirayama disease. Clinical features included forearm amyotrophy sparing the brachioradialis muscle, cold paresis and causalgia. Neck magnetic resonance imaging was normal in neutral and flexion position. Electromyography showed denervated patterns in the extensor digitorum communis, and conduction studies ruled out multifocal motor neuropathy. Motor evoked potentials were normal. Serum IgG anti-GM1 antibodies were moderately raised but were negative 8 months later. Outcome

Phantom pain in a patient with brachial plexus avulsion injury.

OBJECTIVE: Phantom limb pain is a painful sensation perceived in the absent limb following surgical or traumatic amputation. Phantom limb sensations, which are nonpainful, occur in nearly all amputees. Deafferentation can also produce similar symptoms. Here we report the presence of phantom pain in a deafferented limb. DESIGN: Case report. SETTING: Hospital-based outpatient clinic. PATIENT: A 65-year-old man was referred to the pain clinic for management of upper extremity pain secondary to brachial plexus avulsion (BPA) following a motor vehicle accident. Initially he noticed a feeling of

Deficiency neuropathy in wartime: the "paraesthetic-causalgic syndrome" described by Manuel Peraita during the Spanish Civil War.

This paper discusses the contribution of Spanish neurologist Manuel Peraita (1908-1950) to the study of deficiency neuropathy in the setting of the Spanish Civil War (1936-1939). The clinical characteristics of "paraesthetic-causalgic syndrome" or "Madrid syndrome" as described by Peraita are discussed, and the syndrome is presented in relation to other similar conditions, including Strachan's syndrome and burning feet syndrome.

Use of High-Frequency 10-kHz Spinal Cord Stimulation in the Treatment of Persistent Spinal Pain Syndrome Type 2: A Systematic Review.

OBJECTIVES: In 2015, 10-kHz high-frequency (10-kHz) spinal cord stimulation (SCS) received Food and Drug Administration approval to treat persistent spinal pain syndrome type 2 (PSPS-2). The outcomes of 10-kHz SCS on PSPS-2 have not been systematically reviewed. We systematically reviewed the relevant literature and hypothesized that 10-kHz SCS is safe and effective in managing pain in patients with PSPS-2. MATERIALS AND METHODS: PUBMED and EMBASE data bases were searched with the query ((spinal cord stimulator) OR (spinal cord stimulation) OR (SCS)) AND (pain) AND ((10 khz) OR (10-khz) OR (H

[Pain syndromes with causal participation of the sympathetic nervous system].

The efferent sympathetic nervous system is organized into subsystems that innervate and regulate via separate peripheral sympathic pathways the different autonomic target organs. This review discusses mechanisms through which this efferent system may be causally involved in the generation of pain. Clinical pain syndromes in which this may be the case are "complex regional pain syndromes" (CRPS) type I (previously reflex sympathetic dystrophy) and type II (recently causalgia). The "sympathetically maintained pain" (SMP) is a symptom (and not a clinical entity) that can principally also be prese

Dorsal Root Ganglion Stimulation for Chronic Postoperative Abdominal Pain: A Case Report.

BACKGROUND: Chronic postoperative abdominal pain (CPAP) is a common condition that significantly affects patients' lives with increased gastrointestinal symptoms and decreased quality of life (QoL). Management of CPAP continues to be challenging, and adequate analgesia remains difficult to achieve. CASE REPORT: A 77-year-old woman with a history of multiple abdominal surgeries presented with chronic right-sided abdominal wall pain with possible causalgia. After failing multiple interventions, dorsal root ganglion stimulation (DRG-S) at the T8-T9 level provided significant relief. CONCLUSIONS
