Nicotinic acid: treatment for the hyperchloremic acidosis following urinary diversion through intestinal segments.

The syndrome of hyperchloremic metabolic acidosis following urinary diversion through intestinal segments has posed a problem for urologists for more than 50 years. Recent work demonstrates that chlorpromazine, an intestinal cyclic-AMP inhibitor, partially corrects the metabolic derangements associated with this syndrome. Nicotinic acid has also been shown to be a potent inhibitor of intestinal cyclic-AMP. The present investigation employs nicotinic acid in a rat vesico-cecostomy model to examine its efficacy in the management of this syndrome. Rats with vesico-cecostomies treated with nicotin

Volvulus of the cecum.

A series of 37 patients with cecal volvulus treated at three different Swedish hospitals during the years 1952-1973 is presented. The symptoms, physical findings and radiologic features are presented. The associated factors found at operation are described and their possible role in provoking torsion is discussed. In 5 patients the idagnosis did not become clear untio autopsy. Thirty-two patients were subjected to operation. The operation consisted of detorsion in 11 cases, cecopexy in 10, cecostomy in 3 and cecopexy plus cecostomy in 3 patients. The remaining 5 patients were subjected to righ

Variation in Practice Surrounding Antegrade Colonic Enema Channel Placement.

PURPOSE: Antegrade colonic enemas (ACE) can be an effective management option for defecation disorders and improve quality of life. Best practice regarding channel placement is unclear and variation may exist around preferred initial type of channel, age at placement, and underlying diagnoses. We aimed to describe practice patterns and patient characteristics around ACE channel placement. METHODS: We conducted a multicenter retrospective study of children with an ACE channel cared for at sites participating in the Pediatric Colorectal and Pelvic Learning Consortium (PCPLC) from 2017 to 2022.

Cecal volvulus. Low mortality at a city hospital.

The case records of 12 patients with cecal volvulus over a 20-year period were reviewed. The patients averaged 46.7 years old; 75 per cent were white and 67 per cent were male. Patients presented with signs and symptoms of intestinal obstruction. Pain and distention were present in more than half of the patients. X-rays assisted in making the diagnosis of cecal volvulus in only five of the patients. Diagnosis was made in the remainder at the time of surgical exploration. At operation, one cecum was necrotic and required resection. Five patients underwent cecostomy, two cecopexy, and four simpl

[Intermittent cecal volvulus. Report of 2 cases].

In our country, acute cecal volvulus is responsible of 3.3% of mechanical large bowel obstructions. More than half of these cases have a history compatible with intermittent cecal volvulus, an usually overlooked diagnosis. The author reports two cases of intermittent cecal volvulus, treated in the last ten years. Both patients are female, aged 45 and 71 years old, with a history of 3 and 5 years of recurrent abdominal distention located in the right lower quadrant, that relieves spontaneously after the expulsion of gases or defecation. The symptomatology of the first cases was erroneously attr

[Spontaneous perforation of the cecum after cesarean section].

The literature concerning perforation and imminent perforation of the caecum after cesarean section is reviewed. One case has been observed by the author. Increasing caecal diameter above 10 cm gives rise to suspicion of an imminent perforation of the caecum. Medical treatment and colonoscopy may be tried. In case of unresponsiveness or if occurrence of a cecal perforation, surgical intervention (cecostomy) is indicated.

Outcomes of children with constipation and autism spectrum disorder treated with antegrade continence enemas.

BACKGROUND: Treatment of functional constipation (FC) in children with autism spectrum disorder (ASD) is challenging due to sensory and behavioral issues. We aimed to understand whether antegrade continence enemas (ACEs) are successful in the treatment of FC in children with ASD. METHODS: A single-institution retrospective review was performed in children diagnosed with ASD and FC who underwent appendicostomy or cecostomy placement from 2007 to 2019. Descriptive statistics regarding soiling and complications were calculated. RESULTS: There were 33 patients included, with a median age of 9.7

[Acute pseudo-obstruction of the colon as complication of hip surgery].

Pseudo-obstruction of the colon is characterized by a massive colonic dilatation without mechanical obstruction. It is rarely after hip surgery, however, untreated it may be complicated by a life threatening perforation of the colon. In a retrospective analysis we reviewed 11 patients (4 women, 7 men, mean age 75.2 years, range 67 to 86 years) with pseudo-obstruction of the colon after hip surgery in the years 1995 to 1998. In relation to all patients aged 65 years and older who underwent surgery of the hip, the prevalence was 1.06%. The leading clinical symptom was abdominal distension. On pl

Sacral nerve stimulation allows for decreased antegrade continence enema use in children with severe constipation.

BACKGROUND: Sacral nerve stimulation (SNS) can be beneficial for children with constipation, but no studies have focused on children with constipation severe enough to require antegrade continence enemas (ACEs). Our objective was to evaluate the efficacy of SNS in children with constipation treated with ACE. METHODS: Using a prospective patient registry, we identified patients <21years old who were receiving ACE prior to SNS placement. We compared ACE/laxative usage, PedsQL Gastrointestinal Symptom Scale (GSS), Fecal Incontinence Quality of Life Scale (FIQL), Fecal Incontinence Severity Index

Cecal volvulus.

A review of 561 cases of cecal volvulus that were published between 1959 and 1989 along with 7 new cases, was performed to characterize the clinical and laboratory profile and to evaluate the various surgical options in treating this life-threatening condition. The age and sex distribution of these patients have changed over the years and shifted toward older patients (mean, 53 years) and female predominance (female:male ratio, 1.4:1). The clinical presentation was usually of distal closed-loop small bowel obstruction. Forty-six percent of the plain abdominal radiographs were suspected for cec

Laparoscopic cecopexy for cecal volvulus after laparoscopy. Case report and a review of the literature.

Cecal volvulus is a rare cause of large bowel obstruction. Surgical therapy depends on visceral circulatory conditions at diagnosis. Possible options include endoscopic decompression, detorsion, cecopexy with or without cecostomy, right colectomy with immediate or delayed anastomosis. The present paper describes a case of cecal volvulus in a 40-year-old woman after laparoscopic removal of a pelvic mass successfully treated by laparoscopic cecopexy.

Colonoscopic decompression of pseudo-obstruction and volvulus.

Flexible fiberoptic endoscopy has an integral role in the management of colonic pseudo-obstruction and volvulus. Colonoscopic decompression is the primary method for diagnosis and treatment of colonic pseudo-obstruction. Some patients require repeat endoscopic decompression, but few require tube cecostomy. In the case of sigmoid volvulus, endoscopic examination is useful as a temporizing measure to allow preparation of the colon and patient for elective definitive operative treatment.

Pneumatosis coli: a case presentation and review of the literature.

The case of pneumatosis coli presented here was felt to have occurred concomitantly with chronic obstructive pulmonary disease in combination with prolonged intestinal distention. This condition usually requires no treatment because it is often self-limiting. In this case, because of radiographic and clinical findings suggestive of intestinal obstruction, the patient underwent an exploratory laparotomy and tube cecostomy for decompression and drainage of the markedly distended cecum and transverse colon.

Jejunocecostomy in horses-a review of its unique complexities.

Jejunocecostomy (JC), or jejunocecal anastomosis, is arguably the most controversial surgical procedure in equine small intestinal surgery. It is also the only option for resection and anastomosis for a third of small intestinal strangulating lesions in horses. Critics of the procedure identify its low survival and high complication rates as reasons to avoid its use. This review is intended to highlight findings of recent research and clinical studies that validate use of the JC, and to provide some insights into technical aspects of the procedure that might improve its survival rates. Colonic reservoirs.

During the last decades several types of surgical techniques have been utilized in order to provide the patient with a continent colostomy using the colon as a reservoir. None of the methods have gained wide acceptance and continence is usually dependent upon successful irrigation. By constructing a cecal reservoir with an intussuscepted nipple valve, using the same principle as for the continent ileostomy, a colostomy with perfect continence for both gas and feces can be created. Although improving life quality significantly when successful, the method is hampered by a high rate of complicati

The management of cecal volvulus.

Cecal volvulus is second only to sigmoid volvulus in its frequency of occurrence. Diagnostic doubt is not uncommon in cecal volvulus; nonoperative decompression is rarely achievable; and if gangrene supervenes, mortality rises appreciably. Resection is mandatory for gangrene and a grossly distended, thin-walled cecum. Cecopexy and cecostomy seem less-effective and more morbid options than resection and anastomosis for viable bowel. However, their role needs reappraisal in the light of advances in minimally invasive techniques.

[The Ogilvie syndrome].

The authors observed 3 patients with the Ogilvie syndrome. The Ogilvie syndrome is a peculiar form of acute intestinal obstruction characterized by acute dilatation of the colon without obstruction of its distal portions and often developing against the background of other grave diseases. For a timely diagnosed pathology the conservative treatment including a colonoscopic decompression of the colon and epidural anesthesia++ proved to be effective. In cases of ineffective conservative measures a cecostomy was put, resection of the injured portion was used for necrosis and perforation of the ove

Perforation of the cecum from ingested foreign body in a 14 year old adolescent with autism spectrum disorder and epilepsy: A case report.

INTRODUCTION AND IMPORTANCE: Although foreign body ingestion is a common medical issue, intestinal perforations following foreign body ingestion are rare. Diagnosing foreign body ingestion is challenging especially in children with neurodevelopmental disorders such as Autism Spectrum Disorder (ASD). It is believed that approximately 80-90% of ingested foreign bodies pass through the gastrointestinal tract without causing any luminal damage, and only approximately 1% of foreign bodies are known to remain lodged within the bowel and cause luminal erosion and perforation. CASE PRESENTATION: A 14
