Cefepime Neurotoxicity in Acute Kidney Injury: The Importance of Renal Dosing.

Cefepime is widely used in sepsis owing to its broad spectrum of activity with coverage against Pseudomonas. Though it is usually well-tolerated, in the presence of certain risk factors, renal impairment, high doses, and critical illness, it is associated with the development of neurotoxicity. Those most susceptible are at high risk for developing complications of their underlying illness and the clinical manifestations of cefepime-related neurotoxicity are nonspecific, with reduced level of consciousness, confusion, myoclonus, and seizures, thus a high index of suspicion is required to make a

Cefepime-induced encephalopathy in a COVID-19 patient: a case report.

Prolonged neurological symptoms such as "brain fog" and cognitive impairment have occurred after coronavirus disease 2019 (COVID-19) infection. In this report, we describe impaired consciousness caused by cefepime hydrochloride (CFPM) in a patient with cognitive sequalae of COVID-19. A 56-year-old male patient was diagnosed with penile abscess after COVID-19 infection, and a blood culture detected two drug-resistant Pseudomonas aeruginosa strains. Therefore, CFPM 2 g × twice/day was administered on day 71 after intensive care unit admission. Approximately 48 h after CFPM administration, the pa

A Comparison Between Cefepime and Piperacillin-Tazobactam in the Management of Septic Shock.

Introduction Septic shock is defined as a dysregulated host response to infection characterized by hemodynamic instability. Concern for the increased risk of acute kidney injury (AKI) with piperacillin-tazobactam in combination with vancomycin may prompt more use of alternative broad-spectrum antipseudomonal beta-lactam antibiotics, such as cefepime. This study assessed whether cefepime was associated with improved outcomes compared to piperacillin-tazobactam in patients with septic shock. Methods This retrospective cohort study included veterans treated for septic shock between September 1, 2

A Treatable Encephalopathy in a Peritoneal Dialysis Patient - Cefepime-Induced Encephalopathy.

We report a patient with end-stage renal disease on peritoneal dialysis, who developed encephalopathy after receiving a few doses of cefepime. He recovered clinically and electroencephalographically after having discontinued the culprit agent and undergone hemodialysis. This case highlights the importance of promptly recognizing this reversible encephalopathy, which can lead to the avoidance of unnecessary workup, reduce the length of hospital stay, and thereby improve the patients' outcome.

Influence of plasma exchange on the disposition of the fourth generation cephalosporin cefepime.

Cefepime, a fourth generation cephalosporin, is widely used in hematology and oncology patients. These patients may require plasma exchange (PE) for indications such as chemotherapy- or cancer-induced thromobotic thrombocytopenic purpura to name a few. To date, no pharmacokinetic evaluation has been conducted assessing cefepime's disposition during PE. A 2 g IV cefepime single dose was given to patients undergoing therapeutic PE. Two hours from cefepime dose administration, plasma concentration was measured. PE was then instituted and cefepime plasmapheresate concentration was measured at the

Cefepime: a review of its use in the management of hospitalized patients with pneumonia.

UNLABELLED: Cefepime (Maxipime), Maxcef, Cepimax, Cepimex, Axepim, a parenteral fourth-generation cephalosporin, is active against many organisms causative in pneumonia. Cefepime has in vitro activity against Gram-positive organisms including Staphylococcus aureus and penicillin-sensitive, -intermediate and -resistant Streptococcus pneumoniae similar to that of cefotaxime and ceftriaxone. Cefepime also has good activity against Gram-negative organisms, including Pseudomonas aeruginosa, similar to that of ceftazidime. Importantly, cefepime is stable against many of the common plasmid- and chrom

Cefepime monotherapy for the empirical treatment of fever in granulocytopenic cancer patients.

In a pilot study, we evaluated the efficacy and the safety of cefepime, a new cephalosporin with extended-spectrum activity against both Gram-positive and Gram-negative bacteria, as empirical monotherapy for 108 febrile episodes in 84 granulocytopenic cancer patients. Cefepime (2 g tds) was given for a minimum of 7 days or until resolution of infection. Of the 108 episodes, 91 were evaluable. Microbiologically documented infections occurred in 25 patients (27%) (18 Gram-positive, 7 Gram-negative), of whom 18 had bacteraemia. Infection was clinically documented in 47 patients (52%) and fever wa

Comparative in vitro activity of cefpirome and cefepime, two new cephalosporins.

In in vitro tests the broad-spectrum cephalosporins cefpirome and cefepime were highly active against Enterobacteriaceae, although often less so against strains resistant to amoxicillin-clavulanate and ticarcillin-clavulanate, and against most strains of Acinetobacter spp. and Aeromonas hydrophila. They were also active against Pseudomonas aeruginosa, although strains with non-plasmid mediated beta-lactam resistance were sometimes less sensitive. Other Pseudomonas spp. varied in their sensitivity. Both agents were highly active against Haemophilus influenzae, but beta-lactamase-producing Branh

Overview of novel cefepime-based β-lactam/β-lactamase inhibitor combinations.

INTRODUCTION: The global rise of multidrug-resistant (MDR) Gram-negative pathogens challenges therapy, driving development of novel β-lactam/β-lactamase inhibitor (BL/BLI) combinations. Cefepime-based combinations offer mechanistically diverse options against a broad range of resistance phenotypes. AREAS COVERED: This review summarizes approved and investigational cefepime-based BL/BLI combinations, including cefepime/enmetazobactam (FEP/EMT), cefepime/taniborbactam (FTB), cefepime/zidebactam (FPZ), and cefepime/nacubactam (FEP/NAC). We discuss microbiological activity, resistance mechanisms,

Impact of porin deletions on cefepime-taniborbactam activity against Klebsiella pneumoniae.

We determined the frequency of resistance to cefepime-taniborbactam in NDM-1-producing Klebsiella pneumoniae at <10-10. Isolated mutants that were less susceptible to cefepime-taniborbactam had an increased copy number of the blaNDM-1 gene or disruption of major porins OmpK36 and OmpK35. Antibacterial susceptibility testing using K. pneumoniae isogenic strains indicated that while cefepime penetrates into the periplasm mainly through the major porins, taniborbactam does not have a strong dependence on OmpK35 or OmpK36 for periplasmic accumulation.

Cefepime-Induced Mixed Hepatocellular and Cholestatic Liver Injury: A Case Report.

Drug-induced liver injury (DILI) presents significant diagnostic challenges, particularly in patients with multiple comorbidities. We report a case involving a 72-year-old female treated with cefepime for urosepsis, who developed markedly elevated liver enzymes after two weeks of therapy. After excluding other potential causes, including viral hepatitis, ischemia, and autoimmune hepatitis, cefepime-induced mixed pattern liver injury was determined to be the likely etiology of the elevated liver enzymes. This case underscores the importance of considering DILI in the differential diagnosis and

Ceftazidime and cefepime antagonize 5-fluorouracil's effect in colon cancer cells.

BACKGROUND: Drug-drug interaction (DDI), which can occur at the pharmacokinetics and/or the pharmacodynamics (PD) levels, can increase or decrease the therapeutic or adverse response of a drug itself or a combination of drugs. Cancer patients often receive, along their antineoplastic agents, antibiotics such as ß-lactams to treat or prevent infection. Despite the narrow therapeutic indices of antibiotics and antineoplastic agents, data about their potential interaction are insufficient. 5-fluorouracil (5-FU), widely used against colon cancer, is known for its toxicity and large intra- and inte

Potential of high-dose cefepime/tazobactam against multiresistant Gram-negative pathogens.

BACKGROUND: Early β-lactamase inhibitors were combined with established penicillins, but different combinations may be more appropriate to counter current β-lactamase threats, with development facilitated by the US Generating Antibiotic Incentives Now (GAIN) Act. Cefepime/tazobactam is especially attractive, combining an AmpC-stable cephalosporin with a clinically established inhibitor, active against ESBLs and suitable for high-dose administration. METHODS: Organisms (n = 563) were clinical isolates submitted to the UK national reference laboratory. MICs were determined by CLSI agar dilution

Kidney injury, dialysis, and mortality with vancomycin plus piperacillin-tazobactam or cefepime.

BACKGROUND: Retrospective studies have consistently reported higher rates of acute kidney injury (AKI) when vancomycin is combined with piperacillin-tazobactam compared with other β-lactams, raising concerns about nephrotoxicity. However, recent prospective studies and randomized trials have not demonstrated increased risk of clinically meaningful kidney injury, suggesting that creatinine-based endpoints may overestimate harm. We applied a multistate modelling approach to evaluate whether vancomycin-piperacillin/tazobactam is associated with progression to severe renal outcomes beyond creatini

Pharmacokinetic Analysis of Intravenous Push Cefepime in Critically Ill Patients With Sepsis.

BACKGROUND: Intravenous push (IVP) administration of cefepime increases ease of preparation and limits the need for small volume parenterals. The data on IVP pharmacokinetic/pharmacodynamic (PK/PD) parameters are limited, but may be especially relevant in critically ill patients with altered PK/PD for time-dependent antibiotics like cefepime. The objective of this study was to characterize the PK profile and PD target attainment of IVP cefepime in critically ill patients with sepsis. METHODS: In this institutional review board-approved, prospective, noninterventional PK study, hospitalized ad
