BACKGROUND/OBJECTIVES: We conducted this study to evaluate the genotypic and phenotypic profiles of carbapenem-resistant Klebsiella pneumoniae (CRKP) isolates, exhibiting resistance to cefiderocol (FDC), focusing on antibiotic susceptibility, β-lactamase production, the genetic environment of blaCARB and blaESBL genes and molecular epidemiology. FDC is now a last-line antibiotic for severe infections due to CRKP.
METHODS: Susceptibility to a wide range of antibiotics was determined by the disk diffusion and broth microdilution method. Carbapenemases were screened by a modified Hodge test whil

Background: Increasing antimicrobial resistance (AMR) is one of the leading causes of death worldwide. The predominant pathogens that exacerbate the AMR problem are multidrug-resistant (MDR) Gram-negative bacteria (GNB). Due to the increasing adaptation of MDR GNB to commercially available antimicrobial drugs, such as carbapenems as well as third- and fourth-generation cephalosporins, pharmaceutical companies around the world have been forced to produce increasingly innovative chemotherapeutics. Cefiderocol (CFDC) is a novel injectable cephalosporin 5 generation developed by Shionogi, directed

Introduction and Objectives The availability of a limited arsenal of antibacterial agents effective against Burkholderia pseudomallei, the causative agent of melioidosis, together with sporadic reports of emergence of resistance necessitates an evaluation of in vitro activity of new antimicrobials against clinical B. pseudomallei isolates. Cefiderocol (CFDC), a novel siderophore cephalosporin, and ceftazidime-avibactam (CZA), a new β lactam combination agent, have shown promising results for the treatment of difficult-to-treat Gram-negative bacilli infections with limited treatment options. OBJECTIVES: Ceftazidime-avibactam (CZA) and cefiderocol are recently commercialized molecules active against highly drug-resistant bacteria, including carbapenem-resistant members of the Enterobacteriaceae. Mutants resistant to CZA have been described, notably in Klebsiella pneumoniae carbapenemase (KPC) producers. Considering the structural similarities between ceftazidime and cefiderocol, we hypothesized that resistance to CZA in KPC-producing members of the Enterobacterales may lead to cross-resistance to cefiderocol.
METHODS: CZA-resistant mutants from three clinical isolates of the Enter

Objectives: Carbapenem-resistant Enterobacterales (CRE) limit treatment options, and cefiderocol's efficacy can be compromised by regional carbapenemases. In this study, we aimed to characterize cefiderocol susceptibility distribution among Enterobacterales in Japan-specifically endemic strains producing IMP-type carbapenemases (IMP)-and explore the effect of IMP in mediating resistance. Methods: The susceptibility of 556 evaluable Enterobacterales isolates (from an initial 560 screened) to cefiderocol was assessed using disk diffusion and broth microdilution methods based on Clinical and Labo

The global spread of multidrug-resistant (MDR) Gram-negative bacteria, particularly carbapenem- and colistin-resistant strains, represents a critical public health crisis with limited therapeutic options. Cefiderocol, a first-in-class siderophore cephalosporin, addresses this challenge through an innovative Trojan horse mechanism. Its unique catechol-containing C-3 side chain enables iron chelation and active transport into bacterial periplasmic space, bypassing traditional resistance pathways. This design confers stability against all Ambler class β-lactamases (A-D), including carbapenemases

Cefiderocol represents a significant advancement in the treatment of multidrug-resistant Gram-negative bacterial infections. Its unique uptake mechanism, leveraging bacterial iron transport pathways for membrane permeation, sets it apart from conventional β-lactam antibiotics. Through its siderophore-mediated drug uptake, cefiderocol efficiently bypasses outer membrane barriers, allowing it to target and penetrate resistant pathogens, including those resistant to carbapenems and other antibiotics. This distinct property positions cefiderocol as a promising therapeutic option for infections cau

BACKGROUND: Post-lung transplantation pneumonia (pLuTP) is among the most frequent complications following lung transplantation, predominantly caused by Gram-negative bacteria (GNB). The emergence of multidrug-resistant (MDR) non-fermenting GNB as aetiological agents of pLuTP represents a major therapeutic challenge due to limited treatment options.
METHODS: We present a retrospective case series describing 20 episodes of pLuTP successfully treated with cefiderocol.
RESULTS: Thirteen patients, with a median age of 56 years (IQR 44-59), experienced a total of 20 episodes of pLuTP. In 3 cases

Antimicrobial resistance is currently recognized as an urgent concern against public health in worldwide. Carbapenem-resistant (CR) Gram-negative bacteria, such as Enterobacterales, Pseudomonas aeruginosa and Acinetobacter baumannii are listed as critical pathogens which are widely spread and can cause severe and often deadly infections in WHO guidance. Cefiderocol (Fetroja®), a novel and first siderophore cephalosporin, was approved for the infections caused by these problematic CR Gram-negative bacteria in Japan on November 30, 2023. Cefiderocol has unique mechanisms to be incorporated into

OBJECTIVES: Cefiderocol is a new siderophore cephalosporin for treating severe bacterial infections by Gram-negative pathogens in adults. Significant elimination of the drug will probably occur as a result of kidney replacement therapy, but there are currently insufficient pharmacokinetic data to guide dosing during prolonged intermittent kidney replacement therapy (PIKRT) or the use of the pathogen adsorber Seraph 100.
MATERIALS AND METHODS/CASE REPORTS: Pharmacokinetic data were obtained from two critically ill patients undergoing PIKRT and hemoperfusion with the pathogen adsorber Seraph 10

Cefiderocol (CFDC) is a siderophore-conjugated cephalosporin that hijacks bacterial iron uptake pathways to traverse the outer membrane, offering potent activity against carbapenem-resistant Acinetobacter baumannii (CRAB). Although mutations in pirA, a TonB-dependent siderophore receptor, have been linked to CFDC resistance, the broader genetic basis remains poorly defined. Using Himar1 transposon mutagenesis in a CFDC-susceptible sequence type 2 (ST2) CRAB strain, we identified ten genes whose disruption reduced CFDC susceptibility, spanning siderophore-mediated uptake (pirA and puiA), oxidat

Cefiderocol (FDC) is a last-resort antimicrobial for the treatment of carbapenem-resistant Klebsiella pneumoniae (CRKP). Although cirA inactivation is a recognized mechanism of FDC resistance, whether different forms of cirA disruption influence bacterial adaptation and persistence remains unclear. By analyzing 1,965 publicly available long-read genomes and 480 clinical isolates, we found that cirA loss-of-function mutations were rare (1.1%) but accumulated over time among isolates from China, with four independently emerged cirA-inactivated ST11 isolates identified across distinct phylogeneti

Cefiderocol (FDC), a siderophore-cephalosporin conjugate, is the newest option for treating infection with carbapenem-resistant gram-negative bacteria. We identified a novel mechanism contributing to decreased FDC susceptibility in Klebsiella pneumoniae clinical isolates. The mechanism involves 2 coresident plasmids: pKpQIL, carrying variants of blaKPC carbapenemase gene, and pKPN, carrying the ferric citrate transport (FEC) system. We observed increasing FDC MICs in an Escherichia coli model system carrying different natural pKpQIL plasmids, encoding different K. pneumoniae carbapenemase (KPC

Cefiderocol, a novel β-lactam antibiotic, exhibits potent activity against carbapenemase-producing Enterobacterales (CPE). While its clinical efficacy has been reported for infections caused by KPC-type CPE and Stenotrophomonas maltophilia, evidence regarding its effectiveness against IMP-type CPE remains primarily derived from in vitro studies, with limited clinical data available. This is the first pediatric case successfully treated with cefiderocol for IMP-type CPE bacteremia. He was a 6-year-old boy with inherited glycosylphosphatidylinositol deficiency and acute lymphoblastic leukemia un
