[Research progress on cerebral hypoperfusion following revascularization for carotid artery stenotic disease].

In clinical practice, a subset of patients with severe carotid artery stenosis fails to achieve the expected improvement in cerebral perfusion despite successful carotid revascularization and restoration of arterial patency. In these patients, cerebral ischemic symptoms persist or even deteriorate, reflecting a mismatch between macrovascular recanalization and cerebral tissue perfusion. This phenomenon is referred to as cerebral hypoperfusion syndrome (CHypoS). The pathogenesis of CHypoS is likely multifactorial, involving unfavorable cerebrovascular anatomy, persistent hemodynamic impairment,

Reduction of systemic hypertension following cerebral revascularization surgery for children with moyamoya.

OBJECTIVE: The aim of this study was to evaluate whether indirect revascularization is associated with a reduction in systemic blood pressure (BP) over time in pediatric patients with moyamoya. METHODS: This is a retrospective cohort study of pediatric patients with moyamoya who were treated at a single institution between June 2019 and January 2025. BP measurements were collected preoperatively (preadmission clinic evaluation) and postoperatively (outpatient follow-up evaluation at ≥ 3 months). BP was categorized as hypertensive or normotensive based on percentile thresholds adjusted for age

Ipsilateral Reversible Cerebral Vasculopathy Secondary to Carotid Artery Revascularization: A Case Report.

BACKGROUND: The emergence of neurologic symptoms after carotid revascularization is not uncommon and typically caused by perioperative ischemic stroke or hyperperfusion. Postoperative vasculopathy, including reversible cerebral vasoconstriction syndrome (RCVS) is a rare complication of carotid intervention and may be an under-identified cause of neurologic deficit after revascularization. We report a case of reversible postoperative vasculopathy following carotid revascularization as well as its management. CASE PRESENTATION: A 74 year old right-handed woman presented to the emergency departm

[Changes in cerebral perfusion in patients undergoing revascularization of carotid artery stenosis].

Ischemic stroke is caused by atherosclerotic lesions in extra- and intracranial arteries in about 25% of cases. Revascularization surgery has been long and widely used for secondary prevention of ischemic stroke in patients with atherosclerotic lesions of the carotid arteries. However, currently the changes in cerebral perfusion in patients with atherosclerotic lesions of brachiocephalic arteries, who underwent surgical revascularization, are still not fully understood. This article highlights the issues of changes of cerebral hemodynamics in patients undergoing surgery on the carotid arteries

Minimum Transpetrosal Retrolabyrinthine Approach for Revascularization of Posterior Cerebral Artery: Operative Nuance.

BACKGROUND: Revascularization of the posterior cerebral artery (PCA) can be essential for treating complex cerebral aneurysms in the posterior circulation, and it is considered technically challenging. To help decrease the difficulty of this technique, we developed the minimum transpetrosal approach (MTPA). OBJECTIVE: The technical nuances of the MTPA were innovated by cadaver head dissections and an actual clinical case. METHODS: Four sides of the formalin-fixed cadaver heads were used to investigate if the posterior cerebral artery could be exposed with this minimum retraction of the tempo

Rapid resolution of brain ischemic hypoxia after cerebral revascularization in moyamoya disease.

BACKGROUND: In moyamoya disease (MMD), cerebral revascularization is recommended in patients with recurrent or progressive ischemic events and associated reduced cerebral perfusion reserve. Low-flow bypass with or without indirect revascularization is generally the standard surgical treatment. Intraoperative monitoring of cerebral partial pressure of oxygen (PtiO2) with polarographic Clark-type probes in cerebral artery bypass surgery for MMD-induced chronic cerebral ischemia has not yet been described. OBJECTIVE: To describe basal brain tissue oxygenation in MMD patients before revasculariza

Flow-based evaluation of cerebral revascularization using near-infrared indocyanine green videoangiography.

OBJECT: Indocyanine green (ICG) videoangiography has been established as a noninvasive technique to gauge the patency of a bypass graft; however, intraoperative graft patency may not always correlate with graft flow. Altered flow through the bypass graft may directly cause delayed graft occlusion. Here, the authors report on 3 types of flow that were observed through cerebral revascularization procedures. METHODS: Between February 2009 and September 2013, 48 bypass procedures were performed. Excluded from analysis were those cases in which ICG videoangiography was not performed during surgery

Percutaneous revascularization of subclavian artery chronic occlusion with dual cerebral artery protection.

Percutaneous intervention for symptomatic subclavian artery chronic occlusion is an occasionally performed, minimally invasive alternative to surgical bypass. Potential complications include stroke, perforation, and recanalization failure. We discuss a case of successful percutaneous revascularization of symptomatic subclavian artery chronic occlusion using coronary devices and dual cerebral embolic protection.

Cerebral revascularization using omentum and muscle free flap for ischemic cerebrovascular disease.

BACKGROUND: Indirect cerebral revascularization has been generally accepted in the management of brain ischemia in moyamoya disease. We performed indirect cerebral revascularization by using omental flap and muscle flap techniques for the treatment of ischemic cerebrovascular disease. METHODS: Ten patients with ischemic cerebrovascular disease including three with adult moyamoya disease underwent this procedure (omental flap on eight sides and muscle flap on five sides). The muscle used for the flap was the serratus anterior muscle on two sides and shaved latissimus dorsi muscle on three side

Prevalence of Systemic Hypertension and the Effects of Cerebral Revascularization in Patients With Moyamoya Disease.

BACKGROUND: Hypertension is often codiagnosed in patients with moyamoya disease (MMD), a progressive intracranial steno-occlusive vasculopathy; this has principally been attributed to renal artery stenosis (up to 10%). Susceptibility MMD genes, including ring finger protein 213/mysterin and GUCY1A3, have also been linked to extracranial vascular disease and increased systolic blood pressure. We aimed to define the prevalence of systemic hypertension in MMD patients and characterize its evolution after cerebral revascularization. METHODS: Patients with MMD treated with extracranial-intracrania

Trends in Cerebral Revascularization in the Era of Pipeline and Carotid Occlusion Surgery Study.

BACKGROUND: In the past, microsurgical bypass was the best option for revascularization of cerebrovascular lesions that required flow replacement or flow augmentation. Over the last 2 decades with advancements in the field of neuroendovascular surgery, especially with the revolution of flow diverting stents as well as the results of the Carotid Occlusion Surgery Study, indications for microvascular bypass have significantly decreased. The purpose of this study was to evaluate trends in cerebral revascularization over the past 8 years. METHODS: We retrospectively reviewed patients with cerebra

Tibial artery autografts: alternative conduits for high flow cerebral revascularizations.

BACKGROUND: Extracranial-to-intracranial (EC-IC) bypass is a valuable tool in treating intracranial diseases requiring flow replacement or parent vessel sacrifice. Radial artery grafts (RAGs) and saphenous vein grafts (SVGs) have been used as conduits to provide adequate high flow revascularizations. It is a therapeutic challenge when these grafts are unavailable. METHODS: All EC-IC high flow cerebral revascularizations performed using conduits other than RAGs or SVGs were identified from a prospective cerebrovascular registry. These patients were retrospectively reviewed for surgical techniq

Risks of stroke and current indications for cerebral revascularization in patients with carotid occlusion.

Preventing further stroke in patients with complete carotid artery occlusion remains a difficult challenge because there is no therapy proven effective for this prevention. These patients comprise approximately 15% of patients with carotid artery territory transient ischemic attacks or infarction. Patients with symptomatic carotid artery occlusion have an overall risk of subsequent stroke of 7% per year and a risk of stroke ipsilateral to the occluded carotid artery of 5.9% per year. The presence of severe hemodynamic failure demonstrated by increased oxygen extraction fraction (OEF) of the br

Correlation of MRI and CT data with outcome of cerebral revascularization after stroke.

Sixteen patients with the occlusion of the internal carotid artery (ICA) underwent extra-intracranial bypass surgery. All patients had stroke in evolution or completed stroke with mild or moderate (n = 9) or severe (n = 7) neurological deficits. In each case, the clinical course, magnetic resonance imaging (MRI), computed tomography (CT) and angiographic (AG) findings were evaluated. Patients were followed up from 9 months to 2.5 years postoperatively. MRI was much more sensitive than CT for appreciating the ischaemic tissue. The infarct volume determined by MRI had greater extent than previou

Distal posterior cerebral artery revascularization in multimodality management of complex peripheral posterior cerebral artery aneurysms: technical case report.

UNLABELLED: OBJECTIVES AND IMPORTANCE: Although controversial, revascularization of the distal posterior cerebral artery (PCA) may become necessary in treating complex PCA aneurysms before proximal parent vessel sacrifice. The revascularization of the distal PCA territories of two patients with peripheral PCA aneurysms is presented. CLINICAL PRESENTATIONS: One patient had an aneurysm of the P2-P3 segment and failed a selective P2 balloon occlusion test by developing homonymous hemianopia before planned parent vessel sacrifice. The other patient with a giant P2 segment aneurysm presented with

Microsurgical Trapping and Dual Revascularization for a Giant Fusiform Posterior Cerebral Artery Aneurysm.

Giant fusiform distal posterior cerebral artery (PCA) aneurysms are rare and complex lesions that require distal perfusion preservation when treated. Standard treatment modalities for complex aneurysms (e.g. coiling, flow-diversion or clipping) may compromise blood flow to eloquent territory or result in incomplete occlusion, particularly when a branch arises from the aneurysm.1-3 When fusiform PCA aneurysms are treated with proximal occlusion or trapping, distal revascularization is essential to achieve complete exclusion while avoiding ischemic complications.1,4-6 We present a giant PCA aneu

A Novel Interhemispheric Dural Inversion Technique for Indirect Parafalcine Cerebral Revascularization: Case Report.

BACKGROUND AND IMPORTANCE: Variety of revascularization strategies have been deployed to treat the deterioration of cerebral blood flow. Blood flow restoration can be achieved by direct or indirect procedures. The indirect reconstructive procedure to augment blood flow involves encephalomyosynangiosis, encephaloduroarteriosynangiosis, or encephalodurosynangiosis, and has been used effectively in ischemic Moyamoya disease. However, the preferred procedure must be evaluated on a case-by-case basis and remains controversial in other cerebral arterial diseases. Here, we report the "interhemispheri

Cerebral hyperperfusion syndrome after stenting for revascularization of intracranial internal carotid artery dissection.

Cerebral hyperperfusion syndrome (CHS) is one of the complications of cerebral revascularization. The main pathophysiology of CHS was considered to be cerebral autoregulation impairment due to long-standing cerebral hypoperfusion. Herein, we describe the case of a 40-year-old man with symptomatic intracranial arterial dissection (IAD) related to internal carotid artery stenosis. The patient underwent intracranial stenting 11 days after onset due to severe cerebral hypoperfusion presenting with neurological symptoms, and CHS presenting with intracerebral hemorrhage, post-operatively. The presen

Carotid revascularization and cognitive impairment: the neglected role of cerebral small vessel disease.

Carotid atherosclerosis is a pathological process that leads to narrowing of the vessel lumen and a consequent risk of stroke. Revascularization procedures such as carotid endarterectomy (CEA) and carotid stenting aim to reduce occurrence of stroke in selected patients. Due to the proven benefit and low intraoperative risk, CEA is currently the preferred choice in candidates for carotid revascularization. However, the risk of cognitive impairment subsequent to CEA has not been fully elucidated and is unclear whether certain conditions, such as frailty, may increase this risk. There is consiste

Comparative efficacy of autologous versus cadaveric saphenous vein grafts in cerebral revascularization surgery.

OBJECTIVE: Cerebral bypasses are performed for the purpose of either flow augmentation for ischemic cerebrovascular disease or flow replacement for vessel sacrifice during complex aneurysm or tumor surgery. Saphenous vein grafts (SVGs) are commonly used interposition grafts. The authors of this study sought to compare the efficacy of autologous versus cadaveric SVGs in a large series of cerebral bypasses using interposition vein grafts with long-term angiographic follow-up. METHODS: All intracranial bypass procedures performed between 2001 and 2018 were reviewed. Demographic, clinical, angiog
