Role of endoscopic third ventriculostomy at infected cerebrospinal fluid shunt removal.

OBJECT: Cerebrospinal fluid shunt infection is distressing, especially in the pediatric population. Usually, infected CSF shunts are removed, and after temporary external CSF drainage, reinsertion of the CSF shunt is necessary. Unfortunately, it is not rare to encounter CSF reinfection after shunt renewal, and furthermore, the reinserted CSF shunt is at a considerable risk of malfunction. Endoscopic third ventriculostomy (ETV) is a potent option in managing CSF shunt infection, although ETV failure may occur more frequently when it is used to remove an infected shunt. The authors retrospective

An unusual parasagittal tumour with acute blindness and response to cerebrospinal fluid shunting.

We report a seven-year-old girl who presented with a sudden tumor apoplexy due to a parasagittal hemangiopericytoma. Following tumor excision, the child was noted to have bilateral optic nerve dysfunction and progressive papilledema, despite rapid overall neurological improvement. Based on the clinical features, we feel that this case represents an unusual form of visual deterioration related to impaired CSF absorption somehow precipitated by the acute tumour apoplexy. This unusual case of blindness responded significantly to CSF shunting. Several reports exist describing raised intracranial p

Cerebrospinal fluid shunt dynamics in patients with idiopathic adult hydrocephalus syndrome.

The objective was to assess CSF dynamics of different shunt constructions in patients with adult hydrocephalus syndrome and correlate these findings to clinical outcome, neuroradiology, and the specifications of the shunts provided by the manufacturer. Thirty four patients with idiopathic adult hydrocephalus (normal pressure hydrocephalus) syndrome were included in a prospective, consecutive case series. A differential pressure valve (Cordis Hakim standard system) was used in 28 patients and a variable resistance valve (Cordis Orbis-Sigma) in six. A constant pressure infusion method was used;

Optimum position for an anti-siphon device in a cerebrospinal fluid shunt system.

The effects on shunt flow from the position of an antisiphon device (ASD) and from changes in posture in hydrocephalic patients were examined. Fifty patients with hydrocephalus (including 36 with normal pressure hydrocephalus) were investigated, using quantitative radionuclide shuntography (99mtechnetium-pertechnetate) in the supine, sitting, and standing positions. The types of shunt valve used were as follows: Mishler dual chamber low pressure without ASD (16 cases), with ASD 40 cm below the level of the foramen of Monro (three cases), and with ASD 10 cm below the level of the foramen of Mon

Cerebrospinal fluid pseudocyst presenting as a hepatic mass: a complication of ventriculoperitoneal shunt.

The case of a 3-year-old male child is reported who had a history of congenital hydrocephalus, treated by VP shunting. Severe complications with VP shunting are rare, but in this case the shunt had apparently perforated the liver, causing a pseudocyst to form and subsequent hepatomegaly. The shunt was revised surgically and the pseudocyst resected.

Radionuclide demonstration of ventriculoatrial shunt-cutaneous fistula with cerebrospinal fluid leak.

Radionuclide techniques have commonly been used to assess ventriculovenous shunt function, cerebrospinal fluid dynamics, and for the diagnosis of cerebrospinal fluid leaks. All of these techniques were utilized in this unusual case of retrograde migration of a ventriculoatrial catheter into the neck, with cutaneous fistulization and CSF leak.

Cerebrospinal fluid shunt infections. A review of 35 infections in 32 patients.

The authors have reviewed the clinical manifestations and therapy of hydrocephalus shunt infections in 32 patients with a total of 35 shunt infections. These 35 infections accounted for 43 hospital admissions. First infections usually developed within 2 months following surgery. At the time of diagnosis, 89% of patients were febrile. Fever and cough as a symptom complex characterized the initial clinical presentation in six of 19 episodes of infection complicating ventriculoatrial (VA) shunts, as compared with none of 21 episodes in which infection complicated ventriculoperitoneal (VP) shunts.

Ventriculosagittal sinus shunt for treating hydrocephalus with elevated cerebrospinal fluid protein.

This study aimed to investigate the feasibility, acceptability, and preliminary efficacy of a ventriculosagittal sinus (VSS) shunt in the treatment of hydrocephalus with elevated cerebrospinal fluid (CSF) protein content. In this single-center retrospective analysis, we enrolled 80 patients with hydrocephalus and elevated CSF protein levels. Based on these procedures, primary cohort was divided into two groups using CSF protein (CSFP) for subsequent analysis to determine the relationship between the clinical effect and CSFP. Preoperative and postoperative computer tomography (CT) scans, clinic

Techniques to Reduce the Rate of Infection in Surgeries for Cerebrospinal Fluid Shunting in Adults.

OBJECTIVE: Cerebrospinal fluid shunt placement is associated with high rates of infection. Multiple standardized protocols, particularly in pediatric populations, have been proposed to mitigate this infection rate. We sought to determine the effectiveness of a standardized shunt infection protocol in a large adult population. METHODS: A retrospective cohort study of adults presenting for primary cerebrospinal fluid shunt placement from 2012 to 2022. The primary outcome of interest was shunt infection. The primary exposure of interest was implementation of the shunt protocol (began October 201

Treatment Strategies and Challenges to Avoid Cerebrospinal Fluid Shunting for Pediatric Hydrocephalus.

Treatment for pediatric hydrocephalus aims not only to shrink the enlarged ventricle morphologically but also to create an intracranial environment that provides the best neurocognitive development and to deal with various treatment-related problems over a long period of time. Although the primary diseases that cause hydrocephalus are diverse, the ventricular peritoneal shunt has been introduced as the standard treatment for several decades. Nevertheless, complications such as shunt infection and shunt malfunction are unavoidable; the prognosis of neurological function is severely affected by

Patient and Treatment Characteristics by Infecting Organism in Cerebrospinal Fluid Shunt Infection.

BACKGROUND: Previous studies of cerebrospinal fluid (CSF) shunt infection treatment have been limited in size and unable to compare patient and treatment characteristics by infecting organism. Our objective was to describe variation in patient and treatment characteristics for children with first CSF shunt infection, stratified by infecting organism subgroups outlined in the 2017 Infectious Disease Society of America's (IDSA) guidelines. METHODS: We studied a prospective cohort of children <18 years of age undergoing treatment for first CSF shunt infection at one of 7 Hydrocephalus Clinical R

Association of Brucella meningoencephalitis with cerebrospinal fluid shunt in a child: a case report.

Brucellosis is an endemic zoonosis in Iran. It is a systemic infection that can involve any organs or systems of the body and have variable presentations. Ventriculoperitoneal (VP) shunt infections due to brucellosis have been rarely reported in the literatures. This is the history of a four years old boy who developed Brucella meningoencephalitis at the age of 42 months, whilst he had a VP shunt in situ for hydrocephalus treatment. Also, he presented brucellosis as acute abdomen. This patient was treated with trimethoprim-sulfamethoxazole, gentamicin and rifampicin. The shunt was extracted an

Signs and symptoms of cerebrospinal fluid shunt malfunction in the pediatric emergency department.

OBJECTIVES: Pediatric patients with cerebrospinal fluid shunts frequently present to the emergency department for evaluation of possible shunt malfunction. Most shunt studies appear in the neurosurgical literature. To our knowledge, none have reviewed presenting signs and symptoms of shunt malfunction in patients who present to the pediatric emergency department. The study objective was to evaluate the medical record of children with cerebrospinal fluid shunts who presented to a pediatric emergency department to determine if any signs and/or symptoms were predictive of shunt malfunction. Shunting procedures in the management of intracranial cerebrospinal fluid cysts in infancy and childhood.

Widely diverging opinions on the optimal therapy for intracranial cerebrospinal fluid cysts (CSF), mainly arachnoid cysts and the Dandy-Walker cysts, exist. Excision of the cyst walls in the treatment of the Dandy-Walker cyst has been replaced by shunting procedures, but the recommended method for primary treatment of arachnoid cysts in childhood is still cyst wall excision. Membrane excision is, however, often complicated by recurrence, subsequently requiring shunting-procedures. In a series of 19 cases primary shunting of intracranial CSF cysts proved to be a reliable method. In those cases

Testing cerebrospinal fluid shunt capacity and adequacy by the lumbar route in adults.

The pressure of the cerebrospinal fluid with the patient in a lateral position, the level of zero pressure in sitting position, and the infusion cisternography were evaluated as a test for determining shunt function. The lumbar infusion test gave information about shunt capacity, while the opening pressure in lateral and sitting position reflected shunt adequacy. A reduction in shunt capacity preceded the rise of the opening pressures when the shunt capacity preceded the rise of the opening pressures when the shunts became inadequate. Cisternography was relatively reliable in determining shunt

Retroperitoneal Migration And Transgluteal Extrusion of a Distal Lumboperitoneal Shunt Catheter: An Unusual Complication of Cerebrospinal Fluid Shunt.

Cerebrospinal fluid shunts provide many and different complications. Only 1-3% of them are rare, related to migration, and perforation of hollow organs. Even more unusual are the complications associated with the distal catheter leaving the peritoneal cavity and the tip migrating through some of the anatomical foramens to the muscle layers on the back. We present a rare complication, migration of the distal lumboperitoneal shunt catheter through the retroperitoneum and the gluteal muscles to the hypoderma. The peritoneal catheter entered the retroperitoneal area, passed through the greater sci

Evaluation of intraoperative glove change in prevention of postoperative cerebrospinal fluid shunt infections, and the predictors of shunt infection.

BACKGROUND: We have previously suggested that surgical gloves could be a possible means for transferring microorganisms from skin flora to shunt material during surgery. The objectives of this study were to examine (1) whether the rate of shunt infections was reduced after introducing intraoperative glove change before handling the shunt material; (2) clinical presentation of shunt infections, microbiological data, and treatment management; and (3) predictors of shunt infections. METHODS: A retrospective study of 432 shunt operations in 295 adults was undertaken over a 7-year period. Study po

Effect of cerebrospinal fluid shunts on intracranial pressure and on cerebrospinal fluid dynamics. 2. A new technique of pressure measurements: results and concepts. 3. A concept of hydrocephalus.

Part 2 describes measurements of intracranial cerebrospinal fluid (CSF) pressure in 18 adult patients with CSF shunts, all pressure measurements being referred to a horizontal plane close to the foramina of Monro. All 18 patients had normal CSF pressure by lumbar puncture; however, in one patient an intracranial pressure of +280 mm was subsequently measured after pneumoencephalography. Twelve patients had pre-shunt CSF pressures measured intracranially: 11 ranged from +20 to +180 mm H(2)O and one was +280 mm H(2)O in the supine position. In the upright posture nine patients had values of -10 t

A route for cerebrospinal fluid flow through leptomeningeal arterial-venous overlaps enables macromolecule and fluid shunting.

The flow of cerebrospinal fluid (CSF) is important for conveying brain-derived macromolecules for signaling and enabling them to be drained from the brain parenchyma. The glymphatic route is the best-characterized means of this CSF flow; however, it does not permit the movement of larger macromolecules. Here, we identify in mice an alternative route whereby intra-CSF-injected macromolecules can traverse from periarterial to perivenous spaces, with transfer occurring at sites of overlap between leptomeningeal perivascular (arteriovenous) spaces dispersed across the surface of the brain's leptom
