Ear diseases in elderly hospital patients in Nigeria.

BACKGROUND: Little information is available about the pattern of ear diseases in the elderly population. Therefore, the present study aims to determine the pattern of ear diseases among elderly Nigerians, so as to provide an objective basis for cost-effective health care planning for the emerging geriatric population. METHODS: A retrospective study of 320 patients aged 60 years or older presenting with ear diseases at the University of Ilorin Teaching Hospital, Ilorin, Nigeria, between January 1996 and December 2001 was carried out. RESULTS: Impacted cerumen was the most common ear disease,

A low-cost self-assembled external auditory canal-tympanic membrane model for early-stage endoscopic ear surgery training: A pilot feasibility study.

BACKGROUND: Endoscopic ear surgery (EES) offers advantages over microscopic techniques but remains challenging for beginners, particularly due to one-handed operation and limited access to training. This study aimed to develop a low-cost, self-assembled model and preliminarily evaluate its feasibility and usability for early-stage EES training. METHODS: An external auditory canal-tympanic membrane (EAC-TM) model was constructed using readily available materials (e.g., paper straw and polyethylene film). Fifteen otolaryngology trainees (postgraduate years 1-7) participated in a 2-h training se

Predictors of hearing loss in school entrants in a developing country.

BACKGROUND: Hearing loss is a prevalent and significant disability that impairs functional development and educational attainment of school children in developing countries. Lack of a simple and practical screening protocol often deters routine and systematic hearing screening at school entry. AIM: To identify predictors of hearing loss for a practical screening model in school-aged children. SETTINGS AND DESIGN: Community-based, retrospective case-control study of school entrants in an inner city. METHODS: Results from the audiologic and non-audiologic examination of 50 hearing impaired ch

Agitation in Nonverbal Patients With Autism: The Role of Sensory Evaluation in Diagnosis and Management.

Autism spectrum disorder (ASD) is a complex neurodevelopmental condition often associated with communication barriers, sensory sensitivities, and behavioral challenges, including agitation. Identifying the underlying causes of acute agitation in nonverbal individuals with ASD can be difficult, leading to diagnostic delays and unnecessary medical interventions. We present a case of a 36-year-old nonverbal male with ASD who was admitted for severe agitation, self-injurious behaviors, and increased aggression towards family members. His medical history included epilepsy, hypertension, and prior o

[Two cases of dermatophytosis of the external auditory meatus].

We report two cases of dermatophytosis of the external auditory meatus. Case 1: A 44-year-old man suffered from severe itching in the right external auditory meatus for a year, and had also had tinea unguium for several years. He visited our outpatient clinic because of scaly erythema which had developed on the auricle. Otoscopic examination revealed yellow-brown dry cerumen and redness from the cartilaginous to the bony portion of the external auditory meatus. Case 2: A 14-year-old boy, the son of Case 1, suffered from severe itching in the left external auditory meatus. He scratched the audi

Earwax, osmidrosis, and breast cancer: why does one SNP (538G>A) in the human ABC transporter ABCC11 gene determine earwax type?

One single-nucleotide polymorphism (SNP), 538G>A (Gly180Arg), in the ABCC11 gene determines the type of earwax. The G/G and G/A genotypes correspond to the wet type of earwax, whereas A/A corresponds to the dry type. Wide ethnic differences exist in the frequencies of those alleles, reflecting global migratory waves of the ancestors of humankind. We herein provide the evidence that this genetic polymorphism has an effect on the N-linked glycosylation of ABCC11, intracellular sorting, and proteasomal degradation of the variant protein. Immunohistochemical studies with cerumen gland-containing t

Building an Otoscopic screening prototype tool using deep learning.

BACKGROUND: Otologic diseases are often difficult to diagnose accurately for primary care providers. Deep learning methods have been applied with great success in many areas of medicine, often outperforming well trained human observers. The aim of this work was to develop and evaluate an automatic software prototype to identify otologic abnormalities using a deep convolutional neural network. MATERIAL AND METHODS: A database of 734 unique otoscopic images of various ear pathologies, including 63 cerumen impactions, 120 tympanostomy tubes, and 346 normal tympanic membranes were acquired. 80% o

Emotional and Cognitive Effects of Simulated Temporary Hearing Deficit with Healthy Adults.

Background/Objectives: Accumulation of cerumen (earwax) in the auditory canal is a common condition, particularly in children and older adults, and often causes temporary hearing loss. While chronic hearing loss is known to affect mood and cognition, little is known about the psychological impact of short-term auditory deprivation. This pilot study aimed to examine the emotional and cognitive effects of simulated temporary hearing loss. Methods: Thirty healthy adults (16 females, aged 18-60) participated. Temporary hearing loss was simulated by placing earplugs in both ears for two hours. Part

Severe audiovestibular loss following ear syringing for wax removal.

Syringing of the ear is one of the commonest procedures performed for cleaning cerumen from the external auditory canal. Common complications following syringing are pain, external auditory canal trauma and otitis externa. Hearing and vestibular loss are often mentioned as a complication in descriptions of this technique, but we have not been able to find a reported case of such an occurrence. We report one such a case.

Management of Conductive Hearing Loss in Children.

Conductive hearing loss (CHL), far more common than sensorineural hearing loss in children, can be acquired or congenital, can range from mild to moderately severe, and can be caused by a simple cerumen impaction, middle ear fluid, or complex middle ear abnormalities with or without the absence of the ear canal (congenital aural atresia). This article presents evidence-based recommendations for the evaluation and management of the child with both acquired and congenital CHL.

[The influence of skin parasite infection on the productivity of gilts at pig testing stations].

On the basis of skin scrapings collected together with cerumen from the inside of ears, the occurrence of a hidden mite and louse infection was assessed to evaluate its effect on the productive traits of gilts fattened during 1996-1997 at Pig Testing Stations (PTS). The productivity was highly influenced by the station factors. The low level of infection had, however, some negative effect in a few cases, and in few it was not clear, probably due to the phenomenon of resilience.

Upper respiratory problems in the yellow nail syndrome.

A case of the Yellow Nail Syndrome is described. The characteristic nail changes were associated with pleural effusions, lymphoedema of the face and legs and upper respiratory symptoms. The finding of extremely hard ear wax has not been reported previously. A satisfactory response to combination diuretic therapy was seen. The case of recognition of the Syndrome is emphasized together with the danger of excessive investigation if the nails are not inspected. Possible mechanisms for the formation of extremely hard cerumen are discussed.

Safety and efficacy of Sofenz ceruminolytic solution.

We conducted an open-label study of 109 untreated patients who had excessive or impacted cerumen. Our goal was to evaluate the safety and efficacy of Sofenz Cerumenolytic Solution, a methyltrypsin-containing earwax removal solution. Each patient's external auditory canal was filled with Sofenz for 15 minutes. The primary measure of efficacy--visibility of the tympanic membrane--was assessed after the solution had been drained from the canal and again after the canal had been irrigated with lukewarm water. If the tympanic membrane was not completely visible following either application, the pro

Ceruminous glands in otosclerosis. A histopathological and histochemical study.

The ceruminous glands of the external auditory canal skin of 20 patients suffering from otosclerosis were examined histologically and histochemically. The histological changes included flattening of the lining cells and widening of the lumina of the glands. Histochemical alterations were in the form of diminished succinic dehydrogenase, alpha esterase and alkaline phosphatase enzyme activities. The histological and histochemical changes indicated that atrophy of the ceruminous glands with diminished wax production occurred in some otosclerotic patients. This atrophy may be secondary to the inv
