Shoulder-hand syndrome in cervical spinal cord injury.

To characterize the occurrence of shoulder-hand syndrome (SHS) complicating the rehabilitation of patients with cervical spinal cord injury, we reviewed the medical records of 43 consecutive patients admitted to the Burke Rehabilitation Center with cervical spinal cord injury, focusing on the clinical features of SHS: shoulder pain, hand/wrist pain, edema, vasomotor changes, trophic changes and osteoporosis on x-ray. Twenty-seven patients (63%) had three or more features of SHS. The number of features correlated with age (r = 0.495, p = 0.0007), but not with the presence of upper or lower moto

Cervical spinal cord atrophy associated with spina bifida.

We treated three patients with spina bifida who had atrophy of the cervical spinal cord. One patient presented with a gradually progressive paresis of the upper right extremity, and two patients developed tetraparesis. MRI (magnetic resonance imaging) of the cervical spine demonstrated cord atrophy with enlargement of the subarachnoid space. The three patients were treated by releasing the tethered spinal cord in the lumbosacral area. Two of the three patients had neurological recovery soon after the operation, but this was transient and was followed by later deterioration; and the third patie

Cervical cord extradural haematoma with familial cutaneous haemangiomas.

This case report confirms the link between familial cutaneous haemangiomas and haemangiomas of the spinal extradural space. Haemorrhage from a cervical extradural haemangioma may cause a haematoma resulting in an anterior cord syndrome. Recovery is possible with early diagnosis and surgical decompression at the involved level. If the diagnosis is not made the patient may die from this extradural cervical compression. Diagnosis involves myelography with spinal angiography to confirm the site and extent of both haemangioma and extradural haematoma.

Histiocytosis-X isolated to the cervical spinal cord.

Histiocytosis-X may involve many tissues, including the central nervous system. This report is the first documentation of histiocytosis-X occurring exclusively in the spinal cord. We present a patient with cervical spinal cord disease causing spastic paraplegia, flaccid right arm weakness, and sensory loss. Magnetic resonance imaging of the spinal cord clearly demonstrated the entire lesion. Following surgical decompression and biopsy, the cervical spinal cord was treated with low-dose radiation, which resulted in complete regression of the lesion by radiologic assessment. The child became neu

Intramedullary Hemangioblastoma in the Cervical Spinal Cord of a Dog.

A 9-year-old neutered male French bulldog presented with gradually progressive tetraparesis. Neurological examination revealed reduced postural reactions in the pelvic limbs and right thoracic limb. Magnetic resonance imaging revealed a 0.9-cm intramedullary mass at the level of the C6 vertebra. The mass was surgically removed via dorsal laminectomy and dorsal midline myelotomy. Histopathological analysis confirmed a hemangioblastoma composed of capillaries and stromal cells. Immunohistochemical analysis revealed that the capillaries were labeled with Factor VIII-related antigen. Stromal cells

Sparganosis mimicking an intramedullary tumor of the cervical cord.

Sparganosis is a rare parasitic infection caused by the migrating plerocercoid larva of the tapeworm Spirometra mansoni that rarely affects the central nervous system. When sparganosis involves the spinal cord, it often affects the thoracic area. Here we report a patient with sparganosis mimicking an intramedullary tumor of the cervical cord. A 51-year-old male presented with slowly progressive paresthesias in all extremities and left-sided motor weakness. MRI of the spinal cord revealed an enhancing lesion from C1 to C4 that was suggestive of an intramedullary tumor. The results of enzyme-lin

Brachial diplegia as a result of cervical cord injury.

OBJECTIVE: To report a case of brachial diplegia with normal lower limb power, after a cervical cord injury. DESIGN: Case report of a 63-year-old man who sustained a neck trauma following a fall. SETTING: Department of Neurology, University Hospitals of Leicester, Royal Infirmary, Leicester, UK. CASE REPORT: The mechanism of the injury was probably one of hyperflexion resulting in central cervical cord contusion. Initial neurological examination revealed brachial diplegia, with posterior column sensory involvement of the lower limbs which had normal power. The patient was also in urinary re

[Thorburn's posture. A sign of lesion of the cervical cord].

In two infants hypertonia of the scapular muscles (C5-C6 level) associated with flacid palsy in other muscles of the upper limb produced an unusual posture. This one, described in adults by Thorburn, suggests a traumatic or ischaemic lesion of the cervical cord.

Neurapraxia of the cervical spinal cord with transient quadriplegia.

The purpose of this study was to define as a distinct clinical entity the syndrome of neurapraxia of the cervical spinal cord with transient quadriplegia. The sensory changes include burning pain, numbness, tingling, and loss of sensation, while the motor changes range from weakness to complete paralysis. The episodes are transient and complete recovery usually occurs in ten to fifteen minutes, although in some patients gradual resolution occurs over a period of thirty-six to forty-eight hours. Except for burning paresthesia, pain in the neck is not present at the time of injury and there is c

Charting Cervical Spinal Cord Morphometry Across the Lifespan.

Spinal cord morphometry provides essential biomarkers of neurological health, but clinical interpretations are confounded by inter-subject variability and a lack of normative references across the full human lifespan. We address this gap by generating the first comprehensive lifespan charts for cervical spinal cord morphometry. From 78,269 T1-weighted brain MRI sessions, we identified a final cross-sectional cohort of 24,251 healthy or cognitively normal controls from 33 datasets (ages 0-100 years) with cervical cord coverage. To overcome contrast variability, we employed a state-of-the-art co

Prospective motion correction for cervical spinal cord MRS.

PURPOSE: To develop prospective motion correction for single-voxel MRS in the human cervical spinal cord. METHODS: A motion MR navigator was implemented using reduced field-of-view 2D-selective RF excitation together with EPI readout. A short-echo semi-LASER sequence (TE  = 30 ms) was updated to incorporate this real-time image-based motion navigator, as well as real-time shim and frequency navigators. Five healthy participants were studied at 3 T with a 64-channel head-neck receive coil. Single-voxel MRS data were measured in a voxel located at the C3-5 vertebrae level. The motion navigator

Late Postoperative Cervical Spinal Cord Herniation with Pseudomeningocele.

We report a middle-aged man with a history of traumatic cervical spinal cord injury with extensive edema. Decompressive surgery of posterior laminectomy and fixation was performed. An irreparable dural rupture on dorsal-lateral side of the cord was found during operation. The patient was managed with a myofascial and fibrin glue assisted closure and underwent watertight closure of fascia and skin. No cerebrospinal fluid leakage was detected in the postoperative period and the wound was healing, thus the patient was discharged. Clinical symptoms resolved. Half a year later, however, the patient

The Restless Spinal Cord in Degenerative Cervical Myelopathy.

BACKGROUND AND PURPOSE: The spinal cord is subject to a periodic, cardiac-related movement, which is increased at the level of a cervical stenosis. Increased oscillations may exert mechanical stress on spinal cord tissue causing intramedullary damage. Motion analysis thus holds promise as a biomarker related to disease progression in degenerative cervical myelopathy. Our aim was characterization of the cervical spinal cord motion in patients with degenerative cervical myelopathy. MATERIALS AND METHODS: Phase-contrast MR imaging data were analyzed in 55 patients (37 men; mean age, 56.2 [SD,12.

Cervical cord and ventricle affection in neuromyelitis optica.

OBJECTIVES: Cervical cord involvement is common in neuromyelitis optica (NMO) and multiple sclerosis (MS), but its impact on disability in NMO has rarely been studied. Recent publications on NMO examined the periventricular system, areas of high aquaporin-4 expression, but not yet by using ventricle volumetry. PURPOSE: To compare cervical cord atrophy, ventricular widening, and supra- and infratentorial brain measures between NMO and MS, and study their impact on clinical disability. METHODS: Magnet resonance imaging-based volumetry of upper cervical cord, third and fourth lateral ventricles
