Bilateral brachial plexus block in a patient with cervical spinal cord injury: A case report.

RATIONALE: Bilateral brachial plexus block (BPB) generally requires a relatively large dose of local anesthetic for a successful block, resulting in a high risk of local anesthetic systemic toxicity. It can also result in inadvertent bilateral phrenic nerve palsy, leading to respiratory failure. Hence, it has not been widely used. However, it can be performed in selected patients. In this report, we present a case of ultrasound-guided BPB for bilateral upper extremity surgery in a patient with cervical spinal cord injury (SCI). PATIENT CONCERNS: A 25-year-old woman with SCI secondary to traum

Cystic schwannoma of the cervical plexus masquerading as a type II second branchial cleft cyst.

This case illustrates the rare occurrence of a cystic schwannoma of the neck, presenting clinically and ultrasonographically as a type II second branchial cleft cyst. Histology demonstrated that it was in fact a rare cystic schwannoma, most likely arising from the cervical plexus. This is the first documented case of a cystic schwannoma of the neck being mistaken for a type II second branchial cleft cyst based on clinical and ultrasound findings alone. It suggests that more sophisticated radiological investigations, such as magnetic resonance imaging, may enable accurate diagnosis of a cystic

Unintentional transfixation of a transverse cervical vein during placement of an interscalene brachial plexus catheter.

Cervical hematoma during placement of a catheter into the interscalene groove is a known complication of the procedure. I describe the occurrence of a hematoma while using a new system that is compatible with a nerve stimulator and offers the ability to aspirate during both needle placement and catheter insertion.

Anesthetic Management for Carotid Endarterectomy Using Superficial and Intermediate Cervical Plexus Block: A Case Report.

Carotid endarterectomy (CEA) is a surgical procedure that is performed to reduce the risk of stroke by removing atherosclerotic plaque from the carotid artery. Regional anesthesia, mainly a combined superficial and intermediate cervical plexus block, is used as an alternative to general anesthesia for maintaining cerebral perfusion, hemodynamic control, cerebral monitoring, and rapid neurological assessment during surgery. Here, we present a case of a 69-year-old male with symptomatic right carotid artery stenosis who underwent a successful right CEA using a combined superficial and intermedia

Interscalene Block Versus Pericapsular Nerve Block and Superficial Cervical Plexus Block for Arthroscopic Shoulder Surgery.

BACKGROUND: Interscalene brachial plexus block (ISB) remains the gold standard for analgesia in arthroscopic shoulder surgery (ASS). However, ISB is associated with a higher incidence of hemidiaphragmatic paralysis (DP). OBJECTIVES: This study compares ultrasound-guided interscalene brachial plexus block (USG-ISB) with a combination of ultrasound-guided pericapsular nerve block (USG-PENB) and superficial cervical plexus block (SCPB) to evaluate analgesic efficacy and the incidence of DP. METHODS: In this prospective, triple-blinded randomized trial, 42 American Society of Anesthesiologists (

Misdiagnosis of supraclavicular brachial plexus metastasis as cervical disc herniation: A lung cancer case report.

Cervical radiculopathy, with an incidence of approximately 85 per 100,000 individuals annually, is characterized by pain originating from cervical nerve root disruption. Accurate diagnosis is challenging because of the absence of a universally accepted gold standard, necessitating reliance on patient history, clinical examination, and diagnostic tests. Brachial plexus metastasis is a rare but clinically significant condition that can mimic common musculoskeletal disorders, such as cervical radiculopathy. Its nonspecific presentation often results in delayed diagnosis, particularly in patients

Quantification of Trapezius Muscle Innervation During Neck Dissections: Cervical Plexus Versus the Spinal Accessory Nerve.

OBJECTIVES: Despite increasing use of selective, nerve-sparing surgical techniques during neck dissections, the reported rate of postoperative paralysis of the trapezius muscle is still high. The aim of the study is to measure and compare motor inflow to the trapezius muscle, in order to better understand the peripheral neuroanatomy. METHODS: Intraoperative nerve monitoring (electroneurography) in patients undergoing routine neck dissection (n=18). The innervation of the 3 functional parts of the trapezius muscle was mapped and quantified through compound muscle action potentials. RESULTS: I

Congestion of epidural venous plexus secondary to vertebral artery occlusion mimicking a herniated cervical disc.

Vascular abnormalities or dissection of the vertebral artery are rare causes for cervical monoradiculopathy. We present the case of a 44-year-old female patient with a short history of radiculopathy of the right C5 root with radicular pain and a severe motor deficit. CT-imaging showed a hyperdense structure at the C4/C5 level suggesting a herniated disc. Because of the neurological deficit the patient was operated by a dorsal approach. Intraoperatively no herniated disc but only a congested epidural venous plexus was found. Postoperative MRI and angiography showed occlusion of the vertebral ar

Anastomoses (Superficial Cervical Ansa) Between the Cervical Plexus and Peripheral Facial Nerve Branches: Implications for Regional Anesthesia in Carotid Endarterectomies - Anatomical Study.

PURPOSE: Sensory innervation in the carotid triangle involves the cervical plexus, cranial nerves, and the sympathetic trunk. This innervation also applies to skin incision, including various anatomical structures with potentially different innervation, such as the skin (dermatomes), the platysma (myotomes), and the superficial layer of the cervical fascia (fasciotomes), as well as retromandibular retractor insertion (co-innervation: V, VII). The aim of this anatomical study was to develop an injection technique for carotid endarterectomies to additionally block anastomoses between the transve

The anatomy of the cutaneous branches of the cervical plexus and its clinical significance: supraclavicular nerves. A systematic review.

BACKGROUND: Supraclavicular nerves (SCNs) emerge from the cervical plexus and provide sensation to the skin over and below the clavicle. Detailed knowledge of their course and variation is especially useful in surgeries of the clavicular and neck regions. Anatomical descriptions of the SCNs remain inconsistent across studies. This systematic review aims to provide a comprehensive, evidence-based analysis of SCN anatomy, including its course, branching pattern, and surgical relevance. MATERIALS AND METHODS: A systematic search was conducted across major electronic databases (PubMed, Scopus, Em

Pain Relief With Peripheral Nerve Stimulation of Superficial Cervical Plexus: A Case Report and Literature Review.

BACKGROUND: Neuropathic head and facial pain are relatively common and difficult to treat pain syndromes encountered in pain clinics throughout the world. Atypical syndromes with overlapping nerve distributions can be particularly frustrating for patients and providers. In recent years, as providers search for alternatives to medications, advances in neuromodulation technology, including spinal cord stimulation and peripheral nerve stimulation (PNS), have shown promise in treating neuropathic pain confined to specific nerve distributions, such as the greater and lesser occipital nerves. Howeve

Superficial cervical plexus block for management of herpes zoster neuralgia in the C3 dermatome: a case report.

INTRODUCTION: Herpes zoster is a well-known reactivating viral disease that gives rise to painful skin lesions. Although this vesicular rash heals up within a few weeks, pain sometimes continues, becoming postherpetic neuralgia. In the case of those at high risk of developing postherpetic neuralgia, early interventional pain management is generally recommended as a preventive measure. Pain specialists usually do not see patients face-to-face for chronic refractory pain until the stage of postherpetic neuralgia. However, active and aggressive management, including antiviral treatment, of herpet
