Cervical rib synostosis with the first rib and an elongated C7 transverse process: a rare variation causing thoracic outlet syndrome.

Morphological variants in the cervical and thoracic regions, although often asymptomatic, can have significant clinical implications when they contribute to conditions such as thoracic outlet syndrome (TOS). We report a rare case of a 39-year-old female patient presenting with vascular TOS. Detailed imaging via high-resolution computed tomography (CT) and magnetic resonance angiography (MRA) revealed a right-sided elongated transverse process (ETP) of the C7 (24.70 mm in length) articulating with a cervical rib (CR), which showed synostosis with the first rib. The coexistence of CR synostosis

[Accessory cervical rib as a cause of arterial thoracic outlet syndrome (clinical case)].

The article presents a clinical observation of successful treatment of a patient with post-thrombotic occlusion of the subclavian artery and critical ischemia of the left upper limb. According to computed tomography angiography, extravasal compression of segment 2 of the subclavian artery by an additional cervical rib was detected. Decompression of the subclavian artery with resection of the additional cervical rib, thrombectomy, and angiography were urgently performed via the left supraclavicular approach. Blood flow to the limb was restored, and no additional endovascular procedures were req

Minimally Invasive Resection of Symptomatic Cervical Rib for Treatment of Thoracic Outlet Syndrome.

BACKGROUND: Neurogenic thoracic outlet syndrome treatments have high morbidity and recurrence rates. We present for the first time to our knowledge a minimally invasive spine surgery technique for complete resection of a cervical rib via a costotransversectomy approach. CASE DESCRIPTION: A patient with an 8-year history of progressive thoracic outlet syndrome presented with right C8 pain, weakness, and atrophy of her right forearm and thenar eminence. After neurogenic thoracic outlet syndrome was confirmed via electromyography and imaging revealed bilateral cervical ribs (right more than left

Venous thoracic outlet syndrome in the presence of cervical rib: report of two cases.

Thoracic outlet syndrome is a rare condition caused by the compression of neurovascular structures in the region between the neck and chest. The venous form accounts for 2% to 3% of cases, with deep vein thrombosis most often caused by compression of the subclavian-axillary vein segment between the first rib and the clavicle. The occurrence of a cervical rib as a cause of venous deep vein thrombosis is even rarer. In this report, we present the diagnosis, treatment, and outcome of 2 cases of venous deep vein thrombosis resulting from 2 different mechanisms. The approach was based on a review o

Neurogenic thoracic outlet syndrome secondary to an unilateral cervical rib in a poodle mix.

A 2-year-old spayed female poodle mix was evaluated for persistent intermittent right thoracic limb lameness and pain refractory to oral analgesic and anti-inflammatory medications. Orthopedic and neurologic physical examinations localized discomfort to the right shoulder and cervical region. Orthogonal right shoulder radiographs did not detect any abnormalities. Magnetic resonance imaging of the right brachial plexus revealed an anomalous bone structure at the level of the right thoracic inlet. Orthogonal cervical radiographs confirmed this finding. Surgical exploration of the right thoracic

Arterial thoracic outlet syndrome caused by cervical ribs-an unusual case report.

RATIONALE: Cervical ribs are rare conditions, occurring in 0.05% to 3.0% of the population. This manuscript reports a case of arterial thoracic outlet syndrome (ATOS) associated with this congenital anomaly. PATIENT CONCERNS: We report a 32-year-old female worker presenting pain in her left upper-extremity for 7 months. Her left hand became paler and cold when the temperature decreased, and the symptoms could not be eased through rest, physiotherapy and drugs medication. DIAGNOSES: Compression of left subclavian artery with axillary and brachial arteries thrombosis was confirmed by duplex ul

Thoracic Outlet Syndrome from Bilateral Cervical Ribs -A Clinical Case Report.

INTRODUCTION: Cervical rib is a mesenchymal or cartilaginous elongation of the transverse process of usually the seventh, rarely the sixth, and very rarely the fifth cervical vertebrae. It is an important cause of thoracic outlet syndrome as it has been reported in 5-% of patients with thoracic outlet syndrome. Bilateral cervical rib is a rare anomaly with a prevalence of 0.56% in Enugu, South Eastern Nigeria. This case is reported to emphasize the possibility of bilateral cervical rib as a cause of thoracic outlet syndrome in our environment. CASE REPORT: A 28-year-old female soldier was ref

Supraclavicular Resection of a Cervical Rib Causing Thoracic Outlet Syndrome: 2-Dimensional Operative Video.

Presence of a cervical rib results from overdevelopment of the seventh cervical vertebrae.1-3 The cervical rib along with scalene muscles can cause neurogenic thoracic outlet syndrome.4,5 Rib resection is typically done via anterior approach, using either supraclavicular or transaxillary route.6,7 We present an operative video detailing supraclavicular resection of a cervical rib causing neurogenic thoracic outlet syndrome with direct decompression of the lower trunk of the brachial plexus. The patient presented with severe symptoms including hand atrophy. We were able to directly visualize th

From compression to denervation: Multimodality imaging of cervical rib-related arterial thoracic outlet syndrome.

Thoracic outlet syndrome (TOS) encompasses a spectrum of disorders caused by compression of neurovascular structures at the thoracic outlet. Arterial thoracic outlet syndrome (aTOS) is rare, accounting for less than 1% of cases, yet carries significant morbidity due to arterial injury, thrombosis, and distal embolization. We present a 34-year-old woman with progressive pain, paresthesia, and bluish discoloration of the left upper limb. Doppler ultrasound demonstrated reduced ulnar artery calibre with a tardus-parvus waveform, indicating distal arterial compromise. CT angiography revealed a fus

Outcomes of Transaxillary Approach to Cervical and First-Rib Resection for Neurogenic Thoracic Outlet Syndrome.

BACKGROUND: Cervical rib can often be symptomatic causing neurogenic thoracic outlet syndrome (nTOS). Surgical treatment involves rib resection through a supraclavicular, transaxillary or combined approach. We review outcomes of different approaches and describe our technique of transaxillary resection through a video. METHODS: A single-center retrospective review of perioperative and short-term outcomes in subjects undergoing cervical rib resection for nTOS between 1994 and 2013 was performed. RESULTS: Of the 75 operations performed for nTOS, 40% (30 procedures in 29 patients) required rese

Fibrotendinous band causing neurogenic Thoracic Outlet Syndrome in adolescent with bilateral cervical ribs.

Thoracic Outlet Syndrome (TOS) describes a variety of symptoms caused by the compression of the neurovascular structures in the cervicoaxillary region as they leave the thorax toward the upper limbs. Causes of TOS are vascular and neurogenic, with neurogenic symptoms being the most common presentation (95% of patients). Symptoms related to neurogenic TOS include intrinsic hand muscle and thenar region atrophy and weakness, as well as medial hand and forearm numbness, which are all suggestive of lower trunk involvement. This condition is very rare in the pediatric population. We are reporting a

An original less-invasive surgical treatment for arterial thoracic outlet syndrome caused by cervical rib.

BACKGROUND: Thoracic outlet syndrome affects 0.3-8% of the population, with arterial thoracic outlet syndrome being particularly rare. The management of arterial thoracic outlet syndrome requires complete thoracic outlet decompression along with arterial reconstruction when necessary, a procedure that is typically associated with significant risks, complications, and symptom recurrence. Our study aims to introduce a less invasive surgical method for treating arterial thoracic outlet syndrome caused by a cervical rib, which involves only cervical rib resection and scalenotomy. METHODS: Patients
