Chylothorax is a relatively rare condition characterized by the accumulation of chyle, a milky lymphatic fluid, within the pleural space. It occurs because of disruption or obstruction of the thoracic duct or its tributaries, leading to chyle leakage into the pleural cavity. We present an interesting case of chylothorax that occurred as a complication post-chest tube insertion. A 66-year-old patient presented with hypotension and shortness of breath. Initial chest X-ray in the emergency room showed a right-sided hydropneumothorax requiring chest tube placement. Later on, the patient was transf

Background: After open or thoracoscopic lung biopsy, it is common to leave a chest tube as a postoperative drain that is typically removed on the first or second postoperative day. Standard technique is to apply an occlusive dressing at the site of chest tube removal using gauze and some form of tape. Methods: We reviewed the charts of children who underwent thoracoscopic lung biopsy at our institution for the past 9 years, many of whom left the operating room with a chest tube. When the tube was removed, the site was dressed, based on attending surgeon preference, with either cyanoacrylate ti

Orbital emphysema refers to the presence of air or gas within the orbital soft tissue space. Although orbital fracture due to trauma is the most common cause, iatrogenic orbital emphysema from chest tube insertion is a rare but potentially serious complication from the treatment of pneumothorax. Symptoms can range from mild crepitus of the periorbital region to permanent vision loss secondary to central retinal artery occlusion or optic neuropathy, a result of orbital compartment syndrome. Management is dependent on the presence of orbital signs and vision loss. Herein, we present a rare case

BACKGROUND: Over the last decades, several techniques have been implemented to reduce the invasiveness of thoracic surgery. Omitting chest tubes can lead to less postoperative pain and a shorter length of hospital stay. This study examines the extent to which German surgeons use the tubeless technique and what experience they have had with it.
MATERIALS AND METHODS: We conducted a nationwide survey, supported by the German Society of Thoracic Surgery (DGT). A digital questionnaire was sent to all leading thoracic surgeons with DGT membership between July and September 2020.
RESULTS: 63 of 16

BACKGROUND: Optimal chest tube position in the pleural cavity is largely unexplored for the treatment of primary spontaneous pneumothorax (PSP). We investigated whether type, size and position of chest tubes influenced duration of treatment for PSP.
METHODS: A retrospective follow-up study of all patients admitted with PSP over a 5-year period. Traumatic, iatrogenic and secondary pneumothoraxes were excluded. Gender, age, smoking habits, type and size of chest tube used (pigtail catheter or surgical chest tube) were recorded from the patients' charts. All chest X-rays upon admittance and imme

STUDY OBJECTIVE: The aim of the study was to compare the effectiveness of drainage via a single-lumen (5F catheter) central venous catheter (CVC) to a conventional (14-20 F catheter) chest tube (CT) for the management of pneumothoraces, including primary spontaneous pneumothorax (PSP), secondary spontaneous pneumothorax (SSP), and traumatic and iatrogenic pneumothoraces.
PATIENTS: All consecutive patients admitted to the intermediate intensive care unit of a university hospital for pneumothorax were retrospectively screened over an 8-year period. Patients were preferentially treated using CT

A 87-year-old woman was hospitalised because of a third-degree atrioventricular block. After the insertion of a temporary pacemaker lead through the left subclavian vein, she developed an ipsilateral pneumothorax. Although there were clinical and echocardiographic signs of cardiac tamponade after chest tube placement for pneumothorax, a second echocardiogram performed after transportation for surgical drainage failed to demonstrate the presence of any pericardial fluid, while the patient showed an unexpected clinical improvement. A new X-ray showed a collection of left pleural fluid. Over the

OBJECTIVES: Patients with cirrhosis can acquire pulmonary conditions that may or may not be related to their illness. Although posing a greater risk for complications, chest tubes are sometimes placed as treatment for hepatic hydrothorax and other pulmonary conditions. The aim of this study was to analyze the outcomes of chest tube placement in cirrhotic patients.
METHODS: A retrospective analysis was performed of 59 adults with cirrhosis undergoing chest tube placement. Variables that were investigated included reason for chest tube placement, complications developing while having the tube i

Tube thoracostomy is an invasive procedure caring about 1% complication rate. Most confronted complications include diaphragm or lung laceration [1-3], damage to intrabdominal organ, intercostal artery bleeding and unilateral pulmonary edema. Here we present another rare complication of perforation of the right atrium which occurred in a patient with rheumatic heart disease (RHD) who had received mitral valve replacement (MVR) and tricuspid annuloplasty. Severe tricuspid regurgitation (TR) with huge right atrium was noted. Chest tube was inserted for pleural effusion drainage. The lesion was p

The two major acute thoracic complications of closed chest tube thoracostomy are pulmonary laceration and vascular compression. We have noted that closed chest tube thoracostomy can also perforate an esophageal anastomosis or myotomy site. Clinically, such a perforation produces a profuse discharge of gas and/or fluid through a chest tube positioned at the level of the anastomosis or myotomy site. Plain films demonstrate an accumulation of extrapleural gas and/or fluid adjacent to the distal portion of the chest tube. If untreated, these accumulations may form into an extrapleural abscess.

PURPOSE: Chest tube placement and subsequent removal is a routine step in patient management after cardiovascular surgery. The purpose of this retrospective study is to determine the necessity of routine chest radiography following chest tube removal in order to detect potential complications in pediatric patients after cardiovascular surgery.
MATERIALS AND METHODS: We retrospectively reviewed the hospital records of all consecutive children up to 5 years of age who had cardiovascular surgery at our hospital between January 2015 and December 2020. Two radiologists independently evaluated rout

BACKGROUND: The need for chest X-rays (CXR) following large-bore chest tube removal has been questioned; however, the utility of CXRs following removal of small-bore pigtail chest tubes is unknown. We hypothesized that CXRs obtained following removal of pigtail chest tubes would not change management.
METHODS: Patients < 18 years old with pigtail chest tubes placed 2014-2019 at a tertiary children's hospital were reviewed. Exclusion criteria were age < 1 month, death or transfer with a chest tube in place, or pigtail chest tube replacement by large-bore chest tube. The primary outcome was che

Rib fixation has become a strategy for patients with displaced rib fractures and hemo/pneumothoraces (HTX/PTX). Rib plating improves pain control and respiratory mechanics, thereby reducing recovery times and morbidity/mortality. Current treatment consists of chest tube placement, pain control, and pulmonary toilet. The addition of rib plating should reduce time till HTX/PTX resolution and chest tube removal. The study compares chest tube stay time in rib-plated patients with those managed with current treatment. We hypothesize that patients undergoing rib plating will have a reduction in ches

OBJECTIVES: We hypothesized that published performances of algorithms for artificial intelligence (AI) pneumothorax (PTX) detection in chest radiographs (CXRs) do not sufficiently consider the influence of PTX size and confounding effects caused by thoracic tubes (TTs). Therefore, we established a radiologically annotated benchmarking cohort (n = 6446) allowing for a detailed subgroup analysis.
MATERIALS AND METHODS: We retrospectively identified 6434 supine CXRs, among them 1652 PTX-positive cases and 4782 PTX-negative cases. Supine CXRs were radiologically annotated for PTX size, PTX locati

Thoracostomy tubes (chest tubes) are the standard of care for thoracic trauma, including hemothorax. However, traditional chest tubes are susceptible to occlusion and lack the ability to be repositioned once inserted. This can lead to further life-threatening issues, such as retained hemothorax. Here we describe the investigation of a novel steerable chest tube, the SAM repositionable chest tube (RCT), which is designed with two-axis manipulation for effective drainage of the pleural space while maintaining sterility. We hypothesized that the treatment of hemothorax with RCT would result in an

This case report describes an uncommon occurrence of cardiac injury and perforation resulting from chest tube placement. The patient, a 44-year-old female, presented to the ED with severe blunt force trauma following a pedestrian motor vehicle collision. Imaging revealed a large pneumothorax and multiple fractures. Pulsatile bleeding was observed during chest tube placement, and the tube's tip was later found on CT of the thorax to be located within the heart. Although the patient recovered by postoperative day 13 and was subsequently discharged, this case highlights the importance of performi

The placement of chest tubes in Bülau position is an established method for treating pneumothorax resulting from thoracic trauma. While complications involving nerve structures are rare, they can be clinically relevant. This case report describes a complication not previously mentioned in the literature: recurrent laryngeal nerve palsy caused by the position of the tube tip near the recurrent laryngeal nerve at the upper lung apex.A 47-year-old patient with right-sided traumatic pneumothorax and rib fractures developed progressive hoarseness following initial chest tube placement. Clinical exa
