Psychiatric outcomes in young children with a history of institutionalization.

Children raised in institutions, considered an extreme example of social deprivation, are one group through which we can better understand the impact of neglect on child health and development. The Bucharest Early Intervention Project (BEIP) is the first randomized, controlled trial of foster care as an intervention for institutionalized children. In this review we describe the mental health outcomes from the BEIP. Specifically, we report findings on attachment styles, attachment disorders, emotional reactivity, and psychiatric symptomatology for children in the BEIP. We describe the impact of

Attachment disturbances in young children. I: The continuum of caretaking casualty.

OBJECTIVE: To determine whether signs of disordered attachment were greater in young children being reared in more socially depriving caregiving environments. METHOD: Three groups of children were studied by means of structured interviews with caregivers that were administered over several months in Bucharest, Romania, in 1999: (1) 32 toddlers living in a typical unit (standard care) in a large institution in Bucharest; (2) 29 toddlers living in the same institution on a 'pilot unit" designed to reduce the number of adults caring for each child; and (3) 33 toddlers living at home who had neve

Cryptosporidiosis among orphanage children in Thailand: a one year prospective study.

The prevalence of cryptosporidiosis and other parasitic infections was studied among the orphanage children in Nonthaburi province, Thailand, during May 1986--April 1987. Cryptosporidium oocysts were detected in fecal specimens of 22 out of 303 diarrheal cases (7.3%) by using a modified cold Kinyoun acid-fast technique. None were found among 513 children without diarrhea. Most of the cryptosporidiosis cases were those under 3 years old. The duration of diarrhea in 20 cases varied from 1 to 25 days (mean = 6.6 days). All cases recovered uneventfully without repeated infection. No sexual prepond

Vitamin D Supplementation at a Dose of 10 µg/Day in Institutionalized Children with Severe Motor and Intellectual Disabilities.

Vitamin D (VD) deficiency can lead to health-related consequences. This study determined the effects of VD administration in VD-deficient children with severe motor and intellectual disabilities (SMID). Twenty-eight subjects were included. Among them, 25 subjects with parental consent for VD administration were given 10 µg/day (400 IU/day) of VD in April 2021. Serum 25-hydroxyvitamin D [25(OH)D] levels were measured at least 30 days after the start of VD administration. The total VD intake, serum 25(OH)D levels, and ultraviolet (UV) exposure before the blood tests were investigated. The result

Heightened sensitivity to the caregiving environment during adolescence: implications for recovery following early-life adversity.

BACKGROUND: Adolescence has been proposed to be a period of heightened sensitivity to environmental influence. If true, adolescence may present a window of opportunity for recovery for children exposed to early-life adversity. Recent evidence supports adolescent recalibration of stress response systems following early-life adversity. However, it is unknown whether similar recovery occurs in other domains of functioning in adolescence. METHODS: We use data from the Bucharest Early Intervention Project - a randomized controlled trial of foster care for children raised in psychosocially deprivin

Parent-child separation and cardiometabolic outcomes and risk factors in adulthood: A systematic review.

BACKGROUND: Parent-child separation has been associated with negative mental health across childhood and adulthood, yet little is known about the long-term impacts for cardiovascular health. This systematic review synthesized and evaluated the quality of the literature examining the association between exposures to parent-child separation and cardiometabolic outcomes in adulthood. METHODS: Following a registered protocol, online databases (Pubmed, PsycInfo, and Web of Science) were searched for relevant studies. Studies were included if they (a) defined the exposure before age 18 as instituti

Catch-up growth, metabolic, and cardiovascular risk in post-institutionalized Romanian adolescents.

BACKGROUND: Reduced prenatal growth followed by rapid postnatal weight gain are risk factors for developing metabolic and cardiovascular disease. Children reared in institutions experience a similar pattern of growth restriction followed by catch-up growth after removal. We explored whether patterns of catch-up growth affect metabolic and cardiovascular outcomes in previously institutionalized adolescents. METHOD: A longitudinal study of institutionalized infants randomized to care as usual (n = 68) or foster care intervention (n = 68), and never institutionalized controls (n = 127). Body mas

Determinants of Child Malnutrition and Infant and Young Child Feeding Approaches in Cambodia.

Women's diets often decrease with regard to amounts per meal and day as well as diversity if a household's access to food is limited. The result is a monotonous diet that, in particular, negatively affects women's nutritional status during pregnancy and lactation and, thus, the infant. The infant's diet is of utmost importance, as it needs to meet the nutrient requirements especially during the first 2 years of life, a critical window for the child's healthy development. In Cambodia, infant and young child feeding (IYCF) practices are poor. Preparation of a special complementary meal in additi

Institutional care: associations between overactivity and lack of selectivity in social relationships.

BACKGROUND: The behaviour of children raised in institutional care in their early years is typified by heightened levels of inattention and overactivity irrespective of the quality of the care. There is some evidence that this behaviour may be specifically associated with forms of attachment disorder behaviours, but to date studies have been restricted to institutions characterised by high levels of malnutrition and lack of active experiences. METHODS: Nineteen primary school age children admitted to good quality residential group care before the age of 1 year were compared with 19 children o

Introducing new tools for assessment of parent- and child-reported outcomes in paediatric rheumatology practice: a work in progress.

BACKGROUND: To study causal associations between factors occurring during pregnancy, birth, and infancy and global quality of life (QOL) in adulthood 31-33 years later. MATERIAL/METHODS: Prospective study from the Copenhagen Perinatal Birth Cohort 1959-61. Two sets of questionnaires, one filled out by physicians during pregnancy, birth, and infancy and a validated, self-administered questionnaire on global quality of life (QOL) filled out by the person 31-33 years later (7,222 people). RESULTS: Only a few of the factors examined showed association with later QOL. Regarding the mothers, assoc

Role of health communications in Russia's diphtheria immunization program.

As part of a broader program in health communication assistance, project staff from Basic Support for Institutionalizing Child Survival worked with staff from Russia's oblast (regional) public health agencies to design and implement communication activities supporting local diphtheria immunization efforts. Because aggressive community outreach efforts and strong administrative sanctions had already achieved impressive adult coverage rates for first doses of diphtheria toxoid vaccine, communication interventions emphasized the need for second and third doses. Outcomes were assessed through vacc

The devolution of eugenic practices: Sexual and reproductive health and oppression of people with intellectual disability.

Early 20th century eugenicists propagated a system of ideas, values and dispositions that constituted adults with intellectual disability as the antithesis of the paradigmatic citizen, and a biological threat to society. The eugenic schema was encoded in sex-segregated institutionalization and, in many places, forced sterilization. These eugenic practices are no longer sanctioned. However, eugenic practices did not disappear. In this paper we argue that the eugenic schema is now encoded and purveyed through a multiplicity of social arrangements and practices that deny adults with intellectual
