Evidence-based treatment of attention deficit/hyperactivity disorder in a preschool-age child: a case study.

This case study illustrates a behavioral treatment of "Peter," a 4-year-old male with attention deficit/hyperactivity disorder (ADHD) and oppositional defiant disorder. Multiple evidence-based treatment procedures were implemented, affording the opportunity to explore issues common to the clinical application of empirically supported interventions. Among the strategies utilized were behavioral parent training, school consultation and behavioral training of educators, school-based contingency management, and a behavioral daily report card. Numerous issues are discussed, including the limited ev

Prediction of the severity of child abuse using nationwide survey data from Child Guidance Centers in Japan: focus on infancy and preschool age.

INTRODUCTION: The present study conducted a secondary data analysis of a comprehensive survey from Child Guidance Centers in Japan to identify factors that are associated with child abuse severity in infancy (0-3 years, 1,868 cases) and preschool age (4-6 years, 1,529 cases). A predictive model for abuse severity was developed. METHODS: The data originated from a nationwide survey that was conducted in April 2013, consisting of details of abuse cases, including child characteristics, abuser attributes, and family situation. Abuse severity was assessed on a five-level scale (suspected, mild, m

Observed teacher-child interaction quality as a predictor of motor competence in preschoolers: A sex-differentiated analysis.

BACKGROUND: Motor competence in early childhood enables children to engage in physical activity which further benefits academic and social outcomes. While the quality of teacher-child interactions is well established as a determinant of cognitive and social development, its specific association with motor competence remains underexplored. OBJECTIVE: This study examined the association between observed teacher-child interaction quality assessed via Classroom Assessment Scoring System (CLASS) and motor competence in 5-6-year-old children, with particular attention to sex-specific patterns in th

Eating in the Absence of Hunger in Hispanic Preschoolers: Relationships with Child Sex, Food Preference, and Weight Status.

Background/Objectives: This study examines the relationship of eating in the absence of hunger (EAH) with child sex, food preference, and body mass index (BMI) percentiles in primarily Hispanic preschoolers, an understudied population. Methods: This was a secondary analysis of data from 211 children (79% Hispanic) aged 3 to 5 years from low-income families who completed a cluster randomized controlled trial from September 2017 to June 2020. Weight and height were used to calculate BMI percentiles. Sweet (animal crackers) and salty (pretzels) snacks were used to conduct a validated classroom-ba

Testing reciprocal associations between child anxiety and parenting across early interventions for inhibited preschoolers.

BACKGROUND: Given the robust evidence base for the efficacy of evidence-based treatments targeting youth anxiety, researchers have advanced beyond efficacy outcome analysis to identify mechanisms of change and treatment directionality. Grounded in developmental transactional models, interventions for young children at risk for anxiety by virtue of behaviorally inhibited temperament often target parenting and child factors implicated in the early emergence and maintenance of anxiety. In particular, overcontrolling parenting moderates risk for anxiety among highly inhibited children, just as chi

The Differential Contribution of the Components of Parent-Child Interaction Therapy Emotion Development for Treatment of Preschool Depression.

OBJECTIVE: An adaptation of Parent-Child Interaction Therapy (PCIT) with a novel Emotion Development (ED) module has shown efficacy for the treatment of early childhood depression. Children who received PCIT-ED also showed healthy alterations in neural response to reward. We investigated whether the novel ED module made a unique contribution to the treatment of depression and neural response to reward, and whether child-directed intervention (CDI) and parent-directed intervention (PDI) modules (standard elements of PCIT) were also effective. METHOD: Dyads who participated in a randomized cont

Stability and Predictive Validity of the Parent-Child Sleep Interactions Scale: A Longitudinal Study Among Preschoolers.

Little research has examined the processes underlying children's persistent sleep problems and links with later psychopathology. The current study examined the stability of parent-child sleep interactions as assessed with the parent-reported Parent-Child Sleep Interactions Scale (PSIS) and examined whether sleep interactions in preschool-age children predict sleep problems and psychiatric symptoms later in childhood. Participants included 108 preschool-age children (50% female) and their parents. Parents completed the PSIS when children were 3-5 years (T1) and again when they were 6-9 years (T

Maternal stress and family quality of life in response to raising a child with autism: from preschool to adolescence.

While the impact of raising a child with an Autism Spectrum Disorder (ASD) is well documented, with mothers reporting higher levels of stress than mothers of children with other disabilities, positive maternal outcomes have also been identified. What remains unclear, however, is the role of child age on maternal outcomes. We sought to clarify the role of child age in maternal stress and family quality of life (FQoL) in mothers raising a child with ASD. Participants included 140 mothers of children aged 3-16 years grouped to represent four key stages of childhood (preschool, early school years,

A foreign body aspiration in a preschool child mimicking a multitrigger wheezing: a case report and review of the literature.

We report a child with a history of recurrent episodes of wheezing. At 3 months of age, the frequency of these episodes began to increase. Wheezing was associated with persistent cough and dyspnea with nocturnal awakening. The skin prick test was positive for pollen and pet dander. Airway endoscopy revealed the presence of a peanut, which obstructed the left main bronchus. Chronic cough and recurrent wheezing with symptoms between acute exacerbations could be signs not only of asthma but also of other disorders. Therefore, coexisting conditions should be evaluated whenever symptoms seem to be

Subgingival foreign body embedment in a preschool child: management with three and a half years follow-up.

CASE REPORT: This is a case report of diagnosis and management of a foreign body embedded in the gingivae of a preschool child. A 3 year-old girl presented with pain, mobility and intraoral oedema of the upper left primary canine (#63). An angular, diffuse radiolucency on the mesial aspect of the primary canine was observed in the periapical radiograph and within this radiolucency, a linear radiopacity was noted in contact with the mesial surface of the root. A clinical diagnosis of severe localized periodontal involvement due to foreign body embedment was made. It was initially attempted unsu

Mindful parenting, parent-child relationship and preschoolers' internalizing behaviors: A cross-lagged panel analysis.

The present study examined the bidirectional relationships among mindful parenting, parent-child relationships, and child internalizing behaviors (e.g., anxiety, depression, and social withdrawal) using a longitudinal design. A total of 1,201 Chinese preschoolers (50.3 % boys; age: M = 3.90 years, SD = 0.78) were assessed through parent reports at two time points, one year apart. Cross-lagged panel analysis revealed that mindful parenting at T1 positively predicted parent-child relationships (β = 0.17, p < 0.001) and negatively children's internalizing behaviors (β = -0.07, p < 0.05) at T2. Prenatal Exposure to Organophosphorus Pesticides and Preschool ADHD in the Norwegian Mother, Father and Child Cohort Study.

Prenatal organophosphorus pesticide (OPP) exposure has been associated with child attention-deficit/hyperactivity disorder (ADHD) in agricultural communities and those that are exposed to residentially applied insecticides. To examine this association in populations that are exposed primarily through diet, we estimate the associations between prenatal OPP exposure and preschool ADHD in the Norwegian Mother, Father and Child Cohort Study (MoBa), and describe modification by paraoxonase 1 (PON1) gene variants. We used participants from the MoBa Preschool ADHD Sub-study (n = 259 cases) and a rand

Should a Preschool Child with Acute Episodic Wheeze be Treated with Oral Corticosteroids? A Pro/Con Debate.

Traditionally, preschool-aged children with an acute wheezing episode have been treated with oral corticosteroids (OCSs) based on the efficacy of OCSs in older children and adolescents. However, this practice has been recently challenged based on the results of recent studies. The argument supporting the use of OCSs underscores the observation that many children with recurrent preschool wheezing develop atopic disease in early life which predicts both an increased risk to develop asthma in later life and response to OCS therapy. Further, review of the literature demonstrates heterogeneity of s

Child care problems and employment among families with preschool-aged children with autism in the United States.

BACKGROUND: The impact of childhood autism on parental employment is largely unknown. OBJECTIVE: The purpose of this work was to describe the child care arrangements of children with autism and to determine whether families of preschool-aged children with autism are more likely to report that child care arrangements affected employment compared with typically developing children and children at high risk for developmental problems. METHODS. Parents of 16282 preschool-aged children were surveyed by the National Survey of Children's Health. An autism spectrum disorder was defined as an affirmat

Clinical prescribing of allergic rhinitis medication in the preschool and young school-age child: what are the options?

Allergic rhinitis (AR) is the most common chronic condition in children and is estimated to affect up to 40% of all children. It is usually diagnosed by the age of 6 years. The major impact in children is due to co-morbidity of sinusitis, otitis media with effusion, and bronchial asthma. AR also has profound effects on school absenteeism, performance and quality of life. Pharmacotherapy for AR should be based on the severity and duration of signs and symptoms. For mild, intermittent symptoms lasting a few hours to a few days, an oral second-generation antihistamine should be used on an as-need

Examining the Biological Impacts of Parent-Child Relationship Dynamics on Preschool-Aged Children who have Experienced Adversity.

Parent-child relationship dynamics have been shown to predict socioemotional and behavioral outcomes for children, but little is known about how they may affect biological development. The aim of this study was to test if observational assessments of parent-child relationship dynamics (cohesion, enmeshment, and disengagement) were associated with three biological indices of early life adversity and downstream health risk: (1) methylation of the glucocorticoid receptor gene (NR3C1), (2) telomere attrition, and (3) mitochondrial biogenesis, indexed by mitochondrial DNA copy number (mtDNAcn), all
