Delirium in a Child in Pediatric Intensive Care Unit.

Delirium is a syndrome with an acute onset that is accompanied by fluctuation and is associated with behaviors that indicate impaired consciousness and cognition. It is common and costly and is associated with severe functional decline and distress in an adult. However, its detection and diagnosis are so challenging in children. Herein, we report a 2-year-old girl who was admitted in the pediatric intensive care unit (PICU) with pneumonia and was intubated because of respiratory failure. She needed a lot of benzodiazepine and opioid drugs to be sedated. During hospital course after extubation,

Orthopaedic care of the child with arthrogryposis: a 2020 overview.

PURPOSE OF REVIEW: The orthopaedic treatment of children with arthrogryposis multiplex congenita has evolved steadily over the past two decades. Interrelated factors have spurred this on, including better appreciation of the functional potential of persons with arthrogryposis, development of newer procedures specific for the arthrogrypotic deformities, and outcomes studies that provide understanding of the overall capabilities of adults with arthrogryposis and follow-up to determine which treatments were beneficial and which were not. This article briefly sketches out of some of these advances

Anesthetic Care of a Child With Acute Flaccid Myelitis.

Acute flaccid myelitis (AFM) is a neurological disorder that affects the anterior horn of the spinal cord, resulting in progressive weakness, loss of motor function, hypotonia and hyporeflexia. Given the progressive and frequently long-term neurological involvement of AFM, patients may require anesthetic care during radiological imaging or other surgical procedures, such as a tracheostomy or the placement of a gastrostomy tube. We present a 4-year-old girl with AFM who required anesthetic care during a tracheostomy. The end-organ involvement of the disease is discussed, pathogenesis and treatm

Caring for the mother-child dyad as a family physician.

Infant social, emotional and neurological development is shaped by the mother-child dyad. Dysfunction in this bond, as well as maternal mental health problems, can negatively impact child development. The family physician is well-placed to spot dysfunction in the mother-child dyad and screen for postnatal depression during well-child visits. If any issues are identified, the family physician can provide support and help the mother-child dyad to access community resources and specialist psychiatric services.

The family impact of caring for a child with juvenile dermatomyositis.

Background Juvenile dermatomyositis (JDM), a rare autoimmune disease, accounts for more than 80% of idiopathic inflammatory myopathy childhood cases, making it the most common idiopathic inflammatory myopathy among children. The average age of onset is approximately 7 years and commonly leads a chronic course. Symptoms of JDM include cutaneous features (Gottron's rash, heliotrope rash, or nail fold capillary changes), musculoskeletal features, calcinosis and lipodystrophy (a symmetrical deficit of subcutaneous fatty tissue), and acanthosis (thickening of the skin). Despite improvement in treat

Perioperative care of a child with transposition of the great arteries.

Because a minority of patients with D-transposition of the great arteries are diagnosed in utero by ultrasound, most present after delivery with cyanosis. In the absence of apparent lung disease, cyanotic neonates suspected of having a cardiac lesion should be immediately transferred to an intensive care unit at a pediatric tertiary care center for monitoring, resuscitation, and to define the cardiac anatomy and physiology. A prostaglandin E-1 infusion is usually initiated to maintain ductal patency and promote intra-cardiac mixing. In the past, balloon atrial septostomy (BAS) was routinely pe

An outbreak of tinea capitis in a child care centre.

We describe an outbreak of tinea capitis in a kindergarten. Individuals with relation to the institution were examined clinically and mycologically (n-98), Microsporum audouinii was cultured from 12 people, out of whom eight patients had tinea capitis, four patients had tinea corporis, and three patients had both. The problems with spread of anthropophilic infections and the difficulties with treatment of Microsporum dermatophytes are discussed.

Effect on families and caregivers of caring for a child with laryngomalacia.

We used a retrospective chart review and telephone interviews using a standard format to 1) measure the outcome and psychosocial costs of caring for a child with upper airway obstruction due to laryngomalacia and 2) develop an epidemiologic instrument that measures the impact on the lives of families who have a child with laryngomalacia. Forty-four patients had laryngomalacia documented by endoscopy between 1993 and 1997. Complete follow-up information through 1998 was available on 26 patients. Of the 44 patients, 37 were managed expectantly and 7 (16%) required surgical intervention. Polysomn

The epidemiology of injuries in 4 child care centers.

OBJECTIVES: (1) To describe the pattern of injury in preschool-aged children in 4 child care centers as compared with the results of other studies; (2) to compare injury rates by sex, age, and child care center; and (3) to examine environmental and child factors contributing to injury severity. DESIGN: A 2-year cohort study of 362 preschool-aged children attending 4 urban child care centers. Teachers completed standardized injury forms on the type of injury, body location, site of injury, and contributing factors. RESULTS: During the 2 years of the study, 1886 injuries were reported. The role of the physician in the care of the child with Rett syndrome.

In caring for a child with Rett syndrome, the physician as an advocate for the child can provide, access and facilitate medical, therapeutic, psychosocial and community services. A coordinated effort in the management of children with multiple medical and psychosocial problems is best provided by an interdisciplinary team.

Care of the critically ill child: the bleeding neonate.

Serious bleeding episodes in newborn infants can usually be diagnosed following careful clinical assessment and a few simple laboratory tests. Certain conditions are found almost exclusively in "sick" infants, whereas other coagulation abnormalities occur in otherwise "Работает" neonates. Successful management of hemorrhage necessitates a correct diagnosis which thereby dictates appropirate therapy. In some cases, such as in DIC, successful outcome ultimately depends on correction of the underlying pathophysiology which triggered the coagulation disturbance.

Complications and total care of a child with acute leukemia.

The complications which occur in a child with acute leukemia depend on the stage of the disease and the therapeutic regiman. Most children will present with some manifestation of marrow failure. An occasional child will have marked leukocytosis and disturbance of organ function due to massive leukemic infiltrates. Metabolic disturbances such as hyperuricemia and hyperphosphatemia-hypocalcemia may develop, expecially after therapy is initiated. The myelosuppression and immunosuppression due to drug toxicity may result in opportunistic infections. Other toxicities which can occur with a chemothe

Challenges in care of the child with special health care needs in a resource limited environment.

To identify challenges encountered in the care of children with special health care needs in a resource limited environment a 10 year-old child with a diagnosis of Tuberous Sclerosis was studied. Challenges identified were in: making a definitive diagnosis, provision of adequate care, cost of care, meeting parental expectations and accessing community support for the child and family. Available specialist health care and related services, including community rehabilitation, were provided for the child and family. The study highlights the need for improved community awareness, development in th

Editorial: Pragmatic Precision Child Psychiatry: An Opportunity to Advance Quality of Child Mental Health Care.

The quality of publicly funded child mental health care is chronically poor. In this edition of the Journal, the largest study to date examining antidepressant medication adherence among US Medicaid-enrolled youth both validates prior concerns and prompts a critical examination of how we assess the quality of child mental health care. The study by Fontanella et al. is a retrospective longitudinal cohort study of 196,364 youth aged 9 to 24 years with a new episode of major depression who were prescribed antidepressant medication.1 The analysis used Medicaid claims data from 46 states and the Di
