DSM-5 symptom expression in toddlers.

Children with autism show more social-communication symptoms and repetitive behaviors than children with typical development or those diagnosed with other developmental disorders; however, non-autistic children often show some behaviors that are associated with autism. We compared the behavioral reports from caregivers of children in these three groups to identify the behaviors that were specific to autism. Children with autism were found to show more of these behaviors, and behaviors that are particularly indicative of autism were identified. These behaviors included social symptoms (approach

The roles of comorbidity and trauma exposure and its timing in shaping HPA axis patterns in depression.

BACKGROUND: Much work has documented hypothalamic pituitary adrenal (HPA) axis abnormalities in major depressive disorder (MDD), but inconsistencies leave this system's role in the illness unclear. Comparisons across studies are complicated by variation in co-morbidity (Posttraumatic Stress Disorder-PTSD, anxiety disorders), exposure to trauma, and timing of trauma (child vs. adult). Here, we examined the impact of these factors on HPA axis profiles in depression. METHODS: We recruited 5 groups of participants: MDD (n = 14), comorbid MDD + PTSD following adulthood trauma (MDD + PTSD-Adult; n

Oppositional defiant disorder symptoms in relation to psychopathic traits and aggression among psychiatrically hospitalized children: ADHD symptoms as a potential moderator.

Oppositional defiant disorder (ODD) is associated with elevated rates of psychopathic traits and aggression. However, it remains unclear if attention-deficit/hyperactivity disorder (ADHD) symptoms exacerbate these relations, particularly in samples of children who are severely clinically distressed. The purpose of the present study was to test ADHD symptoms as a potential moderator of the relations of ODD symptoms to psychopathic traits (i.e., callous-unemotional [CU] traits, narcissism) and to aggressive subtypes (i.e., proactive, reactive aggression) in a large sample of children in an acute

A randomized controlled trial of an online mindfulness program for adolescents at risk for internalizing problems.

OBJECTIVE: The purpose of this randomized controlled trial was to test the effects of an online, coached mindfulness intervention on momentary negative affect (mNA) for youth with high levels of trait negative affectivity. METHOD: Participants were 111 youth ages 12 to 17 years old (M = 14.17, SD = 1.60). Youth self-identified as 68% female, 29% male, and 4.5% gender diverse; 54.55% identified as White; 31.82 reported being Hispanic/Latinx. Participants were selected for having high levels of trait negative affect and were randomized to receive either the mindfulness program or no interventio

Psychological reactions of disaster victims.

A general review of psychological reactions to a disaster situation in four groups of people is presented. Adult reactions (and management thereof) vary according to the phase of the disaster, viz (i) the pre-impact phase characterized by underactivity, development of anxiety symptoms or involvement in adaptive activities; (ii) the warning phase typically manifested by overactivity; (iii) the impact phase characterized by bewilderment, confusion or hysteria; (iv) the turmoil-recoil phase manifesting in emotional expression relating to the immediate past; and (v) the emotionally wounded or post

Post-Traumatic Stress Disorder Symptoms among Pediatric Healthcare Workers.

Post-traumatic stress disorder symptoms related to work in pediatric departments aremajor public health problems, as they directly affect health organizations, healthcare workers, and, due to the poor quality of care, the patients as well. The post-traumatic symptoms that a healthcare worker may experience are related to intrusion, avoidance, negative changes in cognition and mood, and changes in arousal and reactivity. The aim of the present investigation was to identify risk factors that contribute to the development of PTSD in pediatric healthcare workers, in order to implement necessary wo

Peripheral immune system activity in young psychiatry patients.

The interaction between the immune system and the central nervous system has been implicated in the development of psychiatric disorders in adult patients. However, comprehensive data regarding pediatric psychiatry are still lacking. This study aims to describe the distributions of various markers of immune system activity in a large clinical sample of children and adolescents and summarize the immunological profiles associated with different psychiatric disorders. We analyzed blood samples from 1543 patients aged 6-18 years (62 % female), admitted to the Child and Adolescent Psychiatry at the

Latent class analysis reveals five homogeneous behavioural and developmental profiles in a large Dutch population sample of infants aged 14-15 months.

Precursors of child psychiatric disorders are often present in infancy, but little is known about the prevalence and course of general psychopathology in population-based samples of children 0-3 years. We examined whether homogeneous behavioural and developmental profiles could be identified in children aged 14-15 months (M = 14.84; SD = 2.19), and we explored whether or not these profiles corresponded with existing classifications of DSM-IV-TR, ICD-10, and DC 0-3R. Parents of 6,330 children answered 74 items about externalizing, internalizing, and social-communicative behaviour. Exploratory f

Health Outcomes of Children Born to Liver Transplant Recipients.

Leveraging the Transplant Pregnancy Registry International, we conducted a retrospective study of children born to liver transplant (LT) recipients between 1986-2023 to evaluate long term health outcomes of offspring. Child data were collected primarily from bi-yearly maternal phone interviews. Descriptive statistics and multivariate analyses were used to evaluate risk factors for adverse child outcomes. There were 599 children with follow-up data, born to 435 LT recipients of whom 73% were white and 13.1% Hispanic with a median age at conception of 29.4 yrs. The most common LT indications wer

In utero exposure to endogenous maternal polyclonal anti-Caspr2 antibody leads to behavioral abnormalities resembling autism spectrum disorder in male mice.

The concept that exposure in utero to maternal anti-brain antibodies contributes to the development of autism spectrum disorders (ASD) has been entertained for over a decade. We determined that antibodies targeting Caspr2 are present at high frequency in mothers with brain-reactive serology and a child with ASD, and further demonstrated that exposure in utero to a monoclonal anti-Caspr2 antibody, derived from a mother of an ASD child, led to an-ASD like phenotype in male offspring. Now we propose a new model to study the effects of in utero exposure to anti-Caspr2 antibody. Dams immunized with

Possible conservation-withdrawal reaction in two infants.

Two breast-fed infants with weight loss are described. Despite their mothers' assertions that their breast-feeding was adequate, the infants gained weight rapidly on complementary feeding and became more responsive. Previously they had rather passively accepted suboptimal intakes, crying little and sleeping excessively. Discussion centers on whether these infants illustrated the conservation-withdrawal state, since apart from their poor weight gains, they were otherwise well.

Pathogens of the Human Central Nervous System-the Major Protozoan Parasites and Helminths.

Numerous pathogen infections can initiate potentially fatal neurological inflammation, including encephalitis, meningitis, meningoencephalitis, and myelitis. Several protozoan parasites and helminths can initiate neurological disorders. Pathogens can achieve this either by their original active infections, or by reactivated infections after a period of latency. The pathogens can be transmitted between humans or to humans by respiratory droplets, ingestion, or physical contact. Or the infections can be transmitted to humans by nonhuman vectors, by means of bites from mosquitoes, ticks, flies, o

Emotional reactivity in referred youth with disruptive behavior disorders: the role of the callous-unemotional traits.

Deficits in emotional reactivity are frequently reported in Disruptive Behavior Disorders (DBDs). A deficit in prosocial emotions, namely the callous unemotional traits (CU), may be a mediator of emotional reactivity. Our aim is to investigate subjective emotional reactivity towards visual stimuli with different affective valence in youths with DBDs and healthy controls. The clinical sample included 62 youths with DBDs (51 males, 8 to 16 years, mean 11.3±2.1 years), the control group 53 subjects (36 males, 8 to 16 years, mean 10.8±1.5 years). The groups were compared using the Child Behavior C

Reappraisal is an effective emotion regulation strategy in children with Tourette syndrome and ADHD.

BACKGROUND AND OBJECTIVES: Difficulties in emotion regulation (ER) have been associated with several psychiatric disorders, emphasizing a need for a greater understanding of the concept and its associations with disruptive behavior. We aimed to study the ER strategy of cognitive reappraisal with an experimental test to increase our knowledge of emotional processes in child psychopathology. METHODS: In the present study, we examined emotional reactivity and cognitive reappraisal with a computer task in 160 medication-naïve children aged 8-12 comprising four groups: Fifty-eight children with To

Reactive attachment disorder in maltreated toddlers.

OBJECTIVE: To determine if Reactive Attachment Disorder (RAD) can be reliably identified in maltreated toddlers in foster care, if the two types of RAD are independent, and to estimate the prevalence of RAD in these maltreated toddlers. METHODS: Clinicians treating 94 maltreated toddlers in foster care were interviewed regarding signs of attachment disorder at intake in an intervention program. RESULTS: Using categorical and continuous measures, both types of RAD can be reliably identified in maltreated toddlers. Both continuous scores and categorical diagnoses indicated that a substantial m

A modified video-feedback intervention for carers of foster children aged 6 years and under with reactive attachment disorder: a feasibility study and pilot RCT.

BACKGROUND: Looked-after children are at risk of suboptimal attachment patterns and reactive attachment disorder. However, access to interventions varies widely and there are no evidence-based interventions for this disorder. OBJECTIVES: (1) To adapt an existing video-feedback intervention to meet the specific needs of foster children in the UK with reactive attachment symptoms, (2) to conduct a case series to road-test the treatment manual and study procedures, (3) to conduct a scoping study of the key hurdles in a pilot trial and (4) to conduct a pilot randomised controlled trial of the ada

[Pathological protective and defensive reactions in early childhood].

A group of pathological defenses has been observed in infants between three and eighteen months of age who have experienced danger and deprivation to an extreme degree. The early defenses, "avoidance", "freezing", and "fighting", are apparently summoned from a biological repertoire on the model of "flight or fight". Before there ist an ego, pain ca bei transformed into pleasure of obliterated from consciousness while a symptom stands in place of the original conflict.

Response to Allen (2018): Points of agreement and disagreement on reactive attachment disorder.

Reactive attachment disorder (RAD) is a very rare, understudied, and controversial disorder. Research in Developmental Disabilities (RIDD) recently published our research study, "Reactive attachment/disinhibited social engagement disorders: Callous-unemotional traits and comorbidity" (Mayes, Waschbusch, Calhoun, Breaux, & Baweja, 2017) investigating comorbidity in children with RAD and demonstrating a high prevalence of conduct disorder and callous-unemotional traits, consistent with previous research. Allen (2018) responded with a paper published in RIDD criticizing our study and offering his
