BACKGROUND: In 2010, Brazil introduced child restraint legislation (CRL). We assessed the effectiveness of CRL in reducing child (aged 0-8 years) injuries and fatalities by race. We performed an evaluation study with an interrupted time-series design.
METHODS: We measured the effect of CRL on two outcomes-number of child deaths and number of child injured in traffic collisions per child population, stratified by race, from 2008 to 2014. We controlled for time, unemployment rate and oil consumption (barrels/day in thousands).
RESULTS: The CRL was associated with a 3% reduction in the rate of

Preterm and low birth weight (LBW) infants are at risk for cardiorespiratory instability in the semi-upright car seat position. Other "at-risk" infants include those with critical congenital heart disease (CCHD), though there are no formal recommendations for screening in this population. The objectives of this study were (1) to assess car seat tolerance screening (CSTS) practices in patients with CCHD at our institution and (2) to understand the epidemiology of this population, including incidence and risk factors for CSTS failure. We performed a retrospective medical record review of neonate

Discharging neonates in a proper car safety seat is standard of care in the United States and many other countries. However, not every neonate can be safely positioned in a standard semi-upright car seat. In these cases, providers may opt for a travel device that allows the infant to lie flat, either supine or prone, known as a car bed. Minimal evidence exists to guide providers on car bed safety and help determine which infants would benefit from discharge in a car bed. In this article, we provide a comprehensive summary of existing literature on the safety of car beds for motor vehicle trave

OBJECTIVE: The purpose of this work was to compare the incidence of apnea, hypopnea, bradycardia, or oxygen desaturation in healthy term newborns placed in hospital cribs, infant car safety beds, or infant car safety seats.
METHODS: A consecutive series of 200 newborns was recruited on the second day of life. Each subject was studied while placed in the hospital crib (30 minutes), car bed (60 minutes), and car seat (60 minutes). Physiologic data, including oxygen saturation, frequency, and type of apnea, hypopnea, and bradycardia were obtained and analyzed in a blinded manner.
RESULTS: The m

BACKGROUND: The Eastern Mediterranean Region suffers disproportionately from paediatric traffic-related injuries. Despite governmental laws, Lebanon-an eastern Mediterranean country-has low child restraint (CR) use prevalence. This study examined the impact of using car seat distribution, and child passenger safety education and awareness intervention to improve child passenger safety knowledge and practices among caregivers.
METHODS: This study recruited Lebanese caregivers with one child or more, using a 4-wheel motor vehicle, and not using a car seat. The intervention comprised an educatio

OBJECTIVE: Motor vehicle crash (MVC)-related spinal injuries result in significant morbidity and mortality in children. The objective was to identify MVC-related injury causation scenarios for spinal injuries in restrained children.
METHODS: This was a case series of occupants in MVCs from the Crash Injury Research and Engineering Network (CIREN) data set. Occupants aged 0-17 years old with at least one Abbreviated Injury Scale (AIS) 2+ severity spinal injury in vehicles model year 1990+ that did not experience a rollover were included. BACKGROUND: Premature and other at-risk infants can experience cardiorespiratory problems when positioned in their car seats. The American Academy of Pediatrics recommends that all premature and at-risk infants undergo a period of observation in their car seat to monitor for apnea, bradycardia, and oxygen desaturation before hospital discharge. This Infant Car Seat Challenge (ICSC) is used to determine readiness for infant travel in a car seat. Infants failing the ICSC are discharged home in car beds and referred for a follow-up screen in the outpatient clinic.

OBJECTIVE: One of the main causes of death worldwide among young people are car crashes, and most of these fatalities occur to children who are seated in the front passenger seat and who, at the time of an accident, receive a direct impact from the airbags, which is lethal for children under 13 years of age. The present study seeks to raise awareness of this risk by interior monitoring with a child face detection system that serves to alert the driver that the child should not be sitting in the front passenger seat.
METHODS: The system incorporates processing of data collected, elements of de

BACKGROUND: Cervical nerve root avulsion after trauma is a well-known occurrence. It is associated with traction injuries to the brachial plexus, commonly after high-speed motor vehicle collisions. Traumatic nerve root avulsion occurs when traction forces pull the nerve root sleeve into the intervertebral foramen with associated tearing of the meninges. The proximal nerve root retracts, and the neural foramen fills with cerebrospinal fluid and eventually forms a pseudomeningocele. Although imaging characteristics often include nerve root edema and pseudomeningoceles, there has only been one de

Despite significant reductions in the number of children killed in motor vehicle crashes over the past decade, crashes continue to be the leading cause of death for children 4 years and older. Therefore, the American Academy of Pediatrics continues to recommend inclusion of child passenger safety anticipatory guidance at every health-supervision visit. This technical report provides a summary of the evidence in support of 5 recommendations for best practices to optimize safety in passenger vehicles for children from birth through adolescence that all pediatricians should know and promote in th

Pediatric trauma plays a crucial role in pediatric mortality, with traffic injuries and falls frequently cited as leading causes of significant injuries among children. A comprehensive investigation, including geographical factors, is essential for developing effective strategies to prevent injuries and alleviate the burden of pediatric trauma. This study involved a retrospective analysis of clinical data from pediatric patients admitted to our hospital's intensive care unit (ICU) due to trauma over a 10-year period. Comprehensive analyses were conducted to elucidate trends, demographics, inju

The dynamic impact tests at Southwest Research Institute for the first time exposed human volunteers to production-like driver air cushion system depolyments at impact levels equivalent to a 30 mph barrier crash (48 kph). No significant injury was produced. At no time was it necessary for the secondary restraint systems to be utilized and the entire program schedule was carried out as planned. All tests were conducted under the most safe and controlled conditions possible, and typify only what the air cushions might accomplish in direct, head-on impacts up to 30 mph (48 kph). BACKGROUND: Motor vehicle crashes are a significant source of pediatric mortality and morbidity. Studies indicate that booster seats significantly improve seat belt fit for children who have not attained a height of 145 cm (4' 9"). This study examined injuries occurring in booster age children up to age 12, as the majority of children do not attain 145 cm until this age. The purpose of the study was to identify differences in injuries due to the type of restraint used, with attention to musculoskeletal injuries.
METHODS: Vehicle and occupant data were obtained from a publically available stat

This case study describes a fatal head injury in a 1-year-old child involved in a motor vehicle accident in Japan. The child, secured in a rear-facing child seat, was a passenger in a car driven by their mother when the offset car-to-car collision occurred. The car rotated counterclockwise before coming to a stop. Despite remaining secured in the child seat, the child suffered severe head trauma, leading to cardiac arrest. Autopsy computed tomography revealed a right open depressed fracture, left head contusion, traumatic subarachnoid hemorrhage, intraventricular hemorrhage, and pneumocephalus

BACKGROUND: The AAP recommends that infants born <37 weeks gestation or with risk factors for positional cardiorespiratory instability undergo a car seat tolerance screen (CSTS) before discharge. Understanding characteristics of CSTS failure may inform screening practices.
OBJECTIVE: Compare clinical, oxygen saturation, and heart rate features in infants who failed vs. passed CSTS.
METHODS: We retrospectively reviewed CSTS data from a single level IV NICU (2018-2024). Clinical characteristics of infants who failed vs. passed their first CSTS were compared. For a subset with SpO₂ and HR data,

OBJECTIVES: To describe temporal trends in car seat tolerance screening (CSTS) failure within a large hospital system (2014-2018).
METHODS: We conducted a retrospective cohort study using electronic medical record data for infants who underwent a CSTS. Our primary outcome measure was the CSTS failure rate. Covariates included year, CSTS location (well nursery or NICU), gestational age (GA), race, sex, birth weight, CSTS date, and age at CSTS. Associations of covariates with CSTS failure were examined by using χ2 tests, t tests, analysis of variance, and Wilcoxon rank tests. Multivariable logi
