[Evaluation of a new cholecystokinetic agent].

Boyden's test meal (egg yolk beaten with cream), used during cholecystography, remains in the stomach for a long time and could interfere with the radiological findings on barium meal examination performed after cholecystography. A new preparation based on corn oil emulsion (G.P. Prep) was evaluated in 33 patients with and without symptoms of gallbladder disease. The criteria used in the evaluation of results were three: reduction in the gallbladder dimensions, variations in the cholecystovertebral angle, and visualization of the extrahepatic bile ducts. The gallbladder dimensions were determi

Delayed gallbladder rupture following percutaneous cholecystostomy.

Percutaneous cholecystostomy has become an accepted therapeutic alternative for high-risk patients with acute cholecystitis. However, some authors have cautioned that patients with gallbladder wall necrosis and gangrene may not be effectively treated by means of percutaneous drainage alone. A case is reported in which gallbladder wall necrosis progressed following technically successful percutaneous drainage. Spontaneous gallbladder rupture ensued, necessitating emergent cholecystectomy. Cholecystography 2 weeks following tube placement and 1 week prior to rupture showed a markedly abnormal, i

Gallstones, cholecystectomy, and duodenogastric reflux of bile acid.

It has earlier been suggested that cholecystectomy, by eliminating the reservoir function of the gallbladder, will induce reflux of bile to the stomach. In the present study 23 patients were studied for duodenogastric reflux of bile acid before and 3 months after cholecystectomy. At the test the gastric contents were continuously aspirated via a nasogastric tube, collected at 15-min intervals for 2 h in the fasting patient, and analyzed for volume and bile acid concentration. The results were compared with those in 14 control subjects. Significant duodenogastric reflux of bile acid (greater th

Acute cholecystitis: the diagnostic role for current imaging tests.

Acute cholecystitis is a relatively common clinical entity characterized histopathologically by obstruction of the cystic duct due to either edema or stone or both. Thorough clinical assessment and selection of the appropriate diagnostic tests are crucial in making an early diagnosis before surgical treatment. Many diagnostic tests are available for imaging the gallbladder. Hepatobiliary imaging using technetium Tc 99m IDA is the test of choice to either exclude or confirm the diagnosis of acute cholecystitis and it carries a discriminating power greater than that of cholecystography or ultras

Endoscopic Transpapillary Gallbladder Drainage for Recurrent Cholecystitis after Covered Self-expandable Metal Stent Placement for Unresectable Malignant Biliary Obstruction.

A 76-year-old woman with advanced pancreatic cancer developed recurrent cholecystitis after covered self-expandable metal stent (CSEMS) placement. The cholecystitis was refractory to repeated percutaneous transhepatic gallbladder drainage (PTGBD). Cholecystography showed a patent cystic duct with right and cranial side bifurcation, which is indicative of an increased likelihood of success of endoscopic transpapillary gallbladder drainage (ETGBD). We were able to manage the cholecystitis by ETGBD without further recurrence. ETGBD is considered an effective internal drainage method for the manag

How to image the gallbladder in suspected cholecystitis.

As a result of important advances in medical imaging, the oral cholecystogram is no longer the primary test of gallbladder function and anatomy. Real-time ultrasonography and cholescintigraphy, both highly sensitive and specific tests, are the two major methods for assessing gallbladder pathology. Oral cholecystography, endoscopic retrograde pancreatography, and percutaneous gallbladder puncture serve as supplementary tests. Decisions about which test to use depend on the kind of gallbladder disease that is suspected as well as the estimated likelihood of the disease before the information is

[Sonographic studies of gallbladder kinetics in childhood--a comparison between healthy and mucoviscidosis patients].

In mucoviscidosis, the gallbladder can show signs of disease even in childhood. In 10 of 35 mucoviscidosis patients the authors found thickening of the walls, gallstones and micro-gallbladders on sonographic examination. Compared with healthy children (N = 70), the mucoviscidosis patients showed impairment of the kinetics of the gallbladder to an increasing extent in step with progressing age. X-ray films of the gallbladder were preferably taken while using a preparation containing bile acids for better absorption of the contrast medium. When using this preparation, the gallbladder was found t

Ultrasonic examination of the gallbladder: a review.

Ultrasonography is a helpful, non-invasive, and relatively inexpensive adjuvant to oral cholecystography for evaluating the gallbladder and its diseases. This method can be highly accurate in detecting gallstones and is specially useful when there is non-visualisation in radiographic contrast studies. Various ultrasound patterns of gallstones are described. All these patterns should demonstrate a sonic shadow in association with an intraluminal defect. Sonography is particularly useful in demonstrating cholelithiasis and organ dilatation in suspected acute cholecystitis. In addition to gallbla

Comparison of real-time and static-mode gray-scale ultrasonography in the diagnosis of cholelithiasis.

Real-time and conventional articulated-arm gray-scale static-mode scanning were performed on 100 patients in a prospective study to determine the complementary or duplicative roles of these two ultrasound modalities in the confirmation of clinically suspected cholelithiasis. Twenty-five abnormal findings were documented ultrasonographically, including 21 cases of cholelithiasis (12 confirmed by oral cholecystography and/or surgery), two cases of biliary sludge, one case of a gallbladder polyp, and one case of a probable blood clot. There was no instance of gallbladder lumen nonvisualization. E

[The Dubin-Johnson syndrome: case report and review of literature].

The Dubin-Johnson syndrome is a hereditary deficiency in the excretion ofconjugated bilirrubin by hepatocytes characterized by chronic hyperbilirubinemia, alteration in coproporphyrin metabolism, and intracellular deposition of a dark melanin-like pigment giving the liver a typical black cast. We report a 28-year-old male patient who presented conjunctival jaundice and conjugated-hyperbilirubinemia without no other alteration in hepatic biochemistry. The diagnosis of this syndrome was perfomed by using the low-risk methods of laparoscopy-facilitated hepatic biopsy and oral cholecystography In

Sonographic diagnosis of bicameral gallbladders: a report of three cases.

Three cases of bicameral gallbladder are reported; the fundal chamber could be visualized by oral cholecystography in only one of these cases. However, all structures were well visualized sonographically. Ultrasonography is thus regarded as essential for the diagnosis of bicameral gallbladders and for detecting any calculi within them.

Large solitary gallbladder calculus resembling multipolypoid cholesterolosis: case report.

A condition diagnosed as multiple cholesterolosis was discovered at surgery to be caused by a very unusual large solitary gallstone. This case demonstrates the potential hazard of omitting the plain KUB film and provides another possible etiology for the appearance of multiple fixed intraluminal defects on oral cholecystography.

[Contribution to the diagnosis of Dubin-Johnson syndrome].

Eight patients with Dubin-Johnson's syndrome have been reported. It appoints the greatest casuistry about the theme in our country. It doesn't agree with the class descriptions in which these exams have negative results. So w

Chronic acalculous gallbladder disease: multiimaging evaluation with clinical-pathologic correlation.

Despite the recent advances in hepatobiliary imaging, the diagnosis of chronic acalculous gallbladder disease remains difficult. A retrospective study was undertaken to assess the value of a multiimaging approach in detecting chronic acalculous gallbladder disease and in predicting which patients would obtain symptomatic relief after cholecystectomy. Of 199 patients with chronic cholecystitis, 26 (13%) had no gallstones. Of these 26, only 17 (65%) had symptoms related to chronic cholecystitis; in the remainder, the histologic diagnosis was made incidentally. After cholecystectomy, 13 (76%) of

[When cholangiocholecystography should and should not be performed].

Little-know contraindications for i.v. cholangiocholecystography with water-soluble hexaiodated contrast media are presented. Particular reference is made to suspected cholecystic lithiasis and dysprotidaemic patients or those undergoing treatment with sulphamides, salicylic acid, barbiturates, etc. Lastly, reference is made to the suitability of cholegraphy in suspected acute cholecystitis. Here, in fact, this method is preferable to oral cholecystography.

[Ultrasonics in the diagnosis of hyperplastic cholecystoses of focal and segmental extension type].

The authors describe the sonographic (US) patterns of hyperplastic cholecystoses observed from January 1983 to April 1987 - 39 cases of focal extension and 12 of segmental extension--and confirm the higher sensibility of US versus oral cholecystography. Sonography (SG) allows a straight visualization of the gallbladder wall and its lesions, both in case of limited thickening of the wall (focal cholecystosis) and in case of more extensive thickening (segmental cholecystosis). To cholecystographic findings of focal lesions, SG gives additional information, adding a fundamental diagnostic element

Rotor's syndrome: report of one case.

Rotor's syndrome is a rare hereditary non-hemolytic hyperbilirubinemia. This report concerns an 11-year-old boy who presented with fluctuant jaundice since birth. Liver function tests showed conjugated hyperbilirubinemia with normal serum aminotransferase and globulin level. The diagnosis was confirmed by laboratory workup which included normal liver histology and oral cholecystography and typical sulfobromophthalein excretion curve.
