A rare case of placentae site trophoblastic tumor (PSTT) studied by immunohistochemistry and nuclear DNA analysis is reported. The patient, a 24-year-old Japanese female, complained of amenorrhea. Dilatation and curettage revealed a small specimen that contained trophoblastic cells and caused intractable bleeding. Pelvic sonography revealed a 5-cm mass in the posterior uterine wall with multiple cystic lesions of several sizes. This gestational neoplasm arises from the trophoblast, the ectodermal layer of a fertilized ovum. Although benign, hydatidiform moles have the potential to progress to malignant trophoblastic disease. Molar pregnancy must be recognized promptly. The clinical presentation is usually vaginal bleeding, which may contain characteristic tissues. Diagnosis is confirmed by identification of hydropic avascular villi obtained by evacuation of uterine contents. The beta subunit of human chorionic gonadotropin is a valuable tumor marker.

Presented are two cases of recurrence of gestational trophoblastic neoplasia more than 1 year after apparently successful therapy. Both patients initially had had nonmetastatic disease, were clinically free of disease, and had had repetitively negative serum radioimmunoassay titers for the beta subunit of human chorionic gonadotropin for 2 to 3 years. The development of these late recurrences re-emphasizes the need for prolonged follow-up monitoring of patients after apparently successful therapy for gestational trophoblastic neoplasia.

Choriocarcinoma is a highly aggressive beta-subunit of human chorionic gonadotropin (betaHCG)-producing germ cell tumor. In men, it is a rare neoplasm and can arise in the testes or in various extragonadal locations such as the retroperitoneum, mediastinum, and pineal body. We present a highly unusual case of a middle-aged man with primary metastatic betaHCG-producing choriocarcinoma of the right thigh along with right lower-extremity venous thrombosis. We comment on prognostic variables as well as discuss several theories to account for the unusual location of his choriocarcinoma.

Epidermoid carcinoma of the cervix cell lines DoT and CaSki were examined for the production of human chorionic gonadotropin (HCG) and its subunits (HCG-alpha and HCG-beta). We evaluated the medical history, physical examination, and laboratory tests done on 245 patients with laparoscopically proven ectopic pregnancies. The absence of abdominal pain was the only clinically useful negative predictive value (91%) regarding tubal rupture. We measured human chorionic gonadotropin (hCG) and its subunits in 76 patients with islet-cell tumors. Seventeen of 27 patients with functioning islet-cell carcinomas had elevated plasma levels of hCG or one of its subunits (hCG-alpha and hCG-beta).
