The cough reflex is modulated throughout growth and development. Cough-but not expiration reflex-appears to be absent at birth, but increases with maturation. Thus, acute cough is the most frequent respiratory symptom during the first few years of life. Later on, the pubertal development seems to play a significant role in changing of the cough threshold during childhood and adolescence resulting in sex-related differences in cough reflex sensitivity in adulthood. Asthma is the major cause of chronic cough in children. Prolonged acute cough is usually related to the long-lasting effects of a p

Chronic cough is a common symptom that can be a daunting challenge for clinicians since treatment of the underlying cause does not always provide adequate relief, an obvious cause can remain elusive, and current antitussives have fairly poor efficacy and undesirable side-effects. Patients with chronic cough typically describe a range of sensory symptoms suggestive of upper-airway and laryngeal neural dysfunction. Additionally, patients often report cough triggered by low-level physical and chemical stimuli, which is suggestive of cough-reflex hyperresponsiveness. Pathophysiological mechanisms

BACKGROUND: Chronic cough in children is a common problem, and sinusitis is a common etiology. The diagnosis of sinusitis is often clinical, but confirmation is thought to require a CT scan due to the difficulty of interpreting a Water's view sinus X-ray.
OBJECTIVES: The purposes of the study were (1) to examine the frequency of an abnormal sinus X-ray in children with a chronic cough of more than 4 weeks duration; (2) to compare the interpretation of the sinus film between allergy/pulmonary clinicians and radiologists; and (3) to correlate symptoms with X-ray results.
METHODS: A chart revie

Initial evaluation of the patient with chronic cough (i.e., of more than eight weeks' duration) should include a focused history and physical examination, and in most patients, chest radiography. Patients who are taking an angiotensin-converting enzyme inhibitor should switch to a medication from another drug class. The most common causes of chronic cough in adults are upper airway cough syndrome, asthma, and gastroesophageal reflux disease, alone or in combination. If upper airway cough syndrome is suspected, a trial of a decongestant and a first-generation antihistamine is warranted. The dia

Successful treatment of a voice problem requires an accurate diagnosis. The clinician often needs to piece the diagnosis together from previous treatments that failed, from an accurate and involved case history, and from examination of the larynx during speech and vegetative tasks. This case challenges the clinician's ability to question appropriately, to listen intently, and to observe critically before making the diagnosis. With the correct diagnosis, the patient found relief from her chronic cough.

Cough is one of the most common symptoms among children. While most acute cough episodes are viral and resolve within 4 weeks, chronic cough persisting beyond 4 weeks warrants further evaluation. Pediatric-specific cough guidelines emphasizing algorithmic evaluation assist in early and accurate diagnosis for preventing long-term pulmonary complications. This article reviews evidence-based approaches to evaluating and managing acute and chronic cough in children, highlighting key diagnostic strategies and targeted treatment options.

In recent years, there has been a substantial increase in the development of antitussive therapies and the first new therapy, gefapixant has been licenced in Europe. This review describes current unlicenced treatments for chronic cough and details treatments currently in development for refractory chronic cough and cough in idiopathic pulmonary fibrosis, as well as compounds previously explored.

Cough lasting more than 8 weeks is considered chronic. If the classic causes of chronic cough have been discarded, vagus nerve sensory disturbances are currently considered the most important etiological cause. Patients with chronic cough of laryngeal origin have associated symptoms such as globus, dysphagia, dysphonia, dyspnoea and/or stridor. These patients are more likely to have paradoxical vocal fold movement. There is a higher cough reflex sensibility and neuropathic laryngeal response, mainly caused by viral infection or reflux. The cough associated with reflux has 2 mechanisms: Exposur

A previously healthy woman in her forties with a six-month history of persistent coughing, breathlessness and fatigue was referred to our hospital for further evaluation. She was initially treated with antibiotics for a possible respiratory tract infection but with only minor effect. A chest x-ray and computer tomography (CT) of the thorax demonstrated a solid tumour in the right lung hilus. Bronchoscopy revealed slight oedema of the bronchial mucous membrane in the area in question. Cytological examination of bronchoalveolar lavage fluid (BAL) showed normal respiratory epithelial cells. Chronic cough is a common presentation in paediatrics. We describe a case which highlights the need for careful history taking and summarize the key clinical features which should prompt a clinician to perform a chest X-ray.

Cough is an essential protective mechanism for the airways and lungs. Cough receptors are situated in the larynx and tracheobronchial tree, and are mediated by rapidly-adapting (irritant) Adelta fibers, although other receptors such as C-fiber receptors may contribute. Cough plasticity and interactions of cough pathways may occur centrally to enhance the cough reflex. The presence of an increased cough reflex as measured by a tussive response to capsaicin or citric acid in patients with a chronic cough indicate that there is sensitisation of the cough reflex. The most common cause of acute cou

Management of chronic cough is complex and warrants careful evaluation. Some patients cough for years without help because of the indiscriminate use of antitussive medications. All patients should be evaluated in a specific manner. The cause of cough can be found and effectively treated in almost all patients. Antitussive therapy should only be used as adjuvant therapy or when a cause cannot be found and the patient is not harmed by decreasing the cough. When used, the safest, least expensive antitussive drug should be instituted for a limited time pending re-evaluation of the patient's clinic

PURPOSE: Only limited studies have depicted the unique features and management of refractory chronic cough (RCC) and unexplained chronic cough (UCC). These led to the initiation of this study, which reported the demographic characteristics, manifestations, and long-term outcomes on a large series of consecutive RCC/UCC patients, providing a guideline-led real-world clinical experience.
METHODS: Retrospective baseline information was obtained from Clinical Research Database (January 2016 to May 2021). At least 6 months after the last clinic visit, included subjects were prospectively followed

OBJECTIVE: To evaluate clinical and imaging character of chronic rhinosinusitis in the children with chronic cough and to discuss the treatment of them.
METHOD: Fifty-three children with chronic cough were examined by otolaryngologists and evaluated by computed tomography (CT) scan. Some of them were also evaluated by nasal endoscope. There was not any finding of lung in chest X-ray in Adhese children and children with immunodeficiency were ruled out from the study. The children with chronic sinusitis were treated according to general protocols for 6 to 8 weeks, among them those with adenoid

Chronic cough is defined as a cough lasting no less than 8 weeks, and 10%-42% of patients with chronic cough retain their cough after comprehensive examination and treatment. The terms idiopathic chronic cough (ICC), unexplained chronic cough (UCC), and refractory chronic cough (RCC) are frequently used to describe this clinical condition. However, these terms are often used interchangeably in articles, leading to heterogeneity in the populations included in the studies and compromising the value of the findings in guiding clinical practice. This article aims to clarify the current terminologi

Continuous cough monitoring using a wearable device analysed by machine learning for automated cough detection has revealed considerable day-to-day variability in cough counts. Single-day recording is thus subject to error. https://bit.ly/3yYCRbX.

PURPOSE: Chronic refractory cough (CRC), defined as cough lasting over 8 weeks despite medical intervention, is a prevalent condition with a number of associated comorbidities. Cough suppression therapy (CST) has been demonstrated to be a promising avenue for treating CRC by improving airway control and coordination. However, little is known about the effects of CST in CRC patients diagnosed with comorbid oropharyngeal dysphagia (DYS) despite a large subset of patients with both conditions. The purpose of this study was to determine if CST affects self-assessment of DYS severity in patients di
