Coronary arterial obstruction associated with congenital aortic valve disease is rare in children. We studied two children with aortic valve disease and symptoms of coronary insufficiency. Cineangiography revealed localized obstruction of the proximal left coronary artery. In one patient, a membrane-like structure partially covered the left sinus of Valsalva, almost isolating the ostium of the left coronary artery; the other patient had severe stenosis of the proximal left coronary artery and a congenitally hypoplastic left coronary cusp. Surgical repair was successful in both cases. Symptoms

In the months following balloon atrial septostomy for complete transposition of the great arteries (TGA), clinical deterioration is often ascribed to closure of the septostomy. Of 16 neonates with TGA, 5 showed early clinical deterioration. Each had an adequate septostomy at operation. Deterioration was characterized clinically by increasing cyanosis and radiographically by progressive diminution in pulmonary vascularity. Subsequent catheterization and axial cineangiography demonstrated that pulmonary flow had decreased, probably secondary to a progressive form of left ventricular outflow trac

A 4-year-old boy admitted to our hospital due to congestive heart failure one year after the intracardiac repair of tetralogy of Fallot. Echocardiography and cineangiography revealed severe aortic regurgitation. The aortic valve replacement was performed with a prosthesis larger than the aortic annulus. Operative findings showed that the cause of aortic regurgitation was infective endocarditis. The aortic root was enlarged with pericardial patch and a 21 mm St. Jude Medical valve prosthesis was inserted in supraannular position. This technique was very useful for insertion of a prosthetic valv

Thirty-five patients with massive atrial thrombus complicating mitral stenosis and atrial fibrillation were treated surgically. The mean age in this series is lower than that reported in the West. Furthermore, the incidence of preoperative embolic manifestations has been less frequent in our experience. We believe it is desirable to document the presence of a thrombus by selective left atrial cineangiography. Surgical treatment was accomplished with an acceptable mortality. The vital role played by the use of low-molecular-weight dextran and early anticoagulation in decreasing the incidence of

Thirty-five patients with typical Prinzmetal's variant angina were studied by coronary cineangiography. There was no demonstrable stenosis of the major coronary arteries in 19 patients. Nine patients with single coronary stenosis underwent aortocoronary bypass and had recurrence of the symptoms postoperatively. Administration of nifedipine effected complete cessation of the symptoms among patients formerly treated medically. Although surgical treatment did not effect permanent relief of pain, all patients initially treated surgically experienced relief of pain when nifedipine was administered.

In 7 cases of Ebstein's anomaly the right cineangiogram disclosed a big right atrium, the tricuspid valve positioned to the left of the spine and tricuspid regurgitation. The right atrial appendage was nearing the outflow tract of the right ventricle and the pulmonary artery so as the normal U appearance of the right aniograms was abolished. In addition to these signs a very characteristics and almost pathognomonic rocking motion of the radiopaque material, moving from the right atrium towards the right ventricle during ventricular diastole and backwards to the atrium during the next ventricul

A case of atrial septal aneurysm associated with combined valvular disease and coronary-pulmonary fistula (C-PA fistula) was presented. The patient successfully underwent mitral valve replacement, excision and closure of the aneurysm, tricuspid annuloplasty and closure of C-PA fistula. Atrial septal aneurysm is a rare anomaly and its natural course is thought to be generally good without significant clinical symptoms. However, complications such as cerebral and pulmonary embolism, or occlusion of the atrioventricular vales by the prolapsed aneurysm were reported to occur in small number of cas

The pulmonary valve was examined following balloon dilation of pulmonary valve stenosis in six children. Stenosis was due to commissural fusion in two. In these patients, cineangiography showed that the valve was slightly thickened and "domed" during systole. Balloon valvoplasty produced complete separation of fused commissures in one and partial separation in the other. Commissural splitting is likely to be the most common mechanism of relief of pulmonary stenosis by balloon dilation. Incomplete separation could account for the partial reduction of the gradient in most children treated with t

Congenital absence of the pulmonary valve is an uncommon combination of anomalies. Recently, we encountered two such cases, one associated with tetralogy of Fallot (case 1), the other with ventricular septal defect (case 2). Clinically, our cases had minimum symptoms. The congenital cardiac anomalies were diagnosed with physical examination, chest x-ray film, echocardiography, and confirmed with cineangiography. These procedures disclosed a rudimentary pulmonary valve, pulmonary regurgitation and aneurysmal dilatation of the main and right pulmonary artery. In case 2, the right lower pulmonary

We report a case of a blood cyst originating from the pulmonary valve cusp of a 43-year-old Japanese woman with pulmonary stenosis. Cineangiography revealed a pedunculated tumor on the arterial surface of the pulmonary valve. It was successfully removed by a transpulmonary artery approach. The cyst contained old blood and calcified thrombi, and its wall consisted of collagenous fibrous tissue. Immunohistochemically, a monolayer of flat cells lining the cyst was confirmed as endothelium using antibodies to von Willebrand factor and CD34, as well as UEA-I lectin. To the best of our knowledge, bl

A 63-year-old man with a history of hypertension and coronary artery bypass grafting (1 year ago) was admitted with acute onset severe chest pain suggesting bypass dysfunction. Biplane cineangiography revealed acute aortic dissection Stanford Type A without involvement of the aortic valve, the coronary arteries or the proximal anastomoses of the two venous bypass grafts, one of which was occluded. Urgent repair of the aorta by a prosthesis and reinsertion of the patent venous graft in the innominate artery by interposition of saphenous vein was performed without complications. Sixteen months l

The posterior or inlet ventricular septal defect may occur as an isolated anomaly or in combination with other defects. Axial cineangiography is the best means to delineate the anatomy of this defect as well as to exclude the presence of other ventricular septal defects. The posterior ventricular septal defect can be recognized in the four chamber view by obliteration of the septum by contrast medium flowing across the defect. Anterosuperior extension into the perimembranous region is identified by obliteration of the septum in the long axial oblique view. The presence of a posterior ventricul

The Mobin-Uddin umbrella filter was placed into the inferior vena cava of mongrel dogs. Studies on hemodynamics and embolization were performed using blood flow and pressure determinations, cineangiography, and radiopaque emboli. Initially flow and pressure remained unaffected by the filter. At 70 minutes flow was only one fourth of the initial IVC flow. Immediately after positioning of the filter emboli up to 6 mm in diameter bypassed the filter, and smaller emboli passed through the perforations in the filter. Thrombus formation on the surface of the umbrella was already noted within one hou

Non-rheumatic mitral incompetence may be caused by a variety of conditions affecting the relationships between the papillary muscles, chordae tendineae and mitral valve cusps. Among the commonest of these conditions are ventricular dilatation, ischaemia, congenital and familial abnormalities, senile atrophy of the muscles and spontaneous rupture. There is a very wide range of severity of the resultant clinical syndromes. Some patients are asymptomatic, others are moderately or severely incapacitated, while the severest cases may have acute pulmonary oedema which may be rapidly fatal. The diagn

BACKGROUND: Effective transcatheter or surgical closure of apical muscular ventricular septal defects (VSDs) requires accurate delineation of variable and often complex anatomy. These defects have generally been considered as communications between the apexes of both left and right ventricles.
METHODS AND RESULTS: Among 50 consecutive patients with multiple muscular VSDs referred for transcatheter device closure between October 1987 and April 1993, a subset of 10 patients (aged 7 days to 28 years) with apical muscular VSDs shared a unique set of anatomic characteristics: (1) large and often s

Aortic dissection limited to one sinus of Valsalva has been described exclusively as an iatrogenic complication during catheterization interventions. This life-threatening subtype had a very small area of dissection, when coronary ostia are frequently involved. We report a 43-year-old man with dissection limited to left sinus of Valsalva, involving the left main coronary artery and causing non-ST-myocardial infarction, including severe reversible ST-depression, maximum of 9 mm in V5 lead. Dissection was suggested by cineangiography, transesophageal echocardiogram, and contrast-enhanced multide

Myocardial bridging is a common and usually benign inborn coronary anomaly. We report on a 51-year-old man who presented with recent angina on minimum physical effort. Cineangiography showed myocardial bridging of the mid-left anterior descending artery (LAD), and intracoronary ultrasonography excluded atherosclerotic disease. Gated single-photon emission computed tomography (SPECT), with exercise stress, showed an extensive anterior perfusion defect, and remarkable ST-segment elevation (up to 10 mm) in recovery. Vasospasm of the LAD was the main hypothesis. Additional oral drugs did not bring

A 59-year-old male was admitted because of severe chest pain and diagnosed as inferior myocardial infarction by ECG and enzymologically, who had a Levine grade II continuous murmur in the left second intercostal space at the left sternal border. Cardiac catheterization revealed a 8% left to right shunt at pulmonary artery. Selective coronary cineangiography revealed left anterior descending-to-main pulmonary artery and left circumflex-to-main pulmonary artery fistulas, and severe atherosclerotic stenosis at right coronary and left anterior descending arteries. Operation was performed simultane

Noninvasive graphic techniques are an invaluable tool in the diagnosis, management, and followup of patients with valvular heart disease. They are both sensitive and specific and therefore often eliminate the need for more invasive and more expensive diagnostic procedures. When cardiac catheterization and/or cineangiography are necessary to obtain definitive hemodynamic or anatomic information, prior noninvasive graphic evaluation frequently directs the cardiologist to the appropriate type of study to be performed. This is especially true of subtle tricuspid valvular disease, unsuspected silen
