Call for integrated smart alarm management strategies for electrocardiographic alarm administration in neonatal intensive care units: a nationwide cross-sectional survey.

INTRODUCTION: The high sensitivity and low specificity of electrocardiogram (ECG) alarms can not only lead to alarm fatigue or neglect, but also interfere with the daily treatment and care of healthcare providers and pose potential harm to newborns. The effect of current clinical ECG alarm management strategies and their impact on healthcare providers warrant further assessment. OBJECTIVES: To evaluate the current clinical ECG alarm management initiatives in NICUs and their impacts on healthcare providers' alarm fatigue levels, alarm perceptions and decision-making confidence. METHODS: A nat

False Asystole Alarms Post-Temporary Pacemaker Placement Due to Pseudo-fusion.

An infant with congenital heart block and hemodynamically significant bradycardia underwent therapeutic temporary pacing wires placement. Post-operatively, frequent "asystole" alarms were observed on telemetry causing distress to both the family and the nursing staff. Investigation of these alarms showed that pacemaker malfunction led to monitor pseudo-malfunction. The alarms were alleviated with mindful setting of the pacemaker and telemetry monitor parameters. This case highlights the challenges of pacemaker placement and monitoring of very small infants in the intensive care setting. Awaren

The influence of patient characteristics on the alarm rate in intensive care units: a retrospective cohort study.

Intensive care units (ICU) are often overflooded with alarms from monitoring devices which constitutes a hazard to both staff and patients. To date, the suggested solutions to excessive monitoring alarms have remained on a research level. We aimed to identify patient characteristics that affect the ICU alarm rate with the goal of proposing a straightforward solution that can easily be implemented in ICUs. Alarm logs from eight adult ICUs of a tertiary care university-hospital in Berlin, Germany were retrospectively collected between September 2019 and March 2021. Adult patients admitted to the

Learning three sets of alarms for the same medical functions: a perspective on the difficulty of learning alarms specified in an international standard.

Three sets of eight alarms supporting eight functions specified in an international medical equipment standard (IEC 60601-1-8) were tested for learnability using non-anaesthetist participants. One set consisted of the tonal alarms specified in the standard. A second set consisted of a set of abstract alarms randomly selected from a database of abstract alarm sounds held by the authors. A third set of alarms was designed as indirect metaphors of the functions. Participants were presented with the alarms and then asked to identify them across ten blocks of eight trials. The results indicated a s

Contactless and continuous monitoring of respiratory rate in a hospital ward: a clinical validation study.

INTRODUCTION: Continuous monitoring of respiratory rate in hospital wards can provide early detection of clinical deterioration, thereby reducing mortality, reducing transfers to intensive care units, and reducing the hospital length of stay. Despite the advantages of continuous monitoring, manually counting every 1-12 h remains the standard of care in most hospital wards. The objective of this study was to validate continuous respiratory rate measurements from a radar-based contactless patient monitor [Vitalthings Guardian M10 (Vitalthings AS, Norway)] in a hospital ward. METHODS: An observa

The Impact of Medical Technology Alarms on Critical Care Nurses: Emotional Responses, Occupational Fatigue, and Implications for Well-Being.

In this focus group study, we examined the challenges associated with sensory overload resulting from medical technology alarms and alerts experienced by intensive care unit nurses. The prevalence of excessive warnings has a detrimental impact on the well-being of critical care nurses. Participants reported a variety of emotional responses to the alarms, predominately negative feelings, such as anger, frustration, stress, and nervousness. Thus, it is imperative to implement strategic measures to address the issue of alarm fatigue and associated noise levels to foster a more conducive work envi

Implementation of a novel postoperative monitoring system using automated Modified Early Warning Scores (MEWS) incorporating end-tidal capnography.

Modified Early Warning Scores (MEWS) provide real-time vital sign (VS) trending and reduce ICU admissions in post-operative patients. These early warning calculations classically incorporate oxygen saturation, heart rate, respiratory rate, systolic blood pressure, and temperature but have not previously included end-tidal CO2 (EtCO2), more recently identified as an independent predictor of critical illness. These systems may be subject to failure when physiologic data is incorrectly measured, leading to false alarms and increased workload. This study investigates whether the implementation of

Development and Validation of a Prediction Model for Alarm-Triggered Interventions in Pediatrics.

BACKGROUND: Alarm fatigue in pediatric wards stems from frequent, nonactionable alarms that do not lead to clinical intervention. Predicting alarm-triggered clinical interventions is essential for optimizing monitoring strategies. OBJECTIVE: To identify factors associated with alarm-triggered interventions and to develop a predictive model to reduce the alarm burden through an action-oriented approach. METHODS: This prospective cohort study was conducted in 2 pediatric wards of a tertiary hospital (2018-2019). A person-day data set was created by linking alarm logs, alarm-triggered response

Home monitor follow-up of persistent apnea and bradycardia in preterm infants.

We reviewed our experience with home monitor observations of 83 preterm infants (postconceptional age, 36 to 44 weeks) who had persistent apnea, bradycardia, or cyanosis. Polygraphic recordings before discharge showed that 92% of these infants had cardiorespiratory abnormalities that included prolonged (greater than 20 s) apnea, excessive periodic breathing (greater than 15%), bradycardia (greater than 80 beats per minute), feeding hypoxemia, or elevated carbon dioxide values. At home, 70 infants had either no alarms or alarms that resolved spontaneously, while 13 (16%) had more serious episod

Reducing Night-Time Sound Levels Through Bed-Level Quiet ICU Bundle: A Randomised Counterbalanced Crossover Study in Adult Intensive Care.

BACKGROUND: Elevated night-time noise in intensive care units (ICUs) may reduce alarm salience, increase cognitive workload and contribute to alarm desensitisation during bedside surveillance. Most evaluations of Quiet ICU interventions have used uncontrolled or quasi-experimental designs, limiting causal inference. AIM: To evaluate whether a structured bed-level Quiet ICU bundle reduced nocturnal acoustic burden in an adult ICU while preserving alarm visibility for clinical surveillance. STUDY DESIGN: This prospective, randomised counterbalanced crossover study was conducted in an adult int

False Alarms in Wearable Cardioverter Defibrillators-A Relevant Issue or an Insignificant Observation.

Background: The wearable cardioverter defibrillator (WCD) has emerged as a valuable tool used for temporary protection from sudden cardiac death. However, since the WCD uses surface electrodes to detect arrhythmias, it is susceptible to inappropriate detection. Although shock conversion rates for the WCD are reported to be high for detected events, its efficacy in clinical practice tends to be degraded by patient noncompliance. Reasons for this include wearer discomfort and frequent false alarms, which may interrupt sleep and generate anxiety. Up to now, data on the incidence of false alarms e

A Scoping Review: The Impact of IV Smart Pump Air-in-Line Alarms in Oncology Care.

BACKGROUND: IV smart pumps (IVSPs) are essential for antineoplastic infusion delivery. However, air-in-line (AIL) alarms, a common IVSP alarm, may contribute to alarm fatigue, treatment delays, and workflow inefficiencies, affecting patient safety, wasted time, and nursing workload. OBJECTIVES: This scoping review examines the literature on IVSP alarms, with a focus on AIL alarms. The review sought to identify the frequency and causes of these alarms, their relationship with alarm fatigue, and their implications on patient safety, time burden, and nursing practice. METHODS: The primary autho

Electrocardiogram failure in the operating room - bench testing to prevent bed-side disaster.

Electrocardiogram (ECG) false alarms are common in electrically-hostile peri-operative environments. Newer integrated monitoring, with sophisticated hardware and software, has the potential to minimise artefacts. However, monitoring issues continue to occur, with the potential for critical incidents and unnecessary and harmful interventions. We describe the root cause analysis of a series of apparent ECG flatline asystolic events that appeared in the operating room shortly after the introduction of new intra-operative monitoring systems. Clinical events and biomedical laboratory testing reveal
