Endotracheal suctioning: an example of the problems of relevance and rigour in clinical research.

Endotracheal suctioning is a routine but potentially dangerous procedure. The literature documenting approaches to minimizing the cardiopulmonary complications of endotracheal suctioning is reviewed. Hyperoxygenation, hyperventilation, hyperinflation and the use of adaptors are all evaluated. The effects of endotracheal suctioning on haemodynamics and oxygen transport are also examined. The traditional dualist approach to the respiratory and cardiovascular systems is contrasted with the recent emphasis on oxygen transport by the cardiopulmonary system. The trade-off between the rigour of labor

Administering heparin subcutaneously: an evaluation of techniques used and bruising at the injection site.

This experimental study tested the effects of four different subcutaneous injection techniques on bruising at the injection site. The independent variables of aspiration of the syringe prior to injecting heparin and pressure on the site following injection, and the dependent variable of size of bruising were studied. Aspiration of the syringe had little effect on bruising. After the first injection, fewer subjects who received pressure following the injection had bruising, the difference being significant for females. However analysis of subsequent injections did not support the conclusion tha

LGBTQIA+ Competence: A Pedagogical Paradigm Shift in Graduate Nursing Education.

BACKGROUND: The omission of lesbian, gay, bisexual, transgender, queer, intersex, asexual (LGBTQIA+) content in graduate nursing education leaves people who identify as sexually or gender diverse (SGD) with poorer health across the life span and a 12-year shorter life expectancy relative to heteronormative counterparts. METHOD: An educational intervention was paired with an optional academic-community-based clinical immersion in LGBTQIA+ health with the goal of improving health equity for people who identify as SGD. Masters of Nursing students (N = 11) from adult specialties participated in a

Instruments for assessing geriatric nursing competence among nursing personnel: protocol for a COSMIN-based systematic review.

INTRODUCTION: Accelerated population ageing has resulted in increasingly diverse and complex care needs among older people, posing a major challenge to healthcare systems worldwide. Nursing personnel represent a core workforce in the delivery of healthcare and nursing services for older people, and their geriatric nursing competence (GNC) is closely associated with the quality of care provided, care outcomes and older people's quality of life. Therefore, accurate assessment of GNC is essential for nursing education, workforce development and quality improvement. Although several instruments ha

Counteracting fragmentation in the care of people with moderate and severe dementia.

Symptoms such as amnesia, agnosia, apraxia, and aphasia may lead to a fragmented experience and actions among people with moderate and severe dementia. The aim of this study was to explore the interactions where fragmentation occurred and how caregivers counteract fragmentation. The observation notes from participant observations were analyzed using interpretive content analysis. Fragmentation was noted if the patients showed that they did not recognize what was going on, the people involved, the things used in the action, or did not recognize themselves in the situation. Care providers could

Basing catheter care on research principles.

This review outlines the research-based principles which should be considered when caring for any patient with an indwelling urinary catheter. As principles they are not prescriptive, but should encourage reflection and allow nurses to make informed decisions regarding their care delivery based on current research.

Associations Between Nursing Students' Knowledge, Skills, and Attitudes and Participation in Experiential Learning on Care for People With Disabilities.

BACKGROUND: Experiential learning activities on care for people with disabilities (PWD) would help nursing students develop the knowledge, skills, and attitudes (KSAs) needed to provide quality care for PWD. PURPOSE: The purpose of this study was to evaluate the perceived changes in nursing students' KSAs related to the care for PWD following participation in health assessment and prevention (HAP) experiential learning activities. METHODS: This was a quasi-experimental, pre- and posttest educational intervention study. Nursing students (n = 160) attended multiple experiential learning activi

The cognitive processes employed by undergraduate nursing OSCE assessors: A qualitative research study.

OBJECTIVES: The Objective Structured Clinical Examination (OSCE) is an assessment format commonly used to assess undergraduate nursing students. However, in spite of its prominence, relatively little research has been conducted into how OSCE assessors form judgements about student performances, and whether divergent processes of judgement formation have the potential to negatively impact the inter-rater reliability (IRR) of awarded scores. This qualitative study aimed to uncover the cognitive processes which assessors employ when assessing OSCE performances. DESIGN, SETTING, PARTICIPANTS: In

Nursing processes used in the treatment of patients with bipolar disorder.

Psychiatric nurses are increasingly being involved in the provision of care for outpatients with bipolar disorder. The establishment of a body of knowledge for the nursing of these patients is vital for the development of integrated evidence-based treatment. The literature for the period January 1980 to January 2006 is reviewed. A sensitive literature search included not only the nursing care based on research results but also nursing care based on experiences acquired by practising nurses. The results show that formal research on the nursing of patients with a bipolar disorder is remarkably l

Women with gastrointestinal symptoms: implications for nursing research and practice.

Irritable bowel syndrome and functional bowel disorder are diagnoses used to describe chronic GI symptoms for which no overt pathological condition can be identified. Symptoms are more common in women and are frequently followed in gastroenterology clinics. The purpose of this article is to provide an overview of the research linking GI symptoms and reproductive cycling and to discuss implications for practice. GIGl symptoms such as stomach pain and nausea are highest during menses compared with other cycle phases; also, stool consistency is loosest at menses. This pattern is present in contro

Pain from sickle-cell crisis.

A qualitative questionnaire survey was carried out to identify the perceptions and expectations of pain management of patients with sickle-cell disease, and of nurses. The study illustrates that the present sample of nurses contributed to the poor pain control of sickle-cell patients because of their misconceptions about narcotic addiction, inadequate formal pain assessment and their neglect of attention to the psychosocial implications of this chronic disease.

Confirming nasogastric feeding tube position versus the need to feed.

OBJECTIVE: To review current methods for confirming nasogastric tube position and their efficacy in relation to the need to feed. DESIGN AND METHODS: This paper reviews current guidelines and techniques to safely confirm tube position balanced against the need to provide nutrition quickly. All new information was incorporated from electronic database searches up to 7.4.2012. FINDINGS: Tube misplacement per se, feeding through undetected misplaced tubes and the potentially fatal complications that arise from both appear to be underestimated. Misplacement occurs due to failure to confirm tube

Defining risk from the perspective of nurses in clinical roles.

Although the nursing literature suggests that nurses generally avoid risk, their willingness and ability to take risk are fundamental to their professional advancement, job satisfaction and patient advocacy. The definition of risk as it pertains in clinical practice, however, may not be synonymous with risk as defined in other fields. This study defines risk in nursing from the perspective of three registered nurses in clinical roles. Grounded theory methodology indicated that 11 risk categories comprised six core concepts. The latter formed the basis for the following: risk in clinical nursin

Using visual inquiry to reveal differences in nursing students' perception of case management.

An exploratory, qualitative study of 24 senior generic nursing students (GNSs) and 23 practicing registered nurse baccalaureate students (PRNSs) was conducted to evaluate students' perceptions about case management for community-based children with disabilities, and to examine the differences in the two student groups' perceptions utilizing the visual inquiry method. For a class project, the students used words and photographs to show how they solved one problem for a family caring for a child with a disability in the community. All students agreed on the overall theme, home environment; howev

Perineal dermatitis risk factors: clinical validation of a conceptual framework.

As part of a larger study previously published in December of 1994, the question was asked whether commonly used pressure ulcer assessment tools (the Braden and Medley) were applicable to a broader definition of "skin risk assessment" and altered skin integrity related to perineal dermatitis (PD), and, if so, which risk factors were actually related to PD. The three site randomized clinical trial compared the use of diapers and underpads for 166 adult patients hospitalized on medical and surgical floors who were incontinent of urine and/or feces. Variables related to skin breakdown were the nu

The bladder model: clinical implications.

The first two articles in this series (1, 2) discussed the importance of the experimental approach within nursing research, and described the development of a bladder model which could be used to investigate errors in practice. The final article will document the use of this model in examining the design features of four urinary drainage bags and in investigating the effect of a potential error in practice.
