Clinical reasoning: a 12-year-old boy with ascending weakness.

A 12-year-old boy presented with 3 weeks of calf pain, tripping, and progressive inability to walk. The onset was preceded by a sore throat 4 weeks prior, but no recent immunizations and no sick contacts. He began having problems "catching his toes" for 2 weeks. He had no visual complaints and no bowel or bladder incontinence. He had no recent travel and there were no heavy metal or solvent exposures. He had no prior medical history and he was on no prescription medications. Developmentally, he was on track and had just successfully completed fifth grade. However, he was reported to be behavio

Clinical Reasoning: a 57-year-old man with jaw spasms.

A 57-year-old man presented with spasms of his left jaw. Two years prior, he had developed left-sided facial numbness followed by development of left-sided shock-like pain and then involuntary and repetitive movements of the jaw-closing muscles. Jaw muscle contractions were episodic, interfered with chewing and talking, and led to frequent tongue biting. Individual spasms varied from seconds to minutes and he reported that function in between episodes was normal, although his wife felt he spoke with reduced mouth opening. He sometimes awoke with a bloody tongue, suggesting that contractions oc

Clinical Process Modeling: An Approach for Enhancing the Assessment of Physicians' Clinical Reasoning.

To ensure the validity of test interpretation and use, educators must be able to connect the dots between the tasks examinees perform on a test and the decisions educators make based on the results of that test. Toward that end, in this article, the authors introduce an approach called clinical process modeling, which combines clinical reasoning and principled assessment design. The goal of this approach is to better align test item development with the assessment of physicians' application of knowledge. Clinical process modeling involves creating complex decision trees that can mirror clinica

Clinical reasoning in feline spinal disease: which combination of clinical information is useful?

OBJECTIVES: The aim of this study was to evaluate if a combination of discrete clinical characteristics can be used to identify the most likely differential diagnoses in cats with spinal disease. METHODS: Two hundred and twenty-one cats referred for further evaluation of spinal disease were included and categorised as follows: non-lymphoid neoplasia (n = 44); intervertebral disc disease (n = 42); fracture/luxation (n = 34); ischaemic myelopathy (n = 22); feline infectious peritonitis virus myelitis (n = 18); lymphoma (n = 16); thoracic vertebral canal stenosis (n = 11); acute non-compressive

Creating hospital-specific customized clinical pathways by applying semantic reasoning to clinical data.

OBJECTIVE: Clinical pathways (CPs) are widely studied methods to standardize clinical intervention and improve medical quality. However, standard care plans defined in current CPs are too general to execute in a practical healthcare environment. The purpose of this study was to create hospital-specific personalized CPs by explicitly expressing and replenishing the general knowledge of CPs by applying semantic analysis and reasoning to historical clinical data. METHODS: A semantic data model was constructed to semantically store clinical data. After querying semantic clinical data, treatment p

Clinical Reasoning-Led Physiotherapy Intervention for Biceps Tendinopathy: A Case Report.

Long head of biceps tendinopathy (LHBT) is a common source of anterior shoulder pain, often misdiagnosed due to overlapping symptoms with other shoulder disorders. This case report presents a 56-year-old female with chronic shoulder pain managed through a clinical reasoning-guided physiotherapy program. Assessment using clinical reasoning, including the International Classification of Functioning, Disability, and Health (ICF) framework, confirmed LHBT. The intervention included pain management, manual therapy, range of motion exercises, strengthening exercises, and patient education over 6 wee

Data-Informed Intuition in Hepatology: Integrating Evidence, Context, and Clinical Reasoning.

Artificial intelligence and data-driven models are changing hepatology, but expert clinical judgment remains essential. Liver diseases are complex and evolve over time, requiring reasoning that links numerical data with clinical context. We propose data-informed intuition (DII), a conceptual framework bridging evidence-based medicine with clinical experience and reflective reasoning. Instead of presenting a validated prediction model, it proposes DII as an integrative scaffold for clinical reasoning that can be examined and revised. DII is viewed as a trainable skill that improves with data fe

The impact of sleep on resident clinical reasoning: a scoping review.

Residency training is an essential part of medical education, characterized by long work hours, high cognitive demands, and insufficient sleep. Given the ACGME duty-hour reforms and the ongoing debate over work-hour restrictions, we conducted a scoping review on the relationship between sleep and clinical reasoning learning and performance in graduate medical education, using the Arksey and O'Malley framework (between 2003 and Dec 2024). Our review aimed to: (1) identify and map the existing literature linking sleep and clinical reasoning learning and performance in medical residents, (2) exam

Osteoarthritis pain masterclass: Moving beyond structure to mechanism-based clinical reasoning.

INTRODUCTION: Osteoarthritis (OA) is a leading cause of pain, disability, and reduced quality of life worldwide. Although OA is commonly interpreted through a structural lens, pain severity is often only weakly related to radiographic severity, suggesting that structure-based models are insufficient to explain many clinical presentations. PURPOSE: This masterclass aims to challenge persistent structural assumptions surrounding OA pain, summarizes the peripheral, central, behavioral, and systemic mechanisms that may shape pain presentation, and provide a mechanism-based clinical reasoning fram

Evaluating large language models in specialized myopia knowledge and clinical reasoning.

BACKGROUND: Myopia is a major global public health concern, and the growing use of LLMs for health consultation makes the accuracy of their myopia-related responses increasingly important. Existing ophthalmic LLM studies have largely focused on image-based applications and mixed ocular diseases, with limited evaluation of myopia-specific reasoning. Yet myopia-related inquiries span both breadth and depth, from specialized differential considerations to common issues in prevention, control, and public education. This study aimed to assess mainstream LLMs across multiple levels of myopia-related

Clinical Reasoning: A 58-Year-Old Woman With Painless Blurry Vision.

A 58-year-old woman presented with painless progressive bilateral blurred vision, worse in the right eye, over several days. Two months prior, she developed a diffuse pruritic rash that spared her face. On examination, she was found to have reduced visual acuity bilaterally, optic nerve edema, anterior uveitis, and scattered intraretinal and peripapillary nerve fiber layer hemorrhages. Diagnostic evaluation demonstrated optic nerve head enhancement on MRI. Her serum workup showed positive antibodies for Borrelia burgdorferi with a negative Lyme disease Western blot and positive Treponema palli

Clinical Reasoning: A 26-Year-Old Woman With Headache and Eosinophilia.

Headache accompanied by eosinophilia in blood or CSF presents a complex clinical challenge. We present a case of a 26-year-old woman with headache, peripheral eosinophilia with meningeal enhancement, and intracranial vasculopathy on imaging. This case illustrates a systematic approach to a patient with meningitis, peripheral eosinophilia, and cerebral vasculitis, which includes comprehensive clinical history taking and appropriate investigative workup to differentiate between infectious and noninfectious causes and timely tissue sampling to conclusively determine the causative agent.

Clinical Reasoning: A 21-Year-Old Patient Presenting With Penile Hemidystumescence.

This case presents a 21-year-old male patient with recurrent episodes of lost consciousness that exhibited both features of seizure and syncope that were ultimately diagnosed as syncope secondary to autonomic seizures. Notably, review of systems revealed stereotyped erectile dysfunction, characterized by unilateral penile tumescence ("hemidystumescence") occurring peri-ictally. Neurologic workup revealed left temporal slowing on EEG, and antiseizure medication led to resolution of both the loss-of-consciousness events and the hemidystumescence. This case represents a novel manifestation of foc
