Evaluation of efficacy and safety of clobazam as an add-on drug in the treatment of generalized tonic-clonic seizure not controlled with valproic acid monotherapy.

BACKGROUND: Epilepsy is one of the major neuron-damaging neurological disorders. Generalized tonic-clonic seizure (GTCS) is the commonest one. Refractory patterns cannot be controlled by simple monotherapy with antiepileptic drugs (AEDs). Valproic acid (VPA) is one of the widely prescribed AEDs but it may not control many cases up to its maximum tolerable doses. In this study, we have seen the safety and efficacy of clobazam to control seizure in the adult population as an add-on drug over valproate, in cases of valproate uncontrolled seizures. MATERIALS AND METHODS: Patients on VPA monothera

Effects of antiseizure drugs zonisamide, oxcarbazepine, clobazam, and lacosamide on FOXO1, LIF, and E-cadherin- mediated molecules in rat embryo implantation.

AIMS: This study aimed to evaluate the effects of commonly used antiseizure drugs-lacosamide (LCM), zonisamide (ZNS), oxcarbazepine (OXC), and clobazam (CLB)-on uterine implantation in a non-epileptic juvenile-to-adult rat model. MATERIALS AND METHODS: Fifty female Wistar rats were divided into five groups: control, ZNS (100 mg/kg/day), LCM (30 mg/kg/day), CLB (12.5 mg/kg/day), and OXC (100 mg/kg/day). Drugs were administered orally for 90 days. Mating was synchronized with estrous cycles, and implantation sites were assessed histologically using hematoxylin-eosin staining. Immunohistochemist

GABAergic modulation in central sensitization in humans: a randomized placebo-controlled pharmacokinetic-pharmacodynamic study comparing clobazam with clonazepam in healthy volunteers.

Positive allosteric modulators of GABAA receptors (GAMs) acting at specific subtypes of GABAA receptors effectively restore compromised spinal pain control in rodents. Studies addressing a similar antihyperalgesic effect in humans are sparse and are hampered by sedative effects of nonselective GAMs available for use in humans. We present results from a randomized controlled double-blind crossover study in 25 healthy volunteers, which addressed potential antihyperalgesic actions of clobazam (CBZ) and clonazepam (CLN) at mildly sedating equianticonvulsive doses. Clobazam was chosen because of it

Effectiveness of adjunctive low-dose clobazam in adults with focal drug-resistant epilepsy and incomplete response to cenobamate: A real-world study.

OBJECTIVE: Cenobamate (CNB) is a highly effective antiseizure medication (ASM) for focal drug-resistant epilepsy (DRE); nevertheless, even with CNB treatment and optimized therapy, a substantial proportion of patients continued to experience seizures. Preliminary observations suggest that clobazam (CLB), a 1,5-benzodiazepine, may enhance CNB efficacy. We aimed to evaluate the real-world effectiveness and safety of adjunctive low-dose CLB in adults with focal DRE who continued to have incomplete seizure control despite CNB. METHODS: We conducted a retrospective single-center study including ad

Neuroethological evaluation of audiogenic seizures and audiogenic-like seizures induced by microinjection of bicuculline into the inferior colliculus. II. Effects of nigral clobazam microinjections.

Male Wistar rats were classified as susceptible (S) and resistant (R) to audiogenic seizures (AS) by evaluation of their response to high-intensity sound stimulation (110.3 dB). R rats injected with bicuculline into the inferior colliculus (IC) preferentially displayed audiogenic-like seizures with gyri, jumping and atonic falling, without important tonic-clonic components but with postictal contralateral asymmetry and hyperreactivity. These audiogenic-like seizures were blocked by clobazam microinjection into the substantia nigra (SN) and partially modified by SN vehicle injection. Injection

The effects of clobazam treatment in rats on the expression of genes and proteins encoding glucronosyltransferase 1A/2B (UGT1A/2B) and multidrug resistance-associated protein-2 (MRP2), and development of thyroid follicular cell hypertrophy.

Clobazam (CLB) is known to increase hepatobiliary thyroxine (T4) clearance in Sprague-Dawley (SD) rats, which results in hypothyroidism followed by thyroid follicular cell hypertrophy. However, the mechanism of the acceleration of T4-clearance has not been fully investigated. In the present study, we tried to clarify the roles of hepatic UDP-glucronosyltransferase (UGT) isoenzymes (UGT1A and UGT2B) and efflux transporter (multidrug resistance-associated protein-2; MRP2) in the CLB-induced acceleration of T4-clearance using two mutant rat strains, UGT1A-deficient mutant (Gunn) and MRP2-deficien

Clobazam and Valproate, but Lamotrigine, a Sodium Channel Inhibitor, Reduce the Incidence of Hyperthermia-Induced Clonic Seizures in Dravet Syndrome Mice: Assessment of Anti-Seizure Effects in Mice Using a Stabilized Ambient Temperature System.

BACKGROUND: Dravet syndrome (DS) is a severe and treatment-resistant epileptic encephalopathy, most commonly caused by de novo mutations in the sodium voltage-gated channel alpha subunit 1 (SCN1A) gene. Patients with DS often present with febrile seizures. It has been reported that Scn1a mutant mice have the risk for hyperthermia-induced clonic seizures similar to those observed in patients with DS. Using our heating protocol with an incubator, we evaluated the effects of antiseizure medications on the incidence of hyperthermia-induced clonic seizures in mutant Scn1a knock-in (KI)/+ mice. MET

Utilizing an acute hyperthermia-induced seizure test and pharmacokinetic studies to establish optimal dosing regimens in a mouse model of Dravet syndrome.

OBJECTIVE: The current standard of care for Dravet syndrome (DS) includes polytherapy after inadequate seizure control with one or more monotherapy approaches. Treatment guidelines are often based on expert opinions, and finding an optimal balance between seizure control and adverse drug effects can be challenging. This study utilizes the efficacy and pharmacokinetic assessment of a second-line treatment regimen that combines clobazam and sodium valproate with an add-on drug as a proof-of-principle approach to establish an effective therapeutic regimen in a DS mouse model. Influence of arachidonyl-2'-chloroethylamide, a selective cannabinoid CB1 receptor agonist, on the anticonvulsant and acute side-effect potentials of clobazam, lacosamide, and pregabalin in the maximal electroshock-induced seizure model and chimney test in mice.

The influence of arachidonyl-2'-chloroethylamide (ACEA - a selective cannabinoid CB1 receptor agonist) on the anticonvulsant potency and acute adverse-effect potentials of clobazam, lacosamide, and pregabalin was determined in the maximal electroshock-induced seizure model and chimney test in mice. ACEA (2.5 mg/kg, i.p.) significantly enhanced the anticonvulsant potency of pregabalin in the mouse maximal electroshock-induced seizure model by decreasing the median effective dose (ED50 ) of pregabalin from 125.39 to 78.06 mg/kg (P < 0.05). In contrast, ACEA (2.5 mg/kg) had no significant impact
