Nonspecific bronchial hyperreactivity after exposure to Western Red Cedar.

A 55-year-old nonatopic man presented with a 2-year history of progressively severe conjunctivitis, rhinitis, and asthma related to exposure to freshly cut red cedar. Chest roentgenogram, lung volumes, diffusing capacity for carbon monoxide, and expiratory flow rates were normal. A histamine inhalation test demonstrated mild, nonspecific bronchial hyperreactivity. After a 35-min cumulative exposure to Western Red Cedar sawdust in the laboratory, the patient developed a late asthmatic response. Bronchial reactivity to inhaled histamine increased significantly after exposure to red cedar in the

Adaptation of lateral pterygoid and anterior digastric muscles after surgical mandibular advancement procedures in different vertical craniofacial types: a magnetic resonance imaging study.

OBJECTIVE: Surgical mandibular advancement procedures induce major adaptations of jaw-closing muscles. In this study, adaptation of antagonist muscles, the lateral pterygoid (LPM) and anterior digastric (DigA) muscles, was evaluated. STUDY DESIGN: Eighteen adult patients with mandibular retrognathia and individually varying vertical craniofacial dimensions were treated with bilateral sagittal split osteotomies (BSSO), in some cases combined with a Le Fort I osteotomy (LF Bimax). The sample was divided into 1 short-face (SF, n = 7) and 2 long-face groups (LF BSSO, n = 3; and LF Bimax, n = 8).

Pharyngoglottal closure reflex: identification and characterization in a feline model.

Earlier studies in humans have shown that pharyngeal stimulation by water at a threshold volume induces a brief vocal cord adduction, i. e., pharyngoglottal closure reflex. The present study was undertaken to 1) develop a suitable animal model for physiological studies of this reflex and 2) delineate its neural pathway and effector organs. Studies were done in cats by concurrent videoendoscopy and manometry followed by electromyographic studies. At a threshold volume (0.3 +/- 0.06 ml), injection of water into the pharynx resulted in a brief closure of the vocal folds, closing the introitus to

[Influence of disturbed lung hemodynamics on the early bronchial obstruction in mitral disease (author's transl)].

32 subjects affected with mitral stenosis were investigated. All the subjects were nonsmokers, and none had any histories of lung disease. Mean pulmonary artery pressure (PAp) and mean wedge pressure (Pc) were obtained by right catherization and compared with a number of sensitive tests for the detection of incipient bronchial obstruction: dynamic and static lung compliance (CLdyn, CLstat); maximal expiratory flows at 50% and 25% (MEF50%, MEF25%) of the forced vital capacity (FVC): closing volume (CV); airway resistance (RAW), and residual volume (RV). It was established that most sensitive te

Small Airways Dysfunction and Lung Hyperinflation in Long COVID-19 Patients as Potential Mechanisms of Persistent Dyspnoea.

BACKGROUND: Reticulation, ground glass opacities and post-infection bronchiectasis are present three months following hospitalisation in patients recovering from SARS-CoV-2 infection and are associated with the severity of acute infection. However, scarce data exist on small airways impairment and lung hyperinflation in patients with long COVID-19. AIM: To evaluate small airways function and lung hyperinflation in previously hospitalised patients with long COVID-19 and their association with post-COVID-19 breathlessness. METHODS: In total, 33 patients (mean ± SD, 53 ± 11 years) with long COV

Volume-pressure properties of the upper airway in normal subjects and patients with obstructive sleep apnoea.

The aim of this study was to examine the volume-pressure (V-P) characteristics of isolated upper airways in normal subjects and patients with obstructive sleep apnoea (OSA) and to ascertain whether an increase in upper airway muscle activity affects these characteristics. We studied upper airway pressure changes during volume changes by inflation and deflation of air volumes of 5, 10, 15 and 20 mL without and with submental electrical stimulation, during voluntary closing of the glottis, in seven normal subjects and 13 OSA patients. Volume-pressure properties of the upper airway were assessed

Electrical-thermal coupling characteristics of pre-insertion resistors in AC filter circuit breaker for UHV grid.

The ultra-high voltage (UHV) AC/DC grid can provide a platform for sustainable power worldwide. To improve the bus voltage quality of the UHV AC system, AC filters are frequently switched into the UHV grid through circuit breakers with pre-insertion resistors. The pre-insertion resistors suppress inrush currents and operate over-voltage during switching. In this paper, we establish a macro and micro model of the pre-insertion resistor based on its temperature coefficient and micro-morphology. We simulate and analyze its electric-thermal coupling characteristics under standard closing and short

Prosthetic mitral valve motion during cardiac dysrhythmias as determined by echocardiography.

To assess the changes and physiologic mechanisms of prosthetic valve motion during cardiac dysrhythmias as well as the role of atrial systole in the closure of the mitral valve, M-mode echocardiography was performed in 36 patients with normally functioning prosthetic mitral valves (Björk-Shiley, Starr-Edwards, and Beall valve). Premature closure of the prosthetic mitral valve in diastole with a "sharp" closing motion was seen during first-degree atrioventricular block, atrial fibrillation with ventricular rates less than 60 beats/min, and atrial flutter. A "rounded" premature valve closure due

Traffic volume and collisions involving transit and nontransit vehicles.

This study reports an analysis of collisions occurring between public transit vehicles operated by the San Francisco Municipal Railway System (Muni), the public transit agency for the City of San Francisco, and nontransit vehicles. The analysis, focusing on weekday collisions during 1987, demonstrated a strong association between hourly transit collisions rates and hourly traffic volume. The collision rate varied from 0.01 per 1,000 Muni vehicle-hours of operation during the interval 5 A.M. to 6 A.M., a time of very low traffic volume, to 0.93 (approximately 1 collision per 1,000 Muni vehicle-

[Dose-dependence of the carboxymethylcellulose-papain combination in the prevention of peritoneal adhesions. Study in rats].

The action of a low volume (one drop) of sodium carboxymethylcellulose (CMC)-papain (PP) association gel to prevent peritoneal adhesions were studied in female Wistar rats. After ether anesthesia and a midline laparotomy incision, the right parietal peritoneum was pinched with a fine hemostat and the pinched peritoneal fold was then ligated. This maneuver was repeated thrice creating four points as if they were little "polyps" with a standardized size. Before closing the incision it was deposited on each point 0.05 ml (one drop) of CMC 2% (group A) or CMC + PP 0.4% (group B), with a total volu

Sustainable Postpartum Hemorrhage Training for Rural Obstetric Nurses in the Absence of Routine Patient Care Experiences.

Critical access birthing units are essential for the well-being of maternal and neonatal patients in rural areas; however, many rural hospitals are closing their birthing units due to a variety of complex factors, including the difficulty of maintaining highly trained staff. These closures worsen the already vast "maternity care deserts" found across much of the rural United States. Postpartum hemorrhage is one of the most common complications during childbirth, but nurses providing care at low-birth-volume hospitals in rural areas often lack routine exposure to postpartum hemorrhage and have

The lungs and airways in achondroplasia. Do little people have little lungs?

Achondroplasia is a unique model of the effects of skeletal dysplasia and dwarfism on the respiratory system. We measured chest dimensions, spirometry, lung volumes, maximal expiratory flow volume curves, nasal and airways resistance, closing volume, maximal inspiratory/expiratory pressures, and tracheal area by acoustic reflection in 12 healthy subjects with achondroplasia. Anterior-posterior thoracic diameter was mildly reduced in men. Vital capacity for all subjects was 108 percent +/- 18.6 percent (SD) of that predicted for achondroplastic subjects, but was reduced when compared with value

Altered respiratory physiology in obesity.

The major respiratory complications of obesity include a heightened demand for ventilation, elevated work of breathing, respiratory muscle inefficiency and diminished respiratory compliance. The decreased functional residual capacity and expiratory reserve volume, with a high closing volume to functional residual capacity ratio of obesity, are associated with the closure of peripheral lung units, ventilation to perfusion ratio abnormalities and hypoxemia, especially in the supine position. Conventional respiratory function tests are only mildly affected by obesity except in extreme cases. The

Automated emergency ventilation devices in a simulated unprotected airway.

BACKGROUND: Automated ventilation devices are becoming more popular for emergency ventilation, but there is still not much experience concerning the optimal ventilation mode. METHODS: In a bench model representing a non-intubated patient in respiratory and cardiac arrest, we compared a pressure-cycled with a time- and volume-cycled automated ventilation device in their completely automated modes. The main study endpoints were inspiratory time, respiratory rate, stomach inflation, and lung tidal volumes. RESULTS: The pressure-cycled device inspired for 6.7 s in the respiratory arrest setting

Aging and the respiratory system.

All the components of the respiratory system are affected by aging, though at different rates: i) the lung elastic recoil decreases; ii) PaO2 decreases and the D(A-a)O2 increases; iii) the chest wall becomes stiffer; iv) the inspiratory muscles loose strength; and v) the respiratory centres are less sensitive. Residual volume, closing volume and function residual capacity increase, whereas vital capacity and FEV1 progressively decrease. The flow volume curve becomes more convex to the volume axis at low lung volume. Whether these changes are due to aging or associated with aging is a matter of

The Effect of Pressure on Organic Reactions in Fluids-a New Theoretical Perspective.

This Review brings a new perspective to the study of chemical reactions in compressed fluid media. We begin by reviewing the substantial insight gained from more than 50 years of experimental studies on organic reactions in solution under pressure. These led to a proper estimation of the critical roles of volume of activation (Δ≠ V) and reaction volume (ΔV) in understanding pressure effect on rates and equilibria of organic reactions. A recently developed computational method, the XP-PCM (extreme pressure polarizable continuum model) method, capable of carrying out quantum mechanical calculati
