Dioscin suppresses colitis-associated colorectal tumorigenesis by inhibiting PPARγ-mediated fatty acid oxidation in TAMs.

BACKGROUND: Chronic inflammation plays a key role in colorectal carcinogenesis, where tumor-associated macrophages (TAMs) critically promote tumor progression. Metabolic reprogramming of TAMs, particularly through the modulation of fatty acid oxidation (FAO), represents a promising therapeutic strategy; however, effective agents remain to be identified. Although the natural compound dioscin exhibits anti-tumor properties, its ability to regulate TAM plasticity and metabolism in colitis-associated colorectal cancer (CAC) remains unclear. METHODS: A mouse model of CAC was established using azox

Dysbiotic microbiota trigger colitis-associated colorectal cancer and imprint a distinctive bile acid profile in a PSC-IBD model.

BACKGROUND: Primary sclerosing cholangitis-associated UC (PSC-UC) carries excess colorectal neoplasia despite often mild-appearing endoscopy, implicating persistent microscopic inflammation and microbiota-bile acid (BA) dysfunction. OBJECTIVE: To test whether PSC-UC neoplasia is driven by transferable microbiota-mediated inflammation linked to secondary BA loss. DESIGN: Surveillance colonoscopies (2012-2022) from PSC-UC (n=251) and UC-only (n=8839) were compared for segmental endoscopic/histological activity and dysplasia. We generated multidrug resistance protein 2 (MDR2)-/- × interleukin (

Behavioral and emotional abnormalities in mice with orthotopic colitis-associated colorectal cancer.

BACKGROUND: Colorectal cancer (CRC) is usually accompanied by emotional and behavioral changes in patients, yet the underlying neurobiological mechanisms remain unclear. There is a lack of robust experimental models to investigate how tumor progression may influence brain function and behavior. METHODS: The azoxymethane (AOM) and dextran sulfate sodium (DSS) were used to induce the orthotopic colitis-associated CRC (CAC) mouse models. Then we performed a range of behavioral tests including the sucrose preference test (SPT), elevated plus maze (EPM), tail suspension test (TST), novel object re

Proctocolectomy for ulcerative colitis after liver transplantation for primary sclerosing cholangitis.

BACKGROUND: Sclerosing cholangitis is a chronic cholestatic liver disease of unknown etiology. Immunologic and genetic factors may be involved in the pathogenesis of the disease, characterized by fibrosis involving bile ducts, which can progress to biliary cirrhosis and cholangiocarcinoma (8%-30%). Sclerosing cholangitis is frequently associated with inflammatory bowel diseases, especially ulcerative colitis (60%-80%), which may require a proctocolectomy with ileal pouch anal anastomosis if there is no response to therapy or the appearance of colonic carcinoma. PATIENTS AND METHODS: Among 162

Machine Learning for Colitis-Associated Cancer in Inflammatory Bowel Disease: Evidence and Future Directions Toward Precision Medicine, a Narrative Review.

Colitis-associated cancer (CAC) represents a major long-term complication in patients with ulcerative colitis (UC) and Crohn's disease (CD), the two main forms of inflammatory bowel disease (IBD). Unlike sporadic colorectal cancer, CAC develops through a distinct inflammation-dysplasia-carcinoma sequence driven by chronic inflammation and complex molecular alterations. Early detection of dysplasia and accurate risk stratification remain critical challenges in IBD management. Conventional surveillance strategies, including endoscopy, histopathology, and immunohistochemistry, are time-consuming,

Adenocarcinoma below stapled ileoanal anastomosis after restorative proctocolectomy for ulcerative colitis.

A case of adenocarcinoma arising in a 39-year-old patient after restorative proctocolectomy is reported. The patient underwent an ileal pouch-anal anastomosis with double-stapled technique for severe ulcerative colitis 18 years earlier, without evidence of associated neoplasm or dysplasia in operative specimen. After endoscopic diagnosis of adenocarcinoma, the patient was submitted to excision of the pouch and permanent ileostomy, followed by combined radiotherapy and chemotherapy. Pathology showed an AJCC stage III moderately differentiated mucinous adenocarcinoma. The patient died 24 months

Pediatric and adult celiac disease: similarities and differences.

Differences between children and adults in celiac disease (CD) presentation and epidemiology are reviewed here. Clinical manifestations, histological changes, serology, and response to gluten-free diet are similar. Differences exist in epidemiology, type of clinical presentations, coexisting diseases, complications, and association with obesity. CD is two to five times more common in children than in adults. Classical CD with gastrointestinal symptoms is more common in children whereas nonclassical CD dominates in adults. A gene dose phenomenon (double-dose HLA-DQB1 02 allele) is postulated to

CV-SCAN (Crystal Violet Staining for Colitis-Associated Neoplasia): A Novel Endoscopic Staining Method to Detect Paneth Cell Metaplasia and Ulcerative Colitis (UC)-Associated Neoplasia in UC.

OBJECTIVES: Paneth cell metaplasia (PCM), a metaplastic change associated with chronic inflammation in ulcerative colitis (UC), may be linked to UC-associated neoplasia (UCAN). However, no endoscopic method currently exists for detecting PCM. This study aimed to develop and validate a novel endoscopic staining technique-CV-SCAN-for identifying PCM and UCAN, and to explore the molecular characteristics of the stained areas. METHODS: This retrospective observational study included 131 patients with UC undergoing surveillance colonoscopy. CV-SCAN involved spraying an ultra-diluted solution (0.00

Ki-67 distribution, α-methylacyl-CoA racemase (AMACR) expression and mucin phenotypes are associated with non-polypoid growth in ulcerative colitis-associated neoplasia.

AIMS: Ulcerative colitis-associated neoplasia (UCAN) is characterised by multifocal tumourigenesis. A wide range of metachronous lesions have been reported to occur after endoscopic treatment of UCAN, which suggests the development of sporadic tumours in lesions treated as UCAN. Therefore, we aimed to evaluate differences of immunohistochemistry (IHC) in features and clinicopathological characteristics of intramucosal lesions in patients with ulcerative colitis (UC). METHODS AND RESULTS: We examined 35 intramucosal lesions resected for carcinoma or dysplasia by total colectomy from patients w

F4/80+Ly6Chigh Macrophages Lead to Cell Plasticity and Cancer Initiation in Colitis.

BACKGROUND & AIMS: Colorectal cancer is a leading cause of cancer death, and a major risk factor is chronic inflammation. Despite the link between colitis and cancer, the mechanism by which inflammation leads to colorectal cancer is not well understood. METHODS: To investigate whether different forms of inflammation pose the same risk of cancer, we compared several murine models of colitis (dextran sodium sulfate [DSS], 2,4,6-trinitrobenzene sulfonic acid, 4-ethoxylmethylene-2-phenyloxazol-5-one, Citrobacter rodentium, Fusobacterium nucleatum, and doxorubicin) with respect to their ability to

Therapeutic role of ursodeoxycholic acid in colitis-associated cancer via gut microbiota modulation.

Inflammatory bowel disease (IBD) is a predisposing factor for colitis-associated cancer (CAC). The association between bile acids and the gut microbiota has been demonstrated in colon neoplasia; however, the effect of ursodeoxycholic acid (UDCA) on gut microbiota alteration in development of colitis and CAC is unknown. Our analysis of publicly available datasets demonstrated the association of UDCA treatment and accumulation of Akkermansia. UDCA-mediated alleviation of DSS-induced colitis was microbially dependent. UDCA treatment significantly upregulated Akkermansia colonization in a mouse mo

Colitis-associated gut-vagus-brain signaling integrates tumorigenesis and neuroinflammation: comparative regulation by vagotomy and atropine.

BACKGROUND: Colitis-associated cancer (CAC) is frequently accompanied by neuropsychiatric comorbidities, yet the underlying gut-brain signaling pathways remain elusive. Emerging evidence highlights the gut-brain axis, particularly the vagus nerve, as a pivotal mediator linking gastrointestinal pathology with neuropsychiatric dysfunction. This study aimed to dissect the dual roles of vagal signaling in a murine CAC model and to evaluate the therapeutic potential of the muscarinic antagonist atropine on CAC. METHODS: The azoxymethane/dextran sulfate sodium (AOM/DSS) was adopted to induce CAC mic

The MicroRNA miR-223 Constrains Colitis-associated Tumorigenesis by Limiting Myeloid Cell Infiltration and Chemokine Expression.

Aberrant intestinal inflammation plays a critical role in the development of colitis-associated colorectal cancer (CAC), yet the mechanisms controlling tumor development by the myeloid immune compartment are not fully understood. Although altered microRNA expression is observed in CAC, it is also unclear how myeloid-specific microRNAs impact the inflammatory process that underpins the continuum from ulcerative colitis to tumorigenesis. In this study, we report that miR-223 acts to limit myeloid-driven inflammation in the azoxymethane (AOM)-dextran sodium sulfate (DSS) model of CAC in mice. In

[Hypermucinous and goblet cell-deficient, IBD-associated, non-conventional dysplasia besides colorectal adenocarcinoma].

Patients with inflammatory bowel disease (IBD) are twice as likely to develop colorectal carcinoma as the general population. In addition to invasive malignancy, these patients are also more likely to develop dysplasia, as well. In recent years, several subtypes of the so-called IBD-associated, non-conventional dysplasias have been identified, and the amount of information about these laesions show an increasing tendency, due to the many ongoing investigations. We present the case of a 62-year-old woman, diagnosed and treated with relapsing ulcerative colitis for 14 years. In the patient’s sub

Clinical features by disease duration in ulcerative colitis-associated cancers.

AIM: Ulcerative colitis (UC) is a known contributor to the development of colitis-associated cancer (CAC), although the exact mechanism remains to be elucidated. CAC typically presents as a flat type macroscopically and manifests histologically as mucinous carcinoma and signet ring cell carcinoma. While the relationship between disease duration and chronic inflammation has been studied, the impact of disease duration on CAC outcomes has yet to be thoroughly investigated. The aim of this study is to examine the effect of UC duration on the clinicopathological features of CAC. METHOD: This stud

Persistent Gene Activation as a Molecular Signature of Ulcerative Colitis Progression to Colorectal Cancer.

INTRODUCTION: Ulcerative colitis (UC) is an inflammatory disease characterized by colonic epithelial damage. With prolonged disease duration, the risk of malignant transformation increases significantly. However, the mechanisms driving the progression from chronic inflammation to tumorigenesis remain incompletely understood. This study aimed to identify a set of genes that may contribute to the transition from healthy controls (HC) to UC and ultimately to colitis-associated colorectal cancer (CAC). METHODS: We conducted bioinformatic analysis of four Gene Expression Omnibus (GEO) datasets to

Long-Term Outcomes After Endoscopic Resection for Strictly Defined Ulcerative Colitis-Associated Neoplasia: An Exploratory Retrospective Cohort Study.

OBJECTIVES: Ulcerative colitis (UC)-associated neoplasia (UCAN) carries a risk of metachronous development; however, risk stratification based on inflammatory origin remains unclear. This exploratory study aimed to classify UCAN arising in colitis-affected areas and to evaluate long-term outcomes after endoscopic resection (ER). METHODS: We retrospectively analyzed the data of 102 patients with UC who underwent ER for colorectal neoplasia between 2004 and 2024. Lesions in colitis-affected areas were operationally classified as confirmed UCAN, based on non-polypoid morphology and characteristi

Preclinical cancer chemoprevention studies using animal model of inflammation-associated colorectal carcinogenesis.

Inflammation is involved in all stages of carcinogenesis. Inflammatory bowel disease, such as ulcerative colitis and Crohn's disease is a longstanding inflammatory disease of intestine with increased risk for colorectal cancer (CRC). Several molecular events involved in chronic inflammatory process are reported to contribute to multi-step carcinogenesis of CRC in the inflamed colon. They include over-production of free radicals, reactive oxygen and nitrogen species, up-regulation of inflammatory enzymes in arachidonic acid biosynthesis pathway, up-regulation of certain cytokines, and intestina
