Bipolar Disruption of the Lateral Ulnar Collateral Ligament of the Elbow: A Case Report.

CASE: The lateral ulnar collateral ligament (LUCL) is one of the primary stabilizers of the elbow. Disruption typically occurs from the humeral origin and may be because of an elbow dislocation or fracture/dislocation. If not identified and properly managed, posterolateral rotatory instability may result from LUCL insufficiency. We describe the case of a patient with bipolar LUCL disruption consisting of an avulsion fracture of the crista supinatoris and a soft-tissue avulsion from the lateral epicondyle. CONCLUSION: Bipolar disruption of the LUCL is a previously unreported injury pattern tha

Sprains, Strains, and Partial Tears of the Medial Ulnar Collateral Ligament of the Elbow.

Medial ulnar collateral ligament (MUCL) insufficiency is becoming common in younger, nonprofessional athletes. In contrast to elite athletes who develop valgus extension overload syndrome and associated chronic pathologic changes in the MUCL, younger patients present with sprains and partial tears that can often be managed non-operatively with successful outcome and rapid return to play. In the younger throwing athlete with medial-sided elbow pain, a hinged elbow brace and rehabilitation of dysfunctional muscles often lead to successful recovery and return to play within 1-2 months. Reconstruction of the lateral ulnar collateral ligament of the elbow: a comparative biomechanical study.

PURPOSE: Posterolateral rotatory instability (PLRI) of the elbow is the result of an insufficient lateral collateral ligament complex (LCLC). Single-bundle reconstruction of the lateral ulnar collateral ligament (LUCL) represents the standard treatment method for chronic PLRI. However, cases of recurrent instability after LUCL reconstruction have been reported. The dual-reconstruction procedure has been described to anatomically restore the LUCL as well as the radial collateral ligament (RCL). It was hypothesized that anatomic reconstruction of the LCLC provides increased stability compared wi

Reconstruction of the ulnar collateral ligament of the thumb metacarpophalangeal joint: a cadaver study.

PURPOSE: The purpose of this study was to compare the stiffness and strength of the native ulnar collateral ligament with 4 methods of static ulnar collateral ligament (UCL) reconstruction at the thumb metacarpophalangeal (MCP) joint. METHODS: Eleven fresh-frozen cadaver specimens were amputated at the carpometacarpal and interphalangeal joints and all soft tissues were removed except for the extensor pollicis brevis tendon, the proper and accessory collateral ligaments, and the volar plate. Each thumb metacarpal was potted in cement and the native UCL was loaded to failure at 30 degrees of M

Avulsion and Soft Tissue Injuries of the Ulnar Collateral Ligament in Children and Adolescents.

BACKGROUND: Although pediatric medial epicondylar fractures and apophysitis are well studied, patterns of subapophyseal avulsion and ligamentous injuries of the medial elbow in this population merit investigation to inform optimal treatment strategies. PURPOSE: To describe the occurrence and demographic correlates of ulnar collateral ligament (UCL) avulsion and soft tissue injuries of the pediatric and adolescent elbow. STUDY DESIGN: Cross-sectional study; Level of evidence, 3. METHODS: An institutional review board-approved review was conducted to identify consecutive patients with medial

Repair of the Medial Ulnar Collateral Ligament of the Elbow With Internal Brace Augmentation.

BACKGROUND: The medial ulnar collateral ligament (MUCL) is the primary restraint to valgus force across the elbow during throwing. The incidence of MUCL continues to rise, and injuries are common in overhead throwing athletes often leading to elbow pain and instability. While MUCL reconstruction remains the gold standard for high-level throwing athletes with midsubstance tears, MUCL repair remains an option for select athletes and affords good functional outcomes with quicker return to sport compared with reconstruction. INDICATIONS: Indications for MUCL repair include acute injuries, proxima

The Stener Lesion and Complete Ulnar Collateral Ligament Injuries of the Thumb A Review.

A Stener lesion is a complete tear of the ulnar collateral ligament (UCL) from the thumb proximal phalanx at the level of the metacarpophalangeal (MCP) joint that is displaced superficial to the adductor pollicis aponeurosis, leading to interposition of the aponeurosis between the UCL and the MCP joint. The interposition of the adductor aponeurosis distinguishes the Stener lesion from other UCL injuries and impedes healing, thereby necessitating surgery. A thorough clinical examination, including valgus stress testing of the MCP joint, is crucial to the diagnosis. In cases where the clinical e

Biomechanical performance of baseball pitchers with a history of ulnar collateral ligament reconstruction.

BACKGROUND: A relatively high number of active professional baseball pitchers have a history of ulnar collateral ligament reconstruction (UCLr) on their throwing elbow. Controversy exists in the literature about whether professional baseball pitchers regain optimal performance after return from UCLr. It has been suggested that pitchers may have different biomechanics after UCLr, but this has not been previously tested. HYPOTHESIS: It was hypothesized that, compared with a control group without a history of UCLr, professional pitchers with a history of UCLr would have (1) significantly differe

Evaluation and treatment of medial ulnar collateral ligament injuries in the throwing athlete.

Medial ulnar collateral ligament (UCL) injuries are common and are seen most frequently in baseball pitchers. Appropriate recognition, treatment, and rehabilitation are necessary to ensure the best chance for return to preinjury levels of participation. Participation in competitive sports may be disrupted for 6 months to 1 year when treated optimally. Abstinence from play may be prolonged when treatment is delayed or if conservative treatment fails; this delay carries significant consequences to the professional, collegiate, and high school athlete. The orthopedic literature is replete with re

Ulnar Collateral Ligament Reconstruction Through a Mini-Invasive Approach and Suture Anchor Fixation.

Valgus stability of the elbow joint is mostly provided by the ulnar collateral ligament (UCL), which is composed of 3 bundles: anterior, posterior, and transverse. Many surgical techniques for UCL reconstruction have been described since Dr. Jobe's first introduced a technique using a free tendon graft and bone tunnel fixation on both the ulnar and humeral side. The aim of this technique is to describe a UCL reconstruction that focuses on restoring both the anterior and posterior bundle of the UCL, through a mini-open approach, using knotless suture anchors in an onlay fashion.

Short-term outcomes following reconstruction or direct repair of the ulnar collateral ligament.

INTRODUCTION: Medial ulnar collateral ligament (UCL) injuries substantially impair athletic performance due to pain, instability, and diminished throwing capabilities. While UCL reconstruction is the primary surgical treatment due to its reliable restoration of stability and high return-to-play rates, direct UCL repair has re-emerged as an alternative in select cases. For the purposes of this study, both procedures are collectively referred to as UCL reconstruction/repair (ULCR). The long rehabilitation period and complications such as ulnar neuropathy and stiffness remain concerns. Existing l

Association Between Preoperative Patient-Reported Questionnaires and Outcomes After Ulnar Collateral Ligament Surgery.

BACKGROUND: While surgical interventions for ulnar collateral ligament (UCL) injuries in throwing athletes have improved return-to-sport rates, predicting individual recovery timelines and the risk of complications remains a challenge. The influence of a patient's preoperative status, captured through patient-reported outcomes (PROs), on postoperative recovery milestones and complication risks is not well-defined. PURPOSE/HYPOTHESIS: The purpose of this study was to determine if baseline PROs and specific surgical variables predict critical postoperative milestones and adverse outcomes follow

Bone patellar tendon bone graft for ulnar collateral ligament reconstruction: a biomechanical study.

BACKGROUND: Ulnar collateral ligament (UCL) tears are a common injury among overhead athletes, leading to a lengthy recovery period after surgical reconstruction. To explore alternatives to decrease recovery time, we compared the biomechanical properties of the standard palmaris reconstruction with a novel bone-patellar tendon-bone (BTB) reconstruction with interference screw fixation. METHODS: Sixteen fresh frozen cadaver upper extremities and 8 cadaver knees were obtained (elbows 77 ± 9 years, male:female 1:3 ratio; knees 59 ± 7 years, male:female 3:1 ratio). Eight upper extremities were ra

Posterolateral Elbow Dislocation: An All-Arthroscopic Reconstruction of the Lateral Ulnar Collateral Ligament.

The elbow is one of the most commonly dislocated joints. Although simple dislocations of the elbow usually resolve with conservative management, certain patients can experience residual chronic instability. Posterolateral rotational instability accounts for approximately 80% of elbow chronic instability cases. We propose an all-arthroscopic reconstruction of the posterior, or ulnar, fascicle of the lateral ligament complex using an autograft or allograft, performed with a 5-mm-thick and 8-cm-long graft. The graft is first inserted distally into the supinator crest with an Arthrex 4.75-mm Swive

Middle Finger Metacarpophalangeal Joint Locked in Flexion Caused by Entrapped Ulnar Collateral Ligament.

Type 1 finger metacarpophalangeal (MCP) joint locking is characterized by maintained flexion of the MCP, with a lack of active and passive extension. Metacarpophalangeal joint locking is rare but has several identified causes, most commonly involving a radial-sided osteophyte. We report a case of right middle finger ulnar collateral ligament (UCL) entrapment characterized as type 1 MCP joint locking. The physical examination demonstrated that the right middle finger was locked in flexion at 60° at the MCP joint. Magnetic resonance imaging (MRI) demonstrated a possible tear of the distal attach
