Acute colonic pseudo-obstruction: rapid correction with neostigmine in the emergency department.

Ogilvie's syndrome, now known as acute colonic pseudo-obstruction, is characterized by massive dilatation of large bowel in the absence of mechanical obstruction. It is found in a variety of patients, although elderly and immobile patients make up a large portion of the afflicted population. This article discusses the case of a 64-year-old bedridden, paraplegic, male nursing home resident who presented to the Emergency Department with a chronic history of abdominal distention that acutely worsened on the day of his arrival. A diagnosis of acute colonic pseudo-obstruction was made and 2 mg of i

Retrospective study of neostigmine for the treatment of acute colonic pseudo-obstruction.

Acute colonic pseudo-obstruction (ACPO) typically develops postoperatively or after severe illness. Studies suggest that pharmacologic manipulation with intravenous (i.v.) neostigmine (NSM) may be an effective and less invasive treatment modality for ACPO with minimal side effects. The purpose of this study was to retrospectively assess the efficacy and incidence of complications of an i.v. NSM bolus in patients with ACPO. Eight patients with ten episodes of ACPO were treated with a bolus dose of NSM. Rapid and effective decompression of the colon was achieved in six episodes after a single do

CHRONIC PSEUDO-OBSTRUCTION OF THE SIGMOID COLON: A CASE REPORT.

Chronic intestinal pseudo-obstruction (CIPO) is a rare syndrome characterized by signs of intestinal obstruction lasting for 6 months or more, in the absence of a definitive cause of obstruction. We report a case of CIPO in a 49-year-old female patient with a 6-month history of ongoing irregular bowel movements, manifested as constipation and diarrhea accompanied by abdominal pain and bloated feeling. Contrast-enhanced abdominal computed tomography and magnetic resonance enterography revealed focal thickening of a segment of the lienal flexure and intermittent areas of wider and narrower calib

[Acute pseudo-obstruction of the colon as complication of hip surgery].

Pseudo-obstruction of the colon is characterized by a massive colonic dilatation without mechanical obstruction. It is rarely after hip surgery, however, untreated it may be complicated by a life threatening perforation of the colon. In a retrospective analysis we reviewed 11 patients (4 women, 7 men, mean age 75.2 years, range 67 to 86 years) with pseudo-obstruction of the colon after hip surgery in the years 1995 to 1998. In relation to all patients aged 65 years and older who underwent surgery of the hip, the prevalence was 1.06%. The leading clinical symptom was abdominal distension. On pl

Pseudo-obstruction of the colon following cesarean section. A review.

Pseudo-obstruction of the colon is characterized by an adynamic unobstructed colon which rapidly progresses to marked dilatation of the cecum and transverse colon. Disagreements exist regarding the etiology or pathogenesis of this syndrome; it has been associated with metabolic, traumatic, postoperative, and idiopathic causes. In reviewing the literature we have concluded that in pseudo-obstruction of the colon after cesarean section, the mean age of occurrence is 35 years. The symptoms occur in the first 72 hours after operation. Straight x-ray examination of the abdomen is the most useful di

Epidemiology of Acute Colonic Pseudo-obstruction (Ogilvie Syndrome) Based on Disease-Specific Data.

PURPOSE: Prior epidemiologic assessment of acute colonic pseudo-obstruction (ACPO) used nonspecific ICD-9 coding preventing precise case identification. Implementation of disease-specific ICD-10-CM code K59.81 (Ogilvie syndrome) in 2020 enables more accurate characterization. METHODS: Adults with ICD-10-CM code K59.81 in the 2021-2022 National Inpatient Sample were identified. Patients were classified into four treatment groups: medical management, colonoscopy alone, surgery alone, and combined colonoscopy/surgery. Multivariable regression models assessed associations between treatment group

Subcutaneous neostigmine appears safe and effective for acute colonic pseudo-obstruction (Ogilvie's syndrome).

BACKGROUND: Intravenous neostigmine is a well-established treatment for acute colonic pseudo-obstruction (ACPO). Its use is hampered by the perceived requirement for continuous cardiac monitoring, and patients are often transferred to high-dependency units for close observation during treatment. Subcutaneous neostigmine has the potential to minimize adverse cardiovascular effects while maintaining efficacy. This study aims to assess the safety of subcutaneous neostigmine on ward inpatients with ACPO monitored with standard nursing care. METHODS: This is a retrospective case series of 30 patie

Favorable surgical treatment outcomes for chronic constipation with features of colonic pseudo-obstruction.

AIM: To determine long-term outcomes of surgical treatments for patients with constipation and features of colonic pseudo-obstruction. METHODS: Consecutive 42 patients who underwent surgery for chronic constipation within the last 13 years were prospectively collected. We identified a subgroup with colonic pseudo-obstruction (CPO) features, with dilatation of the colon proximal to the narrowed transitional zone, in contrast to typical slow-transit constipation (STC), without any dilated colonic segments. The outcomes of surgical treatments for chronic constipation with features of CPO were an

Does neostigmine improve time to resolution of symptoms in acute colonic pseudo-obstruction?

A best evidence topic was written according to a structured protocol. In [patients with acute colonic pseudo-obstruction] is [neostigmine] superior to [conservative treatment] with respect to [duration of symptoms and complications]. In total 51 papers were found using the reported search, and ten of these represented the best evidence to answer the clinical question. The authors, date, journal, study type, population, main outcome measures and results are tabulated. We conclude that intravenous neostigmine is associated with significantly reduced duration of acute colonic pseudo-obstruction (

Treatment of acute colonic pseudo-obstruction with ultrasonography-guided percutaneous drainage. Case report.

In an 81-year-old woman acute colonic pseudo-obstruction occurred 3 days after operation for right femoral neck fracture. When conservative treatment failed, ultrasonographically guided percutaneous drainage of the caecum was performed to relieve progressive caecal dilatation. Bowel function was thereafter restored. The method may be a therapeutic option for acute colonic pseudo-obstruction in selected cases.

Silent Megacolon: Fulminant Clostridioides Difficile Infection Without Diarrhea Mimicking Acute Colonic Pseudo-Obstruction.

Clostridioides difficile infection (CDI) typically presents with diarrhea in elderly or antibiotic-exposed patients. We report a 40-year-old immunocompetent man with alcohol use disorder who presented with abdominal pain and jaundice, rapidly progressing to refractory constipation and colonic dilation (14.7 cm) mimicking acute colonic pseudo-obstruction. Conservative management including bowel regimens and neostigmine (generally contraindicated in suspected toxic megacolon) were attempted. Colonoscopic decompression revealed cecal pseudomembranes whereas rest of colon was grossly normal. Stool

Pneumatosis intestinalis with portal, mesenteric and renal gas due to colonic pseudo-obstruction.

OBJECTIVES: Pneumatosis intestinalis is a rare condition with subserosal or submucosal gas-filled cysts of the gastrointestinal tract. It is often associated with acute mesenteric ischemia, but also non-ischemic causes are described. CASE PRESENTATION: A 27-year-old male patient with severe congenital spastic tetraparesis presented to the emergency room with fever and reduced general condition. The patient was hypotonic and tachycardic, had a fever up to 39.7 °C and reduced peripheral oxygen saturation. The laboratory analyses revealed leukocytosis (16.7 G/L) and elevated CRP (162 mg/L).The p

Ogilvie's syndrome--acute colonic pseudo-obstruction. Case report and review of the literature.

We present a rare entity of colonic pseudo-obstruction, characterised by severe colonic dilatation in absence of any organic obstacle. Clinical symptoms, diagnostic approach, and therapeutic measures are analysed and discussed. Many factors have been associated with this syndrome which include electrolyte imbalance, systemic infection, drugs, and occasionally, neurologic disease. Reported here is a case of acute colonic pseudo-obstruction which developed in a patient with restrictive respiratory dysfunction. Colonic decompression by means of colonoscopy, the most effective therapeutic approach

Acute Colonic Pseudo-Obstruction After Posterior Spinal Fusion: A Case Report and Literature Review.

BACKGROUND: Acute colonic pseudo-obstruction (ACPO) or Ogilvie's syndrome occurs in 0.22%-7% of patients undergoing surgery, with a mortality of up to 46%. ACPO increased median hospital days versus control in spinal surgery (14 vs. 6 days; P < 0.001). If defined as postoperative ileus, the incidence was 7%-13.4%. Postoperative ileus is associated with 2.9 additional hospital days and an $80,000 increase in cost per patient. We present a case of ACPO in an adult patient undergoing spinal fusion for correction of scoliosis and review the available literature to outline clinical characteristics
