Multidisciplinary management of a traumatic posterior meningeal artery pseudoaneurysm: A case report and review of the literature.

BACKGROUND: Meningeal arterial injuries represent <1% of all blunt traumatic brain injuries (TBIs). Middle meningeal artery (MMA) lesions comprise the majority. However, there is little clinical data on posterior meningeal artery (PMA) injuries. CASE REPORT: A 69-year-old man was brought to our trauma center after sustaining a fall inside a warehouse. He was GCS (Glasgow Coma Scale) 3 on arrival. Non-contrast CT (computed tomography) brain showed subarachnoid hemorrhage with diffuse cerebral edema and a basilar skull fracture. The patient subsequently underwent emergency ventriculostomy. Imme

Intractable seizures after cranioplasty, a dreadful post-operative complication managed efficiently from Nepal: A case series.

INTRODUCTION AND IMPORTANCE: Cranioplasty is an elective neurosurgical procedure following decompressive craniectomy, often associated with post-operative complications such as hemorrhage, seizures, infection, hydrocephalus, and bone resorption. While seizures post-cranioplasty is not uncommon, Intractable seizures are rare but a dreadful complication following cranioplasty. CASE SUMMARY: A 23 and 17-year-old male underwent decompression craniectomy for traumatic brain injury and subsequently underwent titanium mesh and acrylic cranioplasty respectively, During the post-operative period both

Epidemiology of traumatic brain injury in a South African major trauma cohort.

BACKGROUND: South Africa has one of the highest trauma burdens globally, with unique contextual challenges around health system capacity. Traumatic brain injury (TBI) is a significant contributor to post-injury mortality and morbidity. We aimed to examine TBI prevalence and outcomes in adult major trauma patients. METHODS: We conducted a secondary analysis of the Epidemiology and Outcomes of Prolonged Trauma Care (EpiC) study, a prospective observational study of adult major trauma patients presenting to public-sector health facilities in South Africa's Western Cape. Individuals injured from

CT-based radiomic markers to predict late-onset seizures after traumatic brain injury.

Post-traumatic epilepsy (PTE) is a major long-term complication of traumatic brain injury (TBI), but early risk prediction remains imprecise. Radiomics enables quantitative analysis of subtle abnormalities on non-contrast head CT (NCCT) that are not readily visible on routine imaging and may improve early risk stratification. This pilot study assessed the performance of radiomic features from acute NCCT, alone or combined with clinical variables, to predict late post-traumatic seizures (PTS) within six months of injury, an early marker of PTE. Eighty-two patients with TBI were included, and tw

A Prognostic Model for Predicting One-Month Outcomes among Emergency Department Patients with Mild Traumatic Brain Injury and a Presenting Glasgow Coma Scale of Fifteen.

The lack of well-performing prognostic models for early prognostication of outcomes remains a major barrier to improving the clinical care of patients with mild traumatic brain injury (mTBI). We aimed to derive a prognostic model for predicting incomplete recovery at 1-month in emergency department (ED) patients with mTBI and a presenting Glasgow Coma Scale (GCS) score of 15 who were enrolled in the HeadSMART (Head Injury Serum Markers for Assessing Response to Trauma) study. The derivation cohort included 355 participants with complete baseline (day-of-injury) and follow-up data. The primary

Prediction of Global Functional Outcome and Post-Concussive Symptoms after Mild Traumatic Brain Injury: External Validation of Prognostic Models in the Collaborative European NeuroTrauma Effectiveness Research in Traumatic Brain Injury (CENTER-TBI) Study.

The majority of traumatic brain injuries (TBIs) are categorized as mild, according to a baseline Glasgow Coma Scale (GCS) score of 13-15. Prognostic models that were developed to predict functional outcome and persistent post-concussive symptoms (PPCS) after mild TBI have rarely been externally validated. We aimed to externally validate models predicting 3-12-month Glasgow Outcome Scale Extended (GOSE) or PPCS in adults with mild TBI. We analyzed data from the Collaborative European NeuroTrauma Effectiveness Research in Traumatic Brain Injury (CENTER-TBI) project, which included 2862 adults wi

The clinical value of serum S-100 protein measurements in minor head injury: a Scandinavian multicentre study.

PURPOSE: This study of patients with minor head injury was designed to investigate the relation of S-100 protein measurements to computed tomograpy (CT) findings and patients outcomes. Increased serum levels of this protein were hypothetized to predict intracranial pathology and increased frequency of post-concussion symptoms. METHODS: One hundred and eighty-two patients were studied with Glasgow Coma Scale scores of 13-15. The study recruited patients from three Scandinavian neurotrauma centres. Serum levels of S-100 protein were measured at admittance and CT scans of the brain were obtained

Evaluation of cerebrovascular CO2-reactivity and autoregulation in patients with post-traumatic diffuse brain swelling (diffuse injury III).

The present study was undertaken to elucidate the status of autoregulation and CO2-reactivity soon after injury in patients with a post-traumatic diffuse bilateral brain swelling. A prospective study was carried out in 31 consecutively admitted patients with a severe head injury and a Diffuse Brain Injury type III, following the definition stated by the Traumatic Coma Data Bank classification. To evaluate CO2-reactivity, AVDO2 was measured before and after ventilator manipulations. Assuming a constant CMRO2 during the test, changes in 1/AVDO2 reflect changes in CBF. Patients with changes in es

Social Determinants of Health Influence Mild Traumatic Brain Injury Symptom Burden: A Retrospective Study.

BACKGROUND AND OBJECTIVES: Traumatic brain injury (TBI) affects over 69 million people worldwide, most of whom suffer a so-called "mild" injury. Patients with mild TBI, defined as a Glasgow Coma Scale (GCS) of 13-15 on presentation, often have a significant symptom burden as detected by the Rivermead Post-Concussion Questionnaire (RPQ). In this study, we aimed to determine whether social determinants of health (SDOH) may influence patient self-report of symptoms within a month of injury. METHODS: Patients presenting to an academic Level I trauma center with GCS 13-15 were included in the stud

Chronic post-traumatic headache after head injury in children and adolescents.

This was a prospective, observational study of children aged 3 to 15 years admitted to hospital with head injury (HI). Demographic data and information on the nature of the HI, and history of premorbid headache were collected. A structured telephone questionnaire was used to interview parents and children 2 months after injury and at 4-monthly intervals for up to 3 years, if headache was reported. One hundred and ninety children were admitted with HI. Data were available on 117 children (81 males, 36 females; mean age 8y 5mo [SD 3y 1mo]). HI was minor in 93 patients and significant in the rest

Pediatric Gunshot Wound to Visual Cortex with Retained Bullet: Case Report and Review of the Literature.

INTRODUCTION: Pediatric gunshot wounds (GSWs) to the head are not well studied in the literature, especially in civilians. With a dearth of case-based and clinically relevant information, pediatric neurosurgeons may be challenged when considering the risks and benefits of removing retained bullet fragments in different intracranial locations. We explore the literature and highlight the key factors in the surgical decision-making case of a 16-year-old girl with GSW to the visual cortex. CASE REPORT: A 16-year-old girl was shot in the head in a parieto-occipital trajectory with the bullet cross

Assessment of the predictive factors and outcomes of surgically treated patients with depressed skull fracture at a tertiary hospital, Northwest Ethiopia.

INTRODUCTION: Depressed skull fractures occur when a portion of the skull is displaced inward towards the brain, leading to complications such as intracranial hematoma, brain contusion, and intracranial infection. Managing these fractures necessitates a multidisciplinary approach, with postoperative management and rehabilitation playing crucial roles in optimizing patient outcomes. This study aimed to assess the predictive factors and outcomes of patients who underwent surgical treatment for depressed skull fractures. METHOD: A comprehensive retrospective review was undertaken on the medical

Targeting dual specificity tyrosine-phosphorylation-regulated kinase 1A mitigates tauopathy and enhances recovery after repetitive head injury.

Chronic neuroinflammation and accumulation of phosphorylated Tau (pTau) are hallmark features of several neurodegenerative diseases and are also observed in some individuals who have sustained traumatic brain injury (TBI). Notably, more than 70 % of patients presenting to emergency departments with mild TBI (Glasgow Coma Score of 13-15) exhibit neuropathological alterations despite a normal sensorium, and up to half experience prolonged post-injury symptoms. Dual specificity tyrosine-phosphorylation-regulated kinase 1A (DYRK1A) is a serine/threonine protein kinase that contributes to tau phosp

Posttraumatic tension pneumocephalus.

This case presentation will review tension pneumocephalus as a rare etiology of delayed posttraumatic headache (PTHA). It demonstrates that clinicians must be aware of even the uncommon causes of PTHA if appropriate diagnostic assessment and treatment are to be rendered. The case involves a 26-year-old right-handed white male who was 4 years post severe traumatic brain injury with facial fractures and an initial Glasgow Coma Scale score of 5. The patient's main postinjury functional impairments were cognitive-behavioral dysfunction, dysmetria, left hemiparesis, and posttraumatic epilepsy. Appr
