Practical norms in emerging infectious disease control: lessons for transnational collaboration from a suspected newly emerging zoonosis outbreak in Ghana.

Concern around the emergence of zoonoses with pandemic potential has fuelled significant foreign engagement with domestic infectious disease surveillance and response systems across Africa. These international efforts at augmentation have likely been hampered, however, by an inattention to how such systems actually manifest on the ground and the critical activities and undertakings that take place outside of official structures and protocols. Such deviations from official protocols have previously been treated as inherently detrimental to public service delivery. A growing body of anthropologi

The Impact of Global Environmental Changes on Infectious Disease Emergence with a Focus on Risks for Brazil.

Environmental changes have a huge impact on the emergence and reemergence of certain infectious diseases, mostly in countries with high biodiversity and serious unresolved environmental, social, and economic issues. This article summarizes the most important findings with special attention to Brazil and diseases of present public health importance in the country such as Chikungunya, dengue fever, yellow fever, Zika, hantavirus pulmonary syndrome, leptospirosis, leishmaniasis, and Chagas disease. An extensive literature review revealed a relationship between infectious diseases outbreaks and cl

Impacts of biodiversity on the emergence and transmission of infectious diseases.

Current unprecedented declines in biodiversity reduce the ability of ecological communities to provide many fundamental ecosystem services. Here we evaluate evidence that reduced biodiversity affects the transmission of infectious diseases of humans, other animals and plants. In principle, loss of biodiversity could either increase or decrease disease transmission. However, mounting evidence indicates that biodiversity loss frequently increases disease transmission. In contrast, areas of naturally high biodiversity may serve as a source pool for new pathogens. Overall, despite many remaining q

Decreased Incidence of Pediatric Intussusception during COVID-19.

Coronavirus disease 2019 (COVID-19) changed the epidemiology of various diseases. The present study retrospectively investigates the epidemiologic and clinical changes in pediatric intussusception for ages ≤ 7 years before (February 2019-January 2020) and after (February 2020-January 2021) the COVID-19 outbreak in a single pediatric emergency department of a university-affiliated tertiary hospital. The incidence of communicable diseases-defined as infectious diseases with the potential for human-to-human transmission via all methods, non-communicable diseases, and intussusception were decrease

Patient characteristics of the Accident and Emergency Department of Kenyatta National Hospital, Nairobi, Kenya: a cross-sectional, prospective analysis.

BACKGROUND: Resource-limited settings are increasingly experiencing a 'triple burden' of disease, composed of trauma, non-communicable diseases (NCDs) and known communicable disease patterns. However, the epidemiology of acute and emergency care is not well characterised and this limits efforts to further develop emergency care capacity. OBJECTIVE: To define the burden of disease by describing the patient population presenting to the Accident and Emergency Department (A&E) at Kenyatta National Hospital (KNH) in Kenya. METHODS: We completed a prospective descriptive assessment of patients in

Can You Catch It? Lessons Learned and Modification of ED Triage Symptom- and Travel-Screening Strategy.

INTRODUCTION: Efficient identification and isolation of patients with communicable diseases limits exposure to health care workers, other patients, and visitors. In August 2014, our team developed and implemented an algorithm to triage suspected cases of Ebola virus disease in a midwestern United States emergency department and outpatient clinics based on patient travel history and symptoms. Here, we present the lessons learned and modifications to update the tool. METHODS: Two strategies were developed and utilized to properly identify, isolate, and inform on patients with suspected highly h

Public health implications of emerging zoonoses.

Many new, emerging and re-emerging diseases of humans are caused by pathogens which originate from animals or products of animal origin. A wide variety of animal species, both domestic and wild, act as reservoirs for these pathogens, which may be viruses, bacteria or parasites. Given the extensive distribution of the animal species affected, the effective surveillance, prevention and control of zoonotic diseases pose a significant challenge. The authors describe the direct and indirect implications for public health of emerging zoonoses. Direct implications are defined as the consequences for

Vaccine-preventable diseases in humanitarian emergencies among refugee and internally-displaced populations.

Humanitarian emergencies may result in breakdown of regular health services including routine vaccination programs. Displaced populations including refugees and internally displaced persons are particularly susceptible to outbreaks of communicable diseases such as vaccine-preventable diseases (VPDs). Common VPDs encountered in humanitarian emergencies include measles, polio, and depending on geographical location, meningococcal meningitis, yellow fever, hepatitis A, and cholera. We conducted a review of 50 published articles from 2000 to 2015 concerning VPDs in humanitarian emergencies. This a

Cardiovascular manifestations of the emerging dengue pandemic.

Dengue is one of the most important emerging viral diseases globally. The majority of symptomatic infections result in a relatively benign disease course. However, a small proportion of patients develop severe clinical manifestations, including bleeding, organ impairment, and endothelial dysfunction with increased capillary permeability causing hypovolaemic shock that can lead to cardiovascular collapse. Evidence is increasing that dengue can also cause myocardial impairment, arrhythmias and, occasionally, fulminant myocarditis. No antiviral agents or vaccines are licensed for dengue, and trea

Recognition and Management of Infectious Biothreats and Emerging Pathogens.

Emerging and high-consequence infectious diseases pose substantial critical care challenges because of diagnostic uncertainty and limited targeted medical countermeasures. Presentations are nonspecific, and patients can rapidly progress to shock, ARDS, or multiorgan dysfunction syndrome. Because these conditions can mimic common causes of sepsis or respiratory failure, management focuses on early, optimized supportive care (including volume resuscitation, ventilatory and oxygenation support, and management of potential coinfections), appropriate infection prevention and control practices (util

Neuropsychiatric Complications of Two Re-Emerging Viruses (DENV and MPXV): Current Evidence, Pathophysiological Mechanisms, and Diagnostic Challenges.

Dengue virus (DENV) and Monkeypox virus (MPXV) are two major re-emerging viral pathogen, which have recently gained global attention, mainly in light of their potential for widespread transmission and the ability to cause various complications. While the characteristic manifestations of these infections comprise lymphadenopathy, fever, and rash, recent studies suggest a darker dimension, neuropsychiatric complications, which are a hallmark of neurovirulent viral pathogens. However, current evidence suggests that these complications, which may vary from mild to critical conditions such as menin

The epidemiological profile of incidence and mortality from epidemics in complex humanitarian emergencies from 1990 to 2022 - A scoping review.

AIM: This study aimed to investigate the impact of communicable diseases with epidemic potential in complex emergency (CE) situations, focusing on the epidemiological profile of incidence and mortality and exploring underlying factors contributing to increased epidemic risks. METHODS: Using the Preferred Reporting Items for Systematic Reviews and Meta-Analyses for Scoping Review (PRISMA-ScR) guidelines, we conducted a scoping review of articles published between 1990 and 2022. The search included terms related to complex emergencies, communicable diseases, outbreaks, and epidemics. We identif

Melioidosis: Emerging beyond endemic areas.

Recent cases suggest that melioidosis, an infection caused by Burkholderia pseudomallei, is an emerging infectious disease. Nurses have a key role in the care of patients with melioidosis. This article provides an overview of the epidemiology, clinical presentation, diagnosis, treatment, and prevention of melioidosis, and discusses unusual, non-travel-related cases of melioidosis.

[Histoplasma capsulatum: an emerging agent in Chile?].

In the last eleven months, we have diagnosed 9 cases of Histoplasmosis in our country. All patients affected were from endemic areas of South-America. Here, we wish to inform and prevent to all clinical laboratories from Chile about the presence of Histoplasma capsulatum in clinical samples. In the same way we want to prepare and raising awareness of the strengthening of biosecurity measures.

Vaccines, emerging viruses, and how to avoid disaster.

Rino Rappuoli is a graduate of Siena University, where he also earned his PhD before moving to the Sclavo Research Center, the Italian vaccine institute, also in Siena. He then spent two years in the USA, mostly at Harvard with John Murphy and Alwin Pappenheimer working on a new diphtheria vaccine based on a non-toxic mutant of diphtheria toxin which has since become the basis for conjugate vaccines against haemophilus, meningococcus, and pneumococcal infections, before returning to the Sclavo Research Center where he developed an acellular vaccine based on a mutant pertussis toxin.
