BACKGROUND: Colorectal cancer (CRC) screening by annual fecal immunochemical test (FIT) is an accessible and cost-effective strategy to lower CRC incidence and mortality. However, this mode of screening depends on follow-up colonoscopy after a positive FIT result. Unfortunately, nearly one-half of FIT-positive patients fail to complete this essential screening component. Patient navigation may improve follow-up colonoscopy adherence. To deliver patient navigation cost-effectively, health centers could target navigation to patients who are unlikely to complete the procedure on their own.

INTRODUCTION: Colorectal cancer (CRC) screening utilization is low among low-income, uninsured, and minority populations that receive care in community health centers (CHCs). There is a need for evidence-based interventions to increase screening and follow-up care in these settings.
METHODS: A multilevel, multi-component pragmatic cluster randomized controlled trial is being conducted at 8 CHCs in two metropolitan areas (Boston and Los Angeles), with two arms: (1) Mailed FIT outreach with text reminders, and (2) Mailed FIT-DNA with patient support. We also include an additional CHC in Rapid C

INTRODUCTION: Uncertainty and rapid changes in treatment guidelines are influencing factors for medication use during the COVID-19 pandemic. Therefore, this study aimed to evaluate antibiotic utilization in Community Health Centers (CHCs) between 2020 and 2021.
METHODOLOGY: Prescription data from adult outpatient visits (≥ 18 years) receiving prescription services at 35 CHCs in South Jakarta, Malang regency, and Bengkulu city, comprising 796,598 prescriptions, were examined using the ATC/DDD methodology and DU90% analysis. These regions were purposively selected to represent three distinct he

Community health centers (CHCs) play a critical role in addressing the health needs of diverse and underserved communities. However, there remains a significant gap in the literature examining their resilience, particularly in crises such as COVID-19. This scoping review analyzed 68 articles to identify which performance indicators have been used to assess the impact of the pandemic on CHCs, and to understand what these indicators reveal about the CHCs' performance and resilience. The findings revealed that most performance indicators focused on access, availability, and quality of care, highl

We leveraged electronic health record (EHR) data from the Accelerating Data Value Across a National Community Health Center Network (ADVANCE) Clinical Research Network (CRN) to identify social risk factor clusters, assess their association with obstructive sleep apnea (OSA), and determine relevant clinical predictors of cardiovascular (CV) outcomes among those experiencing OSA. Geographically informed social indicators were used to define social risk factor clusters via latent class analysis. EHR-wide diagnoses were used as predictors of 5-year incidence of major adverse CV events (MACE) using

Colorectal cancer (CRC) is the third most common cancer and third leading cause of cancer-related death among African Americans in the United States. However, when detected early, CRC is treatable and survival rates are high. CRC health disparities for African Americans compared with other groups may be due in part to lower screening adherence and later stage diagnosis. The objective of this research phase was to test predictors of ever having received CRC screening (i.e., self-report of lifetime receipt of CRC screening) using survey measures in the domains of healthcare communication, trust

BACKGROUND: Emergency departments (EDs) are experiencing a growing burden of non-urgent presentations, which frequently signal deficiencies in access to primary and community-based healthcare. Evidence suggests that a substantial proportion of ED visits could be safely and effectively managed within community care settings. The general aim of the study was to identify which ED visits could have been managed within primary and community care services in a scenario of full implementation of community health centers.
METHODS: A retrospective observational study was conducted. This study examined

BACKGROUND: Short inter-pregnancy interval (IPI) is associated with adverse health outcomes for women and infants, and low-income women experience disproportionate rates of short IPI. An essential solution is providing postpartum (PP) women with timely contraceptive care. However, patient-centered approaches for facilitating care access are needed.
OBJECTIVE: To explore Community Health Center (CHC) staff and provider perspectives on the implementation of a clinical trial offering co-scheduled well-infant/maternal contraceptive care for women with infants 0 to 6 months at the Well-Baby Visit

BACKGROUND: Virtual care (VC) services (e.g., telemedicine) could help address transportation barriers to healthcare among rural Veterans. The Veterans Health Administration (VA) Virtual Health Resource Centers (VHRCs) provide in-person and telephone technology support for Veterans to use VC, but few are in rural locations. Our objective was to understand Veteran perceptions of a novel implementation of a rural, community-based VHRC. METHODS: We conducted our study using an established learning health system model and a rapid qualitative analysis approach to collect and analyze data. Data we

This article describes a nurse-run clinic for low-income individuals or individuals experiencing homelessness. We conducted a quality improvement project using data from 111 individuals over 194 visits. Thirty-seven percent of patients had difficulty managing their health and 22% reported being in an active health crisis. The visits resulted in patients leaving with their visit goal being met (86%), and 96% of nonemergent cases being diverted from the emergency room. The nurse-run clinic provided access to care and served as a safety net for a vulnerable population, providing support for this

BACKGROUND: The tremendous burden of cardiovascular risk among persons with serious mental illness underscores a critical need for prevention. Counseling by primary care clinicians increases patient smoking cessation, physical activity, and the consumption of fruits and vegetables. The extent to which community mental health clinicians counsel about cardiovascular risk factors has not been reported.
METHODS: This cross-sectional study examines the rates of counseling about cardiovascular risk factors by mental health providers at an urban community mental health center (n = 154). Logistic reg

Sociodemographics, clinical characteristics, and life stressors of community-dwelling suicidal risk and nonsuicidal risk elders referred to a community aging and mental health provider were compared in this study. Information was collected through case manager surveys and agency records on 683 older adults referred to the Elder Services Program of Spokane Mental Health in 1994 and the first 6 months of 1995. This sample included 109 individuals who were clinically judged to be at suicide risk by case managers at the time of initial assessment. Comparisons between suicidal risk and nonsuicidal

To measure the prevalence of cocaine abuse at a community mental health center's inpatient alcohol unit, a 20 item document was used to obtain information from the medical records of 162 patients admitted from January 1988 through July 1988. The data collected were analyzed to evaluate the assessment, diagnosis, treatment and referral of patients with coexisting alcohol and cocaine disorders. Of the 162 patients in the sample, 154 (95%) had a diagnosis of alcohol abuse/withdrawal and five (3%) had a diagnosis of cocaine abuse. Fifty-five patients had used cocaine for at least one year and 39 (

BACKGROUND: Linkage to community health and social services after incarceration is often inadequate. This is troubling, because risk taking and poor health are common immediately after release, and access to health providers and insurance helps individuals to transition home.
OBJECTIVES: This article describes how faculty and clinicians at a large academic health center in New York City, established partnerships with justice programs to jointly develop services that aimed to increase health care access for men in the justice system.
METHODS: We describe how faculty and students from Columbia

Community Health Centers (CHCs) provide care to 32.5 million low-income patients across high need areas, forming the largest primary care network in the US. CHCs consistently improve outcomes and lower costs, yet face growing financial stress. Rising demand, workforce shortages, and uncompensated care costs are stretching limited budgets at CHCs, while Medicaid payments and federal funding do not reflect the true cost of care. Many CHCs lack financial reserves to modernize or join new payment models. This article argues for long-term investments to maintain access to affordable and comprehensi

OBJECTIVE: This study evaluates completion of treatment for latent tuberculosis infection (LTBI) in Mexican immigrant children aged 1-18 years in a Community Health Center (CHC).
METHODS: Children were screened for LTBI at a CHC. All children with a tuberculin skin test (TST) ≥10 mm had a chest radiograph (CXR). Those with negative CXR had nurse appointments to collect demographic information and to begin 9 months of INH treatment. A minimum 6 months of INH treatment defined completion.
RESULTS: Between November 15, 2006 and March 15, 2009, 157 children had positive TSTs. Three never had a C

Differences that emerge in comparisons of persons applying for psychiatric care at a mental health center with those applying to the private sector are comfounded by marked diagnostic differences beyond obvious social class differences. To circumvent that problem and to ascertain what differences, if any, persist after diagnosis is held constant, the authors examined demographic and psychosocial characteristics of neurotics in a one-year adult applicant population, the only major diagnostic group treated in large numbers and similar proportions by the center and the private sector. Findings sh
