Acute attacks of aquaporin-4 antibody neuromyelitis optica spectrum disorder (AQP4-Ab NMOSD) are typically managed with intravenous methylprednisolone, with therapeutic plasma exchange (PLEX) reserved for severe episodes. Despite these interventions, a subset of patients fails to respond and may sustain significant neurological disability. Complement C5 inhibitors, currently approved for long-term relapse prevention, have recently gained attention as a potential rescue therapy by rapidly inhibiting the terminal complement cascade, thus preventing downstream astrocytic injury. We have previously shown that reoxygenation of hypoxic human umbilical vein endothelial cells (HUVECs) leads to the activation and deposition of complement. In the present study, we investigated whether the terminal complement complex (C5b-9) influences HUVEC nuclear factor-kappaB (NF-kappaB) translocation and vascular cell adhesion molecule-1 (VCAM-1) protein expression after hypoxia/reoxygenation by decreasing endothelial cGMP. Additionally, we investigated the action of anti-human C5 therapy on endothelial cGMP, NF-kappaB translocation, and VCAM-1 protein expression. Reoxygenation (0.5 to 3

A simple and robust assay was developed for the quantification of PEG2000-DMG and TS-202, key components of the lipid nanoparticle (LNP) formulation of E8001-LNP, which encapsulates small-interfering RNA targeting complement C5, in rabbit plasma. The assay utilizes high-performance liquid chromatography coupled with tandem mass spectrometry (LC-MS/MS). To prevent degradation of PEG2000-DMG, plasma samples were treated with a serine protease inhibitor prior to protein precipitation extraction. The analytes were separated on a reversed-phase column under gradient elution conditions. Detection wa

Paroxysmal nocturnal haemoglobinuria (PNH) is a rare clonal disorder caused by PIG-A mutations and characterized by complement-mediated haemolysis. C5 inhibitors are recommended as first-line therapy for PNH with high disease activity. We retrospectively analysed 57 patients treated with eculizumab or crovalimab. Luminex® liquid-phase chip technology was used to detect changes in complement and cytokine levels in patients pre- and post-treatment. Haemolysis control (LDH ≤ 1.5 × ULN) was achieved in 78.9% of patients, and 68.3% became transfusion-independent. Mean haemoglobin rose from 76 to 99

The importance of T-cell receptor (TcR) V beta gene haplotype and complement component C5 deficiency for the resistance to collagen-induced arthritis (CIA) in the SWR mouse was studied. Firstly, the immune responses against heterologous and autologous type II collagen (CII) were compared between SWR mice and arthritis-susceptible and major histocompatibility complex (MHC)-identical DBA/1 mice. Secondly, F1 and F2 crosses were made between the two strains and studied for arthritis development, V beta gene usage and C5 presence. After immunization with heterologous rat CII, both strains reacted

Complement activation contributes to amyotrophic lateral sclerosis (ALS) neuropathology, but whether motor neuron degeneration is driven by proximal or terminal activation products remains unresolved. Failed trials of C5 inhibition - ravulizumab (CHAMPION-ALS) and zilucoplan (ATHLEET/HEALEY ALS Trial), and of systemic C3 inhibition (pegcetacoplan, MERIDIAN) underscore this mechanistic gap. We compared two pharmacologically distinct inhibitors in hSOD1G93A mice. CR2Crry blocks complement at C3 and localizes to sites of active deposition. BB5.1, functionally equivalent to ravulizumab, selectivel

BACKGROUND: Complement-mediated myocardial and vascular injury is a hallmark of ST-elevation myocardial infarction (STEMI). Dysregulated activation of complement component C5 has been implicated in endothelial dysfunction, exacerbated inflammatory responses, and progressive vascular damage during acute ischemia. Experimental data suggest that antagonizing the C5a–C5aR1 axis may attenuate these effects, but clinical evidence regarding the impact of established C5 inhibitors during STEMI remains limited. METHODS: To investigate real-world cardiovascular and thromboembolic outcomes associated wit

Patients with genetically determined deficiency of complement component 5 are usually diagnosed because of recurrent invasive Neisseria meningitidis infections. Approximately 40 individual cases have been diagnosed worldwide. Nevertheless, reports of the responsible genetic defects have been sporadic, and we know of no previous reports of C5 deficiency being associated with a number of independent meningococcal disease cases in particular communities. Here we describe C5 deficiency in seven unrelated Western Cape, South African families. Three different C5 mutations c.55C>T:p.Q19X, c.754G>A:p.

Our previous experiments showed that immune IgG and F(ab')2, but not Fab', mediated serum killing of Escherichia coli 0111B4, strain 12015 (12015), without significantly increasing the extent of terminal complement (C) component attachment to the bacterial surface. We concluded that bactericidal antibody must change either the site or the nature of C5b-9 bacterial attachment. To pursue this possibility, conditions necessary for elution of C5b-9 from the bacterial surface were examined. Forty-two to 44% of 125I-C9 was released from the serum-resistant nonpresensitized 12015 by 1 M NaCl or 0.1%

C3 glomerulopathy is characterized by accumulation of complement C3 within glomeruli. Causes include, but are not limited to, abnormalities in factor H, the major negative regulator of the complement alternative pathway. Factor H-deficient (Cfh-/-) mice develop C3 glomerulopathy together with a reduction in plasma C3 levels. Using this model, we assessed the efficacy of two fusion proteins containing the factor H alternative pathway regulatory domains (FH1-5) linked to either a non-targeting mouse immunoglobulin (IgG-FH1-5) or to an anti-mouse properdin antibody (Anti-P-FH1-5). Both proteins i

The implication of complement in multiple diseases over the last 20 years has fuelled interest in developing anti-complement drugs. To date, the focus has been on C5; blocking cleavage of C5 prevents formation of two pro-inflammatory activities, C5a anaphylatoxin and membrane attack complex. The concept of C5 blockade to inhibit inflammation dates back 30 years to the description of BB5.1, an anti-C5 blocking monoclonal antibody raised in C5-deficient mice. This antibody proved an invaluable tool to demonstrate complement involvement in mouse disease models and catalysed enthusiasm for anti-co

Complement-mediated damage to the neuromuscular junction (NMJ) is a key mechanism of pathology in myasthenia gravis (MG), and therapeutics inhibiting complement have shown evidence of efficacy in the treatment of MG. In this study, we describe the development of a subcutaneously administered N-acetylgalactosamine (GalNAc)-conjugated small interfering RNA (siRNA) targeting the C5 component of complement that silences C5 expression in the liver (ALN-CC5). Treatment of wild-type rodents with ALN-CC5 resulted in robust and durable suppression of liver C5 expression. Dose-dependent serum C5 suppres

Epidermolysis bullosa acquisita (EBA) is an inflammatory subepidermal blistering disease characterized by circulating and tissue-bound autoantibodies specific for type VII collagen of the basement membrane zone. The antibodies consist of both complement- and noncomplement-binding populations and belong to all four subclasses of IgG. We investigated the presence of the membrane attack complex C3b, C5, and S protein in EBA and compared C3b and C5 in EBA and bullous pemphigoid. In 10 patients with EBA, these components were detected at the basement membrane zone as follows: membrane attack comple

Ravulizumab (ULTOMIRIS™) an intravenous glycoengineered humanized anti-C5 IgG2/4 monoclonal antibody (mAb), is a new FDA-approved treatment for Aquaporin-4-antibody (AQP4-IgG) positive Neuromyelitis Optica Spectrum Disorder (NMOSD). Based on the importance of intermediate and terminal components of the complement cascade during disease pathogenesis, in the last few years, a mAb targeting the C5 complement factor (anti-C5, eculizumab) has already been in use for treating AQP4-IgG positive NMOSD. Ravulizumab acts similarly to eculizumab, binding specifically to the complement protein C5, thereby

We studied the interaction of African trypanosomes with human complement. Bloodstream forms of Trypanosoma brucei subsp. gambiense isolated from mice activated the alternative pathway of complement during a 30-min incubation in vitro. In human serum, all cells remained intact and motile during this period. C3 was detected on the surface by a direct binding assay with a monoclonal antibody which recognizes C3b and iC3b. C3 deposition could also be detected by this radioimmunoassay when parasites were incubated with purified C3. Such C3 binding was enhanced by factor B, factor D, and magnesium.
