Decompression sickness in surface decompression breathing air instead of oxygen.

We report an unusual decompression sickness (DCS) incident in a commercial diving project. Eleven divers completed 91 dives to 23.5-36.2 m with bottom times ranging 23-67 min. The divers were breathing compressed air while immersed. Decompression was planned as surface decompression in a deck decompression chamber breathing oxygen typically for 15-30 min. Due to a technical error the divers breathed air rather than oxygen during the surface decompression procedure. Two divers suffered DCS. Both were recompressed on site with the same error resulting in them breathing compressed air rather than

Evaluation of airborne chrysotile concentrations associated with packing replacement during valve repair or overhaul.

Asbestos was used in certain packing products for much of the twentieth century. The objective of this study was to characterize exposure from working with and around chrysotile-containing packing during valve repair and overhaul to better understand the exposure potential of career pipefitters and other tradesmen, as well as bystanders and household contacts. Airborne fiber and chrysotile concentrations during packing (20.91-62.77% chrysotile) replacement were measured during standard and nonstandard work tasks involving valve repair and overhaul, cleanup, and clothes handling. Packing replac

Does CO2 pneumoperitoneum alter the ultra-structuture of the mesothelium?

BACKGROUND: Pneumoperitoneum may be responsible for ultra-structural alterations in the mesothelium during laparoscopy. To characterize the effect of pneumoperitoneum on the mesothelial cells with CO(2) and compressed air; and to compare to laparotomy and control group (anesthesia only). MATERIALS AND METHODS: Forty C-57 mice were divided in four groups of 10 animals each: CO(2), air, laparotomy, and control group. The animals were submitted to pneumoperitoneum at 8 mmHg during 30 min (CO(2) or compressed air). Five animals of each group were sacrificed 2 and 24 h after the procedure. Fragmen

Giant air cell of the petrous apex: a possible cause of facial hypalgesia.

A giant air cell of the left petrous apex was found in a 23-year-old man with ipsilateral facial hypalgesia. The size of the giant air cell depicted on computed tomography was 1.5 x 2.0 x 2.0 cm. A coronal T1-weighted magnetic resonance image showed that the trigeminal nerve was compressed superomedially by a large signal void area that was probably a result of excessive pneumatization of the petrous apex. It is suggested that the facial hypalgesia was caused by the compression by the giant air cell of the petrous apex on the trigeminal nerve.

A case of tension pneumoperitoneum with fecal peritonitis due to high-pressure air insufflation through the anus.

KEY CLINICAL MESSAGE: The reckless or ridiculous usage of high pressure compressed air could lead to disastrous consequences as demonstrated in this case. Injuries from a barotrauma can vary from a simple mucosal laceration to tension pneumoperitoneum causing abdominal compartment syndrome. Decompression by a wide-bore needle can be done as depicted in our patient to provide immediate relief. ABSTRACT: Rectal perforation most commonly occurs due to trauma, but rarely due to a high pressure compressed air passing through the anus as a part of playful joke. Owing to the belief of medico-legal i

Orbital Emphysema Following Ocular Trauma and Sneezing.

Orbital emphysema is typically a benign condition that occurs following forceful injection of air into the orbital soft tissue spaces. In many cases there is a history of trauma and fracture of an orbital bone, which permits air entry. However, other mechanisms of orbital emphysema have been reported including infection, pulmonary barotrauma, injury from compressed-air hoses, and complications from surgery including dental procedures. Here, we describe a report of a teenager who suffered an isolated medial orbital wall fracture while playing basketball, and several hours later developed orbita

Breath-Hold Diving Injuries - A Primer for Medical Providers.

Breath-hold divers, also known as freedivers, are at risk of specific injuries that are unique from those of surface swimmers and compressed air divers. Using peer-reviewed scientific research and expert opinion, we created a guide for medical providers managing breath-hold diving injuries in the field. Hypoxia induced by prolonged apnea and increased oxygen uptake can result in an impaired mental state that can manifest as involuntary movements or full loss of consciousness. Negative pressure barotrauma secondary to airspace collapse can lead to edema and/or hemorrhage. Positive pressure baro

Hollow Notched K-Wires for Bone Drilling With Through-Tool Cooling.

Kirschner wire (K-wire) is a common tool in clinical orthopedic surgery for bone fracture fixation. A significant amount of heat is generated in bone drilling using K-wires, causing bone thermal necrosis and osteonecrosis. To minimize the temperature rise, a hollow notched K-wire in a modified surgical hand drill with through-tool cooling was developed to study the bone temperature, debris evacuation, and material removal rate. The hollow notched K-wire was fabricated by grinding and micro-milling on a stainless steel tube. Bone drilling tests were conducted to evaluate its performance against

Air quality assessment in Delhi: before and after CNG as fuel.

A number of policy measures have been activated in India in order to control the levels of air pollutants such as particulate matter, sulphur dioxide (SO(2)) and nitrogen dioxide (NO(2)). Delhi, which is one of the most polluted cities in the world, is also going through the implementation phase of the control policies. Ambient air quality data monitored during 2000 to 2003, at 10 sites in Delhi, were analyzed to assess the impact of implementation of these measures, specifically fuel change in vehicles. This paper presents the impact of policy measures on ambient air quality levels and also t

Cervicofacial and mediastinal emphysema as the result of a dental procedure.

Cases of cervicofacial subcutaneous emphysema occurring during dental treatment often result from the use of air-water cooled dental drills during tooth extraction. A case is presented in which a compressed air syringe, used to dry the field, caused diffuse cervicofacial emphysema with retropharyngeal and mediastinal extension. The point of entry appeared to be a 4 mm superficial laceration of the buccal mucosa. Despite the size of the wound, a significant amount of air was able to enter the tissues and spread quite distantly. Though many cases of subcutaneous emphysema go unnoticed, diffuse e

Periorbital emphysema in mechanical air blast injury.

A young adult male presented to the emergency department, due to an accidental compressed air injury by high-pressurised air hoses, complaining of left-sided facial pain, swelling and an inability to open his left eye. The left eye appeared oedematous on clinical examination, with swelling extending to the premaxillary region. Additional findings included conjunctival chemosis, palpable crepitus and tears in the inferotemporal bulbar conjunctiva accompanied by subconjunctival haemorrhage. CT of the brain, orbits and facial bones revealed periorbital emphysema around the left orbit, with air po

Cardiovascular changes during acute episodic repetitive hypoxic and hypercapnic breathing in rats.

It has been shown that chronic repetitive ambient hypoxia, simulating pulmonary gas disturbances observed in apnoea, leads to systemic hypertension in rats. However, the relative roles of hypoxia (HO), hypercapnia (HC), gas stress and vigilance on the cardiovascular changes have not yet been demonstrated. The aim of this study was to investigate the acute haemodynamic changes observed during the repetitive inhalation of various gas mixtures in rats for HO alone and HO+HC, and to analyse the effects of vigilance and of the stress of gas administration. We studied 6 unanaesthetized Wistar rats c

The analgesic effect of inhalational Entonox for extracorporeal shock wave lithotripsy.

Extracorporeal shock wave lithotripsy (ESWL) is a non-invasive procedure that allows urinary stones to be fragmented using acoustic shock waves. The impact of the shock waves causes transient stinging pain at the entry site as well as deep visceral discomfort, requiring analgesia during the procedure. The objective of this study was to compare the clinical efficacy of Entonox and pethidine for pain relief during outpatient ESWL. We randomized 150 outpatients undergoing elective ESWL into three groups of 50 patients, each group receiving inhalational Entonox, intravenous pethidine, or inhalatio

[Central nervous system involvement in patients with decompression illness].

Dysbarism or decompression illness (DCI), a general term applied to all pathological changes secondary to altered environmental pressure, has two forms decompression sickness (DCS) and arterial gas embolism (AGE) after pulmonary barotrauma. Cerebral and spinal disorders have been symptomatically categorized as AGE and DCS, respectively. Magnetic resonance images (MRIs) of divers with DCI showed multiple cerebral infarction in the terminal and border zones of the brain arteries. In addition, there were no differences between MRI findings for compressed air and breath-hold divers. Although the p

Elimination of CT-detected gas bubbles derived from decompression illness with abdominal symptoms after a short hyperbaric oxygen treatment in a monoplace chamber: a case report.

We report the case of a 54-year-old male compressed-air worker with gas bubbles detected by computed tomography (CT). He had complained of strong abdominal pain 30 minutes after decompression after working at a pressure equivalent to 17 meters of sea water for three hours. The initial CT images revealed gas bubbles in the intrahepatic portal vein, pulmonary artery and bilateral femoral vein. After the first hyperbaric oxygen treatment (HBO₂ at 2.5 atmospheres absolute/ATA for 150 minutes), no bubbles were detected on repeat CT examination. The patient still exhibited abdominal distension, mild

Therapeutic effects of inhaling aerosolized surfactant alone or with dexamethasone generated by a novel noninvasive apparatus on acute lung injury in rats.

BACKGROUND: Pulmonary surfactant (PS) administration has been attempted for the treatment of adults with acute lung injury (ALI)/adult respiratory distress syndrome. Aerosolized surfactants inhaled by spontaneous breathing may be an effective method of surfactant-based therapies. Using a noninvasive apparatus, we evaluated the therapeutic effects of aerosolized PS alone or together with dexamethasone (Dex) on a rat model of ALI. METHODS: Severe ALI was induced by intravenous injection of 20% oleic acid (0.2 mL/kg) into adult Sprague-Dawley rats. Animals were divided into eight groups: sham (n

[High-pressure air-blast injury of the esophagus].

Blast injuries causing pneumatic damage to the oesophagus are very rare. Patients usually present with respiratory distress, subcutaneous and mediastinal emphysema, and may also have pneumothorax. Ruptures need early repair to avoid serious morbidity and potential lethal complications. We report about a successfully treated 47-year-old patient, who has been admitted to our department with compressed air injury of the oesophagus.

Preventing tablet defects through vacuum-assisted deaeration of a powder bed.

Lamination is a common defect observed during pharmaceutical tablet compression and can be attributed to air entrapment (lamination type 1) within the powder bed. In this study, a novel vacuum-inducing chamber was implemented to remove air from the powder bed prior to compaction, thereby enabling, for the first time, a systematic investigation of lamination type 1 while decoupling air entrapment from other contributing factors. Microcrystalline cellulose (MCC) was compacted on a Huxley Bertram compaction simulator equipped with a vacuum chamber under systematically varied compressive stresses,
