VALID AND INFORMED CONSENT IN ORTHOPAEDIC SURGERY: A MULTICENTRE, REGIONAL SERVICE EVALUATION OF CURRENT UK PRACTICE.

BACKGROUND: In recent years, there has been increasing focus upon tailoring the consent process to reflect patients' individual needs and concerns. Meanwhile, clinical litigation costs for 'failure to warn' as part of 'informed consent' remain staggeringly high. We aimed to investigate the validity of the patient consent process in elective lower limb arthroplasty surgery regionally, with a view to ascertaining how it could be improved. METHODS: Regional data across the East of England was collected retrospectively from seven hospital trusts (fifty data sets per hospital) in 2021 and analyzed

Shortcomings of protocols of drug trials in relation to sponsorship as identified by Research Ethics Committees: analysis of comments raised during ethical review.

BACKGROUND: Submission of study protocols to research ethics committees (RECs) constitutes one of the earliest stages at which planned trials are documented in detail. Previous studies have investigated the amendments requested from researchers by RECs, but the type of issues raised during REC review have not been compared by sponsor type. The objective of this study was to identify recurring shortcomings in protocols of drug trials based on REC comments and to assess whether these were more common among industry-sponsored or non-industry trials. METHODS: Retrospective analysis of 226 protoco

Opening the "black box" of informed consent appointments for genome sequencing: a multisite observational study.

PURPOSE: Little is known about how health-care professionals communicate with patients about consenting to genome sequencing. We therefore examined what topics health-care professionals covered and what questions patients asked during consent conversations. METHODS: Twenty-one genome sequencing consent appointments were audio recorded and analyzed. Participants were 35 individuals being invited to participate in the 100,000 Genomes Project (14 participants with rare diseases, 21 relatives), and 10 health-care professionals ("consenters"). RESULTS: Two-thirds of participants' questions were s

How Does Alcohol Use Relate to Sexual Consent? A Scoping Review.

People consume alcohol prior to both consensual and nonconsensual sexual activity. To better inform education and prevention efforts targeting the coalescing of alcohol, sexual consent, and sexual violence, we conducted a scoping review of 57 articles published between 1999 and 2024 that examined the relationship between alcohol use and sexual consent. Most articles were qualitative and investigated how alcohol related to perceptions of consent, internal consent feelings, and external consent communication. Each article's findings were coded to assess how alcohol was related to sexual consent

Epigenetics in glaucoma: a link between histone methylation and neurodegeneration.

Cervical cancer, caused primarily by human papillomavirus (HPV), causes substantial health burden in the Republic of Palau. Despite availability of the HPV vaccine through a school-based vaccination program for girls beginning in 5th grade, HPV vaccination coverage in Palau has declined since 2020. To understand why, this descriptive cross-sectional study analyzed data from the first opportunity for vaccination for all 5th grade girls enrolled at Koror and Babelthaob schools between August 1, 2020 and June 30, 2024. Data were retrieved from the Ministry of Education, School Health Program, and

[Informed consent in congenital defect heart surgery].

In pediatric cardiac surgery, informed consent is often obtained from the patient's parents. The parents are commonly under great stress due to being in a situation where they have to make the important decision of whether their child will undergo cardiac surgery. Cardiac surgery does not necessarily determine the patient's prognosis if he or she has other organ anomalies or systemic diseases. In such cases, communication among the cardiac surgeon, pediatrician, and family becomes even more crucial. In pediatric cardiac surgery, it is important to obtain informed consent with adequate explanat

Consent and risk disclosure of COVID in the post-pandemic era.

Informed consent is a fundamental component of ethical and legal medical practice in the United Kingdom (UK). As COVID-19 transitions to an endemic phase, this study explores how consistently its ongoing risk is communicated during the surgical consent process. We prospectively reviewed 147 surgical consent forms collected over 2 months in 2024 at a UK tertiary centre. The primary aim was to assess how often COVID-19 risk was documented; secondary outcomes included postoperative infection rates and 30-day mortality. COVID-19 was explicitly mentioned in only 5.4% of consent forms. No significan

A template for broad consent in biobank research. Results and explanation of an evidence and consensus-based development process.

BACKGROUND: Biobanks increasingly presume long-term storage of biomaterials and data that shall be used for future research projects which are today unspecified. Appropriate consent documents for sample donors must therefore explain the breadth of consent and other elements of the biobank governance framework. Recent reviews demonstrated high variability in what issues these documents mention or not and how the issues are explained. This might undermine the protection of sample donors, complicate networked biobank research, create research waste and impact on public trust. METHODS: A systemat

Readability and complexity of written information presented to hospitalised patients for trial consent during the COVID-19 pandemic in the UK: a retrospective document analysis.

OBJECTIVES: Patient information sheets (PISs) and informed consent forms (ICFs) are essential tools to communicate and document informed consent for clinical trial participation. These documents need to be easily understandable, especially when used to take informed consent from acutely unwell patients. Health literacy guidance recommends written information should be at a level between reading ages 9-11. We aimed to assess the readability and complexity of PISs/ICFs used for clinical trials of acute therapies during the COVID-19 pandemic. DESIGN: Retrospective document analysis. SETTING: PI

Postal recruitment and consent obtainment from index cases of narcolepsy.

BACKGROUND: Access to research volunteers may be hampered by low numbers of cases and few eligible participants for rare diseases in clinical settings. METHODS: We recruited volunteers and obtained informed consent by mail from narcolepsy cases in a case-control study, and here in we report feasibility, response rate, timeliness and cost. We invited index cases into the study by mail through their care-giving physicians then mailed study information and consent forms to cases that indicated interest in the study. RESULTS: Of the 33 index cases invited, 15 (45.0%) expressed interest in the st

Use of next generation sequencing technologies in research and beyond: are participants with mental health disorders fully protected?

BACKGROUND: Next Generation Sequencing (NGS) is expected to help find the elusive, causative genetic defects associated with Bipolar Disorder (BD). This article identifies the importance of NGS and further analyses the social and ethical implications of this approach when used in research projects studying BD, as well as other psychiatric ailments, with a view to ensuring the protection of research participants. METHODS: We performed a systematic review of studies through PubMed, followed by a manual search through the titles and abstracts of original articles, including the reviews, commenta

IRB perspectives on the return of individual results from genomic research.

PURPOSE: Return of individual research results from genomic studies is a hotly debated ethical issue in genomic research. However, the perspective of key stakeholders-institutional review board (IRB) professionals-has been missing from this dialogue. This study explores the positions and experiences of IRB members and staff regarding this issue. METHODS: In-depth interviews with 31 IRB professionals at six sites across the United States. RESULTS: IRB professionals agreed that research results should be returned to research participants when results are medically actionable but only if the pa

Documenting informed consent in elective hip replacement surgery: a simple change in practice.

Adequate documentation of informed consent is an important aspect of many doctors' work and failings in this area may result in patient dissatisfaction or litigation. In 2009 the documentation of risks and complications on the consent forms of patients undergoing primary hip replacement was audited at a specialist centre. Following this audit procedure-specific complication stickers were introduced in an attempt to improve the documentation of the consent process. This article discusses the results of a re-audit and aims to determine if this change in practice had resulted in any significant i

Influence of mood state on capacity to consent to research in patients with bipolar disorder.

OBJECTIVES: Experts have debated the ability of patients with bipolar illness to consent to research participation when manic. We compared 25 euthymic bipolar and 26 manic bipolar patients in their understanding of three hypothetical studies of varying complexity. METHODS: Hypothetical studies were presented as written informed consent forms, which were reread up to three times. Questions included assessment of hypothetical study purposes, procedures, risks and benefits, and voluntariness of participation. RESULTS: After adjusting for educational level, subjects with mania had lower mean und

Contemporary human rights violations in female sterilization care: legal and ethical considerations when coerced patients do consent.

In this piece we examine three forms of coercive or otherwise involuntary care that can occur with patient consent. To do so, we examine: (1) uninformed consent, (2) contingency-based consent and (3) constrained-market consent, amongst female sterilization patients. While there is broad recognition that "coercion" in sterilization care can manifest beyond instances of overt force and clarity on what constitutes coercion within clinical care, this has not translated to accountability. The current practice of identifying coercion through discrete civil cases may facilitate a narrow understanding
